1.The diagnostic analysis of the acute mesentery embolism

Objective To summariz the clinical features and therapeutic method of the acute mesentery embolism. Methods 11 clinical cases of the acute mesentery embolism in Xinqiao hospital of the third military medical university and the people's hospital in Hanzhong city,Shaanxi province during 1996 to 2004 were retrospectively analyzed. Results All the patients were finally diagnosed the acute mesentery embolism through operation. Among them,8 cases were the acute mesentery artery embolism,and three cases were induced by the acute mesentery vein mesentery. The embolus had already extracted and the dead simple intestinal ducts had already resected in two cases; Intestinal ducts had already resected in nine cases;Six patients were cured and five patients were dead,and the mortality was 45.5%.Conclusion Early operation can improve the curability of the acute mesentery embolism.
2.Influence of trimetazidine on left cardiac systolic function and plasma brain natriuretic peptide in alcoholic cardiomyopathy

Objective To explore the influence of trimetazidine on left cardiac systolic function and plasma brain natriuretic peptide(BNP)in alcoholic cardiomyopathy.Methods 60 cases of alcoholic cardiomyopathy in our hospital from December 2014 to December 2015 were chosen to research,and they were divided into observation group and control group by random number table,30 patients in each group.The control group was treated with bisoprolol combined with strengthening heart,diuretic and other treatment,the observation group was treated with trimetazidine on the basis of routine treatment.Left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter (LVEDD),each sub output(CO)after 6 months'treatment were compared.Results LVEF of the observation group was from pre -treatment (34.8 ±5.4)% up to (47.4 ±5.5)%,LVEDD value (49.1 ±6.5)min and CO values (5.6 ±0.7)L/min were significantly better than the control group[(53.4 ±7.5)min,(4.9 ±0.7)L/min],the differences were statistically significant(t =2.373,3.873,all P <0.05).BNP level of the observation group after treatment was significantly lower than control group[(455.8 ±133.2)fmoL/mL vs (542.7 ±128.3)fmoL/mL,t =2.574,P <0.05)].The total effective rate of the observation group (96.67%)was higher than that of the control group (80%),and the difference between the two groups was significant (χ2 =13.09,P <0.05 ).Conclusion Alcoholic cardiomyopathy should pay attention to the daily scientific daily routine,take measure to quit smoking and alcohol.Trimetazidine could effectively improve the left ventricular systolic function,decrease the level of BNP,thus is worthy of clinical application.

There are many problems about multiple breast cancer (MBC),such as whether it results from intramammary spread of a single tumor, or from two or more separate neoplastic events? Whether MBC has inferior outcome compared with unicentric lesions? Whether MBC is suit for conservative surgery and the sentinel lymph node biopsy, and so on. Thus: We reviewed the prospect for diagnosis and therapy of MBC.

5.Arthroscopy plus radiotherapy in the treatment of pigmented villonodular synovitis of the knee

[Objective]To explore the technique of arthroscopy plus radiotherapy in the treatment of pigmented villonodular synovitis of the knee and to evaluate its clinical value.[Method]A total of 14 cases of pigmented villonodular synovitis of the knee were treated from January 2000 to July 2003.Intra-articular excision under arthroscope with a combined application of routine intrusments and Bipolar radiofrequency was performed on all these 14 cases,and postoperative radiotherapy for diffused pigmented villonodular synovitis of the knee,13 patients were followed up for 10~45 months,and were evaluated for range of motion,pain,swelling effusion.[Result]Thirteen cases according to the International Knee Documentation Committee(IKDC)criteria,subjective knee score was (58.7+6.4) and (86.5 ?5.7) respectively before surgery and at the last follow-up.[Conclusion]In case of pigmented villonodular synovitis of the knee joint,arthroscopical synovectomy can make good surgical results,and postoperative radiotherapy is an effective treatment for diffused pigmented vilionoclular synovitis of the knee.
7.Expression of endoplasmic reticulum chaperone GRP78 in breast cancer and its prognosis significance

