1.Tonsillectomy and adenoidectomy in children with obstructive sleep apnea hypopnea syndrome postoperative immune function influence.

Sleep apnea hypopnea syndrome is a common respiratory tract obstructive disease in children, which often affects children's mental and physical development. For repeated tonsil inflammation and nocturnal sleep apnea in children with OSAHS, tonsillectomy and adenoidectomy is the most effective treatment. But children's postoperative immune function remain in dispute, this review focuses on immunologic function change and influence in the OSAHS children after tonsillectomy and adenoidectomy, according to the existing research results can be confirmed. The immune parameters in OSAHS children who had tonsillectomy and adenoidectomy will vary differently after a short period of several, considering these manifestations are the stress response after operation, and its long-term immune parameters can recover to the normal level. Thus, the postoperative immune function of OSAHS with tonsillitis in children after the adenoid and tonsil resection has no negative effects, providing theory basis for clinician's treatment for clinicians to parents of children with answers to questions provide the theory basis.

2.Effect of autologous stem cell transplantation on multiple myeloma in the era of targeted novel agents

Multiple myeloma (MM) is malignant plasma cell clonal disease.Autologous stem cell transplantation (ASCT) strengthen the advantages of the high-dose chemotherapy in treatment of MM,which achieves long-term survival in some patients.Even in the era of targeted novel agents,ASCT can still obviously increase the response rate of treatment.To improve the therapeutic efficacy,targeted novel agents were administrated before and after ASCT.However,targeted novel agents still cannot replace ASCT therapy in the treatment of MM.The direction of future research is to find more reasonable,effective and low toxic treatment.

The adoption of the system of multi-technical appraisals of malpractice means that the appraisals made each time, which constitute the key basis and evidence for the health administrative departments and the judicial organs to deal with medical disputes, are of legal validity as proof. By analyzing the causes of the differences in the appraisals of certain cases, the authors argue that a correct understanding of the definition of malpractice is the key to a correct determination of it; an appropriate application of the principle of allocating the burden of proof is the key link in the technical appraisal of malpractice; perfection of Regulations on Handling Malpractice, Standards for the Classification of Malpractices, and Temporary Provisions on the Technical Appraisal of Malpractice is the basis of correct and impartial appraisals; and a good grasp of the gist of rules and regulations and appraisal according to law is the guarantee for a correct determination of the nature of malpractices.

Objective To analyze current situation on the quality of statistical data under hospital information system in order to enhance data supervision and improve data quality.Methods The quality of statistical data in the first half year of 2008 was described,and the quality of the dates that dates of clinic service,dates of patients discharged from hospital and surgical dates were analyzed.Results The measures about how to ensure the authenticity and effectiveness of the data were introduced.Conclusion The high practical value is provided by utilizing "No.1 Military Medical Project" Information System of statistical data for manager to provide decision support.
5.Suppression of the expression of hPOT1 gene in human gastric cancer cell line SGC-7901 with vector-based RNA interference technique

Objective To construct the expression vector of siRNA targeting the human protection of telomeres (hPOT1) gene for the observation of the inhibitory effect of the vector on hPOT1 gene expression in gastric cancer cell line SGC-7901, and to offer a ground work for further study on the roles of hPOT1 in growth and proliferation of gastric cancer cells. Methods 64 base-pair oligonucleotide for small interfering RNA expression targeting hPOT1 gene was designed and chemically synthesized. After annealing, double oligonucleotides were inserted into the downstream of H1 RNA promoter of pSUPER to recombine psiRNA-hPOT1 plasmids, and transiently transfected into gastric cancer cell line SGC-7901 with liposome-mediated transfection after restriction enzyme and sequencing analysis. Semi-quantitative RT-PCR was used to detect the expression levels of hPOT1 gene. Result Recombinant psiRNA-hPOT1 vector was confirmed by sequencing analysis. The results demonstrated that 64nt had been inserted into the expected site. Furthermore, the inserted sequence was exactly correct. The recombinant psiRNA-hPOT1 vector could significantly suppress the hPOT1 expression in gastric cancer cell line SGC-7901. Conclusion The constructed siRNA expression vector of hPOT1 gene psiRNA-hPOT1 recombinant plasmid can block the expression of hPOT1 gene in gastric cancer cell line SGC-7901.
6.Diagnosis of colonic lesions with OMOM capsule endoscopy:an analysis of 46 cases

