OBJECTIVE:To promote the implementation of Prescription Administrative Policy(on trial).METHODS:The factors related to implementation of Prescription Administrative Policy(on trial)were analyzed,These included legal awareness of medical staff,the professional quality of pharmacists,deficiencies of electric prescription system,validity of prescribing right,and lack of standardization in drug names.RESULTS&CONCLUSIONS:Successful implementation of Prescription Admin?istrative Policy(on trial)is based on cooperation of relevant offices,consistent perfection of implementing regulations,and ed?ucational reform of pharmacy.
2.Adverse Drug Reactions Induced by Gatifloxacin:Analysis of 866 Cases During 2003~2006

OBJECTIVE:To study the characteristics of adverse drug reactions(ADR) induced by gatifloxacin.METHODS:A total of 866 ADR cases induced by gatifloxacin collected in Zhejiang ADR Monitoring Center between Oct.30,2003 and Oct.30,2006 were analyzed with Excel.RESULTS:The common ADR induced by gatifloxacin were lesions of skin and appendages,gastro-intestinal system,systemic and nervous system.Most of the cases were slight in symptoms,but gatifloxacin could also induce severe ADR such as dysglycemia etc that resulting in potential risks to vital human organs.CONCLUSION:ADR monitoring should be emphasized when using gatifloxacin so as to decrease or avoid the occurrence of ADR.
3.Effects of Zibu Piyin Recipe Serum on endoplasmic reticulum(ER)stress-induced neuron apoptosis and its mechanism

Objective To explore the inhibitory effects of Zibu Piyin Recipe(ZBPYR)serum on neuron apoptosis induced by tunieamyein(Tm,5 μg/ml)and its mechamsm in vitro by using sero-pharmacological method.Method Totally 12 healthy adult male SD rats(220~250 g)(SPF)were divided randomly into control group and ZBPYR group,6 in each group,then the blank and ZBPYR serum were prepared.The mouse.neuroblastoma cell line Neum2a cells were treated with Tunicamycin(Tin,an inhibitor of N-glycoslytion)to establish the endoplasmic reticulum(ER)stress model.The cells treated by ZBPYR aerum of different concentrations were interventional groups,and the cells treated by blank serum were control group.The viability of Neuro2a cells was meusurcdd by MTT assay.Flow cytometry wus applied to observe the apoptosis of Neuro2a cells.Western blotting was utilized to detect the protein expressions of two molecules,ER molecular chaperone-ucose regulated protein 78(CRP78)and transcriptional factor CCAAT/enhancer-binding protein-homologous protein(CHOP).The results were analyzed by sNK-q test.Results Compared to Tm group(cell viability 0.1673±0.0213,apoptotic rate 62.7050±1.4056),The cell viability of interventional groups(5%0.5295±0.0373,10%0.5843±0.0428,15%0.6274±0.0324)increased significantly(P<0.05);and the apoptotic rate(5%47.8733±2.8166,10%46.3366±1.2748,15%39.8833±1.0524)reduced significantly(P<0.05).The protein expressions of GRP 78(5%2.1228±0.2251,10%1.3293±0.9443,15%;15%0.0931±0.1168)and CHOP(5%1.1776±0.2927,10%0.7290±0.1708,15%0.6577±0.1883)of interventional groups reduced significantly compared with Tm group(GRP78 2.9149±0.5355;CHOP 1.6611±0.2913)P<0.05.Condusions ZBPYR serurn could increase the cell viability of Neuro2a cells treated with Tm and inhibit cell apoptosis.Thereby it may have neuroprotective effects,and the mechanism may be associated with the inhibition of ER stress and apoptosis pathway.

5.Combined management of non-small cell lung cancer:current status and perspective

Lobectomy has been the primary treatment modality for resectable non-small cell lung cancer.However, recent data showed that surgery alone is not adequate for resectable NSCLC treatment. All patients eligible for surgery with curative intent (stage Ⅰ,Ⅱ,and ⅢA) may benefit from adjuvant chemotherapy.Although combined chemoradiation may improve local control for stage ⅢA disease,radiotherapy has been shown to be harmful for patients with stage Ⅰ and Ⅱ NSCLC after complete resection.The role of neoadjuvant chemotherapy or chemoradiation in resectable NSCLC is controversial.RTOG and SWOG have initiated a phase Ⅲ randomised trial to evaluate the effect of preoperative combined chemoradiation followed by surgical resection and consolidation chemotherapy for stage ⅢA disease.In this review,we discuss the current status of combined management of NSCLC and forecast the future direction of treatment by reviewing a number of landmark publications and ongoing clinical trials..
6.Innovative Pharmaceutical Service Practice of Medical Institutes in Zhejiang Area

