Objective To probe into the effect of comprehensive nursing intervention on curative compliance of migrant TB patients. Methods One hundred and eighty migrant patients with tuberculosis smearing positive were divided into control group and observation group in equal number according to the registration time.The patients in the control group received routine nursing care and those in the observation group comprehensive nursing intervention including psychological intervention,health education and family supervision for 6 months. The two groups were compared in terms of the curative compliance.Results After the intervention,the observation group was significantly better than the control group in terms of re-visit,regular mediation,drugs taking from the outpatient section and sputum culture after intervention (P<0.05).Conclusion Comprehensive nursing intervention is effective in improving the treatment compliance of migrant TB patients.

Objective Probing into the rationale of DRGs payment standards for references to making DRGs payment standards.Methods Based on the DRGs grouping results of circulatory diseases cases admitted by a tertiary hospital from 2003 to 2005,two typical combinations were selected to determine the payment standard for each DRGs combination.Unnecessary medical expenses were excluded.Results According to the percentage of unnecessary medical expenses,Angina,non-special nursing,and class 1 nursing were in a group,with unnecessary medical expenses per ease of 758.00 yuan;CAG,single stent implantation,acute myocardial infaction as the main diagnose were in a group,with medical expenses per case of 2814.03 yuan;such expenses account for 6.20% of the total hospitalization expenses of both groups.Conclusions If the average expenses with unnecessary medical expenses excluded are set as the combined payment standard,hospitals will be restrained from unnecessary medical behaviors and expenses so incurred.

BACKGROUND: Alienation is derived from dissimilation in philosophy.It is practically defined as the subjective state of individuals who develop negative feelings such as social isolation, uncontrollable and meaningless sense, oppression, and self-alienation because they are alien to others, the.society, nature and themselves, and even feel controlled.OBJECTIVE: To investigate alienation from psychological perspective,aiming to construct and verify the theoretical hypothesis of alienation,thereby working out alienation scale for adolescent students.DESIGN: Cross-sectional investigation.SETTING:National Key Research Center of Basic Psychology and National Key Liberal Arts of Southwest Normal University.PARTICIPANTS: The investigation was carried out from March to June 2000. The method of cohort stratified random sampling was adopted and the questionnaire was administered to adolescent students in Chongqing City and Lanzhou City. Inclusion criteria: volunteer students of grade one in junior high school to grade four in colleges. Exclusion criteria: Those who had incomplete data or had differences of more than 6 points in the total scores for three matched lie-testing questions in the questionnaire. A total of 1502 pieces of questionnaire were collected (including 1 066 questionnaires used for exploratory factor analysis and 436 questionnaires for verifying factor analysis); the age of the students covered the whole range of adolescence, and the boy-to-girl ratio in each grade was matched.METHODS:We combined literature analysis, investigation research and interview. The details are as follows: First, theoretical hypothesis was put forward based on literature analysis; then related variances were collected through interviews and open questionnaires, which were compared with the items in scales used home and abroad at present so as to work out predicting questions for adolescence alienation questionnaire. Finally, 52 items were screened out and used to construct the preliminary questionnaire. The formal alienation scale was made after standardized designation of behavioral variances, and verified for alienation analysis in adolescent students.MAIN OUTCOME MEASURES: Behavioral apparent variances were collected based on interviews and open questionnaires, and used as psychological potential variances after statistical processing.RESULTS:Data of 1502 pieces of questionnaire were analyzed.① The correlation coefficient of each item in the scale was over 0.35. ② The correlation coefficient of each item with the dimension was over 0.30, with statistical significance(P ＜ 0.01). ③ The reliability coefficient of each item was over 0.39 with great significance(P ＜ 0.01). ④ Alienation was displayed in three dimensions: social alienation, interpersonal alienation and surrounding alienation, which were associated with the correspondingdimensions of related scales (P ＜ 0.05-0.01). ⑤Alienation could be divided into 9 levels: sense of meaninglessness, sense of self alienation, isolation, nature alienation, parent alienation, environmental alienation, sense of uncontrollability and social alienation. Parameters that accorded with the theoretical hypothesis and used for verifying analysis were proved rational.Based on the analysis of the 9 first-rank factors, second-rank structural model of 3 unrelated factors were obtained using main component analysis and the maximum variance cross rotation with the optimal coefficients of simulated parameters of: X2/df=4.87;GFI=0.94;A GFI=0.90;RMR=0.0S;NFI=0.93;NNFI=0.92;CFI=0.95;IFI=0.95.CONCLUSION: Alienation scale has been proved to be reliable and valid. It can be used to assess alienation in adolescent students and has potential value of application in healthy psychological development and psychological education.

