1.Expression and role of CD40/CD40L and B7-1/CD28 interaction in folic acid-induced nephropathy

Objective To explored the role of CD40/CD40L and B7-1/CD28 interaction in the immunopathologic process of folic acid-induced nephropathy(FAN) and investigate the intervention effect of the anti-B7-1 monoclone antibody(B7-1mAb). Methods GD1 mice were administrated by i. p with FA only or co-treated with B7-1 mAb for the intervention study. Kidneys were harvested for immunohistochemical, Western blot assays and serum for renal function evaluation at day 1, 3,5, 7, 14 and 21. Results Continuous expression of CD40, B7-1 from day 1 to day 21 on the renal tubular epithelial cells was detected in this model. 2.Impact of palliative radiotherapy to the prognosis of patients with stage Ⅳ non-small cell lung cancer

Objective To investigate the impact of palliative radiotherapy to the prognosis of patients with stage Ⅳ non-small cell lung cancer (NSCLC).Methods Two hundred and forty-four cases of stage Ⅳ NSCLC were collected from our hospital.The study group (n =129) received the treatment of primary tumor and metastases palliative radiotherapy after chemotherapy.The control group (n =115) received only chemotherapy.Followed-up and compared 1-year and 2-year survival rate of the patients,and explored the factors affecting palliative radiotherapy.Results The 129 patients in the study group were followed up for 2 years,and 16 patients survived and 110 patients died.Three patients were lost to follow-up.The median survival time was 8 months.1-year and 2-year survival rates was 29.46% (38/129) and 12.40% (16/129) respectively.In the control group,7 patients survived,and 106 patients died.Two patients were lost to follow up.The median survival time was 6 months.1-year and 2-year survival rates was 11.30% (13/115) and 6.09% (7/115)respectively.Survival rates between the two groups were significantly different (x2 =8.451,P =0.014)Univariate regression analysis found that factors affecting the prognosis of stage Ⅳ NSCLC were KPS score (x2 =5.057,P =0.031),occurrence of brain metastases (x2 =4.781,P =0.029),number of organs of metastasis (x2 =6.341,P =0.010),and the primary stove radiotherapy dose (x2 =5.893,P =0.015) ; Cox regression model analysis showed that the number of organs of metastasis (HR =1.719,95% CI 为 1.172-3.126,P =0.008),the primary focal radiotherapy dose (HR =1.560,95% CI 为 1.082-2.761,P =0.022) were independent prognostic factors.Conclusion Palliative radiotherapy plays a role of prolonging survival time in the treatment of Ⅳ NSCLC.In palliative radiotherapy,we should pay attention to the control of the primary tumor and metastases radiotherapy dose,therefore prolonging the survival time of patients.
3.Clinical investigation of neuropeptides in patients with trigeminal neuralgia

砄bjective: To observe whether substance P(SP), calcitonin gene related peptide(CGRP), vasoactive intestinal polypeptide(VIP),? endorphin(? EP) are related to the pathogenesis of trigeminal neuralgia. Methods: During the attacks of trigeminal neuralgia, SP,CGRP,VIP,? EP in plasma from both external jugular and cubital fossa vein blood ipsilateral to the pain were assessed by sensitive radioimmunoassay in 16 ptients and 10 health controls, after operations the venous blood from the external jugular vein of the patients was sampled again; the external jugular vein blood of normal volunteers was collected as control. SP like and CGRP like immunoreactive positive granules of pain and painless branches were labelled using ABC immunohistochemistry methods; quantitative analysis was performed using HPIAS 1000 system. Results: During the attacks of trigeminal neuralgia, the levels of SP,CGRP,VIP in the external jugular vein blood were significantly higher than those in the cubital fossa vein blood, postoperative external jugular vein blood and the external jugular vein blood of normal volunteers. The plasma levels of ? EP in patients with trigeminal neuralgia were significantly lower than that in normal volunteers.The number and area of SP like ,CGRP like immunoreactive positive granules in the pain branches was much more and larger than those in the painless branches . Conclusion: Abnormal synthesis and release of neuropeptides in patients may be related to the pathogenesis of trigeminal neuralgia.
4.Effect of multiple IPL treatment on cell senescence markers in skin fibroblasts

