1.Primary and secondary focal segmental glomerulosclerosis and comparison of their subtype clinical characteristics

Objective To compare primary and secondary focal segmental glomerulosclerosis(FSGS)and the clinical characteristics of different subtypes.Methods The clinical and laboratory manifestations of 116 patients with primary and secondary FSGS in China-Japan Friendship Hospital from January of 2001 to December of 2008 were analyzed and compared.Results Of the 116 patients,88 were with primary FSGS(P-FSGS),13 with IgA nephropathy-associated FSGS(IgAN-FSGS),and 15 with obesity-related FSGS(OB-FSGS).Of the 88 patients with P-FSGS,53 were with not otherwise specified(NOS)type(60.2%),18 with cellular type(20.5%),13 with tip type(14.8%),4 with perihilar type(4.5%),and there was no case of collapsing type.Among the total 116 FSGS cases,the frequency of nephrotic syndrome was 41.3%,hypertension 54.3%,hematuria 59.5%,elevated serum creatinine level 44.8% and reduced creatinine clearance 54.3%.More than a half patients presented abnormal proximal or/and distal tubular function.Compared with P-FSGS and IgAN-FSGS,OB-FSGS patients had less proteinuria,higher serum albumin level,lower prevalence of nephrotic syndome,higher CCr and less decreased CCr cases(P
2.Effect of negative life events on tinnitus patients quality of life.

OBJECTIVE:
To study the relationship between negative life events and quality of life of tinnitus patients, and explore the role of negative life events in psychological counseling and rehabilitation of tinnitus.
METHOD:
Sixty-one cases of tinnitus patients and 61 normal controls were tested with tinnitus handicap inventory and life event scale, scores were compared and the associated analysis was made.
RESULT:
Stimulating quantity of negative life events in the tinnitus group, compared with the control group was statistically different (Z = -2.56. P < 0.05). The incidence of negative life events in the tinnitus group compared with the control group was no statistically different (chi2 = 0.05, P > 0.05). The incidence of abnormal scores of negative life events compared with the control group was statistically different (chi2 = 16.68, P < 0.01). Positive correlation in tinnitus group was found between scores of negative life events and scores of tinnitus handicap inventory (r = 0.41, P < 0.01). The higher the score of negative life events, the lower the quality of life with tinnitus patients.
CONCLUSION
Severity of negative life events and tinnitus is closely related, and negative life events can reduce the quality of tinnitus patients life. We should pay more attention to negative life events in psychological counseling with tinnitus patients. The intervention in negative life events can improve the quality of life. providing the targeted and individualized theory basis for tinnitus psychological counseling and rehabilitation.

3.Juvenile xanthogranuloma of the nasal vestibule: one case report.

To Study Juvenile xanthogranuloma (JXG) in the nasal cavity, raise the level of diagnosis and treatment for this disease. We reported a case with JXG and reviewed the literatures. JXG in nasal vestibule is ex tremely rare. Only three cases were reported before. After surgical removal,no recurrence was found over a 10-month follow-up. Clinical manifestations and imaging examination is nonspecific for JXG, and the diagnosis of the disease relies on pathological examination. Surgical resection is an effective treatment method.

4.Protective effects of ouabain conjugated peptide from Ph. D.-7 Library on vascular endothelial cell

AIM: To find one kind of peptide that will conjugate with ouabain and inhibit its biological function, and provide a new treatment strategy for primary hypertension. METHODS: In this study, ouabain was used as a target to screen ouabain conjugated peptide (OCP) from Ph. D. -7 phage display peptide library. After 3 rounds of bio-panning, the products were identified by ELISA and DNA electrophoresis analysis and sequencing. Peptide was synthesized and analyzed the activity by radioligand binding assay. The inhibitory ratio of cell proliferation was measured by MTT and the cell morphology changing was measured by Hoechst 33342/PI staining. The expression of Na~+-K~+-ATPase α1-subunit and β1-subunit were detected by RT-PCR and immunocytochemistry. The levels of the free intracellular Na~+ in EAhy926 cells were measured by laser confocal microscope. RESULTS: The ouabain conjugated peptide was found out, and it was occupied in 0.64(9/14). The analysis of protein showed that the obtained peptides had no homology with Na~+-K~+-ATPase. The amino acid sequence of synthesized peptide was Arg-Cys-Met-Thr-Ser-Arg-Ser. There was binding activity between OCP and ~3H-ouabain. The MTT assay showed that OCP could reverse the inhibition action of ouabain on vascular endothelial EAhy926 cells in a dose and time-dependent manner. The number of apoptotic cells had significantly decreased detected by Hoechst 33342/PI staining. The results of RT-PCR and immunocytochemistry showed that OCP could inhibit the up-regulated expression of Na~+-K~+-ATPase α1-subunit and down-regulated expression of Na~+-K~+-ATPase β1-subunit induced by ouabain in EAhy926 cells. CONCLUSION: The OCP could reverse the growth inhibition and death induction of ouabain in EAhy926 cells, which would provide the basis for studying the interaction between ouabain and Na~+-K~+-ATPase and explore novel anti-ouabain agents.
5.Analysis of Helicobacter pylori infection status in type 2 diabetes mellitus

