1.Primary repair of tetralogy of Fallot and double-outlet right ventricle with anomalous coronary arteries

Objective: To review the results with an approach of primary repair for tetralogy of Fallot (TOF) and double-outlet right ventricle (DORV) with anomalous coronary arteries. Methods: From June 1995 to June 2002, 12 patients with TOF and DORV associated with anomalous coronary arteries crossing an obstructed right ventricle outflow tract underwent primary surgical repair. To avoid injury of the anomalous coronary arteries, some modified surgical techniques were used. Main pulmonary artery translocation in 2 patients, repair under a mobilized left anterior descending coronary artery in 3, displaced ventriculotomy with subcoronary suture lines in 5, and the right ventricle outflow tract was repaired via the arteriotomy and/or pulmonary incision in 2. Results: There were no early or late death, and no myocardial infarction. Follow-up ranged from 3 months to 6 years, there were no residual leakage and obstruction of right ventricle outflow tract. Conclusion: Primary repair of TOF with anomalous coronary arteries can be done with excellent results. A proper surgical technique should be used during operation.
2.Study on the mechanism of asporin in degeneration of intervertebral disc

Objective To explore the molecular regulation mechanism of asporin in the matrix synthesis and secretion of the intervertebral disc,and to clarify its role in degenerative lesions of intervertebral disc.Methods There were 8 cases of intervertebral disc tissue in patients with severe intervertebral disc herniation (including typical clinical symptoms,signs and Pfirrmann's grade Ⅲ).There were 6 male and 2 female with an average age of (20.25 ± 3.37) years old (ranged from 11 to 28 years).After primary culture and redifferentiation in alginate beads,cells were reseeded and treated with different concentrations of TGF-β1 for 6,12,18 and 24 h.Total RNA extracted from the cells and was subjected to real-time PCR analysis to examine the expression of asporin.After silencing the expression of endogenous asporin by siRNA,the cells stimulated 24 h with TGF-β1.Total RNA extracted from the cells and was subjected to real-time PCR analysis to examine the expression of asporin,collagen Ⅱ and proteoglycans.After treatment of specific p38 inhibitor or ERK inhibitor for 12 h,cells were stimulated with TGF-β1 for 24h.Protein extracted from the cells by protein extraction kit to examine the level of asporin.Results In the primary intervertebral disc cell experiment,TGF-β1 stimulation induced asporin transcription significantly in a dose and time dependent manner.After 24 h stimulation,a significant difference between different concentration groups (5,10 and 15 ng/ml) was observed,2.754±0.24,3.651 ±0.319 and 4.583±0.38,respectively (F=24.782,P=0.001).Knockdown of endogenous asporin led to the upregulated expression of aggrecan and collagen]Ⅱ (aggrecan:t=7.387,P=0.002,collagen Ⅱ:t=4.443,P=0.0113).Specific p38 inhibitor was used to block p38 phosphorylation,and TGF-β1 on asporin induction was significantly inhibited.Conclusion Our results have verified a functional feedback loop between TGF-β1 and asporin in human intervertebral annulus cells indicating that TGF-β1 can increase asporin expression,whereas asporin inhibits TGF-β 1 signaling pathway by negative feedback,thereby inhibiting TGF-β1 mediated synthesis of extracellular matrix,and TGF-31 can increase asporin expression by p38 in human intervertebral disc cells.
3.Application of round hepatic and falciform ligaments in acute abdominal repair: a report of 22 cases

Retrospective study was conducted to analyze the efficacies of using round hepatic and falciform ligaments in acute abdominal repair for 22 cases between December 2010 to March 2013.There was no occurrence of perioperative mortality.Twenty patients (90%) were followed up for 3 to 24 months.All cases recovered favorably and no complications of common bile duct stricture,bile leakage,postoperative hemorrhage or pancreatic fistula occurred.The anatomic and morphological advantages of pedicled ligamentum teres hepatic and falciform ligaments have made it a high-quality autologous biological repair material applicable for a variety of acute abdominal repairs.
4.Construction of evaluation index system and development of quality of life scale for liver and kidney transplant recipients

Objective To Establish an evaluation system of the quality of life for Chinese liver and kidney transplant recipients,and develop a specialized scale to measure the quality of life (QOL)for such recipients preliminarily.Methods By combining subjective and objective methods,this study determined the evaluation system and created items of QOL scale based on literature analysis,Delphi method (expert consultation),interviews,and experience of researchers.Then,a nationwide clinical survey was conducted on 5 Level Ⅲ Class A hospitals.454 valid questionnaires were collected,including 153 from liver recipients,and 301 from kidney recipients.Subjective statistical methods such as critical Ratio method,correlation coefficient method,dispersion tendency method,and exploratory factor analysis were used for selection of questionnaire items.Results The study established an evaluation system of QOL for liver and kidney transplant recipients,including 4 primary indexes and 12 secondary indexes,and developed QOL scales that were appropriate for liver and kidney transplant recipients respectively.Conclusions The structure of QOL scale for liver and kidney transplant recipients was basically consistent with theoretical assumption.The QOL scale for patients showed good reliability and validity,therefore,it can fully reflect the quality of their life.
5.Investigation on quality of life among liver and kidney transplant recipients

