1.Distribution of transplanted bone mesenchymal stem cells in colon of ulcerative colitis rat

Objective :To investigate the optimal dose of transplanted bone marrow mesenchymal stem cells (BMSCs) for the ulcerative colitis rat. Methods:The BMSCs of SD rat were isolated,cultured and labelled with DAPI.Colitis of rat was induced with immunecombined TNBS/ethanol in group A,B.and C. Each rat in 3 groups received caudal vein injection of fluids containing 1?106,5?106, and 1?107 BMSCs respectively. 5 rats in each group were sacrificed at day 7 and 14 after injection.Cryostat sections of colon were prepareted.The numbers of BMSCs in focal area of colitis and surrounding normal tissue were counted under fluorescent microscope. Results:The DAPI marked BMSCs could been seen in the colic mucosa in each group on day 7 and 14. The number of BMSCs in the ulcerated area was bigger than that in the surrounding normal tissue (P
2.Clinical effect of telmisartan combined with simvastatin in treatment of early diabetic nephropathy

ObjectiveTo evaluate the clinical efficacy and safty of combined treatment telmisartan plus simvastatin on early diabetic nephropathy. Methods46 patients with early diabetic nephropathy were randomly divided into two groups. Control group ( n = 23 ) received regular insulin therapy, captopril l0mg tid. Study group received telmisartan 80mg qd plus simvastatin 20mg qd on the basis of control group therapy. All patients received 16 weeks treatment. The clinical parameters were measured before and after therapy. ResultsAfter 16 weeks treatment,the urine MAU in both study group and control group were significantly decreased( all P <0. 01 ) ,however,the urine MAU in telmisartan plus simvastatin combination group decreased significantly than control group( P < 0. 01 ). Improvement of lipid level in the study group better than the control group( P <0. 01 ). Kidney function,fasting blood glucose levels and HbAlc had no significant changes before and after treatment. ConclusionTelmisartan plus simvastatin combined treatment was better than the traditional therapy,which can delay the development of early diabetic nephropathy.

A important discovery in tumor molecular biology is that the activation of oncogene and inactivation of tumor suppressor gene play an important role in the mechanism of tumorgenesis. 4.Indicators' weight of competency model in emergency physicians' different career stages

Objective On the basis of previous researches,the paper confirmed the weight of indicators to build emergency physicians'competency models for their different career stages.Methods Using the method of analytic hierarchy process (AHP)to establish evaluation hierarchy for emergency physicians'different career stage and construct the pair wise comparison judgment matrix,checking consistency and judgment matrix at the same time.Results Achieved the construction of competency models for three different stages of emergency physicians,primary,intermediate and advanced included.Conelusion The models can dynamically clarify the career stages of emergency physicians,supporting personalized training and management of the physicians.
5.Observation of the effect of bisoprolol combined with simvastatin in the treatment of elderly patients with unstable angina

Objective To observe the clinical effect of bisoprolol combined with simvastatin in the treatment of elderly patients with unstable angina pectoris.Methods 60 elderly patients with unstable angina pectoris were randomly divided into control group and study group according to the digital table method.The patients in the control group were treated with simvastatin,and the patients in the study group were treated with bisoprolol and simvastatin.The clinical therapeutic effect and adverse reactions of the two groups were compared.Results The total effective rate and the rate of ECG (86.67% and 80%)in the study group were significantly higher than those of the control group (76.67% and 46.67%)(χ2 =4.02,7.1 8,all P <0.05 ).The total incidence rate of adverse reactions (26.67%)in the study group had no significant difference compared with the control group (χ2 =0.08,P >0.05). Conclusion Bisoprolol combined with simvastatin had better clinical effect and small adverse reactions in the treat-ment of elderly patients with unstable angina.It was worthy of clinical use.
6.THE CLINICAL SIGNIFICANCE OF ANTI-CCP ANTIBODY IN PATIENTS WITH RHEUMA-TOID ARTHRITIS

Objective To investigate the clinical application value of Anti -CCP antibody ( CCP) levels in patients with in rheumatoid arthritis ( RA) .Methods 132 patients with rheumatoid arthritis , 42 patients of other categories of connective tissue dis-ease, 40 health cases were collected as the research object.The anti-CCP antibody titers were detected in sera of three patients, and the anti CCP antibody in rheumatoid arthritis in the diagnostic process was calculated .Anti -CCP antibody titers were detected in three group patients, and the specific of anti CCP antibody in diagnosis of rheumatoid arthritis was counted .Results Anti-CCP an-tibody has high specificity for the diagnosis of rheumatoid arthritis , and was correlated with clinical symptoms in patients with rheuma-toid arthritis.Combination detection of anti -CCP antibody and C reaction protein can improve the diagnostic specificity , and the difference was statistically significant (p<0.05).Conclusion The specificity of anti-CCP antibody in diagnosis of rheumatoid ar-thritis is higher and its specificity is higher than that of RF .The combined detection of anti -CCP antibody and RF both can improve the diagnostic specificity of rheumatoid arthritis , and anti-CCP antibodies are related with the degree of clinical symptoms .