Objective To comprehend the status of Helicobacter pylori (Hp) infection in type 2 diabetes mellitus (T2DM) patients, and study the relationship between Hp and T2DM. Methods Seven hundred and sixty-eight in-hospital T2DM patients (T2DM group, including new diagnosed 194 patients and re-diagnosed 574 patients) and 172 sex, age matched health individuals(control group ) were involved in this study. 14C-urea breath test (14C-UBT) was proceeded in both groups to determine Hp infection, and infection rates were calculated. Results Total Hp infection rate in T2DM group was significantly higher than that in control group [56.0%(430/768) vs 37.8%(65/172) ,P< 0.01], delamination analysis according to sex also showed significant differences between T2DM group and control group(P < 0.01). The rate of Hp infection in new diagnosed patients and re-diagnosed patients, female patients and male patients had no significant difference (P > 0.05 ). The rate of Hp infection in different age and sex had no significant difference (P >0.05 ). Conclusions The rate of Hp infection in T2DM patients is higher than that in non-T2DM patients.Hp infection has relationship with T2DM, and has no relationship with age, sex and course of disease.
7.A New Diol from Dimocarpus longan Seeds

Objective To investigate the chemical constituents of Dimocarpus longan seeds in Sapindaceae.Methods The chemical constituents were isolated from the ethanol extract of D.longan seeds by silica gel column chromatography.Their structures were identified on the basis of physical and chemical properties and spectral analysis.Results One compound was isolated and identified as 2-methyl-1,10-undecanediol,named longandiol(1).Conclusion Compound 1 is a new compound.
8.Effect of glucagon-like peptide-1 agonist on insulin signaling pathway in skeletal muscle of mice with insulin resistance

Objective To study the effect of glucagon-like peptide-1 agonist(exendin-4)on insulin signaling pathway in skeletal muscle of mice with insulin resistance(IR). Methods Thirty male KM mice were divided into normal control group(NC group, n=10)and IR group(n=20). The mice in NC group and IR group were fed with routine diet and high-fat diet for 16 weeks, respectively. After IR models were successfully established, 10 mice in IR group were treated with exendin-4(Ex group, 3μg·kg-1·d-1)for 4 weeks. NC group and IR group were given the same dose of normal saline. Body weight, fasting blood glucose(FBG), and fasting insulin(FINS)were measured before and after the intervention, and homeostasis model assessment for insulin resistance ( HOMA-IR ) was calculated . The muscle glycogen content were measured by Periodic Acid-Schiff Stain ( PAS ) . The expressions of insulinreceptor(IRC), insulin receptor substrate-1(IRS-1), and phosphatidylinositol 3-kinase(PI3K)in the skeletal muscle were measured by Western blot and immunohistochemistry. Results Before intervention, the body weight, FBG, and HOMA-IR of IR group and Ex group were higher compared with NC group(all P<0. 05), but FINS did not rise significantly(P>0. 05). Compared with before intervention, the body weight of three groups after intervention were all increased, but increased more significantly in the IR group. After intervention, the FBG and HOMA-IR in Ex group decreased, compared with IR group and Ex group of non-intervention (all P<0. 05), but FINS did not decrease significantly(P>0. 05). The skeletal muscle cells in NC control were colored extensively and more colored granules, while IR group were less colored, and Ex group were between NC group and IR group. Compared with NC group, the expressions of IRC, IRS-1, and PI3K in the skeletal muscle of IR group were significantly decreased(P<0. 05), not in Ex group(P>0. 05). Compared with IR group, the expressions of IRC, IRS-1 and PI3K in Ex group were higher(P<0. 05). Conclusions Exendin-4 improves insulin resistance by up-regulating the expressions of IRC, IRS-1, and PI3K in the skeletal muscle and promoting the synthesis of muscle glycogen.
9.Effect of closed-loop target controlled infusion system for propofol administration guided by the bispectral index during anethesia of the biliary tract and pancreas surgeries

