1.Effect of aminoguanidine and tienam on the bacterial translocation in mice with acute necrotizing pancreatitis

Objective To investigate the effect of aminoguanidine and tienam on bacterial translocation in mice with acute necrotizing pancreatitis (ANP) and the preventive and curative effect on pancreatic infection. Methods 50 SD mice were randomly divided into 5 groups, including group A: normal control (n=10), group B: ANP group (n=10), group C: aminoguanidine treatment group (n=10), group D: tienam treatment group (n=10), and group E: aminoguanidine plus tienam treatment group (n=10). ANP was induced by intrapancreatic injection of 5% sodium tanrocholate (2.5 ml/kg). Aminoguanidine (100 mg/kg) was injected intraperitioneally 30 minutes after ANP induction, tienam (60 mg/kg) was injected intraperitioneally 6 hours after ANP induction. The mice were killed at 48 hours, and serum amylase, serum D- lactate, pancreatic MPO were measured. Pathological alterations in the pancreas were examed. The pancreas, liver, blood, mesenteric lymph node and ascites were collected for microbiological study. Results In group C and E, the levels of serum amylase were 91173.30±199.73) U/L and (1075.00±200.40) U/L; the serum D-lactate were (7.165±1.2533) μg/ml and (6.980±1.060)μg./ml; the pancreatic MPO were (0.8035±0.0711) U/g wet film and (0.7765±0.0843 ) U/g wet film; the average bacterial positive rates were 20% and 16%. In group B, the serum amylase was (2234.60±692.06 )U/L;the serum D-lactate was (12.408±1.779)μg,/ml, the pancreatic MPO was (1.5942±0.1965) U/g wet film; the average bacterial positive rate were 60%. The differences between group C, E and group B were statistically significant (P< 0.05). In group D, the pancreatic MPO was (0.8002±0.0603 ) U/g wet film and the the average bacterial positive rate was 18%, and they were statistically different when compared with those of group B (P±0.05). While in group D, the serum amylase and D-lactate was not statistically different when compared with those of group B (P>0.05). There were lamellar necrosis in the parenchyma, hyperaemia and leukocytic infiltrate in the pancreatic mesenchyma in the ANP group, while no leukocytic infiltrating in group C, D, E was found. Conclusions The aminoguanidine and tienam might decrease gut bacterial translocation and prevent pancreatic infection in ANP.
2.The confirmed inspection and analysis of HCV Different Region ELISA KIT to anti-HCV near S/CO sample

Objective To discuss the confirmed value of HCV Different Region ELISA KIT (ANTI-HCV-C, ANTI-HCV-NS3, ANTI-HCV-NS4, ANTI-HCV-NS5) to anti-HCV CO sample.Methods To inspect the general antibody of anti-HCV with ELISA, which is commonly used in clinic.The serum was separated from the suspicious samples, whose results were equal to S/CO or in the state of grey district ( CO?10%), and stored in the refrigerator in -29℃ low temperature and concenratedly inspected by the HCV Different Region ELISA KIT on the Triturus,the enayme immuno-reactions automatic analyzer. We totally collected 44 suspicious samples,among which there were 24 anti-HCV positive,20 anti-HCV CO?10% negative. Meanwhile we collected 30 samples whose S/CO of general antiboby was more than 5.0 and 20 ones which were healthy and could be compared with.Results In the 44 suspicious cases, whose S/CO was in the state of grey disrtict (CO?10%), the HCV Different Region Kit confirmed positive was 22.7% (10/44),suspicious 6.8% (3/44), negative 70.5 % (31/44).The false positive rate of positive anti-HCV CO?10% was 54.2%(13/24) and the false negative rate of negative anti-HCV CO?10% was 5.0%(1/20). In the 30 cases,whose S/CO of general antibody was more than 5.0,the HCV Different Region Kit confirmed positive was 80.0%(24/30),suspicious 13.3%(4/30), negative 67%(2/30). In 20 cases,which were from healthy people, the HCV Different Region Kit confirmed totally negative.Conclusion Using HCV Different Region Kit is a better evidence laboratory,especially for the uncertain samples,that is,it is very important for the "grey district"(CO?10%) samples to be further confirmed.It can reduce the false positive and false negative result which may emerge when anti HCV EIA inspects the dangerous groups and improve the accuracy of clinical HCV diagnosis.To blood station, it has certain practical value for the blood not only can be safely used, but also avoided wasting. The operation is simple and convenient, easily be spread and applied.
3.Diagnostic value of combined detection of serum CEA, CA125, CA153 and CA199 in patients with lung cancer

