1.Observation on the clinical efficacy of neoadjuvant chemotherapy in the treatment of patients with advanced epithelial ovarian cancer

Objective To explore the clinical efficacy of neoadjuvant chemotherapy on advanced epithelial ovarian cancer.Methods From May 2006 to March 2010,60 patients with Ⅲ-Ⅳ stage advanced epithelial ovarian cancer were collected in Affiliated Cancer Hospital of Shanxi Medical University,which were divided into two groups based on different treatment methods.The observation group used the neoadjuvant chemotherapy,but the control group did not use it.The efficacy,operation time,blood loss,size of residual disease and the level of CA125 were observed to evaluate the safety and clinical efficacy of neoadjuvant chemotherapy.Results The total effective rate in the observation group was 60.0%.The satisfied cytoreductive rate was 70.0%,which was significantly higher than the control group (x2 =5.455,P <0.05).The mean operative time (127.21 ±27.62) min (t =10.113,P < 0.05),intraoperative mean blood loss (408.5 ± 112.62) ml (t =5.047,P < 0.05)and the level of CA125 (165.26 ±43.27) U/ml (t =14.100,P < 0.05) in the observation group were significantly lower than the control group [189.33 ± 45.81) min,(590.6 ± 162.41) ml,(339.13 ± 51.86) U/ml].Five year survival rates of the observation group and control group were 36.7% and 26.7% respectively,with significant difference (x2 =8.492,P =0.036).Conclusion Neoadjuvant chemotherapy can play a positive role in the treatment of the patients with advanced epithelial ovarian cancer.
2.Equivalent Lever Principle of Ossicular Chain and Amplitude Reduction Effect of Internal Ear Lymph.

This paper makes persuasive demonstrations on some problems about the human ear sound transmission principle in existing physiological textbooks and reference books, and puts forward the authors' view to make up for its literature. Exerting the knowledge of lever in physics and the acoustics theory, we come up with an equivalent simplified model of manubrium mallei which is to meet the requirements as the long arm of the lever. We also set up an equivalent simplified model of ossicular chain--a combination of levers of ossicular chain. We disassemble the model into two simple levers, and make full analysis and demonstration on them. Through the calculation and comparison of displacement amplitudes in both external auditory canal air and internal ear lymph, we may draw a conclusion that the key reason, which the sound displacement amplitude is to be decreased to adapt to the endurance limit of the basement membrane, is that the density and sound speed in lymph is much higher than those in the air.

3.Optimizing Two Antiplatelet Regimens Based on the CYP2C19 Gene Detection

Objective To select antiplatelet regimen according to the results of CYP2C19 gene polymorphism,and then compare the major adverse cardiac events,bleeding events and the incidence of adverse reactions between two antiplatelet regimens. Methods Two hundred and seven patients who were diagnosed with acute coronary syndrome(ACS)and underwent elective PCI were tested for CYP2C19 genetic polymorphism,and 94 cases with CYP2C19 intermediate metabolism were randomly divided into high-dose clopidogrel group and ticagrelor group(47 cases each).High-dose clopidogrel group was given clopidogrel 150 mg once daily,and ticagrelor group ticagrelor 180 mg twice daily.Major adverse cardiac events,bleeding events and the incidence of adverse reactions were observed between two groups one month later. Results The average declined platelet aggregation rate was significantly different between the two groups[(6.27±5.65)% and(12.30±10.23)%,P<0.01];Adverse drug reactions ,the incidence of bleeding events and major adverse cardiac events of two groups were not significantly different. Conclusion Ticagrelor has stronger antiplatelet aggregation effects than high-dose clopidogrel.There is no difference in short-term clinical outcomes between the two groups.

