1.Investigation on the extraction technology of Northeast tussah pupa oil with SFE-CO2 and analysis of its composition

Objective To investigate the extracting technology of the Northeast tussah pupa oil and analyze the composition of the oil. Methods The Northeast tussah pupa were crushed and dried at 60℃. The oil was extracted from the Northeast tussah pupa powder with super critical fluid extraction technology-CO2 (SFE-CO2) at the extracting temperature as 45 ℃,pressure as 50 MPa and operating time as 6 hours according to the results of homogeneous design. The composition of the oil was detected by gas chromatography (GC).Results The extracting rate of the oil from the Northeast tussah pupa powder was 17.93% (w/w)and the percent of compositions of the oil were C15:0 fatty acid 37.03%,hexadecanoic acid 5.63%,C17:0 fatty acid 32.81%,octadecanoic acid 1.79%,leic acid 3.75%,octadecadienoic acid 11.98% and α-1inoleic acid 7.00%. Conclusion The technology of SFE-CO2 can be used in extraction of Northeast tussah pupa with High extraction efficiency.
2.Exploration of ophthalmology practice teaching for foreign students majoring in clinical medicine

Ophthalmology belongs to microsurgical subject and anatomy is intricate. In clinical practice, students are required to skillfully use eye examination equipment. The foreign students from Medical School of Vienna University are eager to learn and know theoretical knowledge very well. Department of ophthalmology has developed specific and individualized teaching plans according to the characteristics and needs of foreign students. Through the configuration of high quality teachers, the practice teaching is ensured. Chinese and foreign students are encouraged to share their knowledge and form a study group. Opportunities for hands-on practice of ophthalmology are increased by using Wetlab condition. Problem-based heuristic teaching is used to stimulate students' learning initiative. Through these teaching measures, the students' quality of clinical practice has been enhanced, the teaching level of clinical ophthalmology has been improved and sustainable development has also been achieved.
3.Diagnostic value of velocity parameters of Tardus-Parvus for the detection of renal artery stenosis

Objective To evaluate the diagnostic value of velocity parameters of Tardus-Parvus for the detection of renal artery stenosis ( RAS) ( diameter reduction ≥50%) and to determine the useful cutoffs for these parameters .Methods A study group was composed of 221 renal arteries that were detected Tardus-Parvus by color Doppler flow imaging and were referred to abdomino-aorto-re-nal arteriography afterwards .Five Doppler parameters including the peak systolic velocity ( PSV ) , end-diastolic minimum velocity (EDV), resistance index (RI), acceleration time (AT), and accelerated velocity (AC) from each location including renal aorta ( MRA) , segmental artery ( SRA) , interlobar artery ( IRA) , and arcuate artery ( ARA) were archived and compared among the differ-ent groups .Renal artery angiography showed arterial canon reduced 50% or higher RAS .Arteries were considered stenosed on renal arteriography if there was a diameter reduction of greater than 50%.Statistical analysis to determine the best parameter for predicting a RAS was performed with the receiver operating characteristic ( ROC) curves.The sensitivity, specificity, and negative and positive predicting values at various cutoffs were calculated .Results Renal artery stenosis degree was less than 50% in 16 cases, 50%~99%in 197 cases, completely occluded in 2 cases, and no stenosis in 6 case by renal arteriography .For RAS with inner diameter re-duction of 50%or more, the ROC curve analysis showed renal artery flow velocity had a good sensitivity and specificity , 50%~99%of the RAS had optimal threshold value in PSV 20 cm/s, EDV 10 cm/s, RI 0.50, AT 0.09 s, and AC 1.5 m/s2.Conclusions Tardus-Parvus had high specificity and sensitivity for diagnosis of stenosis rate in more than 50% of the RAS, low sensitivity for the narrow degree in 0~49%, and no clinical value for the renal artery occlusion .
4.Compararive study on two feeding ways in coma patients at gastrointestinal tract complications

