1.Clinical efficacy of oxaliplatin-based systemic chemotherapy in treating advanced hepatocellular carcinoma:a com-parative study of two regimens

Objective To evaluate the therapeutic efficacy and toxicity of oxaliplatin (OXA)-based systemic chemotherapy in patients with advanced hepatocellular carcinoma (HCC).Methods Thirty patients with advanced HCC admitted to our hospital from June 2009 to June 2013 were randomly divided into CapeOX group (15 patients,treated with OXA and capecitabine or CAP)and FOLFOX group (15 pa-tients,treated with OXA and calcium folinate or CF,followed by fluorouracil or 5 -FU).The therapeutic effects were evaluated after two cycles of treatment using the Response Evaluation Criteria in Solid Tumors (RECIST 1 .1 ).Assessment criteria included overall response rate (OR),time to tumor progression (TTP),and overall survival (OS)analyzed by Kaplan-Meier survival curves,as well as the toxicity profile of the combination chemotherapy.Comparison of OR was made by chi-square test;assessment of tumor response and toxicity profile was performed by Mann-Whitney U test;OS and TTP were analyzed by log -rank test.Results In the CapeOX group,the OR was 13.3%,and the mean OS and TTP were 10.4 months and 5.0 months,respectively.In the FOLFOX group,the OR was 6.7%,and the mean OS and TTP were 9.0 months and 4.7 months,respectively.The differences between the two groups had no statistical significance in all three parameters (P=0.543,0.606,and 0.769,respectively).Compared with the FOLFOX group,the CapeOX group had significant-ly lower toxicity rates in gastrointestinal tract and myelosuppression (P=0.006 and 0.002,respectively).Conclusions The OXA-based systemic chemotherapy shows anti-tumor effects for advanced HCC,and there is no significant difference in efficacy between the two regi-mens.Decisions regarding the choice of specific treatment should be based on the patients'clinical conditions.

3.Mechanical analyses on urethra wall after suburethral slings for female pig stress urinary incontinence

Objective To analyze the mechanical mechanism of suburethral slings by measuring the mechanical properties of female pig urethra wall and polypropylene mesh.Methods The uniaxial tension tests of 49 female pig urethra wall strips and 27 polypropylene mesh samples were performed to test the mechanical properties.Results The stress and strain of urethra wall displayed a parabola shape and its elastic modulus was proportional to the square root of stress as well as the strain.The maximum calculated elastic moduli of the urethra wall were 0.806 MPa and 0.818 MPa correspondingly calculated with stress and strain.The elastic modulus of polypropylene mesh was(6.977?1.095) MPa.The ultimate stress of polypropylene mesh was significantly higher than that of the urethra wall,and the ultimate strain of polypropylene mesh was significantly lesser than that of the urethra wall.The calculation results suggested that the suburethral slings treatment of poly-propylene mesh decreased the deformation to 1/3 of the untreated stress urinary incontinence.Conclusion Polypropylene mesh has special mechanical properties that suit suburethral slings and enhance the support to the urethra,and improve the continence.
4.Analysis of Distribution Characteristics and Influencing Factors on Adults Abnormal Blood Pressure

Objective To study the relationship between abnormal blood pressure distribution characteristics and the influencing factors for the prevention of hypertension and low blood pressure to provide reference. Methods Regarding the Guangzhou City adult group as the research object,we analyzed the distribution of the adults abnormal blood pressure in age, gender , urban and rural areas, as well as of occupations. Results The detection rate of blood pressure abnormalities was 15.9 % , which accounted for 12.4% of hypertension, 3.5% of hypotension. On the whole, the detection rate of hypertension was 18% in rural, higher than 9.8% in urban. The detection rate of hypotension in rural areas was 2.3% , 4.0% in urban. The detection rate of hypertension of men was high than that of women, and the detection rate of hypertension gradually increased by the age increased. At the same time, hypertension and hypotension was significantly affected with the factors of education, urban and rural areas, obesity and so on. Conclusion The detection rate of hypertension is high in rural areas than that in urban,and men than women, and the detection rate of low blood pressure is on the opposite. Both of them is significantly affected by the factors of education, urban and rural areas, obesity and so on.

7.Analysis of the results of multifocal visual evoked potential examination in patients with anterior ischemic opticneuropathy before and after treatment

Objective To observe the results of multifocal visual evoked potential (mfVEP) examination in patients with anterior ischemic optic neuropathy (AION) before and ater treatment,and to probe its clinical significance.Methods A total of 90 patients (90 eyes) with AION were examined by mfVEP;the second-order reaction of mfVEP was analyzed.The reaction was divided into upper and lower hemi-field of visual field,or 1 / 4 quadrant visual field (superior nasal,inferior nasal,superior temporal,and inferior temporal).The sum of waves of each response was analyzed and the results in various regions were compared.The features of wave configuration was compared between the AION eyes and the contralateral eye,and between the AION eyes before and after treatment.Results The amplitude and latency of P-wave of mfVEP was 0.198±0.033 and 100.197±7.354 respectively in AION eyes before treatment,and was 0.271±0.024 and 98.567±6.794 in the contralateral eyes;the difference was significant (t=16.556,18.330;P<0.01).The amplitude and latency of P-wave of mfVEP was 0.229±0.016 and 100.104±10.603 respectively in AION eyes after treatment,which differed much from that before the treatment (t=13.649,8.858;P<0.01) and also from that of the contralateral eyes (t=13.649,8.858;P<0.01).Conclusions The amplitude and latency of P-wave of mfVEP may accurately reflect the recovery of local optic nerve damage in AION eyes before and after treatment with good repeatability.AION can be used as a new method for AION diagnosis and detection of the prognosis.
8.Cerebral function assessment in the acute phase of neonatal bacterial meningitis caused by different pathogens

