1.The protection effect of trimetazidine on patients with diabetic cardiomyopathy

To investigate the protection effect of trimetazidine on patients with diabetic cardiomyopathy.Collection in January 2005~2008 in my years in October inpatient and outpatient services and long-term use of trimetazidine in patients with diabetic cardiomyopathy a total of 121 cases,divided by age:A group (the control group,59 people),B group ( treatment group,62 people),both groups with conventional medical treatment before and after treatment measured in fasting blood glucose,OGTT after 2 h blood glucose,blood lipids (total cholesterol,triglycerides),prothrombin time (PT),KPTT activation time (APTT),electrocardiogram QT dispersion,high-sensitivity C-reactive protein,blood pressure,ejection fraction (EF),left ventricular fractional shortening (FS),and compared before and after treatment LVMI (left ventricular mass index) and RWT (relative Room wall thickness).The results compared with those before treatment,B group significantly improved clinical symptoms,EF and FS compared to no significant difference,lowering blood lipids,blood glucose decreased,compared with A group,B group electrocardiogram QT dispersion has improved,B group LVMI,RWT ,high-sensitivity C-reactive protein lower than the A group (P<0.05);conclusions trimetazidine supplement the treatment of diabetic cardiomyopathy.
2.Effect of Yangyinqingfei granula on immune function in mycoplasma pneumonia complicated with Epstein-Barr virus of children

Objective To investigate the effect of Yangyinqingfei granula on immune function in the treatment of mycoplasma pneumonia complicated with Epstein-Barr virus of children.Methods 108 mycoplasma pneumonia complicated with Epstein-Barr virus of children from our hospital were selected and randomly divided into two groups.The control group of 54 cases were treated by simple western medicine treatment and the experimental group of 54 cases were treated on the basis of the control group with Yangyinqingfei granula.The improvement of clinical symptoms, inflammation indexes and the levels of T lymphocyte subsets, clinical efficacy and complications were compared.Results The clinical efficacy in experimental group was 90.74%,which was significant higher than that in control group 75.93%(P <0.05).Compared with the control group,the remission duration of fever, cough and rales,the disappearance time of lung X-ray,the length of stay were shorter(P<0.05), the CRP,IL-6,TNF-αand IL-8 levels were lower ( P <0.05 ) , the CD3 +, CD4 +, CD4 +/CD8 + and NK levels were higher, the CD8 +level was lower ( P <0.05 ) . Conclusion Yangyinqingfei granula could reduce the inflammatory factors and improve immune function in the treatment of mycoplasma pneumonia complicated with Epstein Barr virus infection pneumonia in children.
3.Association between serum level of C-reactive protein and heart function impairment of hypertension patients

Objective To detect the serum level of C-reactive protein in the hypertension patients and analyze its relationship with the heart function impairment. Methods The hypertension patients who had no, or mild, or severe heart function impairment were selected and accordingly divided into 3 groups (n=26 in each group). Meanwhile another 26 healthy subjects served as normal controls. The heart function of all subjects were evaluated and scored according to heart function classified schedule published by New York Heart Disease Association (1928) and CRP levels were measured by scatter turbidimetry within 24 h after diagnosis. Results The scores of heart function for healthy patients and the hypertension patients without heart function impairment were within the normal range and their serum CRP levels were at the basic level. 4.A analysis of clinical value about endoscopic nipple modified resection in duodenal papillary adenoma resection