Objective To explore the factors associated with the quality of life (QOL) in patients after liver and kidney transplantation.Methods A multicenter cross-sectional survey was carried out in 5 Level Ⅲ Class A hospitals.153 liver transplant recipients and 301 kidney transplant recipients of outpatient clinic follow-up from March to December in 2015 were selected and investigated with a self-designed general state questionnaire,and Quality of Life Scale for Liver and Kidney Transplant Recipients which developed by our research group.Results There were significant differences in QOL total score in liver and kidney transplant recipients among groups of marital status and occupation.Divorced,unemployed and low-educated patients showed lower QOL total score than married,employed and high-educated ones (P < 0.05).As compared with non-living-related transplantation group,patients undergoing living-related transplantation presented a better status in QOL total scores (P<0.05).The QOL total score was obviously lower in patients suffering from complications and rejection than in those without occurrence of complications and rejection (P< 0.01).Postoperative time was correlated positively and significantly with QOL scores,and variances existed among different stages postoperation (P < 0.01).Multivariable regression analysis demonstrated that education,marital status,postoperative time,type of donor and chemotherapy were the factors influencing liver transplant recipients' QOL,while marital status,postoperative time,type of donor,medical care assurance,complications and rejection after operation had effect on kidney transplant recipients.Conclusion Attaching importance to QOF among liver and kidney transplant recipients,and implementing scientific and effective nursing intervention based on the characteristics of them are necessary.
6.Sustained-released mixture of vascular endothelial growth factor 165 and fibrin glue strengthens healing of ileal anastomoses in a rabbit model with intraperitoneal infection.

PURPOSE: To investigate the effects of a sustained-released mixture of vascular endothelial growth factor 165 (VEGF165) and fibrin glue (FG) local administration on postoperative rabbit ileal anastomoses. METHODS: One hundred twenty-eight male and female New Zealand white rabbits underwent intraperitoneal infection subsequent ileal anastomosis surgery were divided randomly into 4 groups, including 32 animals in each, applied with saline solution, FG, rhVEGF165 and a mixture of rhVEGF165 with FG (VEGF + FG) on the anastomoses, respectively. The incidences of anastomotic leakage were observed. Histopathological examination for inflammatory infiltration, fibroblast proliferation, and capillary vascular proliferation were performed. Then, bursting pressure and hydroxyproline concentrations were assessed in anastomoses sits on postoperative days 3, 5, 7, and 14. RESULTS: Rabbits in VEGF + FG group had the lowest incidence of leakage (P < 0.05). Histological evaluations revealed that granulation tissue was formed on days 5 after anastomosis; fibroblast proliferation and capillary vascular proliferation were significantly increased on days 7 and 14 in VEGF + FG group. Furthermore, there was a statistically significant difference in the mean bursting pressures between VEGF + FG group and other groups on days 7 and 14 (P < 0.05), and rabbits in VEGF + FG group exhibited a higher concentration than VEGF group (P < 0.05) and FG group (P < 0.05) on day 14. CONCLUSION: Administration of VEGF165 mixed with FG to ileal anastomosis accelerates wound healing and enhances the anastomosis by increased angiogenesis.

7.Intergration of co-immunoprecipitation technology and mass spectrometric analysis for sperm membrane protein screening

Objective To reveal the mechanisms of immunological infertility,the method of coimmunoprecipitation(CO-IP) and liquid chromatogram mass/mass (LC-MS/MS) was used to screen sperm membrane proteins which interacting with antisperm antibodies (ASA).Methods This study was designed as a case-control.The disease group including 56 serum samples from 521 cryptogenic infertile patients were screened ASA positive by ELISA and conformed with mixed antiglobulin reaction(MAR).The controls were 31 serum samples which ASA is negative and already possessed healthy offspring.All subjects were enrolled from September 2015 to December 2015 in China PLA General Hospital.Spermatozoa samples from 48donors with normal sperm parameters were from January 2016 to April 2016 in China PLA General Hospital.The purified human sperm membrane proteins were then mixed with serum from disease group (positive for ASA) and control group (not containing ASA).The binding proteins of antisperm antibodies were enriched using CO-IP assay.The immunoprecipitates were separated on sodium dodecyl sulfate-polyactylamide gel (SDS-PAGE),then the binding proteins were cut from the gel and analyzed by LC-MS/MS after the enzymolysis.These proteins could bc idcntified as definition,biological function (s) and subccllular localization with Uniprot database.Results The serum samples from infertile persons (39 females and 17males) were screened ASA positive by ELISA and conformed with MAR.The healthy controls (17 females and 14 males) were ASA-negative in ELISA.Forty proteins that interact with ASA were obtained from the study and these could be divided into three groups:11 antigens detected by control serum samples only,14antigens recognised by both infertile patients and control sera,and 15 antigens specific for patients with ASA.These 15 proteins are Sperm Cation channcl protein 1,Sperm Cation channel protein 3,Sperm Cation channel protein 4,Sperm associated antigen 9,Apolipoprotein A-I,Dynein heavy chain 14,Cylicin-2,Izumo sperm-egg fusion protein 4,Thioredoxin domain-containing protein 2,IQ domain-containing protein H,IQ domain-containing protein F1,Spermatogenesis-associated protein 5,Spermatogenesis-associated protein 5-like protein 1,Sperm acrosome membrane-associated protein 1,E3 ubiquitin-protein ligase RNF 114.Conclusion Fifteen proteins discovered with CO-IP technology and LC-MS/MS analysis could be referred as male immunoinfertility-related antigens and they may hold the great importance in revealing the secret of immunological infertility.
8.Anatomical and histological observation of ileocecal Treves fold and its clinical significance

