2.Regulatory effect of glycyrrhizin on the expression of interferon (IFN)-γ and tumor necrosis factor (TNF)-β in peripheral blood mononuclear cells of patients with alopecia areata

Objective To observe the regulatory effect of glycyrrhizin on the expression of IFN-γ and TNF-β in peripheral blood mononuclear cells (PBMCs) of patients with alopecia areata.Methods PBMCs were obtained from 18 patients with mild alopecia areata,24 patients with severe alopecia areata and 20 normal human controls,and cultured with phytohemagglutinin (PHA) or the combination of PHA and glycyrrhizin for 24 hours.Then,reverse transcription (RT)-PCR was conducted to detect the mRNA expression of IFN-γand TNF-β in these cells.Results The mRNA expression levels of IFN-γand TNF-β in PBMCs were significantly higher in patients with severe alopecia areata than in those with mild alopecia areata and normal human controls (all P < 0.05),and higher in patients with mild alopecia areata than in normal human controls (both P < 0.05).A significant decrease was observed in the mRNA expressions of IFN-γ and TNF-β in the PBMCs from patients with alopecia areata after stimulation with the combination of PHA and glycyrrhizin (both P <0.05).Conclusion Glycyrrhizin can inhibit the expression of Th1-type cytokines and reverse Th1-type immune response.
3.The animal models of H5N1 avian influenza virus

H5N1 subtype of avian influenza virus (AIV) has been prevailing in many countries over the world, which has threaten human health. The establishment of AIV animal models may provide a tool for the study on the infection, mutation of avian influenza virus and pathogenesis of the disease. The pathogenicity of H5N1 to mammalian such as cynomolgus macaques, ferrets, mice, rats, gerbils, cats were summarized in current review, which may provide meaningful help for the establishment and study on ideal experiment animal models.
4.IODINE NUTRITION STATUS OF POPULATION IN THE AREAS WITH DIFFERENT IODINE CONCENTRATIONS OF DRINKING WATER

Objective:To study the master iodine nutrition status of population living in areas with different iodine in drinking water and the cut-off point of stopping iodized salt supply and the range of adequate iodine intake for the strategy of prevention from iodine deficiency or excess. Method:Thirteen townships points along the Yellow River downstream in Shandong were investigated and divided into six groups(A-F) based on water iodine concentrations of 0-50,50-100,100-150,150-300,300-800 and over 800 ?g/L. The indicators of water iodine,edible salt iodine,urinary iodine(UI) and thyroid goiter were observed. Results:From A,B,C,D,E and F group,the medians of water iodine concentration were 20.3,91.4,143.2,203.6,341.9 and 812.3 ?g/L,of edible salt iodine were 0,25.7,25.8,30.4,36.4 and 33.0 mg/kg,and of UI were 116.8,354.2,400.4,607.9,881.3 and 1 213.8 ?g/L respectively. The goiter rates were 10.8,8.6,15.0,14.2,14.9 and 25.0 % respectively . The sample proportion of UI below 100 ?g/L was 8.2%,of 100-300 ?g/L was 18.2% and of over 300 ?g/L was 73.6%. Frequency excursion of UI had a high trend towards water iodine concentration over 90?g/L in 5 groups. The proportion of UI over 300 ?g/L increased,consistent with higher water iodine level. Median UI of all groups with iodized salt decreased significantly,especially group F. There were significant correlations between UI and water iodine. Iodine nutrition status was markedly excessive in group B,C,D,E and F. Conclusions:Iodine nutrition status of most residents consuming drinking water with iodine concentration about 20 ?g/L should be adequately supplied with eligible iodized salt. Iodine intakes in groups B-F were excessive,and iodized salt supply should be stopped there. The cut-point of water iodine level where iodized salt stopped could possibly be 90 ?g/L,and the range of adequate iodine intake,in terms of drinking water,could be defined at range of over 20 to 90 ?g/L in iodine concentration.
5.The impact of operative timing on clinical outcomes in elderly hip fracture