Objective: To analyze the clinical value of modified endoscopic nipple resection in duodenal papillary adenoma resection Methods: 40 patients with duodenal papillary adenoma were randomly divided into observation group (20 cases) and control group (20 cases)according to the random number table. The patients of control group were treated by conventional resection of papillary adenoma, while the patients of observation group were treated by modified endoscopic nipple resection. The resection rate and the implantation of stent between the two groups were compared. Besides, the occurrence rate of complication and postoperative follow-up between the two groups also were compared. Results: The complete resection rate of observation group (95.00%) was not significant difference with that of control group (90.00%), while en bloc resection rate of observation group (85.00%) significant higher than that of control group (55.00%) (x2=4.28, P<0.05).There was not statistically significant difference between the two groups about the implantation of stent (x2=0.36, P>0.05). In postoperative three months, the difference of recently complication between the two groups was significant (x2=6.14, P<0.05). While the difference of long-term complication between the two groups was no significant (x2=0.36, P>0.05) in more than postoperative three months. After the follow-up of one year, the recurrence rate of observation group (10.34%) was no significant with that of control group (14.29%)(x2=0.17, P>0.05). Conclusion: The modified endoscopic nipple resection can enhance the en bloc resection rate, and reduce the incidence of complications and the recurrence rate. Besides, it has many dominances including higher feasibility in clinical practices. Therefore, its promotion is worthy.
5.Effect of comprehensive intervention residents health literacy in hypertensive patients with diabetes

Objective To investigate the comprehensive intervention residents health literacy with the thera-peutic efficacy of hypertensive patients on diabetes,and to provide a reference for the implementation of clinical inter-ventions.Methods 1 684 cases of community residents were selected as the research objects.Lectures on health,one on one counseling,health manuals mode on the implementation of comprehensive intervention residents health litera-cy.Comparison of the residents after comprehensive intervention China citizen health literacy questionnairesurvey results,and the awareness rate of diabetes,high blood pressure,diabetes hypertension compared after comprehensive intervention,the awareness rate,treatment rate and control rate of blood pressure,blood glucose were investigated. Results After comprehensive intervention,the basic knowledge and concept healthy residents of the ratio of 81.94%,healthy lifestyle and behavior of the ratio of 82.18%to 81.77%the proportion of health,skills,health liter-acy was 54.98%,which were significantly higher than those of intervention,the difference (χ2 =17.88,90.67, 193.72,413.88,all P<0.05);the number of patients of hypertension in 537 cases,the incidence was 31.89%.The prevalence of hypertension in residents after comprehensive intervention,the awareness rate was 64.06%,the stand-ardized treatment rate was 51.02%and the control rate of hypertension was 35.01%,which were significantly higher than those of intervention(χ2 =67.93,24.85,30.66,all P<0.05);prevalence of diabetes in 269 cases,the inci-dence was 15.97%.The prevalence of diabetes in residents after comprehensive intervention,the awareness rate of diabetes was 78.07%,standardized treatment rate was 60.97%,the blood sugar control rate was 37.17%,which were significantly higher than those of intervention(χ2 =68.78,6.33,17.48,all P<0.05).Conclusion The comprehensive intervention residents health literacy is the effective measures to improve the residents'health literacy and health liter-acy,improve help control blood pressure in patients with hypertension and diabetes,glycemic index,which is of posi-tive significance to promoting the control hypertension,diabetes.
6.Comparative study of therapeutic effects of DF and GEM alone regimens of concomitant radiochemotherapy for stage Ⅲ~Ⅳa nasopharyngeal carcinoma

7.Case Analysis and Pharmaceutical Care for a Patient with Chemotherapy-inducedⅣDegree Myelosuppres-sion

Objective:To investigate the breakthrough points and methods of pharmaceutical care performed by clinical pharma-cists for chemotherapy-induced Ⅳ degree myelosuppression. Methods: One advanced lung adenocarcinoma patient suffering from IV degree myolosuppression after being treated with pemetrexed combined with nedaplatin was selected as the example, and the chemother-apy regimen, the cause and treatment of IV degree myolosuppression and the pharmaceutical service could be carried out were ana-lyzed. Results: With the help of clinical pharmacists, the patient conquered chemotherapy-induced myelosuppression, and clinical pharmacists enhanced the awareness of pharmaceutical care and played a positive role in the safe and effective drug use. Conclusion:The participation of clinical pharmacists in clinical pharmaceutical care through providing pharmaceutical service is beneficial to safer and more effective drug therapy.
8.Lymph nodes dissection along bilateral recurrent laryngeal nerve for patients with esophageal carcinoma: thoracoscopic esophagectomy versus open thoracic esophagectomy

