1.Effects of Methylprednisolone Combined with Dexmedetomidine on Cognitive Function under Cardiopulmonary Bypass

Objective To evaluate the protective effects of methylprednisolone combined with dexmedetomidine of cardiac valve replacement in patients with brain cognitive function under cardiopulmonary bypass (CPB).Methods Sixty patients for heart valve replacement in Affiliated Hospital.Inner Mongolia Medical University were randomly divided into four groups (n =15 each):group A control,methylprednisolone group (group M),dexmedetomidine group (Group D) and methylprednisolone combined with dexmedetomidine group (group B).After the induction of anesthesia,the patients were endotracheally intubated and mechanically ventilated.Methylprednisolone 20mg/kg was administered to the CPB circuit prime in group M.A loading dose of dexmedetomidine 1 μg/kg was pumped intravenously over 10 minutes after induction,followed by continuous infusion at 0.05μg/(kg · h) until the end of operation in group D.A loading dose of dexmedetomidine 1 μg/kg was pumped intravenously over 10 minutes after induction,followed by continuous infusion at 0.05μ g/(kg · h) until the end of operation and methylprednisolone 20mg/kg was also administered to the CPB circuit prime in group B.At the same time the equal volume of normal saline was given in group A.After induction (T1),CPB instantly (T2),CPB 30min (T3),l0min after CPB(T4),at the end of operation(T5),6h after CPB (T6),the jugular bulb venous blood samples were taken for testing the serum concentrations of interleukin-6 (IL-6),interleukin-10 (IL-10),S100βprotein and neuron-specific enolase (NSE) using the method of ELISA.Simplified mini-mental state examination (MMSE) is used to assess the cognitive function of patients in pre-operation and 2days,3days as well as 7days after operation.Results At T1-6,IL-6,S100 protein and NSE in group M,group D and group B were significantly lower than those in control group,IL-10 was higher than that in control group,and there was significant difference (P < 0.05).In B group at all time points IL-6 、S100 protein and NSE were lower than those in other groups,IL-10 was higher than other groups,and there was significant differences (P < 0.05).The incidence of postoperative cognitive dysfunction in group M,group D and group B was higher than that in control group,and the incidence of cognitive dysfunction in B group was lowest,and there were significant differences (P < 0.05).Conclusion Methylprednisolone and dexmedetomidine can effectively reduce the concentration of IL-6,S100 protein and NSE in cardiopulmonary bypass,reduce brain injury and the incidence of postoperative cognitive dysfunction.The protective effect on cerebral injury is better when using dexmedetomidine combined with methylprednisolone under cardiopulmonary bypass (CPB).
2.Determination of Nitrate in Air by Wetting Absorption Ion Chromatography

Objective To establish a method for rapid determination of nitrate in air. Methods Nitrate in air was absorbed using Na2CO3-NaHCO3 solution and the content of nitrate in the absorption liquid was determined by ion chromatography. Results The detection limit of NO3- was 0.008 mg/m3 when the sampling volume of air was 100 L. The linear correlation coefficient was 0.999 6. The relative standard deviation of the method was less than 5% and the recovery rates was 92.0%-96.7%. Conclusion The method had good selectivity and was simple and rapid for the determination of nitrate in air.
3.Maxillary protraction and multiple loop edgewise arch wire technology in treatment of early mixed dentition skeletal class Ⅲ malocclusion

Objective To evaluate the orthodontic effect of surgery on mixed dentition period class Ⅲ malocclusion and to investigate the effective therapeutic method of the skeletal class Ⅲ malocclusion.Methods A total of 20 patients aged 8.5 to 9.1 years with early mixed dentition class Ⅲ skeletal malocclusion.The treatment was divided into two phases.During the first phase,the entire stuffy group was treated with a bonded maxillary expansion and protraction.During the second phase,the entire study group received multiple loop edgewise arch wire (MEAW) treatment until an ideal occlusion relationship achieved.Comparison of the pre-and post-orthodontic treatment cephalometric radiographs had been done after maxillary protraction and MEAW were applied for patients.Results In the 20 patients after orthodontic treatment,Class Ⅲ skeletal facial profile was improved,the retraction of anterior teeth was obvious,and there were significant changes in dentition; the profile of soft tissue and satisfactory occlusion relationship were significantly improved,as compared with the changes between pre-treatment and post-treatment.Conclusions Skeletal Ⅲ malocclusion in early mixed dentition can be successfully treated.The results can be satisfied with improved class Ⅲ skeletal and molar relationship,high-angle and profile of soft tissue.
4.The Impact of Humanistic Spirit Cultivation on Nursing Students' Ethical Cognition

