1.Preparation and in vitro Dissolution Behavior Investigation of Frovatriptan Succinate Film-coated Tablet

OBJECTIVE:To prepare Frovatriptan succinate film-coated tablet,and investigate its in vitro dissolution behavior. METHODS:Using lactose monohydrate,microcrystalline cellulose,dioxide,silica,sodium carboxymethyl starch and magnesium stearate as accessories,Frovatriptan succinate tablet was prepared. Using opadry premix spray-coating liquid,Frovatriptan succinate film-coated tablet was prepared. Single factor test was used,using moisture,angle of repose,rigidity,friability,disintegration time and dissolution rate as indexes,to screen the formulation;using dissolution degree as index,coating material dosage was screened. The dissolution curves in vitro of self-made tablets and imported tablets in water,0.1 mol/L HCL,pH of 5.5,6.8 phos-phate buffer solutions were compared. RESULTS:The optimal formulation of Frovatriptan succinate uncoated tablet was as follow as frovatriptan succinate 3.91 mg,lactose monohydrate 99.18 mg,microcrystalline cellulose 33.06 mg,magnesium stearate 1.40 mg,sodium carboxymethyl starch 1.05 mg,silica 1.40 mg;optimal coating weighed quality was 2.0%-4.0%. In the 4 mediums, the dissolution behavior of self-made tablets and imported tablets were similar. CONCLUSIONS:Frovatriptan succinate film-coated tablet is prepared successfully,and its in vitro dissolution behavior is similar to the imported preparations.
2.Effects of health management of stomatology on the marginal bone resorption of minimally invasive implant in the elderly

Objective To evaluate the effects of health management of stomatology on the marginal bone resorption of minimally invasive implant in the elderly.Method Tottally 54 implants were placed in 37 elderly patients of missing teeth in maxillary molar areas and the patients were randomly divided into health management group and control group.Patients in the group of health management were guided to master the proper techniques of oral hygiene practices before and after the implantation and periodontal maintenance were carried out carefully until 1 year after loading of restoration.In the control group, oral hygiene was applied. X-ray and CBCT examinations were made and implant marginal alveolar bone loss (MBL) was measured 3, 6, 9, 12 months after loading with Planmeca PROMAX3D software. Besides, 3, 6, 9, 12 months after loading, improved plaque index (iPLI), modified bleeding index (mBI) and probing depth (PD) were also recorded. SPSS 15.0 was applied to analyze the data. Result Differences in diastolic and systolic blood pressure and changes in heart rate in patients during the intraoperative period were (8.86 ± 0.31) mmHg(1 mmHg=0.133 kPa), (19.18 ± 0.22) mmHg, (20.19 ± 1.84) times/min, which were significantly lower than those of the control group values [(16.55±1.08) mmHg, (28.31±2.30) mmHg, (20.19±1.84) times/min] (P<0.05 or 0.01);at 12 months after the load around the implant in oral health management group, the values of MBL and mSBI were 2.6 (2, 4), 2.1 (2, 4), which were significantly better than control group [4.3(3, 7), 4.3(3, 7)] (P<0.01);simultaneously, there was significant difference in the values of MBL and PD between experimental group (1.42 ± 0.03, 0.77 ± 0.24) and control group (1.59 ± 0.04, 0.54 ± 0.17) (P<0.01). Conclusion Well-performed health management of peri-implant could reduce the MBL and keep healthy soft tissue environment around implants.
3.Painful heating for evaluating trigeminal small nerve fiber function in patients with migraine

Objective To study any functional changes in the trigeminal nociceptive system in cases of migraine with aura (MWA) using painful heat-evoked potentials (CHEPs),and to explore the value of CHEP in evaluating the trigeminal small nerve fibers of patients with MWA.Methods Sixty patients with MWA were recruited as the MWA group,and 60 healthy persons were selected as a normal control group.Thermal stimulation at 51 ℃ was applied to the skin above the patients' right and left eyebrows.The N wave latencies and N-P amplitudes of the CHEPs were recorded at the vertex (Cz) of the head.The intensity of the stimulus was graded using a visual analogue scale (VAS).Results In the control group the simulation produced no significant differences in the N550 or P750 latencies or in the N-P amplitudes.In the MWA group no significant differences were observed when the headache side was stimulated,but the N550 and P750 latencies were both significantly shorter than those of the control group and the N-P amplitude was significantly greater than that of the control group.The average VAS score on both sides in the patients with migraine was significantly higher than those of the control group.However,there was no significant difference in average VAS scores between the headache and non-headache sides of the patients with migraine.Conclusions CHEP is a reliable method for detecting any pathological changes in the trigeminal small nerve sensory pathway.Increased cutaneous allodynia in acute migraine attacks is detectable using CHEPs.
4.In vivo anticancer effects induced by gene transfer of 4-1BBL into murine hepatocellular carcinoma cell line Hepa1-6

