1.Nursing experience of patients undergoing liver biopsy after liver transplantation

ObjectiveTo summarize the nursing experience of postoperative liver biopsy in patients after liver transplantation. Methods89 patients undergoing postoperative liver biopsy after liver transplantation from June 2010 to April 2011 were retrospectively analyzed.Their nursing experience was summarized. Results81 patients were confirmed of having rejection reaction.89 patients had no severe complications,only 28 cases(31.5%)had mild local pain. ConclusionsSufficient preoperative preparation,good nursing cooperation during the operation,close postoperative observation and nursing can prevent and timely detect and cope complications,thereby reduce the risk of liver puncture.
2.Simultaneous Determination of Berberine and Salvianolic Acid B in Fuyankang Enema by HPLC

Objective To establish a method for the determination of berberine and salvianolic acid B in Fuyankang En- ema.Methods The chromatograhie separation was performed on a Kromasil C_(18) column (250 mm?4.6 mm,5?m) with a gradient elution of acetonitrile-0.04 mol/L potassium dihydrogen phosphate solution containing 0.1% phosphoric acid.The flow rate was 1.0 mL/min and column temperature was set at 30℃.The detection wavelength was 230 nm.Results There was a good linearity in the range of 0.0924~2.3100?g (r=0.999 9) for berbeine,and 0.1000~2.5000?g (r=0.999 9) for salvianolic acid B.The average recoveries of berberine at high,middle and low concentra- tions were 100.40 %,99.51% and 99.15 % respectively with RSD being 0.59 %,0.67 %and 1.02 % (n=3) re- spectively.The average recoveries for salvianolic acid B were 101.52 %,99.32 % and 98.68 % respectively with RSD being 0.60 %,0.92 % and 1.00 % (n=3),respectively.Conclusions This method is convenient,specific and re- producible for the determination of berbeine and salvianolic acid B in Fuyankang Enema by HPLC.
3.Prospective study on the long-term ocular toxicity of hydroxychloroquine in rheumatic diseases

OMective In order to evaluate the long-term ocular safety of hydroxychloorqine,we prospectively study the ocular symptoms,routine ophthalmological screening tests and electrophysiological tests in patients taking hydroxychloroquine.Methotis 170 cases of autoimmune diseases patients who had taken hydroxychlon,qine during August,2005 to June,2007 were enrolled into the study.Visual acuity,intraocular pressure,and fundoscopy examination,visual field testing,electroretinogram (ERG) and electrooculogram (EOG) were examined every six months.Results The average duration of taking hydroxyehloroquine was 16.1 month,ranging from 0.5 to 48 months.The total dose ranged from 6 g to 584 g.All patients'visual acuity and intraocular pressure were normal.Fifty-two cases had performed ERG and EOG.The ERG tests were normal in 52 patients.The EOG test of 2 patients showed that the Arden ratio was lower than normal,in which one had severe myopia.Other tests done for these two persons were normal.Conclusion This observational study has shown that hydroxychloroquine has good ocular safety.Further studies with much longer follow-up period will be useful in evaluating the retinal toxicity of hydroxychloroquine.
4.Significance of ultrasound tissue characteristics of carotid atherosclerotic plaques in predicting cardiocerebrovascular events in patients with type 2 diabetes

