Objective To investigate the role of activated cylic AMP(cAMP) signaling in chemical-induced podocyte injury.Methods Eight-weeks-old male BalB/C mice were randomly divided into three groups:control group,Adriamycin (ADR) group and Forskolin+ADR group.ADR nephropathy models were established by tail intravenous injection,and part of them were injected Forskolin,an agonist of adenylate cyclase,intraperitoneally.Phosphorylation of cAMP response element binding protein (CREB) was detected by laser confocal microscopy,morphology of foot processes were determined with transmission electron microscope,and WT-1 expression in glomeruli were detected by immunohistochemistry.Conditionally immortalized podocytes were treated with puromycin aminonucleoside (PAN),Exchange protein directly activated by cAMP (Epac) agonist 8-pCPT-2-O-Me-cAMP (2Me),protein kinase A (PKA) antagonist H89 and its agonist pCPT-cAMP(pCPT).Western blot was used to detect the expression levels of Epac,caspase3 and cleaved caspase3.PKA activity was assayed using cAMP-dependent protein kinase detection system.Cell viability was determined by a cell count kit and podocyte apoptosis was estimated by TUNEL staining.Mitochondrial membrane potential was evaluated by JC-1 staining.Results (1)Compared with ADR group,the urine albumin decreased significantly (P < 0.05) among Forskolin + ADR group and the WT-1 positive cells per glomerulus increased obviously (P < 0.05).(2)PAN decreased podocyte number in a time-dependent manner (P < 0.05),pre-treatment with pCPT obviously inhibited PAN induced podocyte decrease (P <0.05),but H89 prevented the effect of pCPT in a dose-dependent manner (P < 0.05).(3)JC-1 staining showed that the percentage of podocyte with green fluorescence for control,PAN and pCPT+PAN group were (12.67±2.15)%,(31.35±4.60)% and (16.96 ± 2.51)% respectively (P < 0.05),and pretreatment with H89 inhibited the effect of pCPT (P < 0.05).(4) PAN promoted podocyte apoptosis and cleaved caspase3 expression (P < 0.05),and pretreatment with pCPT significantly prevented PAN-induced podocyte apoptosis and cleaved caspase3 expression (P < 0.05).Conclusions cAMP signaling activation ameliorated podocyte injury in ADR nice and PAN-induced podocyte apoptosis,and cAMP/ PKA pathway may mediate these processes.

4.MR diffusion tensor imaging in the evaluation of neural progenitor cells transplantation to acute injured canine spinal cord

Objective To observe the effect of transplantation of telomerase immortalized human neural progenitor cells to acute injured canine spinal cord by using MR diffusion tensor imaging (DTI).Methods Telomerase immortalized human neural progenitor cells with expression of green fluorescent protein were prepared for transplantation. Eight adult canines with left spinal cord hemisection at the level of T13 were examined by MR diffusion tensor imaging four times sequentially: prior to injury, one week after injury, one week after transplantation (two weeks after injury), and four weeks after transplantation. Results The ADC values of the injured spinal cord were (1.00?0.15)?10 -3 mm2/s, (1.65?0.45)?10 -3 mm2/s, (1.44?0.48)?10 -3 mm2/s, and (1.43?0.26) ?10 -3 mm2/s, respectively. There was statistically significant difference between the data obtained at different times (F=6.038, P=0.005). The FA values of the injured spinal cord were 0.59?0.11, 0.30?0.17, 0.36?0.25, and 0.34?0.11, respectively. There was also statistically significant difference between the data obtained at different times ( F=5.221,P=0.009). The ADC values of the intact spinal cord were (1.01?0.17)?10 -3 mm2/s, (1.32?0.06)?10 -3 mm2/s, (1.10?0.24)?10 -3 mm2/s, and (1.14?0.22) ?10 -3 mm2/s, respectively. There was no statistically significant difference between the data obtained at different times ( F=1.303,P=0.306). The FA values of the intact spinal cord were 0.60?0.09, 0.38?0.25, 0.46?0.15, and 0.50?0.21, respectively. There was also no statistically significant difference between the data obtained at different times (F=2.797,P=0.072).Conclusion DTI can provide useful information for spinal cord injury and regeneration in experimental spinal cord injury.