Objective To evaluate the clinical value of combined detection of serum CEA, CA125,CA153 and CA199 in lung cancer diagnostec. Methods The serum level of CEA,CA125,CA153 and CA199 in 58 patients with lung cancer,38 patients with benign lung diseases and 40 healthy persons were determinated by Chemiluminesent immunoassay. The data were analyzed to evaluate the diagnostic value of the combination use of four markers. Results The serum levels of CEA,CA125,CA153 and CA199 in lung cancer group were 5.6 (1.0-619. 0) μg/L,124. 5 (5.3 - 994. 8) U/ml,14.4(3. 1 - 800. 0) U/ml and 21.8(2.0 - 1200.0) U/ml respectively ;The serum levels of CEA, CA125, CA153 and CA199 in benign lung diseases were 1.9 (0. 6 -14.4) μg/L, 17. 7 ( 1.8 - 303.6 ) U/ml, 6. 8 ( 2. 4 - 20. 1 ) U/ml and 9. 3 ( 2. 0 - 82. 6 ) U/ml respectively. The serum levels of CEA, CA125, CA153 and CA 199 in healthy controls were 1.4 (0. 6 - 5.0 ) μg/L, 9.6 (4. 4 -36. 5 ) U/ml,6. 6 (3.8 - 19. 9) U/ml and 6. 4 (2.0 - 58. 1 ) U/ml respectively. The levels of markers in the lung cancer group were significantly higher than those in patients with benign lung disease and healthy controllers (Ps < 0. 01 ). The sensitivity of combined measurement was 94. 8%, significantly higher than a single measurement ( CEA 48. 3% ,CA125 75.9% ,CA153 20.7% ,CA199 31.0% ,Ps <0. 01 ). Conclusion The combined detection of CEA, CA125, CA153 and CA199 can significantly improve detection sensitivity of lung cancer and might provide valuable laboratory proof for early diagnosis of lung cancer.
4.Comment on Literatures of Clinical Randomized Controlled Trials of Traditional Chinese Medicine for Acute Myocardial Infarction

[ Objective ] To assess the quality of clinical randomised controlled trials (RCT) of traditional Chinese medicine (TCM) for acute myocardial infarction (AMI). [Methods] Eighty - three journals of TCM from the first issues of 1977 to the latest issues of 2002 were manually searched and reviewed under the following inclusion criteria: clinical trials of TCM for AMI with 'randomized controlled or randomised being mentioned. And a questionnaire was designed according to the criteria of RCT with the items of diagnostic standard, consistent of baseline, method of randomization, concealment in allocation, blind method, intention - to - treat analysis, statistic methods and conclusion. Percentages of each item were figured out to assess the quality. [Results] Two thousand five hundred and one trials were associated with heart diseases, among which 102 were associated with AMI and 42 accorded with the inclusion criteria. Calculation method of sample size, concealment in allocation and intention - to - treat analysis after drop - out were mentioned in none of the 42 articles, single - blind method adopted in 4 (9.52%) , diagnostic standard described in 40 (95.24%), baseline consistent illuminated in 27 (64.29%), method of randomization presented in 8 (19.05%) , statistic methods used mistakenly in 6 (14.28%) and statistic methods not described in 18 (42.86%). [Conclusion] There still exist some problems in the design of RCT of TCM for AMI: (1) mistaken use of randomization method; (2) less adoption of calculation method of sample size, concealment in allocation and intention - to - treat analysis after drop - out; (3) improper application of statistic methods; (4) lower rate of utilization for blind study.

