1.Influence of psychological intervention on the mental state of patients before Mammotome minimal-ly invasive biopsy system for excision of benign breast lump

ObjectiveTo study the influence of psychological intervention on anxiety and de-pression of patients before Mammotome minimally invasive biopsy system in resection of benign breast lump. Methods120 patients with breast lumps were divided into the experimental group and the control group with 60 cases in each group. The psychological intervention was used in the experimental group be-fore the surgery, including psychological cognition, psychological guidance and support, family support, etc,and the routine nursing care was given to the control group. SDS and SAS were used to evaluate the psy-chological state of the two groups before surgery using χ2 test. ResultsThe anxiety and depression score in the experimental group was significantly lower than that of the control group. ConclusionsThe psy-chological intervention before Mammotome minimally invasive biopsy system in resection of benign breast lump can effectively ameliorate the anxiety and depression of patients.

Objective:To investigate the current status of,analyze its influencing factors and explore educational countermeasures,thus to provide evidence for establishing prevention model of medical students' soft addiction in medical colleges.Method:Using the self-designed questionnaire,a questionnaire survey related tosoft addiction was conduct among 780 medical students.The results were analyzed statistically.Results:At present,the proportion of soft addiction in medical students is relatively large.These students will affect their study and work because of the addiction to certain behaviors and have a negative impact on personal growth and self-development.Conclusion:University should pay attention to the prevention and treatment of medical students' soft addiction behavior.It should change the social concept of medical students,enhance interpersonal ability;improve their self-cultivation,stimulate the self-education;focus on time management,improve efficiency,overcome procrastination.
3.Efficacy on somatoform pain disorder treated with resuscitation and tranquilization acupuncture technique.

OBJECTIVETo compare the efficacy differences in the treatment of somatoform pain disorder between resuscitation and tranquilization acupuncture technique and oral administration of deanxit.
METHODSForty cases of somatoform pain disorder (complicated with mild and moderate depression) were randomized into an acupuncture group and a western medication group, 20 cases in each one. In the acupuncture group. acupuncture was applied to Shenting (GV 24). Shuigou (V 26). Sishencong (EX HN I) to Baihui ((V 20) (penetrating technique). Feng-chi (GB 20). etc. The treatment was given once every day. 6 times a week. In the western medication group. the anti depression drug. deanxit was prescribed for oral administration, 1 tablet in every morning and lunchtime each day. In 8 weeks of continuous treatment, the visual analogue scale (VAS) and Hamilton depression scale 17 items (HAMD) were used to evaluate the efficacy in the patients of the two groups, the adverse reactions were observed and the recurrence rate was followed up in 3 months.
RESULTSAfter treatment. VAS and HIIAMD scores were all apparently reduced as compared with those before treatment in the patients of the two groups (all P<0.05). The reducing amplitude of VAS score in the acupuncture group was greater than that in the western medication group (P<0.05) and that of HAM) score was not different significantly as compared with that in the western mnedica tion group (P > 0.05). There was no adverse reaction in the acupuncture group and were 3 cases as dizziness, trembling. dry mouth and constipation in the western medication group. The curative and remarkably effective rate was 50.0 (10(20)) in the acupuncture group, better than 15.0% (3/20) in the western medication group (P<0.05). The recurrence rate was 10.0 (2/20) in the acupuncture group. lower than 35.0% (7/20) in the western medication group (P<0.05).
CONCLUSIONThe resuscitation and tranquilization acupuncture technique achieves the definite efficacy on somatoform pain disorder, relieves depression symptoms of patients, especially pain and reduces adverse reactions and recurrence rate. The effects of this therapy are advanced as compared with deanxit.

4.Significance of change in thyroid hormone in patients with heart failure

Thyroid hormone levels were fourd to be changed significantly in 93 patients with heart failure. Their T3 and FT3 decreased, while rT3 increased. The severer heart failure, the more evident change in thyroid hormone. The results suggest that the change in thyroid hormones might be an index in reflecting the severity of heart failure.
5.Effects of Methylprednisolone Combined with Dexmedetomidine on Cognitive Function under Cardiopulmonary Bypass

