1.Clinical analysis of nine patients with presumed tubercular retinal vasculitis

Objective To observe the clinical features and treatment outcomes of presumed tubercular retinal vasculitis.Methods This is a retrospective non-comparative interventional clinical research.A total of nine patients (11 eyes) with major presentation of retinal vasculitis were included in this study.Patients first consulted the eye clinic and were diagnosed presumed tubercular retinal vasculitis.The patients,seven males and two females,aged from 19 to 66 years,with an average of 43.89 years.The time interval from symptoms to diagnosis ranged from two weeks to six months with an average of 76.27 days.Visual acuity,slit lamp ophthalmoscopy,fundus fluorescein angiography (FFA),optical coherence tomography (OCT),hematological and tuberculosis related investigations were examined and analyzed.All patients had standard anti-tuberculosis treatment.Treatment outcomes were followed for six to 37 months with an average of 14.11 months.Results Baseline visual acuity ranged from hand movement to 0.8 with an average of 0.28.Among 11 eyes,six presented mild to moderate vitritis,five presented as retinal vein occlusion with no obvious vitirits.Fundus examination showed six cases with retinal hemorrhage,four cases with macular edema,two with macular epiretinal membrane,and two with vitreous hemorrhage.FFA revealed 11 cases with leakage of vessels,11 with nonperfusion area,four with macular edema,three with retinal neovascularization,and two with choroidal lesions.OCT of nine eyes suggested six eyes with retinal edema,three with macular edema,three with macular epiretinal membrane.TST of seven patients were all strong positive.T-SPOT.TB of four patients were all positive.Three of eight patients who had chest X-ray or chest CT were suggested tuberculosis infection.Four to six weeks after the start of anti-tuberculosis treatment,vitritis,exudates,retinal and macular edema subsided.During follow up,inflammation was stable with no recurrence observed.The visual acuity of last follow-up ranged from 0.15 to 0.8 with an average of 0.51.Conclusions The main presentations of presumed tubercular retinal vasculitis are vitritis,retinal vein occlusion,and retinal hemorrhage.Standard anti-tuberculosis treatment can improve inflammation and retinal hemorrhage.

3.A survey on NICU human resources using modified NAS

Objective To investigate the human resources of neurosurgery intensive care unit(NICU)using modified nursing activity score(NAS).Methods NAS was used to investigate the nursing work load of all shifts and compute the needed personnel for every shift,followed by adaption of nursing staff based on the results.Results Based on the scores by NAS,day shift was the heaviest in work load,followed by early night shift and then by late night shift.The differences between day shit and early night shift or late night one were both statistically different(P<0.05 for both).Conclusions The adjustment based on the results by NAS will reduce nursing risk and improve quality of nursing.
4.Comparative observation on electroacupuncture combined with acupoint-injection for treatment of oculomotor paralysis induced by aneurysm of cerebral posterior communicating artery.

OBJECTIVETo compare clinical therapeutic effects of electroacupuncture (EA) combined with acupoint-injection and simple acupuncture on oculomotor paralysis induced by aneurysm of cerebral posterior communicating artery.
METHODSSeventy-eight cases were randomly divided into an acupuncture group and an EA plus acupoint-injection group. In the acupuncture group, Jingming (BL 1), Qiuhou (EX-HN 7), Chengqi (ST 1), etc., were selected and the EA plus acupoint-injection group were treated by the same method as in the acupuncture group, in combination with EA and local acupoint injection of adenosine cobaltamine. After treatment of 3 months, their therapeutic effects were compared.
RESULTSThe total effective rate was 41.7% in the acupuncture group and 77.8% in the EA plus acupoint-injection group, with a very significant difference between the two groups (P<0.01). The therapeutic effect in the patients who received the operation within 2 weeks after attack was better than that in those who received the operation over 2 weeks after attack (P<0.01); the therapeutic effect in the patients who received acupuncture treatment within 1 month after the operation was better than that over 1 month after the operation(P<0.01).
CONCLUSIONEA plus acupoint-injection is an effective method for oculomotor paralysis induced by aneurysm of cerebral posterior communicating artery, with a better therapeutic effect than that of simple acupuncture, and early diagnosis, surgery and timely acupuncture treatment are closely related with its prognosis.