One hundred and thirty patients with esophageal carcinoma were randomly asigned into two groups:62 cases received thoracoscopic esophagectomy (TE group) and 68 cases received open thoracic esophagectomy group (OE group).All patients underwent lymph nodes dissection along bilateral recurrent laryngeal nerve.Compared with OE group,TE group had a less blood loss during the lymph nodes dissection (P < 0.05) and a lower incidence of pulmonary infection (P < 0.05).There were no significant differences in the number of dissected lymph nodes,dissection time,incidence of temporary hoarseness,permanent hoarseness and mechanical ventilation for respiratory failure between two groups (P > 0.05).The study suggests that lymph nodes dissection along bilateral recurrent laryngeal nerve can be succesfully completed by thoracoscopic esophagestomy,and it has the advantage of less intraoperative blood loss and lower incidence of pulmonary infection,compared to open thoracic surgery.
9.The features of nail fold capillaroscopy of systemic sclerosis patients and comparison between systemic sclerosis patients and patients with other connective tissue diseases

Objective The aim of the study was to observe the features of nail fold microcirculation in systemic sclerosis (SSc) patients and to compare these findings in SSc patients with patients with other connective tissue diseases.Methods Forty patients with SSc and thirty-seven patients with other connective tissue diseases were included in the study and all the patients reported symptoms of Raynaud's phenomenon in the hands were also included.Nail fold capillaroscopy (NFC) was performed and the abnormality of nail fold microcirculation between the two groups were compared.The relations between nail fold capillaroscopic findings and clinicolaboratory parameters in SSc patients were analyzed.Statistical analysis were carried out by t-test and Chi-square.Results The loss of capillaries and dilated and giant capillaries and hemorrhage as well as neoangiogenesis were hallmarks of the scleroderma capillary findings,which could be detected by nail fold capillaroscopy.The abnormalities of nail fold microcirculation in SSc patients were more severe and more specific than those in other connective tissue disease patients.The total scores of nail fold capillaroscopy test were obviously higher in SSc patients with lung or esophagus involvement than those patients without these organ involvement,meanwhile,the total scores of nail fold capillaroscopic findiugs were elevated in SSc patients with anti-Scl70 antibody than those with negative group.Conclusion The nail fold capillaries of patients with SSc have specific abnormalities,and nail fold capill-aroscopy could distinguish between SSc and other connective tissue diseases,therefore it could be used as a promising tool for early detection of patients who may have the potential to develop scleroderma and it is also helpful in assessing disease severity.
10.CT manifestations and clinical pathology features of hepatic focal nodular hyperplasia in children

Objective To investigate the CT features of hepatic focal nodular hyperplasia (FNH) in children.Methods Thirteen patients with FNH,which was confirmed by postoperative pathology,were enrolled retrospectively.Plain and contrast CT were performed on all patients before operation.The CT imaging features of FNH including size,shape,density,style of contrast were analyzed retrospectively and compared with pathology.Results There were 13 patients with 16 lesions,8 lesions were found in the right lobe,5 lesions in the left lobe and 3 lesions involving both lobes.The tumor size ranged from 5.5 cm to 11.5 cm (media size 7.5 cm) in diameter.Histologically,2 cases were typical type,11 cases were atypical type.The lesions were regular morphology in 12 cases and 1 case with capsule.On plain CT,the lesions were isodensity (n=1) or slightly low-density (n=12).In 2 typical type lesions,there were slit-like,stellate-shaped low density central scars.Arterial phase demonstrated that 12 cases were significantly enhanced and 1 case showed mild enhancement.The central scar was not enhanced.In 12 cases,thickened and torturous arteries were seen.The enhancement was reduced at the portal venous phase in all the lesions,with 10 cases showing slightly higher density,2cases isodensity and 1 case low-density.Two cases showed mild enhancement of the central scar.The enhancement of the solid portion in all lesions decreased at the delay phase,with 12 cases showing isodensity and 1 case slightly low density.Two cases with central scar showed delayed enhancement with slightly higher density.Conclusion The CT features of FNH in children are diversified but distinctive which are related with postoperative pathological findings.Combining with clinical symptoms and CT features can be helpful for the early diagnosis of FNH in children.
