1.Multivitamin and Mineral Supplements on the Quality of Llife of Healthy Persons

Objective: To explore the effects of multivitamin and mineral supplements on self-evaluation of mood and health status in healthy volunteers.Methods:149 healthy volunteers took multi-nutrient supplements or placeboes for 8 weeks, and self-evaluation of mood and health status was evaluated by Profile of Mood States and 36-item short-form health survey.Results:Self-comparing POMS scores, active group's scores of vigor were significantly increased, from 14.2? 4.6 to 15.5 ? 4.9(t=2.74,P37)were significantly improved. But the placebo group and young volunteers were not significantly improved. Conclusion: The intervention of multivitamin and mineral supplements can improve elderly healthy persons' mood and healthy status.
2.The clinical experience of single-port laparoscopic surgery for incarcerated inguinal hernia in children of 32 cases

Objective To explore the feasibility and safety of single-port laparoscopic surgery in the treatment of incarcerated inguinal hernia in children. Methods The clinical efficacy of single-port laparoscopic and epidural anesthesia needle treatment for incarcerated inguinal hernia in children of 32 cases were analyzed retrospectively. Results Thirty-two children were operated successfully. The operation time was (16.24 ± 4.67) min for unilateral hernia (in 25 cases) and (21.69 ± 5.85) min for bilateral hernia (in 7 cases). The blood loss was (2.50 ± 0.56) ml. The recovery time of intestinal peristalsis was (16.42 ± 4.86) h after surgery, and the postoperative hospital stay was (3.25 ± 0.82) d. Intraoperative exploration found 7 cases had concealed contralateral hernia;incarcerated hernia contents included bowel in 27 cases, ovaries in 3 cases, and greater omentum in 2 cases. All the incarcerations were successfully restored by hand and no-damage clamp assistance. The incarcerated organ had good blood flow in all the 32 cases. The follow-up time was (15.62 ± 5.34) months, and there was no recurrence and atrophy of testis or ovaries. Conclusions The single-port laparoscopic surgery for incarcerated inguinal hernia in children has advantages of preservation of anatomy of the inguinal canal easily to perform high ligation of hernial sac, and reduction under direct visual control. Meanwhile, occult inguinal hernia in the opposite side can be cured. It was a safe, effective, and minimally invasive procedure, being worthy of further clinical application.
3.Clinical research of umbilical single-port laparoscopic surgery in the treatment of children ovarian cysts of 36 cases

Objective To investigate the feasibility and safety of umbilical single- port laparoscopic surgery in the treatment of children ovarian cysts. Methods The clinical efficacy of umbilical single-port laparoscopic surgery in the treatment of children ovarian cysts of 36 cases was analyzed retrospectively. Results Thirty-six children successfully completed surgery. The operative time was (27.8 ± 8.6) min, the diameter of ovarian cysts was (7.2 ± 2.4) cm, and the blood loss was (6. 5 ± 1.6) ml. All the children were confirmed to have benign ovarian cysts by pathological examination. Thirty-four children retained ovarian tissue after removal of the cyst, and 2 children underwent oophorectomy because of ovarian necrosis. The anus exhaust was recovered (18.4 ± 4.6) h after surgery, and the postoperative hospital stay was (4.2 ± 1.3) d. The children began to diet after anesthesia conscious. All the children recovered well, and no surgical complications occurred. The followed-up time was (24.6 ± 6.3) months, and no recurrence occurred. Conclusions Umbilical single-port laparoscopic surgery in the treatment of children ovarian cysts is easy, minimally invasive, no scar formation, quickly recovered and reliable. It is worth of further clinical application.
4.ANTHROPOMETRY AND FAT STORE DURING PREGNANCY

