1.Analysis of effect of salmeterol,fluticasone propionate combined with tiotropium bromide in the treatment of elderly patients with chronic obstructive pulmonary disease

Objective To evaluate the clinical curative effect of salmeterol,fluticasone propionate combined with tiotropium bromide in the treatment of elderly patients with chronic obstructive pulmonary disease (COPD),thus to provide reference for the clinical treatment.Methods 76 patients with moderate and severe COPD were randomly divided into observation group and control group,38 cases in each group.The control group on the basis of COPD conventional treatment,received salmeterol,fluticasone propionate inhalation therapy.The observation group was trea-ted with the treatment of the control group and treated with the inhalation of tiotropium bromide.The lung function tests,C reactive protein and T lymphocyte subsets were compared between the two groups before and after treatment. Results After treatment,FEV1 ,FEV1 /FVC,FEV1 % in the two groups were significantly higher than before treat-ment (t =4.25,4.43,3.98,4.1 2,4.26,4.07,all P <0.05).After treatment,FEV1 ,FEV1 /FVC,expected value for FEV1 % in the observation group were (2.45 ±0.35)L/s,(63.91 ±7.42)%,(47.29 ±7.52),which were signifi-cantly higher than those of control group (t =3.68,4.03,4.1 8,all P <0.05).After treatment,C reactive protein, CD +8 in the two groups were significantly lower than those before treatment,while CD +4 ,CD +4 /CD +8 were significantly higher than those before treatment (t =4.45,3.99,5.1 3,3.86,4.1 3,4.59,3.89,4.03,all P <0.05).After treat-ment,C -reactive protein,CD +8 of the observation group were (1 0.38 ±3.1 2)mg/L,(22.47 ±2.1 3)%,which were significantly lower than the control group,and CD +4 (31 .62 ±4.22)%,CD +4 /CD +8 (1 .43 ±0.20)were signifi-cantly higher than control group (t =4.46,4.1 7,3.92,4.72,all P <0.05).Conclusion Salmeterol,fluticasone propionate combined with tiotropium bromide in the treatment of elderly patients with COPD can significantly improve the patients'lung function,reduce the inflammation reaction,improve the patients 'immune function,alleviate the patients'disease.
2.Effect of Empathy Nursing on the Anxiety and Satisfaction of Cesarean Parturients

Objective To explore the effect of empathy nursing on the anxiety and nursing satisfaction of cesarean parturients.Methods Seventy-two cases of cesarean parturients treated in our hospital between September 2013 and October 2014 were selected as the study object.These women were randomly divided into an observation group and a control group,with 36 cases in each group.Those in the control group were given routine nursing and those in the observation group were given routine nursing and empathy nursing.Results After nursing,the anxiety and depression of patients in both groups were alleviated.The anxiety score and depression score in the observation group were respectively (31.16 ± 3.94) and (18.21 ± 2.97),lower than those in the control group,with the anxiety score of (43.19 ± 4.45) and the depression score of(43.19 ± 4.45) The differences between the two groups were statistically significant (P<0.01).After nursing,improvement on life quality of the patients was registered in both groups.The scores of cognitive function,physical function,emotional function,role function and social function of patients in the observation group higher than those in the control group.The comparative difference was statistically significant (P<0.01).Besides,the total score of nursing satisfaction in the observation group was (118.53 ± 12.41),significantly higher than that in the control group which was (97.65 ± 6.81),and the difference was statistically significant (P<0.01).Conclusion For cesarean parturients,empathy nursing can better help to identify the patients' psychological changes and to establish closer bond between the nursing staff and the patients,which can provide guidance for them to cultivate positive mental state,help the staff to better understand their needs,pains and their pressure,thus lessen their anxiety and improve the nursing effect and satisfaction.
3.Endovascular graft exclusion for thoracic and/or abdominal aorta aneurysm: a report of 5 cases

Objective To evaluate the effect of endovascular graft exclusion (EVGE) for aorta aneurysm(AA). Methods From October,2000 to August 2002,5 cases of AA were treated by EVGE, 4 of which were thoracic and abdominal aortic dissecting aneurysm and the another one was abdominal AA. Results All the operations were successfully performed. The pseudo cavity in the 4 cases of aortic dissection disappeared after the operation,and the another aortic aneurysm was successfully isolated. Conclusions EVGE is an effective ,less trauma and safe, when the indications are carefully selected.
4.Effect of Health Education and Psychological Nursing Intervention on the Patients with Postpartum Depression

Objective To explore the clinical effect of health education and psychological nursing intervention on the prognosis of patients with postpartum depression. Methods Ninety cases of postpartum depression in our hospital from March 2012 to March 2014, were randomly divided into the control group and the observation group, 45 cases in each group. The patients in the control group were given routine nursing, the patients in the observation group received health education and psychological intervention on the basis of the control group. The anxiety and depression scores change, medication adherence and quality of life in the two groups were observed and compared. Results The scores of SDS and SAS in the observation group were significantly lower than those in the control group (P<0.05) . The medication adherence and quality of life score 3 months and 6 months after discharge in the observation group were significantly higher than those of control group (P<0.05) . Conclusion Health education and psychological intervention has positive effect on the prognosis of postpartum depression patients, and can effectively improve the patient's medication compliance and quality of life, it's one of the effective measures for the prevention and treatment of postpartum depression.
5.Ethacrynic acid promotes apoptosis in lung cancer A549 cells when combined with cisplatin chemotherapy