Standardized management and safe-supply system can be enhanced by oxygen chamber management rules,oxygen safe -supply management,daily maintenance of oxygen supply,solutions on common malfunctions and the management of oxygen humidifier. The importance of the management of oxygen is discussed and the regulations should be strictly abided to reduce accident. The oxygen used on clinic can be standardized to ensure the safe-supply.
6.Inhibition of transdifferentiation of human renal tubular epithelial cells by valsartan

Objective To investigate the possible role of valsartan(Val) in transdifferentiation of human renal tubular epithelial cell line(HKC ). Methods HKC cells were divided into four groups: (1) serum-free (negative control); (2) MCP-1 + AAI-treated (positive control); (3) Val-treated alone; (4) Val-inhibition(treated with MCP-1 + AAI + Val). Then the expression of vimentin, ?-smooth muscle actin (?-SMA ) of HKC cells were assessed by indirect enzyme immunohistochemistry (IEI), and the percentage of ?-SMA(+ ) HKC cells was assessed by flow cytometry. Results No difference in expressions of vimentin and ?-SMA by IEI and the percentage of ?-SMA(+ ) HKC cells by cytometry were found between serum-free control HKC cells and Val treated ones. The expressions of vimentin and ?-SMA in positive controls were markedly stronger than negative controls; while these expressions in Val + MCP-1 + AAI-treated HKC cells were less strong than those in positive controls. 7.Life quality of patients with nasopharyngeal carcinoma after intensity modulated radiotherapy and chemotherapy:a qualitative study

Objective To evaluate the life quality of patients with nasopharyngeal carcinoma after intensity modulated radiotherapy and chemotherapy. Method Using phenomenological research method, we interviewed 10 patients repeatedly to explore their somatization, psychological changes as well as the effect on quality of life. Result The life quality of patients with nasopharyngeal carcinoma after intensity modulated radiotherapy and chemotherapy were affected tremendously , characterized by intense psychological stress, side effects from radiochemotherapy, hunger for emotional support, heavy burden of medical expenses, and the change of family and work roles. Conclusion Nursing staff should understand the mental feelings of patients, and provide supports for the patients psychologically, physically, from the economic, family and social points of view, so as to improve the life quality and physical and mental health of patients.
8.Etiology of ischemic stroke in young adults

The etiology of ischemic stroke in young adults is extensive, and cardiogenic embolism and cervicocephalic arterial dissection are the most common. In recent years, the proportion of atherosclerotic ischemic stroke has increased because of the increased incidence and inadequate control for the traditional vascular risk factors in young adults. However, even after a detailed and comprehensive examination, there is stil about 1/3 unknown etiology.
9.Qualitative research on experiential teaching in the core midwifery curriculum

Objective To understand the practice of experiential teaching in the core midwifery curriculum. Methods Phenomenology as a qualitative research method was utilized and fifteen students participated in the non-structured interviews. Results Four themes were found: comprehension of knowledge;professional thought; teacher- student relationship;gratitude. Conclusions Experiential teaching can increase the teaching effect of core midwifery curriculum, and develop good ideological and professional qualities.

Objective To explore the effect of 25% insulin lispro( Mix 75/25) intensive treatment on plasma glucose and pancreatic function in newly diagnosed type 2 diabets. Methods Sixty patients with newly diagnosed type 2 diabetes were treated by 2 months 25% insulin lispro(Mix 75/25) treatment. The level of fasting plasma glucose (FPG) ,2 hours postprandial glucose (2hPG), glycosylated hemoglobin A1c (HbA1c), the fasting insulin and Cpeptide, insulin resistance index (HOMA-A), insulin secretion index (HOMA-B), insulin sensitivity index (ISI) and the ratio of fasting insulin to fasting glucose(FINS/FPG) were compared before and after 25% insulin lispro(Mix 75/25) treatment. Results After the treatment,FPG,2hPG,HbA1c and HOMA-A were significantly decreased( all P<0. 01 ) ;the fasting insulin and C-peptide,FINS/FPG, HOMA-B and ISI greatly increased( all P<0. 01 ). Conclusion 25% insulin lispro( Mix 75/25) intensive treatment can effectively improve plasma glucose and pancreatic function in newly diagnosed type 2 diabetes.