Objective:To investigate the impact of humanistic spirit cultivation on nursing students' ethics cog-nition. Method:Total 364 internship nursing students in 2010 were taken as the control group with the routine clini-cal teaching method, 381 in 2014 as the experimental group, not only a routine clinical teaching method but also to increase of their humanistic spirit culture. Results:Humanity spirit cultivation before and after practice nursing students in medical security system cognition difference is not significant, but in medical optimization, the patients right of informed consent, the life value differences of cognitive and ethical cognitive dimension score was statisti-cally significant ( P<0 . 05 ) . Conclusions:Humanistic spirit culture promote the practice of nursing students to raise the level of moral cognition foundation for humanity quality education and nursing ethics education, should further strengthen the diversification of internship training nursing students humanistic spirit.
5.The value and feasibility of temporary bedside cardiac pacing in the emergency treatment of severe brady-cardia

Objective To study the value and feasibility of temporary emergency bedside cardiac pacing. Method Two hundred patients with severe witnessed bradycardia were treated with temporary emergency cardiac pacing. We treated 130 patients with emergency bedside pacing and 70 patients with x-ray-guided pacing. Results Emergency bedside pacing was successful in 127 patients except three patients and no postoperative complications occurred. X-ray-guided pacing was successful in all 70 patients but three patients experienced complications: one deep venous thrombosis and two cardiac tamponades due to myocardial perforation. The pacing electrodes were more likely to be displaced in X-ray-guided pacing than in emergency bedside pacing (six cases versus three cases) . The door-to-operation time was 30-90 min for x-ray-guided cardiac pacing and 5-15 min for emergency bedside pacing. Needle-to-pacing times were similar for both procedures (3.5 ± 1.5 min for x-ray guided pacing versus 4± 2.5 min for bedside pacing). Conclusions Temporary emergency bedside cardiac pacing is a rapid, efficient and safe procedure for treating severe witnessed bradycardia. The technique is easily mastered and may prove lifesaving in an emergency.
6.Therapeutic effects of recombinant prourokinase on patients with acute myocardical infarction

Objective To investigate the therapeutic effect and side effect of prourokinase (Pro-uk) on the patients with acute myocardial infarction. Method From May 2004 to May 2005, a total of 68 cases of acute myocardial infarction treated by thrombolytic agents within 6 hours after onset were divided randomly into Pro-uk group ( n = 43) and urokinase (UK) group ( re = 25). In the Pro- uk group, there were 22 eases treated with 50 mg Pro-uk and 21 cases prescribed in 60 mg Pro-uk. The patency of involved coronary artery was evaluated by using selective coronary angiography at 90 minutes after thrombolytic therapy. The incidence of early heart events, bleeding and adverse reaction, were alse observed. Results (1) The total rates of resumed patency of obstructed coronary artery (TIMI grade 2 and 3 flow) were 76.7% in Pro-uk treated patients and 52.2% in UK treated patient ( P = 0. 041). (2) The occurrence of hyporrhea was lowered more significantly in Pro-uk treated patients than in UK treated patients (P = 0.029), but there were no intracranial hemorrhage found in both group. (3) Early cardiac events and adverse reactions were similar between two groups. Conclusions Pro-UK is safe and effective for the patients with acute myocardial infarction.
7.MR Diagnosis of Stage Ⅰ Tumor Thrombus in Renal Cell Carcinoma