5.Acupuncture for tinnitus, hearing loss in patients with type 2 diabetes using the twirling, ;reinforcing-reducing method

Objective To evaluate the efficacy of acupuncture for tinnitus, hearing loss in patients with type 2 diabetes using the twirling, reinforcing-reducing method. Methods A total of 132 tinnitus, hearing loss patients with type 2 diabetes were included and divided into an observation group and a control group by random number table method, 66 in each group. Patients in both groups were acupunctured at Tinggong, Tinghui, Yifeng in the affected side, and Zhongzhu, Waiguan, Sanyinjiao, Taichong and Taixi in the both sides. Patients in the observation group were acupunctured at Taichong using the twirling reducing method, Taixi using the twirling reinforcing method, and those in the control group both of Taichong and Taixi using the needle retention method. Patients in both groups were also treated with semiconductor laser therapy and ultrashort wave therapy. All patients were treated for 1 month. The severity of tinnitus was assessed by the Tinnitus Severity Scale, and the clinical efficacy was evaluated. Results After the treatment, the Tinnitus Severity Scale score in the observation group was significantly lower than that in the control group (5.71 ± 2.19 vs. 7.14 ± 3.36;t=2.897, P<0.01). The significant improvement rate in tinnitus in the observation group was significantly higher than that in the control group (77.3%vs. 63.6%;χ2=22.091, P<0.01). The significant improvement rate in hearing loss in the observation group was no significant difference than that in the control group (72.4% vs. 63.8%; χ2=0.992, P=0.319). Conclusion Acupuncture using the twirling, reinforcing-reducing method may relieve tinnitus in tinnitus, hearing loss patients with type 2 diabetes.
6.Preparation of hydroxypropyl-β cyclodextrin inclusion compound of volatile oil from Houttuyniae Herba

AIM To prepare the hydroxypropyl-β cyclodextrin inclusion compound of volatile oil from Houttuyniae Herba.METHODS In the content determination of volatile oil by GC,2-undecanone was taken as a reference substance,and inclusion rate and drug loading were calculated.After the determination of optimal method for preparing the inclusion compound among techniques of mixing,grinding and ultrasonic methods.the preparation process was optimized by orthogonal test with ratio of volatile oil to hydroxypropyl-β cyclodextrin,inclusion temperature,mixing speed and inclusion time as influencing factors,and inclusion rate and drug loading as evaluation indices.The characterization was performed by TLC,infrared spectroscopy and differential scanning calorimetry.Then high temperature,high humidity and strong light tests were applied to investigating the inclusion compound stability.RESULTS Mixing method brought forth the maximal yield,inclusion rate and drug loading for the inclusion compound.And under the optimal conditions of 1 ∶ 25 for ratio of volatile oil to hydroxypropyl-β cyclodextrin,50 ℃ for inclusion temperature,420 r/min for mixing speed,and 80 min for inclusion time,the inclusion rate and drug loading were identified to be 77.35% and 4.48%,respectively.The obtained inclusion compound was found to be white powder with loose texture and significantly increased solubility,the in vitro accumulative release rate reached 80.85%.Deliquescence and agglomeration,as well as obviously decreased inclusion rate and drug loading,were observed at a relative humidity of more than 75%,despite the good stability at high temperature (60 ℃) and strong light (3 000 lx),and yet the rehydration character was good.CONCLUSION It is possible that the hydroxypropyl-β cyclodextrin inclusion compound of volatile oil from Houttuyniae Herba generates a new phase rather than a simple mixture,which should be kept dry and sealed at the time of storage.

8.Efficacy of follicular unit extraction in the repair of scarring eyebrow loss: a clinical observation

Objective To evaluate the efficacy of follicular unit extraction (FUE) in the repair of scarring eyebrow loss.Methods A total of 30 patients with scarring eyebrow loss were treated with FUE technique in the Department of Plastic Surgery of Zhengzhou First People's Hospital,including 18 females and 12 males with 41 eyebrow defects.Fine soft hairs with a length of 5-10 mm were extracted from the back of the ear and temporal region of the patients,and then were split into individual follicular units under a microscope at 6 × magnification.The transplant density ranged from 20 to 30 FU/cm2.Results During the 3-18 months of follow-up after the surgery,eyebrows in the recipient areas grew in a natural direction and showed good appearance.The survival rate of hair grafts was up to 90%,and the patients were satisfied with the results.Conclusion FUE may be an ideal method for the treatment of scarring eyebrow loss with less injury and high survival rate of hair grafts.
9.Experimental study on the peripheral blood immune cells NK, NKT and T cells induced by rat hepatic vesicular hydatid ablation with nanosecond knife at different energy levels

