1.A study on the relationship of the activity of aryl hydrocarbon receptor and aryl hydrocarbon ;metabolic enzymes in peripheral blood and breast cancer

Background and purpose:Previous studies showed activated aryl hydrocarbon receptor (AhR) might be involved in the development of breast cancer induced by polycyclic aromatic hydrocarbons (PHAs) and AhR which was regarded as transcription factors activated by ligand could induced many enzymes expression. Therefore, this study aimed to explore the protein level of AhR and aryl hydrocarbon metabolic enzymes in vivo with a relationship of the breast cancer and pathology indicators. Methods:A case-control study was 1∶1 matched and 65 pairs blood samples were collected, the ELISA was used in testing blood protein levels of the aromatic hydrocarbon receptor and aryl hydrocarbon metabolic enzymes. Meanwhile pathological data was collected. The Rank sum test statistical analysis was used in analyzing measurement data, Spearman test was used in correlation analysis. Results:Statistical analysis revealed that there was no statistical difference in aldehyde dehydrogenase(ALDH), cytochrome P450(CYP450) between the case group and the control group (Z=0.196,P=0.845;Z=0.269,P=0.788);and the AhR, NQO1, GST control group concentrations were lower than that in the case group. There were statistical differences of AhR, quinone oxido-ereductase 1(NQO1), glutatnione-S-transferases(GST) between the control group and the case group(Z=1.956, P=0.041;Z=2.627, P=0.009;Z=3.272, P=0.001). Correlation analysis showed that AhR has a higher correlation in case group with GST, NQO1 (r=0.665, P<0.01;r=0.704, P<0.01). Grouping with the pathology indicators (ER, PR, HER-2)-,+,++,+++, found that the level of grouping by ER, AhR protein levels and GST protein levels have a discrepancies(P<0.05). Conclusion:These results might suggest that the protein levels of AhR and aryl hydrocarbon metabolic enzymes have a signiifcant impact on breast cancer. Detection of enzyme protein levels may be helpful for early detection of breast cancer, prompt treatment options and prognosis are meaningful.
2.Effects of low dose X-ray irradiation on proliferation and differentiation of mesenchymal stem cells derived from human umbilical cord into adipocytes and osteoblasts

Objective To observe the effects of low dose irradiation (LDR) on proliferation,adipogenesis and osteogenic potential of human umbilical cord mesenchymal stem cells (hucMSCs).Methods hucMSCs were isolated from Wharton's jelly tissue of human umbilical cord by modified tissuepiece inoculation,and flow cytometry was used to detect the expression of specific marker in the hucMSCs.The hucMSCs were randomly divided into two groups:irradiation group undergoing irradiation with the doses 50,100,or 200 mGy respectively,and control group without irradiation.MTT method was applied to evaluate the proliferation of the hucMSCs at different time points with various doses irradiation.The third passage hucMSCs were randomly divided into two groups:irradiation group undergoing low dose irradiation of 200 mGy,and control group without irradiation,and then underwent induction by adipocytic and oesteocytic differentiation induction fluids respectively so as to differentiate into adipocytes and osteoblasts.Oil red O staining was used to detect the activity of alkaline phophatase (ALP),and RT-PCR was used to detect the mRNA expression of core binding factor alpha 1 in human osteoblast.Results After 9-12days,fibroblasts began to swim out of the tissue piece with a confluence rate of 80% 2 weeks later.Within 7 days the absorption values of the hucMSCs undergoing different irradiation doses 2,3,4,5,and 6 days later were all significantly higher than those of the control group(F = 159.17,448.81,265.15,183.93,and 181.83 ,all P <0.01),with the proliferation rates of the 100 mGy subgroup being the highest.After being induced liquid,vacuoles were observed in the irradiated group 2 days later.21 days later,the adipogenic rates of irradiated group was significantly higher than that of the control group (t = 28.25,P <0.01).The ALP activity increased in the irradiated group compared with control group (t=16.87,P <0.01) .The expression level of Cbf-α1 mRNA was up-regulated obviously (t = 14.16,P<0.01).Conclusions LDR promotes the proliferation of hucMSCs,and accelerates the hucMSCs' differentiation into adipocytes and osteoblasts.
3.The clinicopathological analysis of 88 patients with abnormal liver function test of unknown etiology

Objective To evaluate the clinical and histological features of patients with abnormal liver tests of unknown etiology, and then to investigate the diagnosis and differential diagnosis. Methods Patients with abnormal liver function test hospitalized and had liver biopsies during 2008-2009 constituted this retrospective study cohort. After excluding those patients diagnosed with hepatotropic viral hepatitis,space occupying lesions of the liver, alcoholic liver disease and obstruction of bile duct caused by stone or malignancy and AMA/AMA-M2 positive of primary biliary cirrhosis ( PBC ), the clinical and histological characteristics were evaluated. Results Out of the 180 patients who underwent liver biopsy, 88 patients were included in the present analysis. The final diagnosis involved 15 categories of diseases, with druginduced liver injury ( DILI ) [34. 09% ( 30/88 )], autoimmune liver diseases [22.73% ( 20/88 )], and nonalcoholic fatty liver disease (NAFLD) [12. 50% ( 11/88 )] being the most common causes, following by genetic and other rare diseases. Conclusion DILI, autoimmune liver disease and NAFLD were the most common causes of abnormal liver tests in these non-viral liver diseases. Some rare diseases such as hereditary metalbolic liver disease also represent a considerable proportion in patients with abnormal liver function test.
4.Survey on hospital beds utilization of rural and urban community health service centers in Shanghai

