1.Thinking on the Development of Private Hospitals in Shanghai

Private Hospitals are lawful medical institutions which are responsible for their own management decisions,profits and losses.They aim at making profits but also pay much attention to the unification of social benefit and economic benefit.Taking the Shanghai Private Hospitals as an example,by data collection,questionnaire survey and interviews from general public and hospital administrators,it is concluded the overall development of Shanghai Private Hospitals is far from satisfaction.Private Hospitals must define their medical concepts and market orientation for long-term development,and strengthen the monitoring system and attain public′s trust for their healthy development.Since China has a huge demand for medical service,private hospitals will surely have a bright future as long as capturing the market and holding the faith of "serving for patients,and benefiting the society".
2.Current situation and influencing factors of uncertainty in illness of family members of patients with malignant tumor

Objective To investigate uncertainty in illness of family members of patients with malignant tumor,to explore the factors that affect uncertainty in illness of family members of patients with malignant tumor,and to provide clinical basis for clinical doctors and nurses implementing effective interventions for their patients.Methods Questionnaires were conduted to investigate uncertainty in illness of family members of patients with malignant tumor and explore the related factors.The measurements included three parts,Chinese version of Mishel uncertainty in illness scale for family member,self-designed questionnaire about family general condition,patients disease information.Results The total score of uncertainty in illness was 82.71±9.93,more than 50 % (75 score) of the highest PPUS-FM score.There were statistical significances of uncertainty in illness in education (F =8.06,P < 0.05),family income (F=3.14,P < 0.05),residence place (F =3.99,P < 0.05) and frequency of hospitalization (F =4.85,P < 0.05).Conclusion Family members of patients with malignant tumor have a higher level of uncertainty in illness.Education,family income,residence place and frequency of hospitalization have a certain effect on uncertainty in illness of family members of patients with malignant tumor.

4.Preliminary Study on the Thinking Mode of Nephrology Clinical Pharmacists Reviewing Medical Orders

OBJECTIVE:To investigate the thoughts and ways of clinical pharmacist in nephrology department to review medi-cal orders,and to provide reference for rational drug use in nephrology department. METHODS:Based on specific cases of nephrolo-gy department,the thoughts and ways of clinical pharmacist in nephrology department to review medical orders were investigated from several aspects,including whether the therapeutic drugs had covered all the problems,drug duplication,drug selection,usage and dosage and course of treatments,drug interaction and compatibility. RESULTS & CONCLUSIONS:By establishing rigorous and systematic review thinking mode,based on their professional specialty,clinical pharmacists of nephrology department can sort the reviewing thought,fill in the gaps of medical orders reviewing and enhance the ability of participating in the clinical practice.

The anthor has survevyed some medical staff who are from two triple hospitals,six second hospitals and eight grass-roos hospitals in Shanghai district, about modern medical model.Through analynzing and discussing,the anthor proposed some positive countermeasures to improve medical staff's conciouness of modern medical medel and adapt to its tranformation.
7.Lactobacillus on allergic rhinitis rat models of the influence of the immune function *

Objective Explore the effect lactobacillus on allergic rhinitis (AR) rat serum Il-4 ,Il-5 ,IFN-γ concentration and the influence of immune markers .Methods SD rat 60 were randomly divided into 4 groups ,each group of 10 only .Lactic acid bacteria group every day with lactobacillus living bacterium preparations irrigation stomach 3 weeks ,establish rats LuanBai protein sensitiza-tion model ,positive drug control group irrigation stomach loratadine ;Another 20 rats used for passive cutaneous anaphylaxis (PCA) ,inspection in the rat model preparation and treatment .Determination test group rats serum IL-4 ,IL-5 ,IFN-γ,the content of Eos in nasal secretion was detection .Results Model group IL-4 ,IL-5 content was significantly higher than normal control group (P<0 .05) ,IFN-γlevels were low ,lower than the normal control group(P<0 .05) .Lactic acid bacteria group IL-4 ,IL-5 content were significantly lower than the model group(P<0 .05) ,IFN-γcontent increased ,higher than the model group(P<0 .05) .The in-dex of lactobacillus group with positive drug compared difference was statistically significant .Conclusion Lactic acid bacteria can cut AR rats serum IL-4 ,IL-5 level ,increase IFN-γlevel ,adjust Th1/Th2 lymphocyte subsets balance ,reduce nasal mucosa allergic inflammation .
8.A study of risk factors of papillary thyroid carcinoma in the elderly