OBJECTIVE:To provide reference further development of hospital pharmacy in China. METHODS:Innovative pharmaceutical care method of hospital pharmacy was summarized in 32 medical institutions from Zhejiang area,including 18 third-level hospitals,8 special hospitals,6 third-level TCM hospitals and integrated traditional and western medicine hospitals. RE-SULTS & CONCLUSIONS:Medical institutions provide innovative pharmaceutical service to guarantee the safety of drug use,ex-pand medical service space,enrich the connotation of pharmaceutical care,and provide reference for pharmaceutical care in medi-cal institutions of other regions,such as establishing satellite pharmacy of anesthesiology department,opening outpatient depart-ment of pharmaceutical care (elderly comprehensive evaluation and multidisciplinary outpatient pharmaceutical care,anticoagula-tion consultation outpatient department and endocrine consultation outpatient department managed by pharmacists,comprehensive pharmacy consultation outpatient department,outpatient pharmaceutical care of pharmacy unit mode),formulating pharmaceutical ward round mode under multidisciplinary cooperation,developing clinical pharmaceutical care based on pharmacy scientific re-search(drug monitor,gene detection)and evidence-based medicine.
7.Effects of camptothecin on the expression of hypoxia-inducible factor-1α in HaCaT cells

Objective To estimate the effects of camptothecin (CPT) on the expression of hypoxia-inducible factor-1α (HIF-1α) in HaCaT cells under hypoxic conditions (2% O2),and to explore the potential therapeutic mechanism of topical CPT for psoriasis.Methods Some HaCaT cells were classified into 6 groups:5 test groups cultured in Dulbecco's modified Eagle's medium (DMEM) with the presence of CPT at 12.5,25,50,100 and 200 nmol/L respectively,and 1 control group cultured in DMEM with the presence of dimethyl sulfoxide (DMSO).All the 6 groups of cells were cultured under normoxic conditions for 12,24,48 or 72 hours or under hypoxic conditions for 12 hours.Cell counting kit-8 (CCK-8) assay was conducted to estimate the proliferation of HaCaT cells after the normoxic culture,and Western blot to quantify the protein expression of HIF-1α after the hypoxic culture.Some HaCaT cells were classified into a normoxia group (21% O2) and a hypoxia group (2% O2),and each group was divided into a CPT (100 nmol/L)-treated subgroup and a non-intervention subgroup (treated with the vehicle).After 12-hour culture,real-time fluorescencebased quantitative PCR was performed to measure the mRNA expression of HIF-1α.Statistical analysis was carried out by using Levene'.s test,one-way analysis of variance,Dunnett-t test and factorial analysis with the SPSS16.0 software.Results After treatment with CPT at 12.5-200 nmol/L for 12-72 hours,the proliferation of HaCaT cells was inhibited in a concentration-and time-dependent manner.More concretely,the cell proliferation rates were inhibited by 17.66% ± 6.46%,33.11% ± 4.63% and 56.31% ± 1.69% respectively in HaCaT cells after 12-hour treatment with 200 nmol/L CPT as well as 24-hour treatment with 100 and 200 nmol/L CPT compared with the control group at the corresponding time points (all P < 0.05).The protein expression level of HIF-1 α was significantly decreased in HaCaT cells after 12-hour treatment with CPT at 12.5,25,50,100 and 200 nmol/L under hypoxic conditions compared with the control group (0.348 ± 0.065,0.261 ± 0.112,0.115 ± 0.043,0.045 ± 0.024 vs.1.445 ± 0.329,all P< 0.05).The mRNA expression level of HIF-1α (expressed as △Ct) in the CPT-treated subgroup and non-intervention subgroup was-5.575 ± 0.29 and -5.451 ± 0.21 respectively in the normoxia group,significantly higher than that in the hypoxia group (-6.543 ± 0.57 and -6.203 ± 0.31 respectively,F =29.856,P < 0.05),while there was no significant difference between the CPT-treated and non-intervention subgroups (F =1.667,P > 0.05).Conclusions CPT at 100 nmol/L could inhibit the expression of HIF-1α protein,but had no obvious effect on that of HIF-1α mRNA.
8.Effects of camptothecin on the autophagy of HaCaT cells

