1.Literature Analysis of 56 Cases of Adverse Drug Reactions Induced by Monosialotetrahexosylganglioside Sodium Injection

OBJECTIVE:To study the general pattern,characteristics and related risk of adverse drug reaction(ADR)induced by Monosialotetrahexosylganglioside sodium injection,and to provide reference for safe use of drugs in the clinic. METHODS:Us-ing“monosialotetrahexosylganglioside”and“ADR”as keywords,literautres were retrieved from CJFD,Wanfang and VIP database according to inclusion and exclusion criteria in Oct. 2014,and cases reports were extracted and analyzed statistically. RESULTS:A total of 19 literatures were included,involving 56 patients,with the ratio of male to female was 6.17∶1,and the most of patients aged 0-9 years,≥60 years,accounting for 44.64%(25 cases),30.36%(17 cases). Multiple organs or systems were involved in ADR;the most common reactions are fever (29 cases) and shiver (28 cases),etc.,and severe ADR can cause green-barry syn-drome,anaphylactic shock,etc. CONCLUSIONS:Thus,it is important to pay attention to ADR caused by Monosialotetrahexosyl-ganglioside sodium injection,to prevent the occurrence of serious ADR,to perfect drug instructions,and to ensure clinical safe ad-ministration.

3.Early diagnosis of vascular complications following orthotopic liver transplantation

ObjectiveTo establish early diagnosis of vascular complications following orthotopic liver transplantation (OLT).MethodsThe surveillance and diagnosis making of vascular complications in 53 cases of OLT patients from April 1990 to June 2000 were carried out by color Doppler imaging (CDI), selective-angiography and analyzing relevant clinical features.ResultsThe incidence of vascular complication after OLT is 15%(8/53) including 3 cases of hepatic artery thrombosis, 2 of hepatic artery stenosis, 1 of celiac artery stenosis, and 2 of inferior vena cava stenosis. Diagnosis was confirmed by selective angiography in 7 cases and by autopsy in one. The sensitivity and specificity of CDI were 100%(8/8) and 98% (45/46) respectively. Conclusions Clinical feature of vascular complications following OLT lacks specificity. Continuous monitoring using CDI is a sensitive and specific method in the detection and diagnosis of vascular complications.Duration of surveillance should be no less than 2 months.
4.Automated enzymatic determination for serum total and free L-carnitine and its clinical application

Objective To establish an automated spectrophotometic assay for serum total and free L-carnitine with enzymatic method(CAT-BTND). Methods To optimize the reaction conditions of hydrolysis,pH and L-carnitine Acetyl-transferase (CAT)concentration in the assay,to evalute the assay method and to measure the serum L-carnitine of health adults ( n =63) and children ( n =56). Results The calibration curve for L-carnitine was linear between 4 81 and 150 ?mol/L( r =0 999).The inter and intra average CV were 5 52% and 5 90% in free carnitine,5 78% and 6 14% in octanoylcarnitine.The mean recovery rate was 98 5% and 98 4% respectively.This method was free from interference by bilirubin(≤120 ?mol/L),hemoglobin(≤7 g/L),and ascorbate(≤500 ?mol/L).Total and free carnitine and acylcarnitine reference range: adults (48 5?17 7) ?mol/L , (39 2?14 2) ?mol/L and (9 3?5 3) ?mol/L(male, n =32),(42 7?17 3) ?mol/L, (32 4?18 0) ?mol/L and (10 5?5 5) ?mol/L respectively(female, n =31).Children (age between 9~12 years old) (57 3?19 3) ?mol/L , (45 1?14 4) ?mol/L and (12 3?10 4) ?mol/L(male, n =31), (59 4?15 7) ?mol/L, (46 6?12 6) ?mol/L and (12 8?7 8) ?mol/L respectively(female, n =25). Conclusion The procedure is rapid,accurate and automatable,and could be used to diagnose L-carnitine deficiency,and to estimate and monitor the effect of treatments.

6.Effect of four-handed technique in light cured resin veneer in the treatment of dental fluorosis

Objective To observe the effect of four-handed technique in light cured resin veneer in the treatment of dental fluorosis. Methods Eighty outpatients with dental fluorosis in our hospital from January 2011 to January 2014 were randomly divided into two groups: 40 in the study group were treated with four-handed technique and another 40 in the control group with one-doctor manual operation. The two groups were compared in terms of treatment duration and curative effect. Result The treatment duration in the study was significantly shorter than that of the control group and the curative effect was significantly better than that of the control group (all P<0.05). Conclusion As for the light cured resin veneer in the treatment of patients with dentalfluorosis in the process , the four-handed technique is better than one-doctor manual operation.
7.Biliary complications after percutaneous ablation for patients with hepatic malignant tumors:retro-spective analysis of ultrasonic imaging features and clinical outcomes

