1.Analysis of the baseline investigation results of the conformity rate of bacterial culture examination records in a tertiary specialized hospital in 2022

Objective This paper aims to investigate and analyze the baseline conformity rate of bacterial culture exami-nation records in a tertiary specialized hospital.By doing this,this paper seeks to understand the current situation of medical re-cord writing of bacterial culture examination and explore the improvement path and relevant measures for enhancing the conformity rate of bacterial culture examination records.Methods According to the requirements of"Quality Control Index of Medical Re-cord Management(2021 Edition)",a retrospective analysis was conducted on 6 317 medical records that underwent bacterial culture examination in 2022.Results The execution rate of medical orders was 100.00%,the completeness rate of the report sheet was 100.00%,the record rate of medical records was 81.05%,the analysis rate of results was 33.07%,and the conformi-ty rate of bacterial culture examination records was 33.07%.Conclusion It is imperative to enhance the conformity rate of bac-terial culture examination records and improve the quality of medical records in terms of content.
2.Prevalence and associated factors of diabetic retinopathy among type 2 diabetes in Dongcheng District and Tongzhou District, Beijing City: a cross-sectional study

Objective:Diabetic retinopathy (DR) is the most common cause of adult blindness in China. Screening of DR is important for early detection, prevention, and treatment. However, there is still controversy in the research on the prevalence and risk factors of DR in China. This study was designed to evaluate the prevalence of DR and related risk factors in patients with type 2 diabetes mellitus in Beijing City.Methods:A cross-sectional survey was conducted in in Dongcheng District and Tongzhou District, Beijing City. Patients with type 2 diabetes aged 18-80 years were selected from four communities, and all subjects underwent questionnaires, physical examinations, laboratory examinations and fundus photography. The logistic regression model was used to analyze the associated factors of DR.Results:A total of 1 531 subjects were included, with the median age of 66 years old and the average age of (65.6±7.4) years old, and the glycosylated hemoglobin level in the subjects was 7.2%±1.3%, and the glycosylated hemoglobin compliance rate was 56.0%(857/1 531). A total of 254 patients with diabetic retinopathy were detected, and the prevalence of DR was 16.6%(254/1 531). Among them, there were 218 cases of non-proliferative diabetic retinopathy and 36 cases of proliferative diabetic retinopathy. Compared with the non-DR group, there were statistically significant differences in fasting blood glucose ( Z=-3.74, P<0.001), glycosylated hemoglobin( Z=-10.664, P<0.001), urinary microalbumin excretion rate( Z=-7.767, P<0.001), low-density lipoprotein cholesterol( Z=-2.589, P=0.01), and duration of diabetes( Z=-10.189, P<0.001) between the DR group and the non-DR group. Multivariate regression analysis showed that the duration of diabetes ( OR=1.08, 95% CI: 1.06-1.10, P<0.001), glycosylated hemoglobin ( OR=1.38, 95% CI: 1.23-1.55, P<0.001), and FPG ( OR=1.11, 95% CI: 1.03-1.19, P=0.008) were associated factors for DR. Conclusion:In this study, the prevalence of DR in 4 communities of type 2 diabetes in Beijing City was 16.6%. Besides, this study further confirmed that the duration of diabetes, fasting blood glucose levels, and glycosylated hemoglobin are associated factors for DR in patients with type 2 diabetes.

4.Onconephrology and nephrology: convergence of multi-disciplines in medicine in children

Onconephrology is a new subject field that has emerged in the past decade.On the one hand, tumors can cause renal injury.On the other hand, as the research on the pathogenesis of tumors has brought unprecedented progress in tumor treatment, more treatment-related renal damage has occurred.Tumor-associated acute kidney injury is common in pediatrics.Therefore, early diagnosis and treatment are crucial for the protection and prognosis of kidney in children.This article reviews the epidemiological characteristics of pediatric cancer and the concept and pathogenesis of tumor-associated renal injury, so as to better promote the development of the field of pediatric onconephrology.
5.Application of a specialist nurse-led nutrition management program in children with leukemia during induction remission chemotherapy

