2.Nursing of patients with gastrointestinal cancer undergoing continuous hyperthermic peritoneal perfusion chemotherapy in the perioperative period

Objective To study the nursing methods of gastrointestinal cancer patients undergoing continuous hyperthermic peritoneal peffusion chemotherapy (CHPPC) in the peri-operative period.Methods A total of 126 patients with gastrointestinal cancer underwent continuous hyperthermic peritoneal perfusion chemotherapy,and nursing care was implemented continuously,comprehensively and individually.Results All of the patients underwent continuous hyperthermic peritoneal perfusion chemotherapy successfully,without any complication occurred.Conclusions Intensive and effective nursing care is helpful to carry out continuous hyperthermic peritoneal perfusion chemotherapy safely,and avoid the complications.

4.Relationship between serum total bile acid and fetal prognosis in patients with intrahepatic cholestasis of pregnancy

To explore the relationship between serum total bile acid concentration and fetal prognosis in patients with intrahepatic cholestasis of pregnancy (ICP).A total of 131 ICP patients were divided into mild and severe ICP groups according to serum total bile acid concentration.And another 100 normal subjects during the same period were selected as control group.Their medical records and fetal prognoses were analyzed.The serum total bile acid,liver enzyme and lipid concentration of ICP patients were all significantly higher than those of the controls (P < 0.01).The fetal prognoses of ICP patients were significantly different with those of controls (P < 0.05).A high serum level of total bile acid of ICP patients may produce serious adverse effects on fetal prognosis.
5.Three diamensional external fixation frame and limited internal fixation in the treatment of comminuted fracture of the shaft of humerus

[Objective]To explore the curative effect of the three diamen sional external fixation frame and limited internal fixation in treating the comminuted fracture of the shaft of humerus.[Method]Twenty-eight patients of the comminuted fracture of the shaft of humerus were treated from Jan 2000 to May 2006.According to the AO/ASIF,there were 5 cases of the B1 type,3 cases of the B2 type,9 cases of the B3 type,6 cases of the C1 type,4 cases of the C2 type,1 case of the C3 type.All of them were treated by the three diamensional external fixation frame and limited internal fixation of the screws or wires,and all of them were given the reasonable rehabilitation training at 3 day after the operation.The curative effect of the operation,the complications,the fracture healing and the function appraisal were analyzed.[Result]The 28 patients were followed-up for 10~15 months(average 13 months),27 cases had the frature healed and the healing rate was 96.4%.The healing period of the fracture lasted from 12 weeks to 33 weeks,average 19.5 weeks.Only 1 case was not healed.The functions of the shoulder and the elbow were normal,the excellent rate was 92.8%.[Conclusion]The three diamensional external fixation frame and limited internal fixation is a better method in treating the comminuted fracture of the shaft of humerus.It fits the rule of minimal-invasive-biology. The operation is simple,and the fixation is reliable,the function is satisfactory.
7.Comparison of placement of peripherally inserted central catheters using vascular ultrasound guidance system and traditional method in 938 tumor patients

Objective To compare the placement of peripherally inserted central catheters(PICC)by using vascular ultrasound guidance system and traditional method.Methods Totally 938 patients undergoing PICC were divided into the traditional method group and the vascular ultrasound system and microintroducer techniques group according to the puncture time.The differences in the one-attempt success rate and the overall success rate of the placement and the incidences of complications were compared between the two groups.Results The one-attempt success rate was 93.01% in the traditional method group and 98.76% in the vascular ultrasound system and microintroducer techniques group(P =0.005).The successful rate of PICC placement was 100%.The incidence of complication was 11.29% in the traditional method group and 2.47% in the vascular ultrasound system and microintroducer technique group(P =0.000).Conclusion The PICC placement using vascular ultrasound guidance system and microintroducer techniques can increase one-attempt success rate and decrease complications.
8.Role of Nav1.7 in dorsal root ganglia in a rat model of diabetic neuropathic pain

