Multiple myeloma is a monoclonal plasma cell malignant proliferative disease,accounting for about 10％ of malignaut tumors of blood system.In recent years,due to the application of hematopoietic stem cell transplantation and the application of MDT by combining new drug such as thalidomide,lenalidomide and bortezomib,the average survival of MM patients was significantly longer than before.The treatment strategy should be individualized before treatment according to the patient's age,physical condition and complications.In this paper,the treatment strategy of multiple myeloma patients were reviewed.

OBJECTIVE:To discuss the characteristics and regularities of adverse drug reactions (ADR) in our hospital. METHODS:ADR reports of 618 cases collected via ADR network system and submitted to state ADR database from 2005 to 2006 were analyzed statistically in respect of patients' age and sex,rating of correlated factors and outcome,clinical manifestations,routes of administration,and the involved drugs etc. RESULTS:Anti-infective drugs dominated the first place on the list of ADR incidence rate,followed by antineoplastic agent and Chinese patent medicines. The main symptoms of ADR were drug eruption,allergic,athma,followed by gastrointestinal reaction. There were fewer incidences of serious ADR such as bone marrow depression,hypoleukemia and shock. CONCLUSION:The ADR monitoring in Fuzhou General Hospital is lack and should be strengthened,and knowledge of ADR must be publicized for guiding the rational,safe and effective drug use in clinic.

Objective: To get the information about freshmen's mental health and it relationship with sex, city and countryside difference, and to provide the reference for effective mental health education and consultation. Methods: The situation of 1351 freshmen's mental health was surveyed by using UPI. Results: ①11.91% of the freshmen had serious mental derangement, 26.15% of them had a little psychological problem. ②The distinct difference existed not only between male and female, but also between city and countryside. The mental health of male students surpassed the female students, the city students surpassed the countryside students. Conclusion: The situation of medical freshmen's mental health should not be neglected, and its influence factors are various. Mental health education and consultation should be pointed to the students.

Objective To investigate the association between preoperative serum TSH level and differentiated thyroid cancer (DTC).Methods The relationship between TSH concentration and differentiated thyroid cancer in 400 qualified patients with thyroid nodules was retrospectively analyzed from January 2009 to December2011 in our hospital.Based on the TSH values,the patients were divided into five groups.Results Preoperative serum TSH level in DTC group was higher than that in benign thyroid nodales group [(2.94 ± 1.74 vs1.92 ± 1.45) μ IU/ml,P＜0.01].The risk of DTC increased as serum level of TSH rose in the patients with thyroid nodule.The incidences of DTC were 27.8％,34.0％,40.0％,63.4％,and 70.2％ respectively in groups with TSH＜0.27,0.27-1.44,1.45-2.19,2.20-4.20,and ＞4.20 μIU/ml groups.TSH level in the group with cervical lymph node metastasis was higher than that without metastasis (P＜0.01).Conclusions The likelihood of thyroid cancer increases with increased serum TSH concentration.High TSH level seems to be associated with lymph node metastasis.Preoperative serum TSH level might be a risk predictor for DTC.

Objective To investigate the efficacy of PEG-INF alpha-2a(PEG-INF-2a)on patients with chronic hepatitis C.Methods 60 patients with hepatitis C were included in this study.30 patients were treated with PEGINF-2a(180μg or 135μg/week)and RBV 900mg/d,30 patients were treated with IFNα-2a(5 MIU/qod)and RBV 900 mg/d.The time of treatment was 48 weeks,and all the patients were followed up for 24 weeks after treatment.There was no significant difference between the two groups in pretreatment HCV-RNA,HCV genotype and other clinical data.The main parameters to evaluate the efficacy were virological and biochemical responses.The side effects were intensively observed.Results Sustained vimlogical response(SVR)rate in PEG-IFNα-2a group was significantly higher than that in IFNα-2a group(56.5%and 19.5%respectively,P＜0.01).As the patients were divided according to HCV genotype 1 and high virus load.the SVR rate of PEG-IFNα-2a group Was higher than IFNα-2a group(P＜0.01).However,there was no significant difference between two groups in the patients with non-genotype 1 and low viral load(P=0.664,0.116).Similar side-effects were observed in PEG IFNα-2a group and IFNα-2a group,but the rate of weisht decline and the degree of leukocyte decrease were more significant in PEG IFNα-2a group than in IFNα-2a group(P＜0.01).Conclusion The efficacy of PEG IFNα-2a in the treatment of chromic hepatitis C is superior to that of conventional IFNα-2a,PEG-IFNα-2a had good tolerance and safety profiles.