Objective To investigate the clinical features,diagnostic modalities and treatment of duodenal Brunner′gland adenoma(BGA).Methods Fifteen patients with duodenal BGA were evaluated base on clinical presentations,the results of serologic test,barium meal examination,gastroscopy and endoscopic ultrasonography(EUS)as well as treatment.The post-operative histological results were compared with pre-operation EUS findings.Results Eight out of 15 patients with duodenal BGA had presentations,including duodenal ulcer(4 patients),upper gastrointestinal hemorrhage (4 patients). The serologic tests of all patients were within the normal limits.Six patients underwent barium meal examination and 13 patients received gastroscopy.Seven patients underwent surgical treatment and 8 patients received endoscopic resection.The post-operative histological results were compatible with the pre-operative EUS findings.Conclusions Duodenal BGA is a benign tumor.EUS was helpful in making the diagnosis and determination of treatment methods(surgical operation or endoscopic resection).
7.Analysis of preventive measures and pathogenic related factors of peri-odontitis

Objective To investigate preventive measures and pathogenic related factors of periodontitis. Methods A total of 138 patients with periodontitis from August 2012 to March 2014 in our department were selected as the experi-mental group. Meanwhile 138 healthy cases matched with the patients within 2 years age difference in the same regions were selected as the control group. Questionnaire survey was used to inquire the basic information, educational status, economic income, acquisition of knowledge in periodontal disease, brushing habits, negative life events and so on. The differences in results between two groups were compared. Results Among 138 patients with periodontitis, there was 1087 disease parts on maxillaries, 872 parts on mandibles, 1243 parts on molars, and 876 parts on anterior teeth and bicuspids. According to Amaitage standard, 138 patients with periodontitis, the rate of mild to moderate periodontitis patients was 81.16% (112 cases), and the rate of severe periodontitis was 18.84% (26 cases). Acquisition of knowledge in periodontal disease, smoking, the frequency and sustained time of brushing per day, negative life events and mouth care between two groups showed statistical significance(P<0.05). There was no significant difference between two groups in age, gender and educational status(P>0.05). Non conditional Logistic regression results showed that: independent pathogenic risk factors of periodontitis included acquisition of knowledge in periodontal disease, the frequency and sus-tained time of brushing per day, negative life events and regular mouth care, and regular mouth care ranked first among these factors(the odds ratio=3.102, P<0.05). Conclusion Periodontitis is the result of interaction of multiple risk factors, therefore, comprehensive measures especially regular mouth care should be adopted to prevent the occurrence of peri-odontitis.
8.Water extraction coupling rectification for isolating volatile oils of Ligusticum chuanxiong.

To improve the oil yield of Chuanxiong volatile oils and keep their quality same as that of oils prepared by steam distillation (SD) for short), a new technology of coupling water extraction and rectification (WER for short) was applied to isolate Chuanxiong volatile oils. The oil yield, as well as GC-MS chromatogram of the volatile oils extracted by WER and SD were compared. The oil yield of WER (0.50%) was that of SD (0.20%). There were 21 common constituents in the two kinds of volatile oils, which occupied 98.94% of the oils extracted by SD, 98.80% of that by WER, respectively. Moreover, the relative percentage of the common constituents was almost the same. The results indicate that WER could not only increase the oil yield of Ligusticum chuanxiong, but also keep the quality of the oils accord with that extracted by SD.