Objective To investigate the expression of GRP78 in breast cancer,the relationships among the expression and the clinicopathological characteristics,prognosis were also investigated.Methods The expression of GRP78 was detected in 172 paraffin-embedded breast cancer samples with immunohistochemistry Envision method,the relationships among the expression and the clinicopathological characteristics,prognosis were investigated.Results Positive expression of GRP78 is common in breast cancer.Strong expression of GRP78 was detected in 71 cases (41.28%),and weak expression was detected in 101 cases (58.72%).GRP78 expression wasn't associated with the clinicopathological characterristics except T stage and Her-2 status.Univariate analysis (log-rank test) showed that GRP78 expression correlated with disease free survival and overall survival significantly.Patients with strong GRP78 expression had poorer prognosis compared to those with weak GRP78 expression(P < 0.05).Multivariate analysis utilizing Cox regression analysis showed that GRP78 is an independent biomarker of disease free survival (P < 0.05),but not an independent biomarker of overall survival (P > 0.05).Conclusions Positive expression of GRP78 is common in breast cancer and strong expression is associated with poorer survival.
8.siRNA against survivin mediated by adenovirus vector up-regulate estrogen receptor expression in breast cancer MCF-7 cell

Objective To investigate the effect on ER expression in MCF-7 cell by siRNA against survivin mediated by adenovirus vector.Methods An adenovirus vector of siRNA against survivin was constructed and used to infect MCF-7 cell.The change of expression of survivin and ER was detected by Western Blot.Results The expression strength of survivin are 0.09 ± 0.04、0.86 ± 0.08、0.82 ± 0.17;expression strength of ER are 1.57 ± 0.09、1.16 ± 0.10、1.23 ± 0.01 respectively in the experimental group,negative control group and blank control group.Statistics analysis shows that the adenovirus vector of siRNA against survivin constructed in the study can suppress the expression of survivin significantly,and suppress the expression of survivin can up-regulate the estrogen receptor (ER) expression.Conclusions The results suggest that there may be a certain regulatory mechanism between survivin and ER signal pathway in MCF-7 cell and siRNA against survivin is of important potential value in the endocrine therapy of hormone receptor positive breast cancer.
9.Prognostic analysis of thoracic esophageal carcinoma treated with postoperative prophylactic radiotherapy

Objective To investigate the effect of the extent of postoperative prophylactic radiotherapy after radical surgery on the long-term survival in patients with thoracic esophageal carcinoma.Methods A retrospective analysis was performed on the clinical data of 201 patients with thoracic esophageal cancer who received postoperative prophylactic radiotherapy after radical surgery from 2000 to 2007.The effect of the extent of radiotherapy on survival was evaluated,and the possible prognostic factors were subjected to multivariate Cox regression analysis.The Kaplan-Meier method was used to calculate overall survival (OS) rates,and the log-rank test was used for survival difference analysis.Results The 5-year follow-up rate was 97.0%.The OS rates for patients who received radiotherapy to the whole mediastinum,whole mediastinum + lymph nodes along the left gastric artery,whole mediastinum + bilateral supraclavicular regions,upper and middle mediastinum + bilateral supraclavicular regions,and whole mediastinum + bilateral supraclavicular regions + lymph nodes along the left gastric artery were 21.7%,37.1%,38.7%,34.8%,and 19.8%,respectively (P =0.406).Multivariate analysis showed that postoperative N stage was the independent prognostic factor (P =0.009).After prophylactic irradiation,metastases to the supraclavicular lymph nodes,lymph nodes in the upper and middle mediastinum,and abdominal lymph nodes were observed in 11,34,and 10 patients,respectively.Conclusions The extent of postoperative prophylactic radiotherapy after radical surgery for thoracic esophageal carcinoma only includes upper and middle mediastinum and bilateral supraclavicular regions.
10.Influence of medicative diet for food therapy on hepatic function of patients with gastric cancer and adjuvant chemotherapy after operation

Objective To approach the alleviating effect of medicative diet for food therapy on hepatic function of patients with gastric cancer and adjuvant chemotherapy after operation and supply references for effective treatment and nursing. Methods We divided 123 patients with gastric cancer after operation into the test group (65 cases) and the control group (58 cases).The control group adopted routine treatment project while the test group received medieative diet for food therapy based on routine treatment project. The changes in hepatic functions were compared between the two groups. Results The changes in hepatic functions of the test group were better than those of the control group (P<0.01 ). Conclusion Medicative diet for food therapy could significantly restore the seriously damaged hepatic function after chemotherapy.