Objective To study the effect of operative timing on clinical outcomes in elderly hip fracture. Methods Two hundred sixty-seven patients with hip fracture were treated from July 2006 to May 2008. The patients who had an operation within two days (the early surgery group) were compared with the outcome for those who had an operation more than two days (the later surgery group). Outcomes measured were mortality, post-operative complications, operative details, the comorbidity and the activities of daily living of patients. Results One hundred and sixteen patients were included in the study. There were 45 cases in the early surgery group, including 13 males and 32 females. The patients ranged from sixty-five to eightyeight years old, with an average age of 78.76 years. While there were 71 patients in the late surgery group,including 24 males and 47 females. The patients ranged from sixty-six to one hundred and three years old,with an average age of 80.00 years. The demographic data of patients show no significant difference in two groups. There was no difference in intraoperative conditions such as anesthesia, amount of bleeding, type of fixation between two groups. Patients in early surgery group experienced shorter length of hospital stay, and fewer pulmonary complications. The mortality of early surgery group do not show a significant increase at discharge, 6 month, 12 month postoperatively compared to that of the later surgery group. However patients in the early surgery group show a significant increase of ADL scores at discharge, 6 month postoperatively,but not at 12 month. Conclusion The timing of surgery has an impact on clinical outcome in elderly hip fracture. Early surgery was associated with improved function in the first 6 month postoperatively, but not 12months, but it was not associated with mortality.
6.Practice and review of the construction of standardized medical resident training bases for the first batch of residents in Xinjiang

Objective Through investigating the current situation of first national Standardized Medical Resident Training Bases construction in Xinjiang to provide the basis for the administrative department of public health to improve their resident training bases.Methods Selecting standardized training related personnel 133 people of 20 base hospital to conduct a questionnaire survey,and selecting the base hospit.al leaders,the management staff of functional departments,teachers and residents,a total of 254 people were given in-depth interviews.We conducted data collection mainly around the basic situation of the base hospitals and departments,to understand the basic situation,difficulties and problems of the standardized training of resident doctors in the base hospitals,and suggestions and opinions on the training work.We used x2 test,Wilcoxon rank test to make statistical analysis.Results Basic conditions of the hospital was not balanced,70% of the base hospital library number was 10 thousand ~5 million,75% of the base hospital computer had a volume of less than or equal to 30.There was statistical significance in the difference of the rate of operation between the cases and the skills in the southem/northeru region teaching/non professional teaching base hospital.Teacher level was not balanced,and only 5 hospital teacher training rate was greater than or equal to 80%,which met the national requirements.Enrollment of each department had a rising trend,including the department of pediatrics,psychiatry and other major shortage of profes-sional year by year,the increase in the proportion of 1 ~2 times.National financial aid was implemented,but lacked local financial input.Conclusion The balanced development of base construction,the continuous improvement of system construction and organization management,the improvement of the teachers' teaching and training quality,the improvement of the resident treatment and striving for more national and local financial allocation will be the focus of the construction and development of Xinjiang resident standardized training base.
7.The effectiveness of wechat-based transitional care in the acoustic neuroma patients with postoperative facial palsy

Objective To evaluate the effectiveness of wechat-based transitional care in the acoustic neuroma patients with postoperative facial palsy.Methods 90 acoustic neuroma patients with postoperative facial palsy were randomly enrolled into 2 groups by tossing coin,45 cases in each group.Control group received routine discharge education,while the experimental group received wechat-based transitional care for three months.The rehabilitation adherence,the level of facial palsy,the ocular infection and the quality of life at patients discharged and three months after operation were compared between the two groups.Results Three months after operation,the cases of high,middle,low level of rehabilitation in the experimental group were 28,12,5,which were more than the control group whose cases were 15,21,9 (x2=7.528,P< 0.05).Facial palsy and the quality of life of the experimental group were 66.7% (30/45) and (71.62±6.36) points,which was significantly higher than 42.4% (19/45) and (63.75±11.28) points in the control group (x2=5.421 and 4.073,P < 0.01).The incidence of ocular infection in the experimental group was 8.9% (4/45) which was significantly lower than 31.1% (14/45) in the control group (x2=6.671,P <0.05).Conclusions Wechat-based transitional care achieves good effectiveness in patients with postoperative facial palsy,which could improve the level of rehabilitation,facial palsy and the quality of life,and reduce the incidence of ocular infection,is worthy of promotion.
8.Curve of labor duration in 2 140 nulliparous women