Weight gain, skinfold thickness, circumference and body density were measured in 30 primiparous women aged from. 23-31 years (averaged 26.3 years old).The average body weight gained 13kg from 6 to 37 weeks of pregnancy.The suprailiac skinfold thickness in 24-37 weeks and the subscapular skinfold thickness in 37 weeks increased significantly from the 6th week of pregnancy, but the triceps and biceps skinfold thickness did not change significantly. The density of the body in 24th and 37th week of pregnancy decreased significantly.The change and the body fat (%) calculated by means of 4 skinfolds increased in 3.7% (5.7kg of fat). The mean values of the hip circumference in 24th and 37th week were significantly different from that in 6th week of pregnancy. The upper thigh in the 24th and 37th week also increased slightly but not significantly different as compared with the 6th week. The mean values of arm, mid-thigh and calf circumference did not change during pregnancy.
5.EFFECT OF VITAMIN A AND IRON SUPPLEMENTATION ON IRON STATUS DURING PREGNANCY

Objective: To observe the effect of vitamin A and iron supplementation on iron status during pregnancy. Methods: 167 pregnant women aged 21~33 years in the second trimester were recruited and randomly allocated to four groups: placebo; vitamin A (1.1 mg/d); iron(60 mg elemental iron); vitamin A and iron, for 10 weeks. Results:The incidences of vitamin A deficiency, anemia and iron depletion were 0.6%,7.8% and 6.2% in the second trimester respectively. Serum VA level was not changed in three supplemented groups but decreased in placebo group. Hemoglobin in all groups before and after supplementation was in normal range, with no significant differences. The improvement in serum ferritin, free erythrocyte proporphyrin and transferrin saturation was better in the group supplemented with VA+Fe than VA or Fe alone. Conclusion: The simultaneous supplementation of vitamin A and iron is better in improvement of iron status than supplementation of vitamin A or iron alone during pregnancy. Improvement of iron status by simultaneous supplementation of pregnant women with vitamin A and iron was better than supplementation with iron or vitamin A alone.
6.Value of neutrophil-lymphocyte count ratio in diagnosing bloodstream infection

Objective To explore the diagnostic value of five infection markers in bloodstream infection.Methods Randomly selected 110 bloodstream infection patients with positive blood cultures and 30 bacterial infection patients with negative blood cultures.Blood was simultaneously drawn with blood cultures;the complete blood count and C-reactive protein (CRP) levels were measured.The white blood cell count (WBC),neutrophil count (NEU),lymphocyte count (LMY),CRP level and neutrophil-lymphocyte count ratio (NLCR) were compared between the two groups.Results The levels of WBC,NEU,NLCR and CRP in bloodstream infection group were significantly higher than those in control group (P<0.05),while LYM was significantly lower than that in control group (P< 0.05).Among these five infection markers,the area under the receiver operating characteristic curve (ROC-AUC) was the highest for NLCR (0.808) and LMY (0.756);when the cutoff value for NLCR was >9.33,sensitivity was 63.6%,specificity was 93.3%;and the cutoff value for LYM was ≤0.97,sensitivity was 58.2%,specificity was 86.7%.Furthermore,the NLCR of patients with gram-negative bloodstream infection was higher than those in patients with gram-positive bloodstream infection.NLCR showed important clinical significance in distinguishing strains of different bloodstream infections.Conclusions NLCR is the better predictors than routine parameters in diagnosing bloodstream infection.
7.Comparison of single- port laparoscopic appendectomy and mult- port laparoscopic appendectomy in children with appendicitis

Objective To compare the clinical effect of single-port laparoscopic appendectomy and traditional mult-port laparoscopic appendectomy in children with appendicitis. Methods The clinical data of 96 appendicitis children who had underwent laparoscopic appendectomy were retrospectively analyzed. Among the children, 48 children received single- port laparoscopic appendectomy (observation group), and 48 children received mult-port laparoscopic appendectomy (control group). The operation time, blood loss, postoperative exhaust time, postoperative hospital stay, incision length and surgery-related complications between 2 groups were compared. Results All children were operated successfully. There were no statistical differences in blood loss and postoperative hospital stay between 2 groups (P>0.05). The operation time, postoperative exhaust time and incision lengths in observation group were significantly lower than those in control group: (19.8 ± 5.3) min vs. (30.2 ± 8.1) min, (15.6 ± 4.7) h vs. (22.7 ± 6.5) h and (1.1 ± 0.2) cm vs. (2.3 ± 0.4) cm, and there were statistical differences (P<0.01). There was 1 case with incision infection in observation group, and the child was cured after symptomatic treatment. There was no other surgery-related complication in 2 groups, and all children recovered smoothly. Conclusions The single-port laparoscopic appendectomy is safe and feasible in children with appendicitis. It has short operation time, fast recovery of gastrointestinal function, and better cosmetic effect.
8.Value of 1H-MRS on SCA3/MJD diagnosis and clinical course