Objective To investigate the predictive value of ultrasonic tissue charateristics of carotid atherosclerotic plaques in cardiocerebrovascular events in patients with type 2 diabetes mellitus (T2DM).Methods From January 2003 to December 2015,patients with T2DM treated in Xi'an Gaoxin Hospital were selected.Their carotid atherosclerotic plaques were examined by ultrasound.Intergrated backscatter (IBS) and thickness of carotid plaques of the patients were documented.The cardiocerebrovascular events were folbwed up prospectively.Kaplan-Meier curve,COX proportional risk regression model,and receiver operating characteristic (ROC) curve were used to compare and analyze the predictive values of calibratedIBS (C-IBS),Framingham Risk Score (FRS) and plaque thickness for the risk of cardiocerebrovascular events in patients with T2DM.Results There was significant difference in the risk of cardiovascular events between the low C-IBS group (n=48,C-IBS <17.1 dB) and the high C-IBS group (n=48,C-IBS ≥17.1 dB) (35.4% vs.12.5%;P=0.015).There was no significant difference in the risk of cardiocerebrovascular events between the thin plaque group (n =48,plaque thickness < 1.3 mm) and the thick plaque group (n=48,plaque thickness >1.3 mm) (14.6% vs.33.3%;P=0.070).COX proportional risk regression model analysis showed that C-IBS (hazard ratio,1.505,95% confidence interval 1.196-1.893,P=0.001) and plaque thickness (hazard ratio,4.794,95% confidence interval 1.142-20.130,P =0.032) were the independent risk factors for the risk of cardiocerebrovascular events.ROC curve analysis found that the combination of C-IBS,FRS and plaque thickness could predict the risk of cardiocerebrovascular events in patients with T2DM.Conclusion Ultrasonic tissue characteristics of carotid atherosclerotic plaque (C-IBS and plaque thickness) in combination with FRS may predict cardiocerebrovascular events in patients with T2DM.
5.Down-regulation of miR-488 targeting to promote Jag1 expression inhibits hypoxia-reoxygenation myocardial H9c2 cell damage.

OBJECTIVE:
To study the effect of down-regulating miR-488 targeting Jag1 on the injury of hypoxia-reoxygenation myocardial H9c2 cells.
METHODS:
A hypoxic-reoxygenated myocardial H9c2 cell injury model was constructed. miR-488 inhibitor was used to transfect the cells. CCK-8 method and flow cytometry were used to detect cell proliferation and apoptosis in each group. Lactate dehydrogenase (LDH), superoxide dismutase (SOD), malonaldehyde (MDA), catalase (CAT) levels were detected. Western blotting was used to detect the expression of Bcl-2 associated X Protein (Bax) and B cell lymphoma/lewkmia-2 (Bcl-2). Target genes of miR-488 were predicted, and a luciferase reporter system was used to verify the targeting relationship between the two. Myocardial H9c2 cells were co-transfected with miR-488 inhibitor and Jag1 siRNA, and treated with hypoxia and reoxygenation, cell proliferation, apoptosis, LDH, SOD, MDA, CAT levels, and Bax, Bcl-2 protein expression were detected.
RESULTS:
The expression of miR-488 in the hypoxia-reoxygenated myocardial H9c2 cells was increased, along with reduced cell proliferation, increased apoptosis, increased Bax protein expression, decreased Bcl-2 protein expression, increased MDA, decreased CAT and SOD, and increased LDH level in the supernatant of cell culture. When myocardial H9c2 cells were transfected with miR-488 inhibitor and treated with hypoxia and reoxygenation, the expression of miR-488 was decreased, along with increased cell proliferation, decreased apoptosis, decreased Bax protein expression, increased Bcl-2 protein expression, decreased MDA, increased CAT and SOD, and decreased LDH level in the supernatant of cell culture. Down-regulation of miR-488 could target and down-regulate Jag1 expression. And Jag1 siRNA could reverse the effect of miR-488 inhibitor on the proliferation, apoptosis, LDH, SOD, MDA, CAT levels and the expression of Bax and Bcl-2 of hypoxic-reoxygenated myocardial H9c2 cells.
CONCLUSION
Down-regulating miR-488 targeted Jag1 can attenuate hypoxia-reoxygenation induced myocardial H9c2 cell injury.

