1.Optimal therapy of advanced Hodgkin' s lymphoma

Advanced-stage Hodgkin's lymphoma (HL) has become a curable disease in the majority of patients.ABVD is considered to be the standard therapy,but debates continue regarding the role of radiation therapy(RT)in this patient population. The incorporation of interim positron emission tomography (PET) imaging, and characterization of HL on cellular and molecular levels are emerging as tools for treatment stratification and predictors of disease status. Therapeutic advances over the past 3 decades have resulted in the cure of the majority of the patients with advanced-stage HL.Several questions have emerged when considering what constitutes optimal therapy with a balance between a high cure rate and minimizing shortand long-term toxicity.This review focuses on 3 key elements:what is the optimal chemotherapy? What is the role of radiation therapy(RT)in advanced HL? Can therapy based on clinical biological risk factors be adapted?
2.Effect of follicle stimulating hormone on the secretion of anti-mullerian hormone in the granulosa cells in patients with polycystic ovarian syndrome

Objective To study the effect of follicle stimulating hormone(FSH)on the secretion of anti-mullerian hormone(AMH)in granulosa cells in patients with polycystic ovarian syndrome(PCOS).Methods From Aug.2008 to Dec.2009,33 patients with PCOS in Reproductive Medicine Research Center,Sixth Affiliated Hospital of Sun Yat-sen University were collected from 8-10 mm antral follicles and classified into the following three groups: FSH-unstimulated granulosa cells(unstimulated group,n =12),exogenous FSH-stimulated granulosa cells(exo-stimulated group,n =12)and internal FSH-stimulated granulosa cells(int-stimulated group,n =21).ELISA and real-time PCR were relatively used to measure protein and mRNA level of AMH.Luciferase reporting vector was constructed to detect the promoter activity of AMH.Results The levels of AMH secretion in PCOS granulosa cells were(11.4 ± 4.0)μg/L in unstimulated group,(7.9 ± 1.1)μg/L in exo-stimulated group and(5.6 ± 1.7)μg/L in int-stimulated group.Both the external and internal stimulation of FSH may suppress AMH secretion significantly(P <0.05).The mRNA level of AMH in PCOS granulosa cells were 2.5 ± 1.2 in unstimulated group,which were higher than 1.5 ± 0.5 in exo-stimulated group and 1.1 ± 0.7 in int-stimulated group significantly(P <0.05).The luciferase activity of AMH in PCOS granulosa cells were 11.5 ± 2.3 in unstimulated group,8.7 ± 2.4 in exo-stimulated group and 6.8 ± 2.4 in int-stimulated group.The luciferase activity of AMH in unstimulated group was significantly higher than those in exo-stimulated and int-stimulated groups(P <0.05).Conclusion FSH may inhibit the excessive secretion of AMH and stimulate follicle growth in PCOS granulosa cells by suppressing activity and expression of promoter.
3.Clinical Characteristics and Management of Choriocarcinoma in Postmenopausal Women

Objective To investigate the clinical characteristics,management and prognosis of choriocarcinorna in postmenopausal women. Methods Six cases of choriocarcinoma in postmenopausal women were reviewed retrospectively. Results Four cases in the 6 patients presented with abnormal vaginal bleeding. All of the six cases received chemotherapy and four cases underwent hysterectomy. Three cases achieved complete remission,one partial remission,one stable condition,and one disease progression.One case died,two cases lost follow-up and three cases survived as follow-up so far. Conclusion The diagnosis of pregnancy-related disease should be considered in the elderly women presenting with abnormal vaginal bleeding.Once diagnosed,chemotherapy should be given as soon as possible. Satisfactory effects can be achieved with a combination therapy of chemotherapy and surgery.
4.Preparation and in vitro Transdermal Study of Zolmitriptan-Diclofenac Microemulsion

OBJECTIVE:To prepare zolmitriptan-diclofenac microemulsion,and conduct quality evaluation and in vitro trans-dermal study. METHODS:Using solubility and microemulsion area in pseudo-ternary phase diagram as indexes,the types of oil phase and mixed emulsifier ratio of zolmitriptan-diclofenac microemulsion were screened;the microemulsion quality was inspected using particle size,Zeta potential,appearance and stability. HPLC was used to measure the contents of zolmitriptan and diclofenac. Transdermal diffusion test instrument was used,2 g microemulsion was smeared in cuticle of extracouporeal rats'skin,and cumula-tive transdermal rate in 24 h was determined. RESULTS:The microemulsion formulation was as follow as 10% oil phase(octanoic acid triglyceride),25% mixture emulgator [polysorbate 80-brij 97 (1:1)],8.3% propylene glycol and 25 mg zolmitriptan,1.25 mg diclofenac,and water adding to 100 mL. The average particle size of prepared microemulsion was(28.2±2.5)nm,Zeta poten-tial was(-3.25±0.33)mV,the appearance was rounding;the microemulsion showed no stratification or flocculation at room tem-perature after placed for 1 month. Contents of zolmitriptan and diclofenac were 0.248 mg/mL,12.46 mg/mL(n=3);24 h cumula-tive transdermal rates were 80%,75%. CONCLUSIONS:Zolmitriptan-diclofenac microemulsion is prepared,and its in vitro trans-dermal ability is good.
5.Anatomical variation in the circle of Willis and cerebral ischemia