Objective To evaluate the accuracy of MR in the diagnosis of tumor thrombus in renal cell carcinoma.Methods 22patients with renal cell carcinoma proved pathologically were recruited in this study.All the patients underwent MR examination beforesurgery.The results of the MR were correlated with that of the operation.Results 8 renal veins could be found tumor thrombus during the surgery,and the other 14 renal veins and 22 inferior vena cava were absent of tumor thrombus.MR could found all of the 8 tumor thrombithat detected by the surgeons.In the other 14 cases,13 of them could be excluded of tumor thrombus by MR.Only in 1 patient,the MRresult was equivocal.CT was performed in 9 patients,the diagnosis was accurate in 7 cases,and was inaccurate in 2.Ultrasound wasperformed in 16 patients,the diagnosis was accurate in 13 cases,and was inaccurate in 3.Digital subtraction angiography was performed in 4 cases,the diagnosis was correct in all of them.Conclusion MR is perfect in the diagnosis of tumor thrombus in renal cell carcinoma oncondition that the renal vein is clearly defined by MR.It can be considered as the method of choice in the evaluation of tumor thrombus inrenal cell carcinoma.
8.Anomaly Detection of Multivariate Time Series Based on Riemannian Manifolds.

Multivariate time series problems widely exist in production and life in the society. Anomaly detection has provided people with a lot of valuable information in financial, hydrological, meteorological fields, and the research areas of earthquake, video surveillance, medicine and others. In order to quickly and efficiently find exceptions in time sequence so that it can be presented in front of people in an intuitive way, we in this study combined the Riemannian manifold with statistical process control charts, based on sliding window, with a description of the covariance matrix as the time sequence, to achieve the multivariate time series of anomaly detection and its visualization. We made MA analog data flow and abnormal electrocardiogram data from MIT-BIH as experimental objects, and verified the anomaly detection method. The results showed that the method was reasonable and effective.

9.Epidemic analysis of 1010 cases of brucellosis in Ji'nan from 2010 to 2014

Objective To provide data evidence for brucellosis prevention by a retrospective analysis of brucellosis epidemiologic feature in Ji'nan Infectious Disease Hospital from 2010-2014.Method The brucellosis patients with confinned diagnosis from 2010 to 2014 admitted to Ji'nan Infectious Disease Hospital were included in this study,and their epidemic data were statistically analyzed,including time,age and occupation distributions.Results There were 1 010 brucellosis patients in total in Ji'nan Infectious Disease Hospital from 2010 to 2014,and 12 cases,69 cases,134 cases,274 cases and 521 cases,respectively,in each year,which showed rapid rising trend.There were two peak times,the first was from April to August,the incidence was 9.1% (92/1 010),12.2%(123/1010),13.4% (135/1010),11.1% (112/1010) and 10.7% (108/1010),respectively,in each month;the second was January,the incidence was 9.4% (95/1010).The diagnosed rate within 1 week of onset only accounted for 13.4% (135/1010),and 72.4% (731/1010) patients were diagnosed for more than two weeks after the onset.The male incidence of brucellosis was higher than the female,and the sex ratio was 2.36:1.00.In terms of age distribution,40-49 and 50-59 were both peak ages,with incidence of 29.3% (296/1 010) and 25.0% (252/1010),respectively.The professional was given priority to farmers and herdsmen (85.6%,865/1 010),and 96.4% (974/1010)of the patients had a clear exposure history,mainly related to sheep,cattle,pigs and other livestock,also related to pets and milk.Conclusions The brucellosis patients that received in Ji'nan Infectious Disease Hospital are increased significantly in the recent 5 years,and their transmission is closely associated with livestock breeding and processing.Most patients have failed timely diagnosis in the early days,which should catch enough of our attention.
10.Application of Transparent Cap Assisted Endoscopic Technique in Diagnosis and Treatment of Duodenal Disease

[Objective]To analyze the advantages of transparent cap assisted endoscopy in the diagnosis and treatment of duode-nal diseases.[Methods]From September 2014 to December 2015,62 cases with suspicious duodenal diseases in the endoscopy cen-ter of our hospital randomly divided into transparent cap group(n=31)and routine group(n=31)according to the time sequence. The visual field resolution,success rate of image capture and operating time were compared in 2 groups.[Results]Thirty cases in transparent cap group had clear visual field definition ,which was obviously higher than 9 cases in conventional group (96.8% vs 29%,P < 0.001). Twenty-three cases in transparent cap group and 8 cases in routine group were successfully captured(90.3% vs 25.8%,P < 0.001). The average operating time of the transparent cap group was significantly shorter than that of the conventional group(150+58 vs 95+36 seconds,P=0.004).[Conclusion]Transparent cap endoscope examination is more effective in the treat-ment of duodenal diseases,and the image capture is higher and the operation time is shorter.