6.Association of white matter abnormality with facial emotion cognitive in the first-episode schizo-phrenic patients with never-medicated

Objective To explore the structure changes of white matter in the first-episode schizo-phrenic patients with never-medicated(FESZ)and the relationship between facial emotion perception and white matter(WM)integrity.Methods Sixty-three schizophrenic patients and thirty health control subjects underwent diffusion tensor imaging(DTI)scans.Voxel-based analysis was used to compared fractional ani-sotropy(FA)map between the two groups.Correlations were analyzed with pearson relative analysis between impaired facial emotion perception tested by facial emotion categorization(FEC)and severity of symptoms measured by the Positive and Negative Syndrome Scale(PANSS).Results (1)Compared with controls, FESZ patients showed overall decreased FA in WM of the body of left ventral frontal lobe((MNI(x,y,z):-18,26,-4;t=4.43)),right supramarginal gyrus((MNI(x,y,z):32,-50,26;t=4.27)),left middle oc-cipital gyral((MNI(x,y,z):-26,-60,0;t=4.89)),right middle occipital gyral((MNI(x,y,z):28,-70, 14;t=4.18)),left fusiform gyrus((MNI(x,y,z):-40,-50,0;t=3.92)),left cerebellum anterior lobe ((MNI(x,y,z):-32,-56,-28;t=4.57)),right parahippocampa gyrus1((MNI(x,y,z):32,-10,-14;t=4.16)),right parahippocampa gyrus2((MNI(x,y,z):16,-6,-14;t=4.56)),left anterior cingulate ((MNI(x,y,z):-2,4,-6;t=4.41)),left extra-nuclear((MNI(x,y,z):-2,-10,-6;t=4.44)),right thalamus((MNI(x,y,z):10,-10,2;t=4.20)),left thalamus((MNI(x,y,z):-22,-28,12;t=4.01)), and right caudate((MNI(x,y,z):14,12,8;t=4.87)).(2)Compared with controls,the patients with schizo-phrenia showed a higher shift point and a steeper slope than control subjects in FEC.Correlational analysis re-vealed that the negative correlations were found between the slope and negative factor(r=-0.298,P=0.036),between positive factor and the FA value in WM of the right middle occipital gyral(r=-0.322,P=0.023)and the left middle occipital gyral(r=-0.288,P=0.043),and between the FA value in the left cere-bellum anterior lobe and shift point(r=-0.393,P=0.005),but the positive correlation was found between disorganized/concrete factor and the FA value in the right parahippocampal gyrus(r=0.429,P=0.002).Con-clusions There are extensive microstructural abnormalities in WM of patients with FESZ.Disrupted WM in-tegrity in these regions may constitute a potential neural pathological basis for the facial emotion perception impairments in schizophrenia.
7.Analysis on medical care seeking of local and non-local pulmonary tuberculosis patients reported in Beijing, 2016-2021.

Objective: To analyze the medical care seeking of local and non-local pulmonary tuberculosis (TB) patients in Beijing from 2016 to 2021 and provide evidence for TB prevention and control in Beijing. Methods: The reported pulmonary TB data from 2016 to 2021 were collected from tuberculosis management information system and standard code management system of Chinese information system for disease control and prevention. The map data were obtained from the Institute of Geographic Sciences and Natural Resources Research. Excel 2016, SPSS 19.0, Python 3.9 and ArcGIS 10.6 softwares were used for data analysis and visualization for the inter-provincial mobility and inter-district mobility of pulmonary TB patient's medical care seeking in Beijing. Results: Among the reported pulmonary TB patients in Beijing from 2016 to 2021, 35.27%(24 307/68 926) were from 30 provinces (autonomous regions, municipalities) and Xinjiang Production and Construction Corps. The top 5 provinces with pulmonary TB patients medical care seeking in Beijing were Hebei (39.58%,9 620/24 307), Shanxi (8.82%,2 145/24 307), Inner Mongolia Autonomous Region (8.66%,2 105/24 307), Heilongjiang (6.95%,1 690/24 307) and Henan (6.88%,1 672/24 307). Patients from these 5 provinces accounted for 70.89% (17 232/24 307) of total pulmonary TB patients from other provinces. The pulmonary TB patients from other provinces mainly flowed to Tongzhou district of Beijing, accounting for 46.72% (11 356/24 307). These pulmonary TB patients mainly visited grade Ⅲ (A) hospitals, such as Beijing Chest Hospital of Capital Medical University with the highest proportion of 44.76% (10 880/24 307). Among pulmonary TB patients with current residence in Beijing, those sought medical care in other districts accounted for 55.06% (24 566/44 619). The outflow and inflow of TB cases' medical care seeking occurred in 16 districts of Beijing. The median proportion of cross district medical care seeking in 16 districts was 59.30% (56.05%, 65.13%). The inflow of the medical care seeking in Tongzhou, Haidian and Xicheng district was greater than the outflow, and the outflow was greater than the inflow in the other 13 districts. The pulmonary TB patients in Beijing mainly went to Beijing Chest Hospital in Tongzhou for medical care seeking, accounting for 42.18%(18 822/44 619). Conclusions: The proportions of non-local pulmonary TB patients seeking medical care in Beijing from 2016 to 2021 and local pulmonary TB patients seeking medical care in other districts in Beijing were high, and the hospitals where non-local pulmonary TB patients and local pulmonary TB patients sought medical care respectively belonged to grade Ⅲ (A) and municipal designated medical institutions of TB.