Objective To evaluate the role of OMOM capsule endoscopy in diagnosis of colon diseases.Methods Forty-six patients with suspected colon diseases,aged 18 to 68,voluntarily underwent the examination with the OMOM capsule endoscopy and colon endoscopy after informed the aim of the study.Patients who were suspected with colon stenosis or obstruction were excluded from the study.The outcome were compared with the results of common colonoscopy.The sensitivity,specificity and accuracy of OMOM capsule endoscopy were analyzed.Results OMOM capsule endoscopic examination were completed successfully in 40 of 46(87.0%) patients.For diagnosis of colon diseases with OMOM capsule endoscopy,the sensitivity was 57.7%,the specificity was 85.7%,and the accuracy was 67.5%.While,the positive predictive value was 88.2%,and the negative predictive value was 52.2%.The positive likelihood ratio was 4.04 and the negative likelihood ratio was 0.494.The bowel preparation had direct influence on examine.Polyethylene Glycol 4000 combined with 50% salamarum had better colon clearness.Conclusion OMOM capsule endoscopy is a safe,non-invasive method,and can provide definitive diagnosis in colon diseases.The bowel preparation is important for successful examination.
7.The confirmed inspection and analysis of HCV Different Region ELISA KIT to anti-HCV near S/CO sample

Objective To discuss the confirmed value of HCV Different Region ELISA KIT (ANTI-HCV-C, ANTI-HCV-NS3, ANTI-HCV-NS4, ANTI-HCV-NS5) to anti-HCV CO sample.Methods To inspect the general antibody of anti-HCV with ELISA, which is commonly used in clinic.The serum was separated from the suspicious samples, whose results were equal to S/CO or in the state of grey district ( CO?10%), and stored in the refrigerator in -29℃ low temperature and concenratedly inspected by the HCV Different Region ELISA KIT on the Triturus,the enayme immuno-reactions automatic analyzer. We totally collected 44 suspicious samples,among which there were 24 anti-HCV positive,20 anti-HCV CO?10% negative. Meanwhile we collected 30 samples whose S/CO of general antiboby was more than 5.0 and 20 ones which were healthy and could be compared with.Results In the 44 suspicious cases, whose S/CO was in the state of grey disrtict (CO?10%), the HCV Different Region Kit confirmed positive was 22.7% (10/44),suspicious 6.8% (3/44), negative 70.5 % (31/44).The false positive rate of positive anti-HCV CO?10% was 54.2%(13/24) and the false negative rate of negative anti-HCV CO?10% was 5.0%(1/20). In the 30 cases,whose S/CO of general antibody was more than 5.0,the HCV Different Region Kit confirmed positive was 80.0%(24/30),suspicious 13.3%(4/30), negative 67%(2/30). In 20 cases,which were from healthy people, the HCV Different Region Kit confirmed totally negative.Conclusion Using HCV Different Region Kit is a better evidence laboratory,especially for the uncertain samples,that is,it is very important for the "grey district"(CO?10%) samples to be further confirmed.It can reduce the false positive and false negative result which may emerge when anti HCV EIA inspects the dangerous groups and improve the accuracy of clinical HCV diagnosis.To blood station, it has certain practical value for the blood not only can be safely used, but also avoided wasting. The operation is simple and convenient, easily be spread and applied.