Objective:To investigate the value of proton magnetic resonance spectroscopy (1H-MRS) on the diagnosis of SCA3/MJD,and to calculate the correlation between 1H-MRS ratio and the clinical score.Methods:Sixteen patients with SCA3/MJD and 19 healthy volunteers were scanned with 1H-MRS.The data of N-acetyl aspartate,creatine,choline-containing compounds,myoinositol,NAA/Cr,Cho/Cr,and mI/Cr ratio were collected,which were grouped for comparative study.The onset patients with SCA3/MJD were evaluated with the International Cooperative Ataxia Rating Scale and Scale for the Assessment and Rating of Ataxia,the correlation between NAA/Cr,Cho/Cr or mI/Cr ratio and the clinical score was calculated.Results:The NAA/Cr in the pons and cerebellar dentate nucleus from the onset patients with SCA3/MJD was significantly reduced compared to that in the normal control group.The NAA/Cr in the cerebellar dentate nucleus of onset patients with SCA3/MJD was obviously correlated with ICARS.Conclusion:SCA3/MJD lesions are mainly located in the cerebellum and brainstem,where gray and white mater are also involved.The cerebellar dentate nucleus may be the earliest involved area.There is a correlation between the ICARS and the cerebellar lesion degree.The ICARS reflects the severity of clinical manifestations.1H-MRS is useful in the diagnosis of SCA3/MJD.

Objective: To investigate the combined preventive effect of folic acid (FA) and vitamin B12 (VB12) on the developmental toxicity of alcohol. Methods: To build animal model with the developmental toxicity by giving 5g/kg bw alcohol (25% ethanol) intragastrically (IG) to ICR mice during gestational day (GD) 6-15. In addition to alcohol, three groups were given FA 60 mg/kg bw, VB12 1 mg/kg bw, FA 60 mg/kg bw + VB12 1 mg/kg bw respectively by IG during GD1-GD16. An alcohol model group and a negative control group were set. All dams were killed at GD18. 10.Diffusion weighted imaging of SCA3/MJD and SPG4.

OBJECTIVE:
To determine the value of diffusion weighted imaging (DWI) in the diagnosis of hereditary spinocerebellar ataxia 3 and the Machado Joseph disease (SCA3/MJD) and hereditary spastic paraplegia 4 (SPG4).
METHODS:
We scanned 13 patients with SPG4, 30 patients with SCA3/MJD (21 onset patients and 9 with only genetic abnormalities), and 27 healthy volunteers with DWI. The processing data were apparent diffusion coefficient (ADC). The above data were grouped for comparative study.
RESULTS:
In the precentral gyrus, posterior limb of the internal capsule, cerebral peduncle, pons, cerebellar cortex and cerebellar white matter, the ADC of onset SCA3/MJD patients increased compared with the control group. The ADC of non-onset SCA3/MJD patients increased only in the cerebellar dentate nucleus compared with the control group. In the cerebellar cortex, the ADC of onset SCA3/MJD patients was significantly higher than the non-onset SCA3/MJD. The ADC of onset SCA3/MJD patients was significantly higher in the posterior limb of the internal capsule, cerebellar cortex, cerebellar white matter and pons than that of SPG4 patients. In the precentral gyrus, the ADC of SPG4 was significantly higher than control.
CONCLUSION
DWI is useful in the diagnosis of SCA3/MJD and SPG4.