Objective To observe the influence of two feeding ways m coma patients at gastrointestinal tract complications.Methods Seventy patients were assigned randomly to two groups according to different feeding ways:entogastric continuing infusion (observation group,n=35) and entogastric manualinfusion (control group,n=35) from June 2006 to May 2007,and observed the ratio of gastrointestinal tract complications in two groups.Results Different feeding ways had significantly correlated with the complications of gastrointestinal tract.The ratio of gastrointestinal tract complications in observation group was lower (20.00%)than that in control group (42.86%).There was statistically significant difference in two groups,P<0.05.Conclusion The ratio of gastrointestinal tract complications could obvious reduce in entogastric nutrition continuing infusion in coma patients.
5.Efficacy Observation ofFufang Shenlu Granule for Kidney-yang Deficiency Aplastic Anemia and Its Effects on CD4+ T Lymphocyte Subsets

Objective To observe the clinical efficacy ofFufang Shenlu Granule for kidney-yang deficiency aplastic anemia (AA) and its effects on CD4+ T lymphocyte subsets, and explore its mechanism.Methods Forty AA patients were randomly divided into treatment group and control group, 20 cases in each group. The treatment group was treated withFufang Shenlu Granule combined with conventional western medicine therapy, while the control group was only treated with conventional western medicine therapy. The treatment lasted for 6 months, and follow-up visits lasted for at least one year. The clinical efficacy and T lymphocyte subsets ratio changes before and after treatment in the two groups were observed.Results The clinical efficacy of treatment group was better than that of control group (P<0.05). After treatment, the blood cell counts of the two groups were improved than those of before treatment (P<0.05,P<0.01). After treatment, the red blood cell count and hemoglobin of treatment group were higher than those of control group (P<0.05). Compared with the data before treatment in treatment group, the ratio of CD4+CD25+ cells in all CD4+ cells, and the ratio of CD4+CD25+FoxP3+ cells in all CD4+CD25+ cells increased (P<0.05); the ratio of Th1 and Th2 cells in all CD3+CD4+ decreased (P<0.05). The ratio of Th2 cells was lower than that before treatment in the control group (P<0.05).ConclusionFufang Shenlu Granule can enhance the treatment efficiency of AA with kidney-yang deficiency syndrome. The mechanism may be related to the decrease of Th1 cells, the increase of CD4+CD25+Tregs and their FoxP3 expression.
6.Cross-validation of the osmotic pressure based on Pitzer model with air humidity osmometry at high concentration of ammonium sulfate solutions.

The osmotic pressure of ammonium sulfate solutions has been measured by the well-established freezing point osmometry in dilute solutions and we recently reported air humidity osmometry in a much wider range of concentration. Air humidity osmometry cross-validated the theoretical calculations of osmotic pressure based on the Pitzer model at high concentrations by two one-sided test (TOST) of equivalence with multiple testing corrections, where no other experimental method could serve as a reference for comparison. Although more strict equivalence criteria were established between the measurements of freezing point osmometry and the calculations based on the Pitzer model at low concentration, air humidity osmometry is the only currently available osmometry applicable to high concentration, serves as an economic addition to standard osmometry.
7.Current status of benign breast diseases and metabolic syndrome in professional women in Chongqing and study on relative risk factors

ObjectiveTo study current status of benign breast diseases and metabolic syndrome in professional women in Chongqing and relative risk factors. MethodsProfessional women (2604 cases )in Chongqing were surveyed by random cluster sampling.The biochemical indicators such as blood lipid were determined by cholesterin oxidase.The indicators such as height and weight were measured by physical examination.Chi-square test and logistic regression were used in statistical analysis. Results The morbidity rate of benign breast diseases, metabolic syndrome, hyperglycaemia, hypertension, hyperlipidemia, and obesity in our study was 19.27% (429/2226), 7.91% ( 176/2226 ), 8.04% ( 179/2226 ), 23.23% ( 517/2226 ), 24.21% ( 539/2226 ) and 20.5% (457/2226) respectively.The difference of mobidity rate between different age group and different career had statistical significance.Office workers and civil servants were high risk population.Age was negatively correlated with benign breast diseases.There was no relation between benign breast diseases and metabolic syndrome.ConculsionsThe morbidity rate of benign breast diseases and metabolic symdrome in professional women in Chongqing is relatively high.A good lifestyle, breast self-examination and regular physicial examination are recommended.
8.Effect of hypoxia on adhesion and invasion of human tongue squamous cell carcinoma cell line Tca8113

