1.A clinical study of short-segment Barrett's esophagus

Objective To investigate the clinical features, management, surveillance and possible pathophysiology of short segment Barrett's esophagus (SSBE).Methods Fifty two cases of SSBE identified by endoscopy and pathology were enrolled in this retrospective study. The endoscopic manifestations, pathological changes, esophageal motor function, management and follow up results were assessed.Results The island pattern was most prominent type accounting for 86.5%, and specialized intestinal metaplasia (SIM) was found in 15.4% of patients with routing H E stain. Abnormal motor function were showed in 8 of 11 cases(72.7%) who received 24 hs esophageal pH and bilirubin monitoring and esophageal manomery. Argon plasma coagulation therapy was carried out in 21 cases and SSBE ablation was achieved in 15 cases based on shor term follow up result. No esophageal adenocarcinoma was found in 49 patients revisited.Conclusions SSBE is associated with abnormal gastroesophageal acid and bile reflux. Island pattern is the most common endoscopic appearance. Specialized intestinal metaplasia and dysplasia may be less frequent in patients with SSBE.
2.Progress of diagnosis and treatment in children with thrombotic microangiopathy

Thrombotic microangiopathy is a group of clinical and pathological syndrome,characterized mainly by hemolytic anemia,thrombocytopenia,and multiple organ dysfunctions,caused by a variety of reasons.Classic types of thrombotic microangiopathy include hemolytic uremic syndrome,and thrombotic thrombocytopenic purpura.Hemolytic uremic syndrome is a critically ill glomerular disease,of which 90% found in children.Depending on the pathogenesis,hemolytic uremic syndrome is currently divided into 2 categories,namely,typical and atypical hemolytic uremic syndromes.Atypical hemolytic uremic syndrome onsets rapidly,is dangerous disease,tends to recurring easily,and has high mortality in its acute phase.Nowadays,great interest is focused on the important role of the excessive activation of the alternative complement pathway in the pathogenesis of atypical hemolytic uremic syndrome.This article report the recent advances on the cause,diagnosis,and treatment of thrombotic microangiopathy.
3.Effect of comprehensive nursing intervention on curative compliance of migrant TB patients

Objective To probe into the effect of comprehensive nursing intervention on curative compliance of migrant TB patients. Methods One hundred and eighty migrant patients with tuberculosis smearing positive were divided into control group and observation group in equal number according to the registration time.The patients in the control group received routine nursing care and those in the observation group comprehensive nursing intervention including psychological intervention,health education and family supervision for 6 months. The two groups were compared in terms of the curative compliance.Results After the intervention,the observation group was significantly better than the control group in terms of re-visit,regular mediation,drugs taking from the outpatient section and sputum culture after intervention (P<0.05).Conclusion Comprehensive nursing intervention is effective in improving the treatment compliance of migrant TB patients.
4.Diagnosis and treatment for anemia of chronic kidney disease in children

There are many reasons for children with anemia of chronic kidney disease (CKD), such as chronic renal insufficiency due to lack of raw materies erythropoietin(iron,folic acid, vitamin B12 deficiency), shortening of the life of red blood cells and blood loss, bone marrow suppression by urine toxins such as erythropoietin.But the main reason is decreased production of renal erythropoietin accompanying chronic renal insufficiency.Anemia affects tissue's oxygen supply, utilization, and cardiac output, and patients often show a series of pathophysiology of immune dysfunction,such as fatigue, difficulty in breathing, enlargement of the hearts, ventricular hypertrophy, heart failure, cerebral insufficiency ,cognitive decline ,affecting the prognosis and quality of life of patients.Renal anemia requires aggressive treatment,which is closely related to the survival and quality of life in patients with chronic renal failure.This paper elaborates on the diagnosis of anemia in children with CKD, the evaluation strategies of anemia,iron therapy and erythropoiesis stimulating agents treatment.
5.Prevention of peritoneal dialysis-associated peritonitis

Peritonitis is a primary complication of peritoneal dialysis.In the past concentration was the treatment after peritonitis became evident.With the study of the causes of peritonitis, it becomes evident that, in order to better protect the peritoneal function, more focus should be on the prevention of peritoneal dialysis-related peritonitis, correction as soon as possible the risk factors of peritonitis, and the reduction of the incidence of peritonitis.The main causes of peritonitis include pollution, outlet exports and catheter-related infections bacterial biofilm formation tunnel.Emphasis should be on the occurrence of malnutrition.Albumin levels are risk factors for peritonitis.To reduce the risk of infection new ideas include VitD supplements that regulate immune function.Endogenous microbes associated peritonitis caused by intestinal infections.Future research directions should focus on reducing the risk of intestinal bacteria translocation, regulating intestinal flora, taking a long-range antimicrobial therapy during the critical period, and implementing fecal bacteria transplant.This paper gives an overview of these issues and how to prevent their occurrence.

7.The single itraconazole and combination with interferon-? against candida albicans infection

Objective:To study the efficacy of single itraconazole and combination with interferon-? against Candida albicans infection.Methods:Murine models of systemic candida albicans infection were established, and treated with different concentrations of itraconazol alone and combination with INF-?. Then the concentrations of colonyforming units(cfu) and histolo gical sections were observed to evaluate the emcacy. Results:The combination of 105U INF-? and itraconazole at the dosage of 10mg/kg showed better effect against Candida albicans than the combination of itraconazole at the dosage of 50mg/kg with 105U INF-?. Conclusion:The combination of itraconazole at a low dose with INF-? shows a good effect against Candida albicans.The single use of itraconazole at a high dose is also anti-infection.
8.STUDY ON THE EPIDEMIOLOGICAL CHARACTERISTICS OF LARGE INTESTINE CARCINOMA IN NORTH ZHEJIANG

Objective:The aim of this study was to explore the epidemiological characteristics of large intestine carcinoma in North Zhejiang.Methods:A retrospective review of clinical records and endoscopic findings of 339 large intestine carcinoma patients (detected from 5320 endoscopic examinees) throught past 8 years was executed.Results:General detection rate was 6.37%.Annual detection rate fluctuated between 4.60% and 8.79%.The detection rate for male was slightly higher than that for female.The ratio of male to female was 1.15∶1.The 41~60 years interval was the susceptible age interval (48.37%);rectum and sigmoid colon were the most frequently sites involved (71.38%).Most (89.38%) of the carcinomas were adenomatous histopathologically.Conclusions:There were susceptible age interval and topological sites for large intestine carcinoma.Most of them were adenomatous.Annual detection rate in this region through the past 8 years showed neither rise nor decline year by year.

10.Role of PI3 K/Akt/mTOR signaling pathway in hepatocellular carcinoma

Phosphatidylinositol 3-kinase,Akt,and the mammalian target of rapamycin (PI3K/Akt/mTOR)signaling pathway is shown to play a key role in the tumorigenesis,proliferation,metastasis,apoptosis,and angiogenesis of hepatocellular carcinoma (HCC)by regulating gene expression.The components and functions of PI3K/Akt/mTOR signaling pathway are briefly described,and the research advances in the action mechanism of PI3K/Akt/mTOR signaling pathway in the progression of HCC and related inhibitors are reviewed.It is disclosed that blocking the PI3K/Akt/mTOR signaling pathway may become a new therapy for HCC.