9.Investigation and Analysis of Clinical Nurses' Satisfactory Degree to PIVAS in Our Hospital

OBJECTIVE:To provide reference for improving the service quality of PIVAS in our hospital. METHODS:Satis-faction questionnaire was designed by us. The survey was conducted among clinical nurses from 40 wards of our hospital about their satisfactory degree to pharmaceutical care of PIVAS and related influential factors. The survey data were analyzed statistically. RESULTS:A total of 280 questionnaires were sent out,and 271 effective questionnaires were collected with effective recovery of 96.79%. The overall satisfaction degree of nurses to PIVAS services was higher [(4.31±0.89)points]. The top 3 items were"drug admixture correction rate""infusion order rationality"and"drug packing quantitative accuracy rate rate",scored (4.57 ± 0.70), (4.56 ± 0.73) and (4.50 ± 0.76),respectively;the last 3 items of satisfaction were"timeliness of telephone answering service""timeliness of temporary medical orders'distribution"and"serviceable time for temporary medical orders'admixture",scored (3.73 ± 1.16),(3.75 ± 1.03) and (3.96 ± 1.01),respectively. The univariate analysis and multivariate analysis showed that there were statistical differences in the satisfaction score among nurses with different ages from different departments(P<0.01). The nurs-es in cardiovascular medicine department and ICU showed lowest satisfaction on service effect,and the nurses aged 20-29 years old showed highest satisfaction on service effect. There was no statistical significance in satisfactory degree among nurses with different gender,nursing ages,education background and professional titles(P>0.05). CONCLUSIONS:The overall satisfaction of clinical nurses to PIVAS service is favorable. The items of temporary medical orders-related service score the lowest,and service effect is related to nurses'departments and age. It is necessary to combine the shortage of work and the demand of clinical services to strengthen the management and promote the continuous improvement of PIVAS service quality.
10.Effect of modified cervicothoracic compression band on physical condition of patients with endoscopic radical thyroidectomy via breast areola approach

Objective:To investigate the effect of modified cervicothoracic compression band on successful hemostasis and postoperative complications of patients with endoscopic radical thyroidectomy via breast areola approach and to provide reference for postoperative nursing of thyroid cancer.Methods:A total of 128 patients with endoscopic radical thyroidectomy via breast areola approach from October 2018 to October 2019 in the Second Affiliated Hospital of Zhejiang University School of Medicine were selected and divided into experimental group(64 cases) and control group (64 cases) by random digits table method. Patients in the control group were fixed with wide adhesive tape for hemostasis, while patients in the experimental group wore modified cervicothoracic compression band. The postoperative complications between two groups were recorded, in addition, the physical condition and comfort of patients was assessed by Edmonton Symptom Assessment System (ESAS) and Kolcaba General Comfort Questionnaire (GCQ), respectively.Results:There was no sigrificance in the score of ESAS, GCQ on the 1st day before the operation between the two groups ( P>0.05). The median score of happiness in ESAS was 6.0 points in the experimental group on the 3rd day after the operation, significantly higher than 4.0 points in the control gorup, the difference was statistically significant ( Z value was 2.919, P<0.01). The physiological, psychological, social, cultural and environmental comfort, total GCQ scores were (10.73±2.35), (24.41±4.99), (23.09±2.11), (20.17±2.32), (78.41±6.49) points in the experimental group and (8.41±2.23), (22.42±4.79), (21.22±2.73), (19.20±2.97), (71.25±6.92) points in the control group, the differences were statistically significant ( t values were 2.058-6.031, P<0.05 or 0.01). The incidence of contact dermatitis, pressure purpura and tension blister were 0, 3.1% (2/64), 0 in the experimental group and 6.3% (4/64), 12.5% (8/64), 7.8% (5/64) in the control group, the differences were statistically significant ( χ2 values were 4.129, 3.095, 5.203, P<0.05). Conclusions:Modified cervicothoracic compression band can significantly alleviate the symptoms of postoperative patients with endoscopic radical thyroidectomy, reduce postoperative complications and improve patient comfort.