Objective To investigate the effect of hyperphosphatemia on type Ⅱa sodium dependent phosphate cotransporter (NaPi-2)mRNA expression in the kidneys of 5/6 nephrectomized (STNx)rats and evaluate the interference of Renagel. Methods Forty-two male Sprague-Dawley rats underwent 5/6 nephrectomy or sham operation. The animals were then divided into seven groups: (1)STNx+high-Pi diet(HP) group (1.2%P); (2)STNx+low-Pi diet(LP) group(0.2%P); (3)STNx+ normal-Pi diet (NP) group; (4)STNx+HP+Renagel-treated (Ren)group; (5)sham+HP group; (6) sham+LP group; (7)sham+NP group. Serum ionized calcium (iCa) , phosphorus, and iPTH levels were detected at day 2,7,14.The kidneys were removed after the rats were sacrificed on day 14 and serum 1,25-(OH)2D3 and FEp were measured. RT-PCR was applied to examine NaPi-2 mRNA. Results Serum P and iPTH levels in STNx+HP group were significantly higher than those in STNx+ LP and 3 sham groups (P0.05)among these five groups mentioned above. OBJECTIVE： To study the method of measuring the in vitro dissolubility of lansoprazole enteric－ coated tablets and to find out the difference between the dissolubility of lansoprazole enteric－ coated tablets and that of lansoprazole enteric－ coated capsules.METHODS： To establish a measuring method of the dissolubility of lansoprazole enteric－ coated tablets and compare the dissolubility of lansoprazole enteric－ coated tablets with that of lansoprazole enteric－ coated capsules and to acquire the dissolving parameters for variance analysis.RESULTS： (1)The accumulative dissolving quantity of the both preparations exceeded 80％ in 30 minutes.(2)The comparison of Weibull parameters,T50,Td and m,between each other in group A showed no significant differences(P>0.05).It was the same for those of group B.(3)The parameters,T50 and Td,of group A,in comparison with those of group B,showed significant differences(P<0.001).The times needed for dissoiving 50％ and 63.2％ of quantity in group B were both shorter than those in group A.The m parameters in different groups had no significant differences(P>0.05).There was no significant difference in slope of equation between two groups.CONCLUSION： This measuring method of the in vitro dissolubility of lansoprazole enteric－ coated tablets is simple,convenient,accurate,stable,feasible and good in repetition.It is concluded that there is significant difference between the dissolubility of the two dosage forms of enteric－ coated preparation of lansoprazole.

The effect of iodinated contrast on thyroid function and auto-antibodies after a single percutaneous coronary intervention (PCI) was explored.In 96 patients with randomly selected PCI,serum TSH,T3,T4,thyroid peroxidase antibody (TPOAb),thyroglobulin antibody (TgAb) were determined preoperatively as well as by 3-6 months postoperatively.After PCI,the level of TSH,T3,T4,showed no significant changes; TgAb,TPOAb which were positive preoperatively,after 3-6 months were statistically increased(P＜0.05).Iodinated contrast in PCI has no significant influence on thyroid function and auto-antibodies after 3-6 months.

OBJECTIVE:To promote hospital rational drug use. METHODS:A total of 1 359 ADR cases submitted to State ADR Center by our hospital from 2005 to 2007 vial network system were analyzed statistically in respect of patients' age and sex,the drugs involved,routes of administration,ADR-involved organs or systems,and turnover etc. RESULTS:Of the total 1 359 cases,25.75% were induced by antimicrobial drugs and 55.33% were induced by intravenous route. The lesions of skin and its accessories were the predominant presentation of ADR,and the majority ADR cases had a good turnover. CONCLUSION:The incidence of ADR can be reduced by strengthening the education on and monitoring of ADR as well as the sense of responsibility of medical staff.