Objective To study the effect of multiple IPL treatment on cell senescence markers of skin fibroblasts using UVA as a control and to make clear whether the multiple IPL treatment may result in cell senescence.Methods Cells were divided into three groups: one group without irradiation as a control,one group receiving IPL treatment with 15 J per cm2,and the last group receiving UVA irradiation with 9 J per cm2.IPL and UVA irradiation were performed once a day during five days.On the sixth day,the cells were collected.Senescence-associated β-galactosidase (SA-β-Gal) staining,cell cycle,reactive oxygen species (ROS) and telomere length were determined.Results Our results showed that five consecutive days of IPL irradiation had no effect on the activity of SA-β-Gal and telomere length and decreased the G1 % of cell cycle and the level of ROS in comparison with the control group (P<0.05).On the contrary,five consecutive days of UVA irradiation increased the activity of SA β Gal and the level of ROS,shortened the length of telomere and no obvious change in the G1 % of cell cycle in comparison with the control group.Conclusions Multiple UVA irradiations induce cell senescence.On the contrary,multiple IPL treatments could not induce cell senescence.
5.Influence of local-anaesthetic combined with dezocine on brachial plexus block and postoperative analgesia

Objective To observe the influence of local-anaesthetic combined with dezocine on brachial plexus block and postoperative analgesia.Methods 60 patients scheduled for unilateral upper limb operation were randomly divided into two groups of C group and D group with 30 cases in each group.The brachial plexus block was performed under ultrasound guiding.Two groups received a bolus injection of local anesthetic 30ml,but the local bolus injection of D group consisted of Dezocine 10mg.The tine of sensory blocked producing effect,anesthesia effect,sensory blocked duration were observed.Two hours after sensory nerve recovered,the scores of VAS pain and Ramsay Sedation were recorded.Adverse reactions of perioperation were also observed(nausea,vomiting,dizzy,respiratory,urine retention,pruritus).Results The sensory blocked duration in D group was(7.1±1.1)h,which was significantly longer than that of C group[(4.5±0.8)h](t=10.47,P<0.01);Two hours after sensory nerve recovered,the score of VAS pain in D group was(1.7±0.7)points,and significantly lower than that of C group[(3.9±1.0)points](t=9.87,P<0.01).Both two groups had no adverse reactions.Conclusion Addition dezocine 10mg to local anesthetic solutions for interscalene brachial plexus block,it could prolong the sensory block duration,provide satisfactory postoperative analgesia,have no adverse reactions.
6.Clinical analysis of intractable epistaxis under nasal endoscope in the treatment of elderly people

Objective To summarize the clinical experience of intractable epistaxis under nasal endoscope in the treatment of elderly people .Methods 312 cases of endoscope of intractable epistaxis patients were retrospectively analyzed.Results Nasal septum and olfactory corresponding head of middle turbinate bleeding site was mainly the upper part of the nasal cavity and the split-plot,back-end middle turbinate at the back of the nose and nasal were fol-lowed,as well as inferior turbinate and the rear end of inferior nasal meatus ,the Littell nasal septum of the front nose were bleeding less .After nasal endoscopic radio hemostatic therapy or local packing treatment with expansive sponge , 310 cases were cured,2 cases were invalid.The total efficiency was up to 99.4%.During the following 3 months,there was no recurrence .Conclusion For the elderly intractable epistaxis hemostatic therapy ,under nasal endoscope oper-ation method,radio hemostatic therapy or local packing treatment with expansive sponge is simple and effective ,which is worthy of popularization and application .
7.Adriamycin increases podocyte permeability: evidence and molecular mechanism.

OBJECTIVETo investigate the increased podocyte permeability by evidence of adriamycin (AD) and its molecular mechanism.
METHODSIn this study, we explored the direct effects of AD on cultured mouse podocytes and the potential protection effects of Dexamethasome (Dex).
RESULTSAfter 24-hour AD (5 x 10(-7) mol/L) treatment, albumin passage through podocyte monolayers was increased by 2.27-fold (P < 0.01). AD caused a 62% decrease in Zonula Occluden-1 (ZO-1) protein (P < 0.05), suggesting that AD might increase podocyte permeability by disrupting tight junctions. Dex (1 x 10(-6) mol/L), co-administered with AD, protected podocytes from AD-induced increased albumin passage. This may be linked with an increased P-cadherin protein level to 1.93 fold of control (P < 0.01).
CONCLUSIONSAD has a direct, detrimental effect on podocyte permeability, probably through disrupting tight junctions; Dex could protect against AD-induced high podocyte permeability by upregulating adherent protein P-cadherin.