Objective:To investigate the changes of natural killer (NK) cells, natural killer T (NKT) cells and T lymphocytes in the peripheral blood after nanosecond pulse ablation of hepatic vesicular hydatid in rats with different energy levels.Methods:A total of 32 SD rats were randomly divided into hepatic vesicular hydatid model group, low voltage group (1 000 V), medium voltage group (1 500 V) and high voltage group (2 000 V). The hydatid model of rats was established by selective injection of 100 μl of echinococcosis head suspension with concentration of 500/100 μl into the left hepatic portal vein in all the 4 groups. After 3 months, nanosecond pulse therapy was applied to the left lobe vesicular hydatid lesions in the low voltage group, medium voltage group and high voltage group. On the third day after treatment, flow detector was used to calculate the ratio of CD 4+ T to CD 8+ T in peripheral blood of four groups by CD 3+ T, CD 4+ T, CD 8+ T, NK cells and NKT cells. Results:CD 3+ T was expressed in the high voltage group with (62.08±2.75)%, the medium voltage group with (63.84±7.73)%, the low voltage group with (55.19±8.55)% and the control group with (54.76±8.28)% ( P<0.05). CD 4+ T number was larger in high voltage group (43.7±6.51)% than medium voltage group (38.82±5.47)%, low voltage group (37.31±6.96)% and model group (38.12±3.04)% ( P<0.05). CD 8+ T ratio in the high voltage group was (20.03±2.40)%, the medium voltage group was (21.22±1.74)%, the low voltage group was (19.00±3.06)%, and the model group (20.56±3.98)%, with no statistically significant difference ( P>0.05). NK cells ratio in high voltage group was (6.49±1.60)%, medium voltage group was (3.02±0.32)%, low voltage group was (3.42±0.91)% and model group was (3.44±0.55)% ( P<0.05). NKT cells ratio in high voltage group was (1.53±0.16)%, medium voltage group was (0.82±0.09)%, in low voltage group was (0.70±0.17)% and model group (0.78±0.10)% ( P<0.05). CD 4+ T/CD 8+ T high voltage group was (2.26±0.65), medium voltage group was (1.90±0.40), low voltage group was (1.98±0.37) and model group was (2.06±0.35) ( P<0.05). Conclusion:High voltage (2 000 V) increased number of T, NK and NKT cells in peripheral blood compared with medium voltage (1 500 V) and low voltage (1 000 V), which may be the immune response of the body caused by nanosecond pulse ablation of hepatic vesicular hydatid in rats.
10.Observation on therapeutic effects of different treatments for sudden deafness.

OBJECTIVETo search for the best acupuncture treatment for sudden sensorineural deafness.
METHODSOne hundred and eighty-seven patients were randomly divided into an acupuncture group (group A), an electroacupuncture group (group B), an acupuncture pius acupoint injection group (group C), an electroacupuncture plus acupoint injection group (group D). Ermen (TE 21), Tinggong (SI 19), Tinghui (GB 2), Yifeng (TE 17), Fengchi (GB 20) on the affected side, and Shangxing (GV 23), Baihui (GV 20), Sishencong (EX-HN 1), Hegu (LI 4), Waiguan (TE 5), etc. were selected. The treatment was given once each day. Hearing was tested after treatment for 40 sessions and the therapeutic effects were compared.
RESULTSThe total effective rate was 47.6% in the group A, 79.2% in the group B, 76.0% in the group C and 94.0% in the group D, the group D being the best and the group A being the worst, and with significant differences between the group D and the other groups (all P < 0.05). In the group D, the therapeutic effect for patients who received the treatment within 2 weeks after attack were superior to those who received the treatment over 2 weeks after attack (P < 0.05); the acupuncture therapeutic effect for the mild and moderate patients was superior to that of severe patients (P < 0.05); the acupuncture therapeutic effect for the patients less than 50 years old was superior to those more than 50 years old (P < 0.05).
CONCLUSIONElectroacupuncture plus acupoint injection is the heat therapy for sudden sensorineural deafness, and the timely intervention of acupuncture, the severity degree of the illness and the patient's age are all related to the prognosis of this disease.
