The medical students, Shanghai Jiaotong University School of medical from 5- year,7-year, 8-year to general of academic, received evidence-based medicine (EBM) education. The students' background and attitudes about EBM were different. Teaching EBM should pay attention to the relationship of EBM and basic courses, especially for deferent students. Teaching EBM should transform from imparting basic knowledge to learning skills on their own initiative.

Objective To assess the diagnostic value of Manning's criteria, Rome Ⅰ criteria and Rome Ⅱ criteria in Chinese patients of irritable bowel syndrome. Methods 724 subjects were enrolled from the outpatient department of gastroenterology and the physical examination center of Xinhua hospital. Self designed questionnaire, routine physical examination and selective accessory diagnostic procedures were undergone in all subjects including 402 cases of irritable bowel syndrome,134 cases of non ulcer dyspepsia,90 cases of ulcerative colitis and 98 healthy controls. According to Manning criteria, Rome Ⅰcriteria and Rome Ⅱcriteria, the sensitivity, specificity, positive likelihood and negative predictive value(PV), positive likelihood and negative likelihood were calculated separately. Results The sensitivity and specificity of Manning's criteria, Rome Ⅰ criteria and Rome Ⅱcriteria were 96.02% (95%CI:95.06%-100.0%), 77.36% (75.31%-79.41%), 68.41% (66.14%-70.68%) and 47.51% (44.78%-50.23%), 73.29%(70.87%- 75.70 %) 83.23% (81.19%-85.27%),respectively. The positive PV and negative PV were 69.55%, 78.33% , 83.59% and 90.53%, 72.17%, 67.85%, respectively. The positive likelihood and negative likelihood were 1.83 , 2.90 , 4.07 and 0.08, 0.31, 0.38, respectively. Conclusion Irritable bowel syndrome could be positively diagnosed by taking an appropriate history and questionnaires,Rome Ⅱ criteria had higher validity in diagnosis of irritable bowel syndrome.

BACKGROUND:During spinal degeneration process, the intervertebral disc and facet joints are interrelated and interacted to impact the normal function and stability of the spine, thereby resulting in low back pain. Moreover, there is always a controversy on the degeneration order of the intervertebral disc and facet joint. OBJECTIVE:To explore the relationship between lumbar facet joint degeneration and intervertebral disc degeneration in patients with low back pain. METHODS: A retrospective analysis was made on the clinical data of 186 patients with low back pain. The facet joint degeneration and intervertebral disc degeneration at L2-S1 motion segments of each patient were evaluated. Enroled patients were divided into three age groups: < 40 years old, 40-60 years old and≥ 60 years old. RESULTS AND CONCLUSION:The incidence of lumbar facet joint and intervertebral disc degeneration was increased with age, and degeneration of the lumbar facet joint and intervertebral disc were the most obvious at L4-5 and L5-S1 segments. The incidence of intervertebral disc degeneration was more than that of facet joint degeneration at each segment in al age groups, except groups of < 40 years old and 40-60 years old at L2-3 segments, but there was no significant difference (P > 0.05). There was a weak correlation between facet joint degeneration and intervertebral disc degeneration (χ2=100.9,P < 0.001, gamma=0.22). These findings show that the incidence of intervertebral disc degeneration and facet joint degeneration is increased with age, and there is a weak correlation between them. However, the lumbar degenerative order is stil unclear and further research is needed.

Objective: To evaluate the superiority in diagnosing DVT between venography and duplex ultrasound, and the effectiveness of C-reactive protein (CRP) as a biomarker. Methods: Firstly, the iliac - femoral vein of the dog left hind leg was isolated, and then, the thrombosis model was established by infusing the thrombin after breaking endangium. The recanalization of thrombosis was assessed by duplex ultrasound and venography, and the expression of serum hsCRP was also examined. From 2006 to 2008, 77 patients with acute DVT proximal to the knee joint were admitted. The interval between the onset of DVT and admission was 1-21 days. They were treated mainly with urokinase and low molecular weight heparin for 2 weeks. The assessment of each patient including clinical manifestation, venography, duplex ultrasound and serum highly sensitive C-reactive protein (hsCRP) were performed immediately after admission and 4 weeks after discharge. Results: After medical therapy for 2 weeks, the clinical features prominently subsided in 49 patients, improved in 23 and didn’t ameliorate in 5.4 weeks after discharge, venography showed clot regression in 15 patients; while in the remaining 62 patients the occluded venous lumen were not visualized, duplex ultrasound showed partial lysis of the thrombosis. At admission, the hsCRP was 28.91?29.4mg/L, and it dropped to 8.13?12.7mg/L at 4 weeks after discharge. Conclusion:Duplex ultrasound was effective to assess DVT. The hsCRP was positively related to the severity of DVT.

Objective To study the association between activity of hand, foot and mouth disease (HFMD) and the meteorological factors in Suzhou. Methods A total of 17 653 children diagnosed with HFMD in Children's Hospital Affiliated to Suzhou University during 2008 to 2011 were enrolled. The meteorological factors in(c)luding mean temperature, relative humidity, rainfall amount, sum of sunshine and mean wind velocity during Jan 2008 to Dec 2011 were collected monthly.Numeration data were analyzed using chi square or Fisher exact test.Normal distribution data were analyzed using Pearson correlation and non-normal distribution data were analyzed using Spearman rank correlation. Results HFMD could be found all over the year and had obvious seasonality which peaked in Summer, followed by Spring and Autumn and HFMD decreased markedly in Winter. The activity of HFMD was positive correlated with mean temperature(r=0.716,P<0.01),wind velocity (r=0.630,P<0.01),rainfall(r=0.477,P<0.01) and sum of sunshine(r=0.311,P<0.05).No statistical significance was observed between relative humidity and HFMD(r =0.276,P>0.05).Multivariate stepwise regression analysis showed that only mean temperature and rainfall were associated with HFMD and mean temperature had stronger influence on HFMD than rainfall (t =4.687,P < 0.01 ; t =2.258,P < 0.05).Conclusions HFMD has obvious seasonality.Mean temperature and rainfall are the main factors affecting this seasonality.

Objective To analyze sonographic imagings of traumatic neuromas of peripheral nerves, and to investigate the applied values of uhrasonography in the traumatic neuroma of peripheral nerve. Methods The sonographic imagings of 38 patients who had evidences of traumatic neuromas in peripheral nerves were retrospectively reviewed by the last diagnostic results of operations. Results Among 38 cases, 2 incomplete traumatic neuromas were not detected, and 1 incomplete traumatic neuroma was misdiagnosised as neurilemoma by ultrasonography. The coincidence of uitrasonography in incomplete traumatic neuroma and complete severed neuroma was 82.4% and 100%. With regard to the echogenicity of 38 traumatic neuromas of peripheral nerves, the sonographic characteristics of neuromas were all hypoechoic. Thirty-three neuromas appearanced like shuttles and 5 neuromas appearaneed like humps. Conclusions The traumatic neuroma has distinctive ultrasonic appearance. Ultrasonography is useful to direct the diagnosis and treatment of traumatic neuroma.