8.Effect of thoracic epidural anesthesia and PCEA on blood coagulation and hemorheology in patients undergoing major thoracic surgery

Objective To evaluate the influence of epidural block on blood coagulation and rhedogy in patients undergoing esophagectomy or lobectomy. Methods Twenty-two ASA Ⅰ-Ⅱ patients (14 male, 8 female) aged 40-65 yr undergoing esophagectomy or lobectomy were randomized to receive either combined general-epidural anesthesia (GEA group , n = 11) or general anesthesia (GA group, n = 11). The patients were premedicated with intramuscular pethidine 50 mg, promethazine 25 mg and scopolamine 0.3 mg. In GEA group epidural catheter was placed at T9-10 interspace before general anesthesia. A test dose of 4 ml 2% lidocaine was given. When correct positioning of the epidural catheter was confirmed, general anesthesia was induced with propofol 1.5-2.0 mg? kg-1 , fentanyl 2?g?kg-1 and rocuronium 0.6 mg? kg-1 , after placement of double- lumen endobronchial tube the patients were mechanically ventilated (VT 10 ml?kg-1 , RR 12 bpm, I: E = 1:2). Anesthesia was maintained with isoflurane inhalation and intermittent i. v. boluses of vecuronium. A mixture of 2 % lidocaine and 0.33% dicaine (1:1) was continuously infused at a rate of 4-6 ml?h-1 during operation. After operation PCEA was commenced with 0.12% ropivacaine and morphine 80 ?g?ml-1 (background infusion 4 ml?h-1 , bolus dose 2 ml, lockout interval 6 min) . In GA group the patients received the same general anesthesia technique. Postoperatively the patients were placed on PCIA with morphine 0.5 mg?ml-1 (background infusion 1 ml?h-1 , bolus dose 2 ml, lockout interval 6 min) Pain intensity was measured using VAS (0-10). Blood samples were taken before induction of anesthesia (T0, baseline), 1 and 3 h after skin incision (T1 , T2 ) and on the morning of 1st and 3rd postoperative day (T3 , T4 ) . Blood samples were tested immediately in a thromboelastograph (TEG) coagulation analyzer. Usual clotting tests (PT, APTT, platelet count, Hb, Hct), fibrinolysis tests (t-PA, PAI-A) and rheology tests were performed at the same time. 9.Assessment of fatigue in patients with ankylosing spondylitis

Objective To investigate the prevalence of fatigue in AS and its relationship with other clinical and functional parameters used for the evaluation of disease activity. Methods The fatigue was assessed in 73 patients with active AS using a single-item fatigue question of BASDAI. The correlation between fatigue and the clinical parameters of disease including the Bath AS disease activity index(BASDAI), Bath AS functional index (BASFI), Bath AS metrology index(BASMI), night pain, spinal inflammation, patient's global assessment(PGA) and enthesis index(EI) were determined. Results A total of 73 patients with ankylosing spondylitis were recruited. The prevalence of fatigue was 90.4% and the fatigue score of 68.5% patients was higher than 5. After controlled for the possible confounding factors such as age, gender, age and disease duration, it was found that fatigue was highly correlated with disease activity, pain and functional disability, BASMI,physical health and mental health. Conclusion Fatigue is an important symptom in AS. Fatigue can be measured with a single-item fatigue question of BASDAI.
10.An application of template in intensity modulated radiotherapy planning

Objeetive To study the application of template in intensity modulated radiotherapy (IMRT)planning.Methods With Philips Pinnacle3(R) version 7.6c treatment planning system(TPS) platform.a template for left breast cancer's IMRT planning was customized by using the function of scripting.Ten patients received IMRT alone after breast-conserving surgery were selected,two IMRT plans were designed for each patient:individual plan and the plan based on the template.Dose parameters of target,including conformal index(CI)and homogeneity index(HI),dose-volume histogram parameters,including V30,V20 of lung and cardiac,and the planning time were compared respectively.Results Of the two IMRT plans based on the template method and individual method.The average time of design an IMRT plan were 7±0.82 min and 36.5±6.84 min(t=13.55,P=0.000),the PTV average CI were 0.851± 0.015 and 0.845±0.032(t=0.59,P=0.560),the PTV average HI were(13.2±1.3)%and(14.4±1.6)%(t=1.804,P=0.08),the lung average V30 were(8.80±0.53)%and(8.31±0.74)%(t= cardiac average V30 were(9.89±1.14)%and(9.66±1.43)%(t=0.41,P=0.688).Conclusion For the tumor with the same location and stage,the application of template can simplify planning process and reduce working time,which was worthy of expansion.