Objective To evaluate the protective effects of methylprednisolone combined with dexmedetomidine of cardiac valve replacement in patients with brain cognitive function under cardiopulmonary bypass (CPB).Methods Sixty patients for heart valve replacement in Affiliated Hospital.Inner Mongolia Medical University were randomly divided into four groups (n =15 each):group A control,methylprednisolone group (group M),dexmedetomidine group (Group D) and methylprednisolone combined with dexmedetomidine group (group B).After the induction of anesthesia,the patients were endotracheally intubated and mechanically ventilated.Methylprednisolone 20mg/kg was administered to the CPB circuit prime in group M.A loading dose of dexmedetomidine 1 μg/kg was pumped intravenously over 10 minutes after induction,followed by continuous infusion at 0.05μg/(kg · h) until the end of operation in group D.A loading dose of dexmedetomidine 1 μg/kg was pumped intravenously over 10 minutes after induction,followed by continuous infusion at 0.05μ g/(kg · h) until the end of operation and methylprednisolone 20mg/kg was also administered to the CPB circuit prime in group B.At the same time the equal volume of normal saline was given in group A.After induction (T1),CPB instantly (T2),CPB 30min (T3),l0min after CPB(T4),at the end of operation(T5),6h after CPB (T6),the jugular bulb venous blood samples were taken for testing the serum concentrations of interleukin-6 (IL-6),interleukin-10 (IL-10),S100βprotein and neuron-specific enolase (NSE) using the method of ELISA.Simplified mini-mental state examination (MMSE) is used to assess the cognitive function of patients in pre-operation and 2days,3days as well as 7days after operation.Results At T1-6,IL-6,S100 protein and NSE in group M,group D and group B were significantly lower than those in control group,IL-10 was higher than that in control group,and there was significant difference (P < 0.05).In B group at all time points IL-6 、S100 protein and NSE were lower than those in other groups,IL-10 was higher than other groups,and there was significant differences (P < 0.05).The incidence of postoperative cognitive dysfunction in group M,group D and group B was higher than that in control group,and the incidence of cognitive dysfunction in B group was lowest,and there were significant differences (P < 0.05).Conclusion Methylprednisolone and dexmedetomidine can effectively reduce the concentration of IL-6,S100 protein and NSE in cardiopulmonary bypass,reduce brain injury and the incidence of postoperative cognitive dysfunction.The protective effect on cerebral injury is better when using dexmedetomidine combined with methylprednisolone under cardiopulmonary bypass (CPB).

7.Effects of propofol on focal cerebral ischemia and the expression of protein kinase C isoform ? in rats

Aim To investigate the effects of propofol on focal cerebral ischemia and the changes of protein kinase C isoform ?(PKC?) expression in rats brain. Methods Male SD rats were randomly allocated into five groups: Ⅰ: sham group, Ⅱ: injury group, Ⅲ: propofol (25 mg?kg-1) plus injury group, Ⅳ: propofol (50 mg?kg-1) plus injury group, Ⅴ: intralipid plus injury group. The focal cerebral ischemia was induced by 3 h of middle cerebral artery occlusion (MCAO) and 24 h of reperfusion. 30 min before reperfusion, propofol and intralipid were infused intraperitoneal of the rats in groups Ⅲ, Ⅳ, Ⅴ, respectively. After 24 h of reperfusion, rats were weighted and the neurological deficit was assessed by 5-point scale. Brain pathologic changes were observed by HE staining, the method of terminal deoxynucleotidyl transferase mediated dUTP-biotin nick end labeling (TUNEL) was carried out for the assessment of neural apoptosis, and immunocytochemistry was used to investigate the changes of PKC?.Results After 3 h of MCAO and 24 h of reperfusion, the weight of rats in group Ⅱ, Ⅲ, Ⅳ, Ⅴ decreased and their neurological deficits scores increased. Objective To know normal reference value of heart-type fatty acid-binding protein (H-FABP) in serum. Methods Serum H-FABP levels of 126 healthy individuals were measured by ELISA. Levels of MYO, cTnI and CK-MB were assayed in the same time as control. The normal threshold value was calculated as mean value?2s. Results The normal concentration of H-FABP in serum was 3 79?3 52?g/L and the difference in age and gender wasn’t significant. Conclusions The normal reference value of serum H-FABP is 0~10 83?g/L.
10.The effect of the variation of cesarean sections during labor and their indications on total cesarean section rate

Objective To investigate the effect of variation of cesarean sections during labour and their indications on the total cesarean section rate. Methods A total of 458 cases of cesarean section was selected from January to June of 1990,1996,2002. They were divided as indication group and no indication group. The change of the indication and cesarean section rate was compared. Results The ratio of cesarean section during labour in total cesarean sections rised. The ratio was 22.0%, 20.8% and 39.4% in 1990, 1996 and 2002 respectively. The proportion of no indication surgery in 2002(63.37%) showed significant growth as compared with that of 1990 (33.84) and 1996 (32.58). Of all cases of cesarean section having indications, the rate of trial produce failure increased in 2002 (25.81%) while it was 11.45% and 16.67% in 1990 and 1996 respectively. The rate of fetal distress decreased from 25.19% (1990), 21.67% (1996) to 12.90% (2002). Conclusion Ascen ding of cesarean section rates during labour was an important factor that increased cesarean section rate in recent years. To decrease the total rates of cesarean section, many important measures should be done such as paying more attention to social factors, reinforcing propaganda and maternal health care , encouraging spontaneous vaginal delivery, emphasizing company during labour and analgesia or pain free delivery, laying stress on the management and monitoring during first stage of labour as well as psychological care.