Objective To survey on utilization of hospital beds in rural and urban community health centers in Shanghai.Methods The hospital beds utilization was investigated in one rural and one urban community health service centers in Shanghai.The data of hospitalized patients in a selected day were surveyed with self-made questionnaire,including the demographic information of patients,the diseases category,the length of hospital stay,self-care ability of daily life (ADL score),the purpose of hospitalization,and the management after discharge.Results Patients aged over 60 y accounted for 100.0% (138/138) in urban center and 98.7% (76/77)in rural center.The three top disease categories were all cardio or cerebrovascular diseases in urban center accounting for 86.9% (120/138),while those in rural center were hypertension,cerebral infarction and acute/chronic bronchitis (or tumor)accounting for 65.0% (50/77).The mean length of hospital stay in urban and rural centers were 609.6 d and 253.8 d,respectively (F =2 604.00,P =0.000).Patients with severe dysfunction in urban and rural centers accounted for the 84.0% (116/138) and 32.5% (35/77),respectively (x2 =80.911,P=0.000).Patients not willing to be discharged in urban and rural centers accounted for 87.7%(121/138)and57.1% (44/77),respectively in city and rural centers(P <0.05).Conclusions The wards in community health centers mainly serve the elderly patients.There are differences in purpose of hospitalization,length of hospital stay,ADL scores of patients between rural and urban community health centers.
5.Helicobacter pylori combined with MNU gavage for preparing balb/c mouse gastric cancer model

Objective To investigate Helicobacter pylori combined with N-methyl-N-nitrosourea (MNU) gavage for preparing balb/c mouse gastric cancer model.Methods Eighty balb/c mice were randomly divided into four groups after 1-week adaptive feed,normal group,model group Ⅰ,Ⅱ and Ⅲ,20 cases in each group.The model group Ⅰ,Ⅱ and Ⅲ were given Helicobacter pylori bacteria liquid (CFU=109/mL) gavage,once every other day for 5 times;then,the freshly configured MNU solution 0.15,0.3,0.6 mL gavages were in turn given,MNU and pure water allocation ratio was 5mg:3mL.Once gavage per week for continuous 10 weeks.Results The model group II had 66.67% adenocarcinoma,the model group I were gastritis with mild atypical hyperplasia,and all mice in the model group III died.Conclusion This method can prepare the gastric cancer model.
6.Effect of Cognitive Therapy and Acupuncture on Insomnia

ObjectiveTo observe the effect of cognitive therapy and acupuncture on insomnia. Methods72 insomnia sufferers were divided into two groups averagely. 36 sufferers were treated with cognitive therapy and acupuncture (treatment group), and other 36 sufferers were treated with acupuncture (control group) only.ResultsA month after treatment, ,15 sufferers were cured, 18 sufferers were obviously improved, 2 sufferers were improved and one did not show any improvement in the treatment group.Otherwise, 6 sufferers were cured, 12 sufferers were obviously improved, 12 sufferers were improved and 6 did not show any improvement in the control group. (P<0.05)ConclusionCognitive therapy and acupuncture used together shows more efficacy on insomnia compared with acupuncture alone.
7.Survey on knowledge, attitude and practice about hypertention in elderly patients from communities of Shanghai Xuhui District

Objective To survey on the knowledge,attitude and practice about hypertention in elderly patients and its relation to blood pressure control.Methods A sampling survey on the knowledge,atitude and practice about hypertention were carried out in a target population of 47 572 elderly hypertensive patients from 5 communities in Xuhui District of Shanghai during May 2011 to July 2011.Total 3800 questionairres were distributed and 3328 responded with a response rate of 87.5%.Results Among 3328 responders,3036 were aged above 65 years with a mean age of (76 ±7) years and 43.3% were males.The rates of knowledge about hypertension were 88.5%-97.8%.General practitioners in out-patient service were the major source of knowledge.Raults revealed that 81.9% had received health education before;81.8% requied a regular health education; 67.2% had self-tested blood pressure weekly; 89.6% controlled salt intake; 66.8% attended outdoor activities weekly with (5.6 ±2.1) times a week and (45 ±28) min each time.The blood pressure control rate in uncomplicated hypertensive patients was higher than those complicated with diabetes,coronary heart disease or stroke (62.3 % vs.17.0%,P < 0.01).Knowing high blood pressure was related to salt intake (OR =3.364,P < 0.01),hoping a regular health education about hypertension (OR =1.718,P < 0.01) were factors contributing to blood pressure control in uncomplicated hypertensive patients; while self-testing blood pressure once or twice a month(OR =2.572,P < 0.01)was contributing factor to blood pressure control in complicated hypertensive patients.Conclusions Elderly hypertensive patients from communities in Shanghai have enough knowledge,positive attitude but lack practices.General practitioners should carry out personalized and stronger health education focued on behavioral changes for those patients.
8.Effects of apolipoprotein A-Ⅰon ATP binding cassette transporter A1 degradation and cholesterol efflux in THP-1 macrophage-derived foam cells