Objective To explore the effects of a specialist nurse-led nutrition management program on the nutritional status of children with leukemia during induction remission chemotherapy.Methods Using the convenience sampling method,45 children with leukemia who received induction remission chemotherapy in a tertiary hospital in Sichuan Province from December 2023 to May 2024 were selected as an experimental group,and the specialist nurse-led nutrition management program was implemented.A total of 45 children with leukemia who admitted to the same hospital to receive induction remission chemotherapy from August to November 2023 were selected as a control group,and routine nursing care was provided.The duration of the intervention was 45 days,and the children's mid-upper arm circumference,BMI,serum albumin and pre-protein concentration were compared between the 2 groups before and after the intervention,and post-intervention satisfaction of their parents was also compared.Results 2 cases were lost to follow-up during the study,and finally 44 cases were included in both the experimental group and the control group.After the intervention,mid-upper arm circumference,BMI,and serum albumin concentration in the experimental group were higher than those in the control group,with statistically significant differences(P<0.05);there was no significant difference in pre-protein concentration between the 2 groups(P=0.199);the satisfaction of parents in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.001).Conclusion Implementation of a specialist nurse-led nutrition management program is helpful in improving mid-upper arm circumference and BMI in children with leukemia during induction and remission chemotherapy,as well as their serum albumin concentration and parental satisfaction.

7.Construction of human TCRP1 gene knockout chronic myeloid leukemia K562 cell line using CRISPR/Cas9 technology and its biological function detection

Objective:To select human chronic myeloid leukemia (CML) cell line K562 as the experimental object, and use lentivirus mediated CRISPR/Cas9 gene editing technology to construct a stable CML cell line K562/TCRP1-KO that knocks out the tongue cancer resistance related protein 1 (TCRP1) gene; and through functional tests such as cell proliferation, apoptosis, and drug sensitivity, compare the phenotypic differences between K562/TCRP1-KO and control cells (K562/cas9-CTL), and preliminarily explore the possible mechanism of TCRP1 gene involvement in the pathogenesis of CML.Methods:The small guide RNA (sgRNA) targeting TCRP1 was designed at a specific location. After annealing, the oligonucleotide fragments were recombined with the linearized Cas9 expression vector, and the lentivirus packaging system was transfected into 293T cells. The purified virus was collected and infected with K562 cells. Positive polyclons were screened for puromycin pressure, and monoclonal K562/TCRP1-KO was further screened by limited dilution method. Stable cell lines were successfully knocked out by sanger sequencing and Western blot detection; Simultaneously, K562 cells transfected with lentiCRISPR vector were constructed as control cell lines (K562/cas9-CTL); Using cell counting method, cell counting kit 8 (CCK8) method, imatinib (IM) gradient dilution method, and flow cytometry cell proliferation, drug sensitivity, and apoptosis analysis were performed on K562/TCRP1-KO and K562/cas9-CTL, respectively.Results:The sgRNA-Cas9 recombinant plasmid vector for TCRP1 knockout was successfully constructed, and after transfection into 293T cells, TCRP1 knockout monoclonal cell lines were successfully screened using limited dilution method. Compared with K562/cas9-CTL cells, the proliferation ability of K562/TCRP1-KO cells was significantly reduced, IM drug sensitivity was significantly enhanced, and the process of cell apoptosis was significantly accelerated (all P<0.05). Conclusions:A CML cell line with TCRP1 knockout was successfully constructed using CRISPR/Cas9. TCRP1 may act as a cancer related gene to affect the proliferation, IM resistance, and apoptosis process of CML cells.
8.Progress on the function and mechanism of pigment epithelium derived factor receptors in the occurrence and development of diabetic retinopathy

Endogenous pigment epithelium derived factor (PEDF) shows great potential as a drug target for the treatment of diabetes retinopathy (DR) due to its anti-angiogenesis, anti-inflammatory, neuroprotective and neurotrophic effects. PEDF plays a biological role by combining with receptor proteins on cell membrane surface and regulating a variety of signaling pathways. Low density lipoprotein receptor related protein 6 plays a role in inhibiting oxidative stress reaction, inflammatory reaction, and neovascularization of DR. Adipose triglyceride lipase, laminin receptor, plexin domain containing 1 (PLXDC) 1, PLXDC2 and F 1-adenosine triphosphate synthase have the effect of promoting endothelial cell apoptosis, among which PLXDC1 also has neuroprotective effect. By clarifying the receptor that PEDF acts on, exploring the affinity between the receptor and PEDF, the difference in the expression level of each receptor in the process of disease, and the specific function that PEDF plays after binding with specific receptors, we can develop fusion protein drugs for the active domain of high affinity of receptors, have a clearer understanding of the pathogenesis of DR, and take PEDF or PEDF receptor as the target to consolidate the theoretical basis for the development of new therapeutic drugs and strategies for DR.
9.The teaching practice and exploration of the course of New Technology of Radiotherapy and Oncology