Objective To study the effects of 4 1BBL on antitumor immunity induced in vivo by murine 4 1BBL gene transfected hepatocellular carcinoma cell line Hepa1 6. Methods Eukaryotic expression vector pcDNA3 1(+) m4 1BBL was transfered into murine hepatocellular carcinoma cell line Hepa1 6 via lipofectamine method. The transfected cells were selected in RPMI1640 containing G418(400~800 ?g/ml) and termed as Hepa1 6 m4 1BBL.The TCV m4 1BBL was obtained by treating with mitomycin (MMC). Three models (immunological model, early model, and later model) were established for the study of anticancer effects of TCV m4 1BBL. Results The syngeneic mice were completely protected and could survive free from tumor for a long period by inoculation with TCV 4 1BBL. In early models, TCV 4 1BBL showed strong immunotherapy effects( P 0 05). Conclusion The antitumor effect against syngeneic murine hepatocellular carcinoma cell line in vivo is significantly enhanced by the treatment with TCV 4-1BBL.
5.Risk factors and prediction model of catheter-associated urinary tract multidrug resistant bacterial infection in elderly patients

Objective To study the infection of multidrug-resistant organism(MDRO)in elderly pa-tients with catheter-associated urinary tract infection(CAUTI)and analyze the infection factors.Methods The medi-cal records of 175 elderly patients with CAUTI admitted to Jiangsu Province Hospital from January 2021 to July 2022 were selected,the distribution of MDRO infection in elderly patients with CAUTI was analyzed,and the influencing factors of MDRO infection were analyzed.The predictive value of independent risk factors for rele-vant nosocomial infections was analyzed by receiver operating characteristics(ROC)curves.Results Among the 175 elderly patients with CAUTI,87 cases(49.71%)developed MDRO infection and were included in the MDRO infection group),and 88 cases(50.29%)had no MDRO infection and were included in the non-MDRO infection group.A total of 432 MDRO pathogens were detected in the MDRO infection group,inclu-ding 415 strains of Gram-negative bacteria,accounting for 96.06%,186 strains of carbapenem-resistant Acine-tobacter baumannii(43.06%),147 strains of carbapenem-resistant Klebsiella pneumoniae(34.03%),79 strains of carbapenem-resistant Pseudomonas aeruginosa(18.29%)and 3 strains of carbapenem-resistant Escherichia coli(0.69%).There were 17 strains of Gram-positive bacteria(3.94%),all of which were methi-cillin-resistant Staphylococcus aureus.Multivariate analysis showed that the use of carbapenems,tigecycline,polymyxin,and days of central venous intubation were considered as independent risk factors.The area under the curve of MDRO infection was 0.883.Conclusion For elderly patients with CAUTI,it is necessary to take strict antimicrobial management measures,master the indications of antimicrobial application,shorten the hos-pitalization time of patients and reduce the incidence of MDRO infection.

Objective To analyze the role of telehealth?based dialysis registration systems in real?time and dynamic reflection of renal anemia in hemodialysis (HD) patients, and discuss the prospect of its application in dialysis registration management. Methods The Red China project was to build up a dialysis registration system based on the WeChat mobile terminal platform. Demographic and baseline laboratory parameters such as age, gender, primary disease, dialysis age, creatinine were recorded in this system. Hemoglobin (Hb) level was monthly recorded. The platform generated Hb statistics report for each HD center monthly, including the detection rate, target rate and the distribution level of Hb, and released it to physicians through the WeChat terminal of mobile phone. After that, physicians could change the treatment of anemia individually on basis of this report. Here the demographic and baseline laboratory parameters, the detection rate, target rate, the average level and the distribution of Hb from June 2015 to October 2017 after the project launched were analyzed. Results From June 2015 to October 2017, 8392 maintenance HD patients from 28 HD centers in Shanghai were enrolled, of whom 5059(60.3%) were male.The average rate age was (60.5 ± 13.7) years old. Baseline average Hb was (108.3±16.0) g/L. Baseline detection rate and target rate were 54.2%and 47.5%, respectively. After 28 months follow?up, the detection rate of Hb increased from 54.2% to 73.6% (P<0.001), the target rate of Hb increased from 47.5% to 56.1% (P<0.001), and the level of average Hb rose from (108.3±16.0) g/L to (110.7±16.0) g/L. The difference between average Hb in two consecutive months was less than 1.3 g/L. Conclusions The telehealth?based dialysis registration system can timely report the anemia situation of HD patients, which may improve the awareness rate of anemia, the degree of attention and the compliance of anemia monitoring, so as to improve the detection rate and target rate of Hb and reduce the fluctuation of Hb, which helps to maintain the HD patients to correct anemia in a timely, stable and long?term way. The telehealth?based dialysis registration system, as an improved mode of dialysis registration is a promising way for long?term management of renal anemia in dialysis patients.