8.Differentiation and Treatment of Bipolar Disorder based on Qi, Phlegm, Fire and Deficiency

Bipolar disorder (BD) is considered to be mainly related to qi, phlegm, fire and deficiency. Binding constraint of liver qi is the initial cause, while phlegm and qi interact obstruction as well as phlegm and fire interact binding is the key pathogenesis of the transformation between depression and mania, and deficiency of both qi and yin is the main reason of the protracted course of disease. In clinical practice, BD is divided into binding constraint of liver qi pattern, phlegm and qi interact obstruction pattern, phlegm and fire interact binding pattern, and deficiency of both qi and yin pattern, which can be treated with Jinyu Shugan Powder (金玉疏肝散), Kaiyu Wendan Decoction (开郁温胆汤), Qingxin Huatan Decoction (清心化痰汤), and Baihe Shengmai Beverage (百合生脉饮) in their modifications respectively; moreover, Guanye Jinsitao (Herba Hyperici Perforati) is usually used to rectify qi, relieve phlegm and clear heat. It is also suggested to put focus on the prevention and treatment of qi, phlegm and heat simultaneously, and modify the medicinals flexibly in accordance with the pathogenesis evolution and the abnormal exuberance.
9.Heart rate variability in obese patients with severe obstructive sleep apnea hypopnea syndrome

【Objective】 To investigate the characteristics of heart rate variability (HRV) in obese patients with severe obstructive sleep apnea hypopnea syndrome (OSAHS). 【Methods】 We retrospectively analyzed 78 patients with severe OSAHS diagnosed by polysomnography (PSG) in The Second Affiliated Hospital of Xi’an Jiaotong University from April 2018 to May 2022. According to body mass index (BMI), the patients were divided into obese with severe OSAHS group (43 cases) and non-obese with severe OSAHS group (35 cases). All patients received 24-hour Holter monitoring while on polysomnography monitoring. The differences in HRV indexes between the groups and the correlation between HRV and clinical indicators were analyzed. 【Results】 In terms of basic data and PSG indexes, the analysis results showed that compared with those in the non-obese OSAHS group, weight, BMI, neck circumference, waist circumference, and AHI in obese with severe OSAHS group were significantly higher, while the standard deviation of the 24-hour normal R-R interval (SDNN), the standard deviation of the 5-minute mean (SDANN), the triangle index (TI), the heart rate deceleration force (DC), the standard deviation of the normal R-R interval (awake SDNN), and high frequency during sleep in the obese with severe OSAHS group were significantly lower (P<0.05). The correlation results showed that among obese with severe OSAHS patients, root mean square of the difference of adjacent R-R interval (rMSSD) was negatively correlated with the course of hypertension; TI and DC were negatively correlated with AHI. After adjusting for neck circumference and waist circumference, the linear regression analysis showed that SDNN, SDANN, and rMSSD were correlated with systolic blood pressure (P<0.05). 【Conclusion】 There is significant decrease in HRV index in obese patients with severe OSAHS, suggesting that deterioration of cardiac autonomic nervous regulation function may increase the risk of cardiovascular disease.