This article reviews the relationship between SCD and neurobiological mechanism of migraine,particular expounding the significance in the gene mutations of familial hemiplegic migraine, CSD in migraine pathophysiological mechanisms and their prophylactic treatment.
5.Effect of aminoguanidine and tienam on the bacterial translocation in mice with acute necrotizing pancreatitis

Objective To investigate the effect of aminoguanidine and tienam on bacterial translocation in mice with acute necrotizing pancreatitis (ANP) and the preventive and curative effect on pancreatic infection. Methods 50 SD mice were randomly divided into 5 groups, including group A: normal control (n=10), group B: ANP group (n=10), group C: aminoguanidine treatment group (n=10), group D: tienam treatment group (n=10), and group E: aminoguanidine plus tienam treatment group (n=10). ANP was induced by intrapancreatic injection of 5% sodium tanrocholate (2.5 ml/kg). Aminoguanidine (100 mg/kg) was injected intraperitioneally 30 minutes after ANP induction, tienam (60 mg/kg) was injected intraperitioneally 6 hours after ANP induction. The mice were killed at 48 hours, and serum amylase, serum D- lactate, pancreatic MPO were measured. Pathological alterations in the pancreas were examed. The pancreas, liver, blood, mesenteric lymph node and ascites were collected for microbiological study. Results In group C and E, the levels of serum amylase were 91173.30±199.73) U/L and (1075.00±200.40) U/L; the serum D-lactate were (7.165±1.2533) μg/ml and (6.980±1.060)μg./ml; the pancreatic MPO were (0.8035±0.0711) U/g wet film and (0.7765±0.0843 ) U/g wet film; the average bacterial positive rates were 20% and 16%. In group B, the serum amylase was (2234.60±692.06 )U/L;the serum D-lactate was (12.408±1.779)μg,/ml, the pancreatic MPO was (1.5942±0.1965) U/g wet film; the average bacterial positive rate were 60%. The differences between group C, E and group B were statistically significant (P< 0.05). In group D, the pancreatic MPO was (0.8002±0.0603 ) U/g wet film and the the average bacterial positive rate was 18%, and they were statistically different when compared with those of group B (P±0.05). While in group D, the serum amylase and D-lactate was not statistically different when compared with those of group B (P>0.05). There were lamellar necrosis in the parenchyma, hyperaemia and leukocytic infiltrate in the pancreatic mesenchyma in the ANP group, while no leukocytic infiltrating in group C, D, E was found. Conclusions The aminoguanidine and tienam might decrease gut bacterial translocation and prevent pancreatic infection in ANP.

8.Application of SMART principle in the management of nursing research

Objective To study the effect of SMART principle on the management of nursing research in hospitals. Methods The SMART principle was used in the management of nursing research,including the development,specification and verification of research goals as well as supervision of the process.The number of nurses having published research papers and the number of published papers were calculated during November 2008 to November 2011. Result The number of published papers went down but the number of scientific research papers went up in the year of 2003.Conclusion The SMART principle is effective for promoting the quality of nursing research.
9.The efficacy and clinical application value of endoscopic transpapillary cyst drainage on the huge pancreatic pseudocysts and pancreatic abscess

Objective To evaluate the efficacy and clinical application value of endoscopic transpapillary cyst drainage(ETCD)on huge pancreatic pseudocysts(HPP)and pancreatic abscess.Methods According to unification standard from 2003 Jun to 2006 Jun,to select 10 disquisitive cases,in which patients with uninfected HPP were 8 cases and that with infected HPP was 1 case,and pancreatic abscess was 1 case.ETCD was adopted in all cases.The clinical treatment process and the change of relative investigation subjects were retrospectively studied.Results Endoscopic transpapillary treatment failed in 1 case,postoperative infection of HPP occurred in 2 cases and acute pancreatitis was found in 1 case.Mean follow-up was 16.5 months in 8 cases.Finally,four patients had complete resolution of HPP,3 cases had partial resolution,and the remains cavity of HPP was formed in 1 caes.Acute grave pancreatitis were cured in 2 cases and Chronic pancreatitis had resolution in 6 cases.Conclusion ETCD was safe and efficient in HPP and pancreatic abscess.Postoperative infection of HPP was the familiar complication,and obstruction of drainage path of HPP was major cause.So the crux of complications precaution and treatment was that the drainage path of HPP was unhindered.
10.Changes of whole hemocyte parameters and hemorrheologic indexes after treatment in patients with cerebral infarction