Objective: To evaluate the effect of synthesized small interfering RNA targeting to HIF-lα on the adhesion and invasion of human tongue squamous cell carcinoma cell line (Tca8113). Methods; A double strand small interference RNA (siRNA) targeting HIF-1α (siRNAH1Fla) was transfected into cultured Tca8113 cells by lipofectamine2000. The expression of HIF-1α was investigated on mRNA level by real time-PCR and protein level by Western blot. The adhesion and invasion of Tca8113 cells to extracellular matrix (ECM) was also analyzed. Results: Exposure to hypoxia induced a prolonged elevation of HIF-lα protein and siRNAHIF.la reduced HIF-la synthesis as measured on mRNA level and protein level compared with the controls. No matter under normoxic or hy-poxic conditions, the adhesion potency of siRNAHIF-1α treated Tca8113 cells was markedly inhibited compared with controls(P<0.05 or P <0.01). So did the invasion potency (P<0.01). The adhesion and invasion potency of siRNAHIF.,a treated Tca8113 cells were inhibited more greatly under hypoxic condition than under normoxic condition ((36.4±2.7)% vs(26±2.35);(44.2±2.2)% vs (35±1.75), P<0.01)). Conclusion; siRNAH1F.lo can knockdown the expression of HIF-la and inhibit the cell adhesion and invasion to ECM in Tca8113 cells. HIF-la may play an established role in the regulation of Tca8113 cells invasion and metastasis. Interfering with HIF-1α pathways by siRNA strategy may provide a therapeutic target for human tongue squamous cell carcinomas.
9.Caraganglioma in thoracic vertebral canal superimposed on dermatomyositis: a case report

A 20-year-old male patient presented with myalgia of upper limbs and myasthenia of extremities for more than 1 month. Physical examination showed diffuse erythema on the cheeks, upper eyelids, upper chest, neck and dorsa of the hands. The myodynamia of the proximal and distal muscles of upper and lower extremities was grade Ⅳ, Ⅴ, Ⅲ and Ⅴ respectively. Laboratory examinations revealed that the serum levels of creatine kinase, CK-MB and lactate dehydrogenase were 2103 U/L, 83 U/L and 489 U/L respectively, which were all above the normal range. Electromyogram revealed myopathic abnormality and normal nerve conduction velocity. Histopathology of gastrocnemius muscle showed hypertrophy and swelling of muscle fibers, disappearance or fuzziness of transverse striation, and intermuscular lymphoid cell infiltration. A biopsy of the skin lesion from the upper chest showed liquefaction degeneration of and colloid bodies in basal cell layer, perivascular lymphoid cell infiltration in the dermis. A diagnosis of dermatomyositis was established based on the clinical and laboratory findings. After management with intravenous prednisolone 80 mg once daily and symptomatic treatment for 4 weeks, the myodynamia of upper limbs was improved, serum levels of creatine kinase,CK-MB and lactate dehydrogenase reached the normal ranges. However, the myodynamia of lower limbs progressively deteriorated with the emergence of paresthesia. Enhanced MRI scan showed a tumor in the vertebral canal at the level of thoracic vertebra 11 to 12. A spherical encapsulated tumor measuring 3 cm in diameter was surgically removed. The tumor was diagnosed as paraganglioma in vertebral canal according to pathological and immunohistochemical findings. The patient was finally diagnosed with paraganglioma in vertebral canal superimposed on dermatomyositis.
10.Pediatric liver transplantation for metabolic liver disease:report of 42 cases

Objective To Analyze the clinical outcomes of pediatric liver transplantation (LT) for liver-based metabolic disorders.Methods We conducted a retrospective analysis on 42 pediatric patients with liver-based metabolic disorders from June 2013 to March 2017,and analyzed the pediatric end stage liver disease model (PELD),growth and development,type of transplant,postoperative complications and prognosis of patients.Results There were 42 children with liver-based metabolic disorders (15.56%) out of all the 270 children who underwent LT.The median age was 51.0 months (range,3.4-160.9 months).Of the 42 children,19 received living donor liver transplantation (LDLT),18 cases received deceased donor liver transplantation (DDLT) and 5 cases received domino liver transplantation.1-,2-and 3-year cumulative survival rate of 42 recipients was 97.7%,93.6% and 93.6%,and that of the grafts was 95.3%,91.4% and 91.4%,respectively.As compared with the 194 children with biliary atresia who underwent LT,significant difference was found in PELD and weight Z-score between the two groups.Conclusion Liver transplantation is a valuable option for children with metabolic disorders,and it has gained a better prognosis.