Objective To retrospectively analyze the ultrasonic imaging features and clinical out-comes of biliary complications after percutaneous ablation for hepatic malignant tumors .Methods The cli-nical data of patients with postablation biliary complications who underwent percutaneous ablation for hepatic malignant tumors in the past 7 years in our department were retrospectively analyzed .The ultrasonic imaging features and the clinical outcomes of biliary complications on follow-up of these patients were analyzed .Re-sults Postablation biliary complications occurred in 65 patients , including 34 patients with bile duct dilata-tion and 31 patients with biloma .In patients with bile duct dilatation , the dilated bile ducts were located dis-tal to the ablation lesions in all these patients .Among 30 patients with bile duct dilatation , the dilatation was progressive in 9 patients (9/30, 30.0%) and stable in 20 patients (20/30, 66.7%), while in one patient (1/30, 3.3%) it resolved on follow-up.The radiologic features of bilomas mainly manifested as four types:crescent , interspersed , circumferential and solitary nodular , and the number of patients with the four types were 13 (13/31, 41.9%), 13(13/31, 41.9%), 3(3/31, 9.7%) and 2(2/31, 6.5%), respectively. In 6 patients (6/31, 19.4%) the bilomas were subjected to interventional or surgical therapy while for the remaining 25 patients (6/31, 19.4%) they were treated conservatively .Three (3/25, 12.0%) of the lat-ter 25 patients had spontaneously disappearance of the biloma and 2 (2/25, 8.0%) of the remaining 22 pa-tients appeared to have progressive increase in size .In 17 (17/25, 68.0%) patients the biloma remained stable in size and in the remaining 3 patients (3/25, 12.0%) the bilomas resolved.Conclusions Postab-lation biliary complications of ultrasound-guided percutaneous ablation for hepatic malignant tumors mainly presented as biloma and bile duct dilatation .Dilated bile ducts were located in the distal side of the ablation zones.Crescent type and interspersed type were the most common morphological types of biloma .Although most patients with postablation biliary complications of ultrasound-guided percutaneous ablation remained sta-ble or resolved on follow-up, interventional or surgical therapy was necessary in the minority of patients .
8.Effect of nitrous oxide on the duration of labor and mother and fetus

Objective To detect the effect of analgesic labor with inhaling nitrous oxide gas for the duration of labor and mother and fetus. Methods Choose 220 cases of normal primipara, 120 cases were observed group, the primipara were inhaled the mixture gas(50% nitrous oxide and 50% oxygen) when the duration of labor entered the progress from active phase to the cervical dilatation were all opened, 100 cases were control group. The effect of anal-gesia, the change of breath and postpartum hemorrhage, neonatal scores were observed. Results The observed group has good effect on anlgesia, the effective rate is 93.3 % . The active phase were obviously shortened and has bad effect to mother and fetus. Conclusion It was safety and effective by inhaling nitrous oxide gas for analgesic labor and worthy to generalized and applied.
9.Metabolites of injected chlorogenic acid in rats.

Chlorogenic acid (5-CQA) is one of the major components in some Chinese herbal injections. However, the metabolism of 5-CQA in rats after intravenous injection has not been determined. An ultra-high performance liquid chromatography/quadrupole time-of-flight mass spectrometry (UPLC/Q-TOF MS) method was applied to identify the metabolites in bile, urine, feces and plasma after a single intravenous administration of 10 mg x kg(-1) 5-CQA to rats. Using MSE and mass defect filter techniques, a total of 35 metabolites were detected in bile, urine, feces and plasma. The predominant metabolites in bile were glutathione conjugates of O-methyl-5-CQA, accounting for approximately 80% of the metabolites excreted in bile. The major components in urine were parent drug, O-methyl-5-CQA, hydrolyzed metabolites and glucuronide conjugates. The major components in feces were O-methyl-5-CQA and its cysteine conjugates. The major component in plasma was the parent drug. The urinary and fecal excretion pathways were equally important to 5-CQA in rats. These results demonstrate that 5-CQA undergoes extensively metabolism in rats and are highly reactive to nucleophiles such as GSH. This finding indicates that attention should be paid on the injections containing 5-CQA, which may covalently bind to proteins, leading to allergenic drug reactions.
10.Clinical observation of laparosopy operation on the treatment of uterine appendages lump

Objective To evaluate the advantage of uterine appendages lump operation by laparoscopy and the choose of the patient. Methods 215 cases of ovrarian tumor operation and fallpian tubes operation by laparoscopy,50 cases of similar period and similar operation indication were chosen by laparoscopy or by abdorminial operation apiece, blood loss, postoperative analgesic rate, the time of using antibiotics, hospitalization day were compared. Results Uterine appendages lump operation by laparoscopy were all operated successfully, compared with abdorminial operation, the laparoscopy group had less blood loss,less drug after operation,less pain, shortened hospitalization day.Conclusion As long as choosing the appropriate cases before the operation, uterine appendages lump operation by laparoseopy has the advantage of minimal trauma, recovery fastly,less postoperative complication and safety.