AIM: To study the effects of apolipoprotein (apo) A-Ⅰon ATP binding cassette transporter A1 (ABCA1) degradation and cholesterol efflux in THP-1 macrophage-derived foam cells. METHODS: After exposure of the cultured THP-1 macrophage-derived foam cells to apolipoprotein A-Ⅰ for different time, cholesterol efflux, ABCA1 mRNA and protein level were determined by liquid scintillation counting, reverse transcriptase-polymerase chain reaction and Western blotting, respectively. The mean ABCA1 fluorescence intensity on THP-1 macrophage-derived foam cells was detected by flow cytometry. RESULTS: ApoA-Ⅰ markedly increased ABCA1-mediated cholesterol efflux from THP-1 macrophage-derived foam cells. This was accompanied by an increase in the content of ABCA1. ApoA-Ⅰ did not alter ABCA1 mRNA abundance. Thiol protease inhibitors increased the level of ABCA1 protein and slowed its decay in THP-1 macrophage-derived foam cells, whereas none of the proteosome-specific inhibitor lactacystin, other protease inhibitors, or the lysosomal inhibitor NH_4Cl showed such effects. The apoA-Ⅰ mediated cellular cholesterol efflux was enhanced by thiol protease inhibitors. CONCLUSION: Thiol protease inhibitors might provide an alternative way to upregulate ABCA1 protein. This strategy is especially appealing since it may mimic the stabilizing effect of the natural ligands apoA-Ⅰ.
9.Effect of mobile network platform in out-of-hospital rehabilitation of patients with total knee arthroplasty: a Meta-analysis

Objective:To evaluate the effect of mobile network platform in out-of-hospital rehabilitation of patients with total knee arthroplasty.Methods:CNKI, Wanfang, VIP, Chinese biomedical literature database, PubMed, Cochrane Library, EMbase and Web of Science database were searched by computer. To collect randomized controlled trials of out-of-hospital rehabilitation management for knee arthroplasty patients using mobile network platform. Two researchers independently screened literature, extracted data and evaluated quality according to inclusion exclusion criteria,and Stata 12.0 software was used for meta-analysis of the required documents.Results:A total of 10 articles were included for meta-analysis. A total of 933 patients were included, 471 in the intervention group and 462 in the control group. Meta-analysis showed that the knee function of patients with total knee arthroplasty managed by mobile network platform were better than those of the control group, and the difference was statistically significant(standardized mean difference( SMD) value was 1.25, 95% confidence interval( CI) 0.84-1.66, P<0.01), but the effect on improving range of motion ( SMD value was 0.29, 95% CI -0.30-0.87, P>0.1) and quality of life( SMD value was 0.90, 95% CI -0.25-2.05, P>0.1) was no difference. Conclusions:There is no obvious effect on the range of motion of the knees and quality of life, while to some extent, the application of mobile network platform in out-hospital rehabilitation management of total knee replacement patients can effectively improve the function of knees. It potentially speed up patients′rehabilitation process.
10.Brushing abrasion of the enamel surface after erosion.

Objective A study was conducted to compare the effect of different enamel remineralization periods after erosion on the depth of brushing abrasion. Methods Ten volunteers were selected for a 4-day experiment. A total of 60 enamels were randomly assigned into six groups (A-F) and placed in intraoral palatal devices. On the first day, the palatal devices were placed in oral cavity (24 h) . On the following three days, brushing experiments were performed extraorally, two times per day. The specific experimental method of brushing follows these next steps. First, the group F specimens were covered with a film of wax, and then acid etched for 2 min. Subsequently, the film of wax was detached. The groups from A to D were brushed after remineralization at the following time intervals: group A, 0 min; group B, 20 min; group C, 40 min; group D, 60 min. Erosion and remineralization were performed on group E, but without brushing. Remineralization was performed on group F, but without acid etching and brushing. The depth of enamel abrasion was determined by a mechanical profilometer. The surface morphology of the enamel blocks was observed using a scanning electron microscope. Results 1) The depth of abrasion was different in varied enamel remineralization time after acid etching. The statistical significant differences between groups were as follows. 2) When the time of enamel remineralization after acid etching was short, the surface depression in the electron microscope was deep, and the surface morphology was rough. Conclusion Brushing immediately after acid etching would cause much serious abrasion to the enamel surface. Brushing after 60 min can effectively reduce the abrasion of acid etching enamel.