Objective To investigate the common pathogens of neonatal bacterial meningitis in the past 5 years,and to evaluate the brain injury in the acute phase through amplitude-integrated electroencephalogram (aEEG),brainstem auditory evoked potential (BAEP) and brain MRI.Methods Sixty children were selected from the past 5 years who were treated in Department of Neonatology,Guangzhou Women and Children's Medical Center in March 2011 to March 2015 as the objects of study.According to the results of etiological culture,the children were divided into streptococcus lactis group (14 cases),escherichia coli group (10 cases),other positive bacteria group (11cases) and culture-negative group (25 cases).The results of aEEG,BAEP and brain MRI of brain injury in the acute phase of these 4 groups were compared.Results Twenty-nine cases of the 60 patients (48.3%) showed positive blood culture,and 14 cases (23.3%) showed positive cerebrospinal fluid (CSF) culture.Streptococcus lactics and escherichia coli were found to be the most common pathogens that caused neonatal bacterial meningitis.By evaluating the cerebral function in the acute phase of 57 cases,it was found that aEEG total abnormal rate was 61.4%,escherichia coli group abnormal rate was 80.0%,while moderate to severe damage seemed to be the most remarkable feature.The abnormal rate was of statistical significance between escherichia coli group and culture-negative group (x2 =3.941,P =0.047).Forty-eight cases manifested potential anomaly evoked by brainstem auditory,with the total abnormal rate as 84.2%.A significant increase in the ratio of severe hearing loss was found in children with bacterial meningitis which was caused by escherichia coli and streptococcus agalactiae.The abnormal rate was of statistical significance between escherichia coli group and culture-negative group (x2 =4.399,P =0.036),and hearing damage caused by escherichia coli was more serious than that in other bacteria group.Of these 57 cases,MRI total abnormal rate was 77.2%,with hydrocephalus as the most common complication.Of the 44 abnormal cases,16 cases showed hydrocephalus,6 cases of which were from the escherichia coli group.The second most common complication was subdural effusion.And another 5 cases showed cerebral softening,3 cases of which were from the escherichia coli group.The data suggested that escherichia coli meningitis easily combined hydrocephalus and brain softening.Conclusions The main pathogens of neonatal bacterial meningitis is streptococcus agalactiae and escherichia coli.The brain injury in neonatal bacterial meningitis caused by escherichia coli is more serious than those caused by other pathogens.
9.Significance of triggering receptor expressed on myeloid cells-1 of cord blood leukocytes in neonates

Objective To investigate the triggering receptor expressed on myeloid cells-1(TREM-1) of cord blood leukocytes in neonates and the transcription level of mRNA, and analyze its promoting function of inflammatory cytokine secretion. Methods During the period from September 2013 to March 2014, cord blood was collected from 20 term neonates at the time of birth, and peripheral blood was collected from 20 healthy adults. The expression of TREM-1 and TREM-1 mRNA on leukocytes was observed using flow cytometry and real-time reverse transeription-polymerase chain reaction, respectively. After the whole cord blood was stimulated by lipopolysaccharide (LPS) or LP17 plus LPS, the contents of interleukin (IL)-6, tumor necrosis factor-α (TNF-α), IL-8 and soluble TREM-1 (sTREM-1) in the supernatant were analyzed by enzyme linked immunosorbent assay. The statistical significance was determined using the one-way ANOVA test, t test, q test and Pearson correlation coefficient. Results The mean fluorescence intensity of TREM-1 on leukocytes of newborns was not different compared with healthy adults (P>0.05), while the percentage of TREM-1 positive on polymorphonuclear cells was lower than that of healthy adults [(82.3±7.1)% vs (98.6±4.8)%, P<0.05]. The level of TREM-1 mRNA in newborns was lower than in healthy adults (1.16±0.13 vs 1.63±0.24, t=7.714, P<0.01). The LPS treatment significantly increased sTREM-1 in newborn whole blood compared with the control treatment [(156.7±36.3) vs (34.6±6.1) pg/ml, t=13.623, P<0.01]. The concentration of IL-6, TNF-αand IL-8 decreased significantly when TREM-1 was blocked by LP17. In addition, the concentration of sTREM-1 showed a positive correlation with the levels of TNF-α(r=0.519, P<0.05), IL-6 (r=0.507, P<0.05) and IL-8 (r=0.538, P<0.05). Conclusions Healthy newborns exhibit expression of TREM-1 on monocytes similar to healthy adults, and most PMNs express TREM-1 at the newborn stage. Blocking the TREM-1 signal transduction pathway may reduce inflammatory responses of neonate leukocytes.
10.Effects of aging on vestibular evoked myogenic potential.

OBJECTIVE:
The purpose of this study is to assess the effects of aging on air-conducted sound elicited cervical vestibular evoked myogenic potential (cVEMP) and ocular vestibular evoked myogenic potential(oVEMP) in normal people.
METHOD:
Sixty normal subjects were recruited for the vestibular evoked myogenic potential examination. Among them, 20-40 year-old group was setted as the first group, 41-60 year-old group was setted as the second group, > 60-year-old group was setted as the third group. Each group included 20 normal subjects. SPSS 16.0 software was used to analyze the data.
RESULT:
As the age growing, P1 latency prolonged, N1-P1 amplitude decreased, and amplitude asymmetric ratio increased in cVEMP; N1 latency prolonged, P1-N1 amplitude decreased, and amplitude asymmetric ratio increased in oVEMP.
CONCLUSION
With the age growing, the time of otolithic organ input pathways prolonged, the function of otolithic organ decreased and the decrease level may be asymeetrical on both sides.