Objective To evaluate effects of camptothecin on the autophagy of HaCaT cells.Methods Some cultured HaCaT cells were divided into several groups to be treated with camptothecin at concentrations of 5,10,25,50,100 and 200 nmol/L,and 0.1% dimethyl sulfoxide (DMSO) (control group),respectively.Cell counting kit-8 (CCK-8) assay was conducted to estimate the proliferative activity of HaCaT cells after 24-and 48-hour treatment,flow cytometry to evaluate cell apoptosis after 24-hour treatment,and Western blot analysis to measure the expression of autophagy-related proteins microtubuleassociated protein 1 light chain 3 (LC3) and p62.Some HaCaT cells were divided into 2 groups to be treated with 10 nmol/L camptothecin and 0.1% DMSO for 24 hours,respectively.Then,indirect immunofluorescence assay (IFA) was performed to determine the LC3 expression.Results Camptothecin at low concentrations of 5 and 10 nmol/L had no significant effects on the proliferation and apoptosis of HaCaT cells.Compared with the control group,the cellular proliferative rates were significantly inhibited by (31.23 ± 1.00)%,(54.21 ± 8.10)% and (66.75 ± 10.70)% in the 50-,100-and 200-nmol/L camptothecin groups after 24-hour treatment respectively,and by (25.81 ± 5.99)%,(44.35 ± 5.32)%,(65.81 ± 8.28)% and (73.23 ± 9.59)% in the 25-,50-,100-and 200-nmol/L camptothecin groups after 48-hour treatment respectively (all P < 0.001).After 24-hour treatment,the apoptosis rates were significantly higher in the 50-,100-and 200-nnol/L camptothecin groups (14.46% ± 2.38%,19.15% ± 1.59%,29.88% ± 1.37%,respectively) than in the control group (3.80% ± 0.13%,all P < 0.001).After 24-hour treatment with 5 and 10 nmol/L camptothecin,the protein expression of LC3 Ⅱ was significantly up-regulated,while p62 protein expression was significantly down-regulated:IFA showed that the percentage of autophagosome-positive cells was significantly higher in the 10-nmol/L camptothecin group than in the control group after 24-hour treatment (36.67% ± 4.55% vs.6.23% ± 0.92%,t =6.546,P =0.003).Conclusions Camptothecin at low concentrations of 5 and 10 nmol/L can induce autophagy of HaCaT cells,but has no obvious effects on cell proliferation and apoptosis.Camptothecin at concentrations of 50,100 and 200 nmol/L can inhibit cell proliferation,promote cell apoptosis,and decrease autophagy levels.
9.The prophylactic effect of methylprednisolone combined with granisetron on postoperative nausea and vomiting after lumpectomy of breast

Objective To explore the prophylactic effect of methylprednisolone combined with granisetron on postoperative nausea and vomiting.Methods Two hundred patients scheduled for lumpectomy of breast were randomly divided into four groups with 50 cases each.The patients in group M1 received a pre-anesthesia intravenous doses of methylprednisolone 25 mg,the patients in group M2 were injected methylpredsisolone 25 mg repeatedly four hours later,in group D received a pre-anesthesia doses of dexamethasone 5 mg,in group N normal saline 2 ml.All the four groups of patients received granisetron 3 mg intravenously at the end of surgery.The incidence of nausea and vomiting in the 24 hours were observed.Results The PONV incidences of group M1,M2,D,N were 36%,18%,38% and 58%.Both group M1,M2 and D significantly decreased the total inci-dence of PONV (P <0.05)in the 24 h.The incidence of PONV was significantly lower in group M2, compared with group M1 and group D respectively (P <0.05).Conclusion Methylprednisolone-gran-isetron combination is as equally effective as dexamethasone-granisetron combination for preventing PONV in lumpectomy,but repeated methylprednisolone after 4 h is more effective than dexametha-sone and single-used methylprednisolone.
10.Clinical study on the treatment of acne vulgaris due to phlegm-heat accumulation with Wendan decoction

Objective To observe the clinical efficacy of Wendan decoction in the treatment of phlegm-heat accumulation type acie vulgaris.Methods 62 patients with phlegm-heat accumulation type ache vulgaris were randomly divided into a treatment group and a control group randomly,with 31 patients in each group.The control group was given dietary advice and skin cleansing techniques,while the treatment group was given oral administration of Wendan decoction on the basis of the control group.Results ①skin damage comparison:after the treatment,skin damage symptoms such as tenderness (0.71 ±0.32),redness (0.47±0.63),cyst(0.59±0.53),and tubercle (0.63±0.54) in the treatment group were significantly improved than the control group [the values were (1.12±0.32),(1.44±0.21),(1.40±0.36),(1.18±0.43),respectively] (P<0.05).②symptoms score comparison:The symptoms score in the treatment group after the treatment (42.95±1.53) was significantly improved than that before the treatment (52.89± 1.98),(P<0.05),and also obviously better than the control group after the treatment.③clinical efficacy comparison:Total efficacy was 96.77% in the treatment group and 54.83% in the control group,showing marked difference(P<0.05).Conclusion Wendan decoction is effective in treating phlegm-heat accumulation type acne vulgaris.