Objective To study the risk factors for thyroid papillary carcinoma in patients aged over 65 years,and to investigate the etiology of thyroid papillary carcinoma.Methods A total of 784 patients with thyroid nodules screened by ultrasound were grouped according to different iodine intakes,gender,age,diet,smoking history and history of radiation therapy.The incidence of thyroid papillary carcinoma was compared among the different groups.Pathogenic factors for thyroid papillary carcinoma were analyzed.Results 55 patients (7.0%) were diagnosed as thyroid papillary carcinoma among 784 patients.Gender,age,iodine intake,smoking history and history of radiation therapy were the pathogenic factors for papillary thyroid carcinoma.Thyroid papillary carcinoma was common in older women,patients with high-iodine intake and patients with the history of radiation therapy.The incidence of thyroid papillary carcinoma was higher in patients with high iodine intake than with normal-iodine intake or low-iodine intake [22.8% (31/136) vs.1.9%% (10/517),10.7% (14/131),x2 =75.07,P<0.01].The incidence of thyroid papillary carcinoma was higher in female than in male elderly patients [8.0% (49/612) vs.3.5% (6/172),P<0.05].The incidence of thyroid papillary carcinoma was higher in patients with the history of radiation therapy than without the history of radiation [11.6% (14/121) vs.6.2% (41/663),P<0.05].Conclusions The main risk factors for thyroid papillary carcinoma are living environment,gender,age,diet,smoking history and history of radiation therapy,which are more influenced by external factors.
9.Reconstruction of LARS artificial ligament and anterior and posterior cruciate ligament sports injury of keen joint

OBJECTIVE: To investigate the effect of LARS artificial ligament on reconstruction of anterior and posterior cruciate ligament of keen joint.METHODS: Articles addressing reconstruction of anterior and posterior cruciate ligament by LARS artificial ligament were retrieved from VIP database (http://www.cqvip.com/) with the key words of "LARS artificial ligament, cruciate ligament,reconstruction" in Chinese between January 2000 and February 2010.Inclusion criteria: advantages and disadvantages of reconstructing anterior and posterior cruciate ligament by LARS artificial ligament; clinical application of reconstructing anterior and posterior cruciate ligament by LARS artificial ligament.Exclusion criteria: Duplicated studies or old references were excluded.A total of 31 articles were included in the final analysis.RESULTS: Reconstruction of anterior and posterior cruciate ligament by LARS artificial ligament characterized by convenient operation, short operative time, mild trauma, engaging in movement early, great strength, simple implantation, and reliable fixation; however, most patients were not able to bear the high medical cost, thus clinical application was limited.Scholars thought that LARS artificial ligament had great effects on reconstruction of anterior and posterior cruciate ligament.CONCLUSION: Patients who had multi-ligament injury, overhauling of ligament, and early movement were treated with LARS artificial ligament.
10.Efficacy of continuous glucose monitoring system combined with insulin pump in diabetic pafients aged 60 years and over

Objective To observe hypoglycemic efficacy of continuous glucose monitoring system (CGMS)combined with continuous insulin infusion(CSII)in the elderly diabetic patients aged 60 years and over.Methods A total 0f 100 elderly diabetic patients were randomly divided into two groups.50 were treated with CGMS or self-monitoring blood glucose(SMBG)and CSII.and whose insulin dose was adjusted based on results of monitoring,and the other 50 with SMBG and insulin pump as controls.whose insulin dose was adiusted based on the results of SMBG for three days.After two weeks.all the patients were monitored by CGMS for blood glucose.Results Mean blood level of glucose and mean amplitude of glycemic excursions(MAGE)were significantly lower in CGMS+CSII group than those in controls[(6.6±2.3)mmol/L vs.(7.5±2.1)mmol/L,and(3.9±0.9)mmol/L vs.(5.1±0.6)mmol/L,respectively,P<0.05 1.Average insulin dose used was significantly lower in CGMS+CSII group than that in controls [(0.64±0.21)U/kg vs.(0.82±0.41)U/kg,P<0.05],and duration of hypoglycemia was less in the former than that in the latter[(20±3)min vs.(40± 9)min,P<0.05 ].Conclnsions CGMS combined with CSII can decrease blood glucose.glucose excursions.average dose of insulin and duration of hypoglycemia in the elderly diabetic patients,as well as prevent occurrence of hypoglycemia.