From December 2011 to August 2017,56 patients with acute abdomen surgery were treated in Affiliated Zhongshan Hospital of Dalian University.The anatomical structure of ileocecal Treves folds was observed during operation,and 15 samples of Treves folds were taken for histological examination.The mean length of the fold was (5.56±0.18)cm for males and (5.22±0.05)cm for female,the width was (1.82± 0.04) cm for males and (1.78 ± 0.06) cm for females;the thickness was (0.18 ± 0.02) cm.HE and verhoeff staining showed that Treves fold was mainly composed of elastic fiber,collagen fiber and smooth muscle with abundant blood supply.Thirty patients were repaired with their own Treves folds,including the fixation with overlay suture to the stump of appendectomy in 26 cases,repairing for perforation of colonic diverticulum in 3 case,and repairing for perforation of colonic diverticulum in 1 case.There were no complications such as fecal fistula,intestinal obstruction and intussusception after operation.The Treves fold has a constant location,which can be used as a clear sign of ileal terminal.It has abundant blood supply,and is suitable for the repair and reconstruction in ileocecal operation.
9.Clinical characteristics of acute renal infarction: an analysis of 15 cases

Clinical data of 15 patients diagnosed with acute renal infarction (ARI) in Affiliated Zhongshan Hospital of Dalian University from Jan 2011 to Dec 2021 were retrospectively analyzed. Of the included 15 patients, there were 14 cases of cardiac origin and 1 case of antiphospholipid syndrome. We found that there were 12 cases of atrial fibrillation, 2 cases of atrial premature beats, 12 cases of elevated level of D-dimer, 15 cases of elevated level of LDH, 11 cases of positive urine occult blood and positive urine protein. Among the 15 patients, catheter-directed thrombolysis was performed in 4 cases, of which 3 cases were revascularized successfully, intravenous thrombolysis in 2 cases and alone anticoagulation therapy in 9 cases. It is suggested that CECT or CTA can assist the early diagnosis of ARI especially in patients with acute onset and persistent abdominal pain with high risk factors of thromboembolism, high levels of LDH, microscopic hematuria and/or proteinuria. Despite prolonged embolic ischemia, try to reconstruct blood flow to save the kidney as much as possible. Late standardized anticoagulant therapy is of critical importance to prevent recurrent embolic episodes.
10.Prevention suggestion based on the correlation between epidemiological characteristics of blood pressure level and glucose metabolism in patients with altitude sickness

Objective To explore the epidemiological distribution characteristics of blood pressure in patients with altitude sickness, and the influence of abnormal glucose metabolism and other factors on abnormal blood pressure in patients with altitude sickness, so as to propose preventive plans. Methods From the 358 patients with altitude sickness diagnosed in our hospital from January 2017 to June 2021, 259 patients who met the inclusion criteria of this study were selected for analysis (99 patients with previously diagnosed hypertension were excluded, and the final 259 patients) The study selected 259 subjects, collected information on blood pressure, blood glucose levels, and other potential influencing factors, analyzed associations, and discussed reasonable prevention strategies. Statistics using SPSS19.0. Results The mean systolic blood pressure of newly diagnosed hypertensive patients was (154.36±12.58) mmHg, the diastolic blood pressure was (93.69±9.54) mmHg, and the pulse pressure difference was (61.25±10.28) mmHg, (compared with the control group t/P value: 16.505/ <0.001, 10.122/<0.001 and 13.644/<0.001); the patients were accompanied by dysglycemia and dyslipidemia at the same time; the regression equation for preventing abnormal blood pressure levels in patients with altitude sickness was fitted based on epidemiological characteristics, and it can be seen that older manual labor practitioners have high altitude The risk of high blood pressure is higher in patients with high altitude sickness, and dyslipidemia, elevated blood sugar and smoking habits are potentially associated with the increased risk of high altitude sickness and high blood pressure. Regression equation: y=-2.611-0.070x1+0.130x2+0.874 x3+0.849x4+0.309x5+0.341x6+0.895x7-0.064x8 . Conclusion Strengthening the publicity and education of health knowledge related to chronic diseases in patients with altitude sickness, enhancing their awareness of high-risk factors such as hyperglycemia, and effective control of blood sugar and blood lipids have positive effects on preventing complicated hypertension.