8.Observation of vasoactive intestinal polypeptide in patients with trigeminal neuralgia: a 16-cases report.

OBJECTIVEThe aim of this study was to observe whether vasoactive intestinal polypeptide (VIP) participated in the attack of trigeminal neuralgia, and to understand further its pathogenetic mechanism.
METHODSSixteen patients were studied. During the attacks the venous blood was sampled from both the external jugular vein and cubital fossa vein ipsilateral to the pain, and after operations the venous blood from the external jugular vein was sampled again, the external jugular vein blood of eleven normal volunteers was collected as the control. Plasma levels of VIP were determined using sensitive radioimmunoassays.
RESULTSDuring the attacks of trigeminal neuralgia the levels of VIP in the external jugular vein blood were significantly higher than that in the cubital fossa vein blood, postoperative external jugular vein blood and the external jugular vein blood of normal volunteers.
CONCLUSIONVIP indeed participates in the attack of trigeminal neuralgia in the local region, the role of VIP may be relating to enhancing the effects of substance P in local neurogenic inflammation.

9.Initial use of trans-ulner or trans-radial approach to coronary revascularization with titanium-nitride-oxide-coated stent

Objective To evaluate the efficacy of trans-ulner or trans-radial approach to coronary revaseularization with titanium-nitride-oxide-eoated stent (TITAN2). Methods 31 patients,who were planed to receive coronary revaseularization,were selected into this study. All patients with coronary revascularization were either by trans-ulner or by trans-radial approach. The pass-through rate of stent, early thrombosis in stent and MACE at followup were recorded for TITAN2. Results 46 lesions with stenosis > 75% with coronary angiograpby in 31 patients successfully underwent coronary revaseularization with TITAN2. One stent didn't go through the lesion in RCA. The pass-through rate of stent was 97.8%. Remaining stenesis ,stent unglues,endarterium laceration ,early thrombosis in stent and coronary rupture were not discovered after stent deployment. MACE was 0 at 1-5 month follow-up. Conclusion Confirmed good efficacy in coronary revascularization with TITAN2 is observed.
10.Protective effect of intense pulsed light on fibroblast injury induced by UVA Ⅰ

Objective To study the protective effects of intense pulse light (IPL) on the injury of normal human skin fibroblasts (FB) induced by ultraviolet A (UVA Ⅰ ) in vitro and to explore its possible mechanism. Methods The human skin fibroblasts were isolated and cultured, and then irradiated by UVA Ⅰ (9 J/cm2) and IPL (15 J/cm2), respectively. The proliferative ability of the cells were detected by CCK-8. Cell cycle was detected by flow cytometry, and cylin D1 and CDK2 protein expression levels were detected by Western blot. Results Different doses of UVA Ⅰ irradiation caused certain damages of cultured fibroblasts. With the increasing of of UVA Ⅰ dose, cell proliferation was decreased. Cells went to death at the exposure to 11 J/cm2 UVA Ⅰ , while the proliferative activity did not change much at 7 J/cm2 UVA Ⅰ . Cells were treated with UVA Ⅰ for other 2 days, then with IPL irradiation for other 2days, showing clear stimulating to the cell proliferation as compared with the cells that received UVA Ⅰ treatment only. Flow cytometry results showed that an increase of cell proliferating index, and cell cycle protein cyclin D1 and CDK2 expression levels were also upregulated after IPL irradiation.Conclusion UVA Ⅰ irradiation may cause cell damage as showed by cell growth index, cyclin D1 and CDK2 expression, and this injury could be protected partly by IPL treatment. The intense pulsed light may regulate the expression of cyclin proteins that may promote normal fibroblast proliferation, which could be one of the mechanisms of IPL skin rejuvenation.