Objective To analyze labor duration in nulliparous women and discuss the change about curve of labor duration. Methods Two thousand, one hundred and forty nulliparous, full-term pregnant, singleton, cephalic presentation and vaginal delivery women who delivered at Peking University First Hospital were included. There were 1 300 cases between January 1, and December 31, 2009, while 840 cases between January 1, and May 31, 2013. A retrospective study was conducted. Data on maternal age, gestational age at delivery, body mass index at delivery, newborn weight, time of labor initiation, cervical dilatation and the amount of bleeding within the first 24 h after delivery were recorded. Data were compared by t test and χ2 test. The median time span corresponding to one-centimeter-increase in cervical dilatation was calculated. Results (1)Compared with data from 2009, the maternal age [(29.0±3.0) vs (29.6±2.8) years, t=4.77], incidence of postpartum hemorrhage [1.8%(24/1 300) vs 4.3%(36/840),χ2=11.17], proportion of induced labor [37.3%(485/1 300) vs 64.0%(538/840),χ2=146.23] and proportion of analgesia during labor [54.2%(705/1 300) vs 61.5% (517/840), χ2=11.15] were all higher in 2013 (all P<0.01). (2)The median (minimum–maximum) time span corresponding to a one-centimeter-increase in cervical dilatation was 3-4 cm which corresponded to 2.2 h(0.8-4.3 h), 4-5 cm which corresponded to 1.9 h(0.6-4.0 h), 5-6 cm which corresponded to 1.8 h(0.5-4.0 h), 6-7 cm which corresponded to 1.6 h(0.5-2.0 h), 7-8 cm which corresponded to 1.8 h(0.8-2.0 h), 8-9 cm which corresponded to 1.3 h(0.2-2.5 h), and 9-10 cm which corresponded to 0.6 h(0.1-2.5 h). The curve of labor duration showed a slow uptrend with time on the horizontal axis and cervical dilatation on the vertical axis. Conclusions The curve of labor duration exhibits a slow uptrend with neither an acceleration phase nor a deceleration phase. It is important to redefine the time span of labor duration in China for appropriate clinical treatment.
9.Effect of Yuzhenxifeng Decoction on the brain ferritin in PD mouse models

Objective To study the effect of Yuzhenxifeng Decoction on the brain ferritin in PD mouse models .Methods All ex‐perimental animals were devided into 4 groups:the control group ,the model group ,the positive drug group and TCM group .Prepare PD model mice with MPTP ,then use the immunohistochemical staining technique to observe the change of expression of Fn .Results the results showed that compared with the model group ,movement coordination disorder of rats in TCM group were relieved ;the Fn level of the model group on 6th ,13th ,20th were higher than control group(P<0 .05) ,and were 2 .21 times ,1 .15 times and 0 .36 times higher respectively .Compared with model group mice ,expression of Fn were enhanced in the treatment group .Conclusion Yuzhenxifeng Decoction can improve the expression of Fn in the brain ,which provide the basis for further study on mechanism of the treatment of Parkinson′s disease .

Objective:
To assess the role of T cell immunoglobulin and mucin domain-containing protein 3 (TIM-3) in regulation of interleukin (IL) -12 secretion by CD14+ peripheral blood monocytes from patients with psoriasis vulgaris.

From November 2017 to March 2018, a total of 47 patients with psoriasis vulgaris (psoriasis group) and 19 healthy volunteers (control group) were enrolled from Shanxi Provincial People′s Hospital.