Objective To evaluate the accuracy of bispectral index (BIS)-guided closed-loop target controlled infusion (TCI) system in comparison with opened-loop manual TCI during anesthesia of biliary tract and pancreas surgeries.Methods Forty adult patients undergoing open surgery of biliary tract or pancreas under total intravenous anesthesia, including 17 males and 23 females, aged 18-75 years, falling into ASA physical status Ⅱ or Ⅲ, were randomly allocated into closed-loop group (group C, n=20) and opened-loop manual group (group M, n=20).In group M, the propofol effect-site concentration was adapted at the discretion of the anesthesiologist to reach and maintain a BIS as close as possible to 42-52.In the closed-loop TCI group, propofol was administered using the closed-loop anesthesia delivery system to reach and maintain atarget BIS of 42-52.The BIS values would be recorded automatically by the system at each second after it began to run.The anesthesia duration, unconsciousness time, endotracheal intubation time, recovery time and endotracheal extubation time were recorded.The total usage of propfol and remifentanil were calculated.The incidence rates of emergence agitation, postoperative nausea and vomiting and intraoperative awareness were recorded.The frequencies of vasoactive drug were recorded.MDAPE, Wobble, GS through BIS values were calculated.Results BIS was maintained within ±10% of target (excellent) for significantly longer time in group C (52.1±10.5)% than that in group M (37.6±5.8)% (P<0.05).BIS was maintained within ±(10%-20%) of target (good) for the same time in both groups.MDAPE in group C (10.1±2.2)% were significantly lower than those in group M (15.3±6.4)% (P<0.05).GS in group C (23.1±8.9)% was significantly lower than that in group M (33.5±15.8)%.The usages of propofol in group C ·kg-1·min-1 were similar to those in group M (0.12±0.03) mg·kg-1·min-1, and the usages of remifentanil in group C (0.12±0.03) μg·kg-1·min-1 were similar to those in group M (0.15±0.05) μg·kg-1·min-1.The frequencies of vasoactive drug were similar in both groups.There was one incidence of emergence agitation in groups M.Postoperative nausea and vomiting and intraoperative awareness didn't occur in both groups.Conclusion The depth of the anesthesia is maitained more appropriately and stable in the closed-loop group than that in manual administration group.
10.The linkage between cell cycle S phase arrest and apoptosis on human hepatocellular carcinoma HepG2 induced by Na~+,K~+-ATPase inhibitors via regulating proteins associated with cell cycle

Aim To investigate the effect of ouabain and cinobufogenin on cell proliferation,apoptosis and cell cycle on HepG2,and explore their molecular mechanism.Methods The anti-proliferative effect on HepG2 cells was determined by MTT assay.The HepG2 cells were stained with Hoechst 33342,and its morphological changes were observed under fluorescence microscope;The cell cycle was measured by flow cytometry.The Cyclin A1,CDK 2,PCNA and p21~(CIP1) expression levels of HepG2 cells treated with ouabain and cinobufogenin were dectected in mRNA and protein by Real time PCR and Western blot.Results Ouabain and cinobufogenin could inhibit cell proliferation on HepG2 cells,and the inhibitory effects were in time and dose dependent manners.The HepG2 cells treated with ouabain and cinobufogenin showed the typical morphological features of apoptosis.Cell cycle analysis showed that the S phase of HepG2 cells treated with ouabain and cinobufogenin increased significantly compared with the control group.Real-time quantitative PCR and Western blot results showed that ouabain and cinobufogenin could down-regulate Cyclin A1,CDK 2,and PCNA expressions(P<0.05)and up-regulate p21~(CIP1) expression(P<0.05).Conclusion Nα~+,K~+-ATPase inhibitor has the anti-proliferative effect on HepG2 cells and induce apoptosis and S phase arrest.These effects might be related with proteins associated with cell cycle closely.