ObjectiveTo investigate the role of Nav1.7 in dorsal root ganglia (DRG) in a rat model of diabetic neuropathic pain (DNP).MethodsThirty-two female Wistar rats aged 3 months weighing 180-220 g were randomly divided into 4 groups ( n = 8 each):control group ( group C),sham operation group ( group S),DNP group and ProTx- Ⅱ (a selective Nav1.7 blocker) group (group E).Diabetes mellitus was induced by intraperitoneal streptozocin 65 mg/kg.Blood glucose level and mechanical paw withdrawal threshold (MWT)to von Froy filamentstimulation were measured 2 weeks later.DNP was confirmed by blood glucose level ≥ 16.0 mmol/L and MWT decreased by more than 50% of the baseline value.Intrathecal catheter was implanted at L5,6 interspace on day 10 after successful induction of DNP.On day 4 after placement of the intrathecal catheter,ProTx- Ⅱ 10 μg/kg was injected intrathecally in group E,while the equal volume of normal saline was given in groups DNP and S.MWT and never conduction velocity (NCV) were measured 1 h after intrathecal injection.The rats were then sacrificed and DRGs of the lumbar segment (L4-6) were removed for determination of Nav1.7 protein expression (by immuno-histochemistry and Western blot) and Nav1.7 mRNA expression (by RT-PCR).ResultsThe MWT and NCV were significantly lower and the Nav1.7 mRNA and protein expression was significantly higher in groups DNP and E than in group C.ProTx- Ⅱ significantly attenuated the diabetes-induced changes in MWT,but had no effect on NCV and Nav1.7 mRNA and protein expression.ConclusionNav1.7 in DRG is involved in the maintenance of DNP in rats.
9.Expression of COX-2 and VEGF in non-hodgkin lymphoma and its clinical significance

Objectiye To explore the expression of COX-2 and VEGF and its clinical significance in non-Hodgkin lymphoma (NHL). Methods The expression of COX-2 and VEGF were detected by immunohistochemistry in 42 cases of NHL and 20 cases of lymph node with benign pathological change. Results The positive rate of COX-2 and VEGF was 45.24% and 73.81% in NHL respectively. The expression rate of VEGF was positively correlated with that of COX-2 in tissues of NHL ( x2 = 4. 63, P < 0. 05).The expression of COX-2 was related to clinical stage and histopathologic grade of NHL ( x2 = 5.43, P <0. 05), but it had no association with gender, age, B symptoms, and IPI. The expression of VEGF was significantly related with aggression, B symptoms and IPI ( x2 =8. 979, 8. 893,6. 434, P <0. 05), but it had no association with age, gender and clinical stages. Conclusion COX-2 and VEGF may be involved in NHL tumorgenesis, and COX-2 may accelerate angiogenesis by increasing VEGF expression. Specific COX-2 inhibitors may be a novel therapeutic approach for NHL.
10.Satge I clinical study of dose escalation of capecitabine during intensity modulated radiotherapy concurrent chemotherapy for local regional advanced nasopharyngeal carcinoma

Objective To decrease radiation induced toxicities especially mucostis in patients with locally advanced nasopharyngeal carcinoma( NPC ) who underwent concurrent radiochemotherapy, the maximum tolerated dose and dose limited toxicities of capecitabine combination with cisplatin were observed. Methods From Aug 2006 to Oct 2007, 24 patients with intensity modulated radiotherapy(IMRT) and concurrent chemotherapy with capecitabine and cisplatin for nasopharyngeal carcinoma(stages Ⅲ-Ⅳ) were enrolled in this study. There were four dose-level groups of Capecitabine[625-1250 mg/(m2 ·d) , d1-14]and fixed cisplatin dose[20 mg/(m ·d) ,d1-5) ]MRI and CT scan were used for evaluation of tumor shrinkage. Treatment related toxicities were evaluated according to the common toxicity criteria( NCI-CTC Version 3.0). Results The acute side-effects include Grade 3 or Grade 4 mucosal toxicity(lasting for at least 5 d) and Grade 3 or Grade 4 non-mucosal toxicity were evaluated. Group 625 mg/m2 and Group 825 mg/m2 had none, Group 1000 mg/m2 had 6 patients and Group 1250 mg/m2 had 3 patients for mucosal toxicity, which were the main dose-limited toxicity and relevant to the dose of capecitabine apparently( P < 0. 05 ). There was also a trend of increase by the dose level of capecitabine for other toxicities. The median follow-up time for all patients was 28. 5 months. The locoregional recurrence occurred in 2 patients and distant metastasis in 2 patients. Two-year overall survival rate and locoregional control rate were 100% and 91.7%, respectively.Complete response and partialresponse were found on MRI or CT scan in patients of 29. 2% at the end of treatment and 83. 3% after three months, respectively. Conclusions The combination regimen of capecitabine and cisplatin is safe and effective according to the preliminary result. Toxicities related to radiochemotherapy for NPC were significantly associated with the dose level of chemotherapy.