Objective:To evaluate and summarize the teaching effect of" New Technology of Radiotherapy and Oncology" in the form of questionnaire, understand the degree of demand for the course of all kinds of medical students and improve the teaching contents and methods.Methods:The course of" New Technology of Radiotherapy and Oncology" was a specialized elective course in Peking Union Medical College. After two rounds of teaching practices, we evaluated the students participating in the course or non-course participants by anonymous questionnaire. The questionnaire items include the course content, setting, teachers and improvement suggestions.Results:A total of 73 questionnaires were sent out, and the recovery rate was 100%. Among them, 52(71%) were from students who chose the courses. 83% of the students" strongly agree" that the teaching content of this course is the latest development, the latest achievement or the problem to be solved, 94% were" satisfied" or" very satisfied" with their learning effect, and 92% and 83% were satisfied with the teachers and teaching plan, respectively. After taking the course, students rated" broadening of mind" (96%) as the biggest gain, followed by" facilitating interdisciplinary collaboration" (79%). 86% of the non-course participants felt the need to supplement the existing curriculum with new techniques in oncology radiotherapy, hoping that the curriculum would" broaden the mind" (76%), improve clinical application (81%) and facilitate interdisciplinary collaboration (71%).Conclusion:The questionnaire results show that the teaching practice of this course covers the different needs of all kinds of students, which is worthy of implementation and further improvement.
10.Protective effect of Radix Astragali seu Hedysari compound on rat retinal DNA damage induced by acute hypobaric hypoxia

Objective:To investigate the rat retinal DNA damage induced by acute hypobaric hypoxia and the protective effect of Radix Astragali seu Hedysari compound on it. Methods:Seventy-two healthy clean grade male Sprague Dawley (SD) rats were randomly divided into normal oxygen control group, hypoxic model group and Radix Astragali seu Hedysari compound gavage group by computer random number distribution, with 24 rats in each group.The rats in the normal oxygen control group were fed in normal condition, while the rats in the hypoxic model group and Radix Astragali seu Hedysari compound gavage group were fed in a hypobaric oxygen chamber, which simulated an altitude of 5 kilometers, and the rats were administered intragastrically with Radix Astragali seu Hedysari compound (0.1 g/kg) or the same volume of normal saline once a day according to grouping.Retinal tissue was obtained on the 7th day of continuous administration of drugs after the rats were sacrificed by euthanasia.Histopathological staining was performed to observe the pathological morphology of retina in each group.Immunohistochemistry was used to identify the expressions of p53 and histone family 2A variant ( γH2AX). Real time-PCR was used to determine the relative expression levels of 8-oxoguanine nucleoside triphosphatase (MTH1) and 8-hydroxyguanine glycolsylase (OGG1) mRNA.The use and care of the animals followed Regulations of the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission. Results:The retina of hypoxic model group was thicker than that of the normal oxygen control group and Radix Astragali seu Hedysari compound gavage group, especially the nerve fiber layer and the ganglion cell layer.The positive staining intensities of p53 and γH2AX were stronger in the retinas of the hypoxic model group than those in the the normal oxygen control group and Radix Astragali seu Hedysari compound gavage group.The relative expression levels of MTH1 and OGG1 mRNA in the hypoxic model group were 0.573±0.081 and 0.772±0.136, which were significantly lower than 0.846±0.160 and 1.013±0.168 in the normal oxygen control group, respectively (both at P<0.05). The relative expression level of MTH1 mRNA was 0.748±0.114 in the Radix Astragali seu Hedysari compound gavage group, significantly higher than that in the hypoxic model group ( P<0.05). No significant difference was detected in OGG1 mRNA expression level between the Radix Astragali seu Hedysari compound gavage group and the hypoxic model group ( P=0.743). Conclusions:The intervention of Radix Astragali seu Hedysari compound plays a protective role in hypobaric hypoxia-induced retinal DNA damage through modulating the expression of p53, γH2AX, OGG1 and MTH1.