BACKGROUND: The abnormal changes of hemorrheologic indexes are closely correlated with vascular injury, disorder of microcirculation, thrombosis and its development.OBJECTIVE: To investigate the changes of 25 indexes of hemorheology and whole hemocytes in patients with acute cerebral infarction.DESIGN: A controlled case study.SETTING: Department of Medical Laboratory, Xinxiang Medical College.PARTICIPANTS: Cerebral infarction group: Totally 127 patients with the first attack of acute cerebral infarction were selected from the Department of Neurology, the Second Affiliated Hospital of Xinxiang Medical College from January to December in 2002. The onsetof disease ranged from 4 to 12 hours, and the lesions were confirmed by cranial CT, the diagnostic standard accorded with the diagnostic guidelines for various cerebrovascular diseases formulated by the Fourth National Academic Conference for Cerebrovascular Diseases. Normal control group: Fifty-three healthy physical examinees with normal blood pressure and without diseases of heart,brain and kidney, as well as diabetes mellitus. There were no sigmficant differences in ages and male to femaleratios between the two groups.METHODS: ① Patients in the cerebral infarction group received fibrinogen reduction therapy, they were injected with two pieces of 10 u defibrin on the day of admission, and injected with one piece of 10 u defibrin again on the 3rd, 5th and 7th days respectively. ② Fasting venous blood (5 mL) was drawn with thromboliquine anticoagulated vacuum blood collecting tube,and 2 mL blood was coagulated with EDTA-K2. The whole blood viscosity,whole blood high-sheafing reduction viscosity, whole blood low-shearing reduction viscosity, whole blood high-sheafing relative index, whole blood lowsheafing relative index, equation K value of erythrocyte sedimentation, plasma viscosity, hematocrit, erythrocyte sedimentation, aggregation index of red blood cells (RBC), rigidity index of RBC, deformation index of RBC, counts of while blood cells (WBC) and RBC, mean volume of RBC, mean corpuscular volume (MCV), RBC distribution width (RDW), content of hemoglobin,mean corpuscular hemoglobin content (MCH), mean corpuscular hemoglobin concentration (MCHC), platelet count, mean platelet volume (MPV) and platelet distribution width (PDW) were detected when the shear rate was 200 s-1, 30 s-1, 5 s-1 and 1 s-1 respectively. ③ The above indexes of the patients with cerebral infarction were detected within 24 hours after attack (exacerbation) and at two weeks after treatment (recovery period) respectively.MAIN OUTCOME MEASURES: The hemorrheologic and whole hemocyte indexes were compared between the two groups.RESULTS: All of the 127 cases in the cerebral infarction group entered the analysis of results at exacerbation, and 5 of them were not reexamined because of discharge and 122 cases entered the analysis of results at recovery period. All the 53 cases in the normal control group were involved in the analysis of results. ① Except RDW and MCHC, all the other indexes in the patients with cerebral infarction at exacerbation were significantly different from those in the normal control group (P<0.05, 0.01). As compared with the normal control group, the whole blood low-shearing viscosity, whole blood low-shearing reduction viscosity, whole blood high-sheafing relative index, whole blood low-shearing relative index, plasma viscosity,hematocrit, aggregation index of RBC, WBC count and MCH all returned to normal in the patients with cerebral infarction at recovery period (P>0.05).② As compared with the exacerbation, the RBC count and MPV were obviously decreased in the patients with cerebral infarction at recovery period (P < 0.05), while the other indexes were similar (P > 0.05).CONCLUSION: Abnormalities of hemorrheologic indexes commonly exist in patients with acute cerebral infarction, which indicates that the abnormalities of hemorrheologic and whole hemocyte parameters may be related to the occurrence of acute cerebral infarction.