AIM:To develop a HPLC method for determing the contents of schizandrol,schisantherin and deoxyschizandrin in Anshen Buxin Pills(Radix et Rhizoma Salviae miltiorrhizae,Fructus Schisandrae chinensis,Rizoma Acori tatarinowii,etc.). METHODS: The samples were extracted with methanol through supersonic wave and then filtered.The determination was performed by HPLC on Shim-pack VP-ODS C_(18) column using methanol-water-acetic acid glacial(65∶35∶0.1) as a mobile phase,the flow rate was 1.0 mL/min,the detection wavelength was at 220 nm. RESULTS: The linear ranges of schizandrol,schisantherin and deoxyschizandrin were 0.001 2-(0.050 0) ?g(r=1.000 0)、0.001 4-0.057 6 ?g(r=0.999 5) and 0.002 1-0.060 8 ?g(r=0.999 8),respectively.The average recoveries of three components were 99.51%,99.40% and 99.06%,respectively.(CONCLUSION:)The method is accurate,quick,sensitive and can be used for measurement of schizandrol,schisantherin and deoxyschizandrin in Anshen Buxin Pills.

The purpose of the present study was to assess the correlation that likely exists among increased portal pressure (Pp), portal blood flow quantity (Qp) and ETA and ETB receptor mRNA expression in human cirrhosis. In situ hybridization and reverse-transcription polymerase chain reactions (RT-PCR) were performed to determined the expression of ETA and ETB receptor mRNA in liver tissues from traumatic subjects (n = 10) and cirrhotic patients (n = 15) in whom hepatic hemodynamic values were measured. The expression of the two transcripts was significantly higher in liver samples of cirrhotic patients than in those obtained from traumatic subjects. It has shown that ETA receptor mRNA predominantly located in hepatic stellate cells (HSCs) and vascular smooth muscle cells of intrahepatic arteries and portal veins, ETB receptor mRNA in HSCs, sinusoidal endothelial cells and Kuppfer cells. There was a highly significant direct relationship between ETA and ETB receptor mRNA and Pp and Qp in cirrhotic patients. It suggests that liver paracrine endothelin system may be overactivated in human cirrhosis accompanied with increased expression of ETA and ETB receptor mRNA which may play an important role in the pathogenesis and maintenance of splanchnic hyperdynamics.

Objective To estimate the risk factors of fall happening in the old inpatient of the department of psychiatry and make up preventing measures.Methods 128 old inpatient(36 male cases and 92 female cases,whose age were older than 60) were surveyed.All of the patients were evaluated for risk factors and were interfered by safe nursing and follow-up in duration of hospital stay and in one year post-discharge in order to prevent the occurrence of accident fall.Results Of the 128 investigation patients there were 49 patients(38.3%) who had fall history in the past year.And the female falling patients were 39,while the male falling patients were 10.There were no significant statistical difference between the male and female cases(P>0.05).Of the 128 investigation patients there were 2 fall patients(1.6%) after 1 year safe nursing interference(P<0.001).The risk factors found by Logistic analysis were non-cooperation,poor action ability,blurred vision and taking antipsychotic drug.Conclusion The risk ratio of old inpatient of the psychiatric department is high.The accident of fall should be prevented efficiently.

ObjectiveTo observe the speech outcome of globle aphasia after rehabilitation.Methods9 patients with globle aphasia from Dec. 2008 to June. 2010 in our hospital were assessed with the Standard Aphasia Examination of Chinese(CRRCAE) and trained using the Schuell aphasic stimulation approach, promoting aphasics communication effectiveness (PACE) and functional communication therapy (FCP).Results9 patients were disabled with their faces, 8 cases were disabled with speech. 9 cases improved in understanding, retelling, naming, and copying after speech training(P<0.05). Boston Diagnostic Aphasia Examination (BDAE) showed 2 cases had obvious improvement, and 2 patients had mild improvement.ConclusionThe speech rehabilitation on globle aphasia is very difficult, but speech function can recover to a certain extent and the type of aphasia can tranffer after speech training, and it can raise the ability of daily exchange by combining nonspeech means.