The circle of Willis is the major cerebral collateral circulation.Its anatomical variation is in populations higher.The previous studies showed that anatomical variation in circle of Willis was an independent risk factor for cerebral ischemia.This article reviews the relationship between the various anatomical variation types of the circle of Willis and cerebral ischemia.
6.Analysis of the Curative Effect of Interventional Therapy for Cervical Cancer

Objective To evaluate the effect of operation associated with uterine arerial chemotherapy and embolization preoperation.Methods 31 patients with pathologically confirmed cervical carcinoma were treated with interventional therapy,followed by uterectomyandpelvic lymphadenectomy.Results Curative effect was obvious.The lymph metastasis of Ⅱ and Ⅲ stage was 12% and 66.67%respectively.27 patients followedup for 3 years,23 were survived(85.19%).Conclusion For cervical carcinoma,before operation,the interventional therapy can reduce the size of tumor and the metastasis of lymph,improve the ectomy by operation and the survival rate.
7.Investigation on Contents of Antibiotics and Antiseptics Added in Cosmetics

Objective To study the contents of antibiotics and antiseptics in the cosmetics sold in markets. Methods The cosmetics sold in professional market and current market (in which cosmetics were retailed) were selected respectively in Guangzhou and the contents of four antibiotics (oxytetracycline, tetracycline, chlortetracycline and chloramphenicol) and five antiseptics (para-hydroxybenzcic acid, methyl para-hydroxybenzoate, ethyl para-hydroxybenzoate, propyl para-hydroxybenzoate and butyl para-hydroxybenzoate) were determined by high performance liquid chromatogram. Results In 111 cosmetics, 30 cosmetics were detected containing oxytetracycline, 62 cosmetics containing tetracycline, 7 cosmetics containing chlortetracycline and 1 cosmetic containing chloramphenicol. The detection rate was 78.4% and the detection rate of antibiotics among various kinds of cosmetics had no significant difference. The antiseptics used in cosmetics were mostly para-hydroxyl compound ester and there were four cosmetics detected containing antiseptics. Conclusion Abusing antibiotics in cosmetics market is very serious. The test standards and methods for antibiotics and antiseptics in the cosmetics should be developed as soon as possible. Cosmetic enterprises need to decrease microbe pollution in the process of production and prohibit adding antibiotics in cosmetics.
8.Application of self-made quality control materials on chemiluminescence analyzer

Objective To investigate the feasibility of self-made quality control materials on chemiluminescence analyzer .Meth-ods Residual serum in blood bags were collected and served as matrixes .Complex quality control materials of tumor markers [car-cinoembryonic antigen ,alpha-fetoprotein ,carbohydrate antigen 125(CA125) ,CA199 ,CA153 ,human chorionic gonadotrophin and total prostate specific antigen ] and hormonal markers (luteinizing hormone ,follicle-stimulating hormone ,estradiol ,testosterone , progesterone and prolactin) were prepared .The monthly average value ,coefficient of variation(CV)% value ,cumulative average value and the number of out-of-control of each marker during the 12 months were calculated .Results All self-made quality control serum which stored and packaged separately showed appearance of clear and transparent after freezing and thawing ,without turbidi-ty and sedimentation .Difference of monthly average value and cumulative average value of each marker had no statistical significance (P>0 .05) .Compared with monthly average value and cumulative average value of each marker ,no obvious drift and trends were found .Monthly indoor imprecision of each hormonal marker were controlled within the allowable range .36 times of out-of-control alarm were monitored during a year ,among them ,25 for system error and 11 for random error .Conclusion Self-made quality con-trol materials has good stability and may find the instability factors of detection system timely to guarantee the reliability of test re-sults of chemiluminescence analyzer .
9.Association between cytochrome P450 3A4 gene 894C>T single nucleotide polymorphism and clopidogrel resistance

T was genotyped by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)in all the patients.Results The occurrence of CR in this population was 23.3%(70/300).There was CYP3A4 894C/T polymorphism in the study population.The frequencies of the three kinds of genotypes(CC,CT,TT)in CR group and non-CR(NCR)group were 45.7%,50.0%,4.3% and 63.5%,31.7%,4.8%,respectively.The frequency of TT genotype was significantly higher in NCR group than that in CR group(OR=2.06,95% CI:1.201-3.547,P=0.020).C allele carriers were more likely to develop clopidogrel resistance compared with that of T allele carriers(OR=1.59,95% CI:1.037-2.442,P=0.023).Conclusion CYP3A4 gene 894C/T polymorphism is associated with the risk of CR,and C allele carriers may be a possible genetic susceptibility factor for patients with CR.
10.Comparative study on particle pollution to the solution in the ampoule wrapped and broken by non-woven fabrics and medical gauze

Objective To compare the particle pollution to the solution in the ampoule wrapped and broken by non-woven fabrics and medical gauze.Methods Totally 200 pcs of sterile water for injection(2 ml/pc) were randomly divided into the experimental group and the control group,with 100 pcs for each.For the experimental group,the ampoules were wrapped and broken by disinfected non-woven fabrics.For the control group,the ampoules were wrapped and broken by medical gauze.The solution was sucked from the ampoule with a 10 ml syringe and injected into a measuring cup for testing.Results The quantity of particles in the solution in the experimental group was less than that in the control group,and the difference was statistically significant (P<0.05).Conclusion Wrapping and breaking the ampoule by disinfected non-woven fabrics can reduce particle pollution to the solution,and this method is easy to practice.