Objective To explore the effect of 25% insulin lispro( Mix 75/25) intensive treatment on plasma glucose and pancreatic function in newly diagnosed type 2 diabets. Methods Sixty patients with newly diagnosed type 2 diabetes were treated by 2 months 25% insulin lispro(Mix 75/25) treatment. The level of fasting plasma glucose (FPG) ,2 hours postprandial glucose (2hPG), glycosylated hemoglobin A1c (HbA1c), the fasting insulin and Cpeptide, insulin resistance index (HOMA-A), insulin secretion index (HOMA-B), insulin sensitivity index (ISI) and the ratio of fasting insulin to fasting glucose(FINS/FPG) were compared before and after 25% insulin lispro(Mix 75/25) treatment. Results After the treatment,FPG,2hPG,HbA1c and HOMA-A were significantly decreased( all P<0. 01 ) ;the fasting insulin and C-peptide,FINS/FPG, HOMA-B and ISI greatly increased( all P<0. 01 ). Conclusion 25% insulin lispro( Mix 75/25) intensive treatment can effectively improve plasma glucose and pancreatic function in newly diagnosed type 2 diabetes.
9.Meta-analysis of the Efficacy and Safety of Glipizide versus Repaglinide in Treatment of Type 2 Diabetes

OBJECTIVE:To systematically review the efficacy and safety of glipiaide versus repaglinide in treatment of type 2 diabetes and provide evidence-based reference for the clinical treatment. METHODS:PubMed,EMBase,Medline,Cochrane Li-brary,CJFD,VIP and WanFang database were retrieved to collect the randomized controlled trials(RCT)of the efficacy and safety of glipizide(test group)versus repaglinide(control group)in the treatment of type 2 diabetes. After the quality evaluation and in-formation collection of clinical studies with inclusion criteria,Rev Man 5.2 software was conducted for Meta-analysis. RESULTS:There were totally 10 RCTs,involving 1 022 patients. Meta-analysis results showed that the HbA1c levels [MD=0.43,95%CI (0.20,0.65),P<0.001],2 h postprandial plasma glucose levels [MD=0.49,95% CI(0.04,0.94),P=0.03] and hypoglycemia inci-dence [OR=2.99,95%CI(1.83,4.88),P<0.001] in test group were significantly higher than control group. There was no signifi-cant difference in the fasting plasma glucose level in 2 groups[MD=-0.14,95%CI(-0.44,0.16),P=0.35]. CONCLUSIONS:Re-paglinide has better efficacy and safety than glipizide in the treatment of type 2 diabetes. Limited by the methodological quality and sample amount of included studies,it remains to be verified by RCT with large sample,strict design and long-term follow-up.
10.Clinical efficacy of strengthening alar groove plasty in nasal tip reduction through internal incision

Objective To investigate the effects of nasal tip reduction with inner incision combined with external fixation,namely,to evaluate the formation of bilateral alar groove which is en hanced by external fixation with small splints,after removing interdomal fat pad as well as adjusting alar cartilage to reduce nasal tip.Methods Alar cartilage and interdomal fat pad were exposed via the bilateral nasal vestibular incision to remove fat pad and part of enlarged lateral foot of alar cartilage.After suture of incision,based on the tissue removal,plastic splint was used for external fixation of bilateral nasal tip and alar groove for one week,to enhance the formation of alar groove.Results A total of 143 patients were followed up for more than 6 months after operation.Photographs taken before and after operation were compared,suggesting obviously smaller nasal tip and significant alar groove.Left and right alar groove space was decreased from preoperative (28.23±3.31) mm to post operative (23.72±2.65) mm,and nasal tip alar angle was decreased from (104.02±9.57)°to (87.78± 6.98)°,with statistical significance (P<0.05).Slightly asymmetric nasal alars were found in 6 patients during follow-up examination,while tiny scars caused by splints (at 0.6 cm from the edge of splints) were found in 1 patient.Conclusions On the basis of nasal tip reduction with inner incision,the method of suture with small splints combined with external fixation is simple,effective and available.