BACKGROUND: Chitosan/poly (3-hydroxybutyrate) (PHB) copolymer has been paid close attention for special biological source of Chitosan and PHB. However, there has been no proper method for them to polymerize effectively.OBJECTIVE: To prepare chitosan/PHB graft copolymer in a homogeneous medium, using a gentle initiator.DESIGN, TIME and SETTING: This study, a single-sample experiment, was performed at the Research Center of Chemistry, Xi'an University of Architecture and Technology, Xi'an, Shaanxi Province, China from August 2007 to October 2007.MATERIALS: Chitosan: DD=100%, Mη=123000, Kyoto, Japan. PHB was purchased from Aldrich chemicals and the molecular weight was 10000.METHODS: Chitosan was grafted with poly (3-hydroxybutyrate) (PHB) in acetic acid/dimethyl sulfoxide system, and benzoyl peroxide (BPO) was used as initiator. The reaction temperature was 85℃ and the reaction continued for 5 hours with nitrogen protection. Grafting reaction and the chemical structure of the copolymer were confirmed by infrared analysis, NMR and elemental analysis. The crystal form and thermal stability of copolymer were characterized with wide-angel X-ray diffraction (WAXD) and thermogravimetric/differential thermal analysis balance, respectively.MAIN OUTCOME MEASURES: The chemical structure of copolymer, crystal form as well as thermal stability.RESULTS: Grafting reaction was confirmed by infrared analysis, NMR and elemental analysis. Wide angle X-ray diffraction and thermogravimetric analysis indicated that graft copolymer was different from chitosan and PHB in crystalline morphology, and had a good thermal stability.CONCLUSION: Using BPO as initiator, chitosan/PHB grafting copolymer is prepared and it has a steady property.

Objective The aim of this study was to evaluate and to characterize the visceral hypersensitivity in non-erosive reflux disease(NERD)through the detection of esophageal distension-cerebral evoked potentials(ED-CEP)and calcitonin gene related peptide(CGRP)and substance P(SP)levels in esophageal mucosa.Methods NERD patients(N 26)and 12 controls were enrolled in this study from Oct.2004 to Mar.2005.Mechanical distention stimulation was performed using the balloon-affixed and polyvinyl multilumen catheter.The ED- CEP was recorded with a muti-channel international 10~20 system of electroencephalograph.The esophageal specimens were immune-histologically evaluated for CGRP/SP-stain positive contents.Results Esophageal distention may evoke recognizable and reproducible muti-peak CEP.CEP morphology of NERD patients was characterized by randomly distributed patterns,and the peak latencies for N1,P1,and N2 were significantly shorter compared with control group(P

Objective.To probe into the influence of Jiannao Anshen Capsule of Miao medicine on the behavioral performance,content of cytokine interleukin-1 (IL-1),neuroendocrine hormone melatonin (MT) and monoamine neurotransmitter 5-hydroxytryptamine (5-HT) of rats with sleep deprival.Methods All of the 60 rats were evenly and randomly divided into 5 groups:the normal control group,model control group,estazolam group,Zaoren Anshen group,and Jiannao Anshen of Miao medicine group,respectively.Every group had 12 rats.Sleep deprivation rat model was established by intraperitoneal injection of para-chlorophenyla lanice (PCPA).After successful modeling,the rats were intragastrically administrated with corresponding medicines.The contents of serum 5-HT,IL-1 and MT and the contents of 5-HT,MT in brain tissue were determined.Results Behavioral score,the contents of 5-HT,MT and IL-1 in serum and the contents of 5-HT,MT in brain tissue in insomnia rats caused by PCPA were significantly decreased as compared with normal control group (P ＜ 0.05).In the Jiannao Anshen group,estazolam group and Zaoren Anshen group,the behavioral score,the content of 5-HT,MT and IL-1 in serum and the content of 5-HT and MT in brain tissue of insomnia rats were significantly increased as compared with model control group (P ＜ 0.05).The behavioral score,the content of 5-HT,MT and IL-1 in serum and the content of 5-HT and MT in brain tissue of insomnia rats were not statistically significant different among Jiannao Anshen group,estazolam group and Zaoren Anshen group (P ＞ 0.05).Conclusion Jiannao Anshen capsule of Miao medicine could obviously improve the sleep quality of rats with sleep deprival.The mechanism may be related to improving the content of IL-1,MT and 5-HT.