Objective To investigate the killing effect of ethacrynic acid (EA) on lung cancer A549 cells derived spheres and explore the underlying mechanism.Methods A549 spheres were cultured in serum-free medium,and the protein expression of CD133,SOX2,EpCAM and ABCG2 was detected by Western blotting.MTT assay was used to evaluate the cell viability of A549 spheres and A549 cells after treated by 1,2,5,10 and 20 mg/mL cisplatin (DDP) for 48 h.The activity of glutathione S-transferase (GST) was measured by colorimetric method after A549 spheres were treated with 10,50,100 and 200 μmol/L EA,respectively.Flow cytometry,Western blotting,real-time PCR and luciferase assay were used to analyze the levels of cellular reactive oxygen species (ROS),formation of A549 spheres,mRNA and protein expression levels of β-catenin,Sox2 and ABCG2,and promoter activity of β-catenin upon 200 μmol/L EA treated cells for 48 h.A549 sphere was infected with β-catenin adenovirus for 24 h,followed by 200 μmol/L EA treatment (in presence or absence of 5 mg/mL DDP) for 24 h.The expression of β-catenin,Sox2 and ABCG2 at mRNA and protein levels was detected by real-time PCR and Western blotting,and cell growth of A549 spheres was evaluated by MTT assay.Results The A549 spheres,with high expression of tumor stem cells markers CD133,SOX2,EpCAM and drug resistance related molecule ABCG2,and resistance to DDP at different doses,were successfully derived.After 200 μmol/L EA had treated A549 sphere for 48 h,the levels of ROS were significantly increased (P < 0.05),and the mRNA and protein levels of β-catenin,Sox2 and ABCG2,and promoter activity of β-catenin were notably decreased (P < 0.05).The treatment of 200 μmol/L EA enhanced the inhibitory effect on proliferation and the promoting effect on apoptosis in A549 spheres induced by 5 mg/mL DDP (P < 0.05).Up-regulation of β-catenin by adenoviral infection partly reversed the effects of 200 μmol/L EA on suppressing the expression levels of β-catenin,Sox2 and ABCG2,compared to the spheres infected with blank adenovirus.Additionally,β-catenin over-expression significantly remitted the inhibitory effect of 200 μmol/L EA and 5 mg/mL DDP on the proliferation in A549 spheres.Conclusion EA exerts inhibitory effect on the proliferation and stemness of A549 spheres through suppressing GST activity and β-catenin expression,and then promotes cell apoptosis.EA might be a novel drug in treatment of lung cancer and cancer stem cells.
6.Interventional regional arterial infusion treatment for severe acute pancreatitis

Objective To evaluate the potential of interventional regional arterial infusion for severe acute pancreatitis (SAP). Methods Regional arterial infusion with somatostatin and antibiotic was performed in 20 cases of SAP within 4 hours after diagnosis confirmed,and pressurized infusion lasted for 5 days. Another 28 cases of SAP as control group received drug treatment all over the body. The SAP clinical data such as symptom,mortality,complication rate and stay time in hospital were analysed in two groups. Results There were significant differences in remission,serum amylase,urine amylase and lipase between the regional and total infusion groups. 7.Inhibition of 5-HT(3) receptors-activated currents by cannabinoids in rat trigeminal ganglion neurons.

This study investigated the modulatory effect of synthetic cannabinoids WIN55,212-2 on 5-HT(3) receptor-activated currents (I(5-HT3)) in cultured rat trigeminal ganglion (TG) neurons using whole-cell patch clamp technique. The results showed that: (1) The majority of examined neurons (78.70%) were sensitive to 5-HT (3-300 μmol/L). 5-HT induced inward currents in a concentration-dependent manner and the currents were blocked by ICS 205-930 (1 μmol/L), a selective antagonist of the 5-HT(3) receptor; (2) Pre-application of WIN55,212-2 (0.01-1 μmol/L) significantly inhibited I(5-HT3) reversibly in concentration-dependent and voltage-independent manners. The concentration-response curve of 5-HT(3) receptor was shifted downward by WIN55,212-2 without any change of the threshold value. The EC(50) values of two curves were very close (17.5±4.5) μmol/L vs. (15.2±4.5) μmol/L and WIN55,212-2 decreased the maximal amplitude of I(5-HT3) by (48.65±4.15)%; (3) Neither AM281, a selective CB1 receptor antagonist, nor AM630, a selective CB2 receptor antagonist reversed the inhibition of I(5-HT3) by WIN55,212-2; (4) When WIN55,212-2 was given from 15 to 120 s before 5-HT application, inhibitory effect was gradually increased and the maximal inhibition took place at 90 s, and the inhibition remained at the same level after 90 s. We are led to concluded that-WIN55,212-2 inhibited I(5-HT3) significantly and neither CB1 receptor antagonist nor CB2 receptor antagonist could reverse the inhibition of I(5-HT3) by WIN55,212-2. Moreover, WIN55,212-2 is not an open channel blocker (OCB) of 5-HT(3) receptor. WIN55,212-2 significantly inhibited 5-HT-activated currents in a non-competitive manner. The inhibition of I(5-HT3) by WIN55,212-2 is probably new one of peripheral analgesic mechanisms of WIN55,212-2, but the mechanism by which WIN55,212-2 inhibits I(5-HT3) warrants further investigation.
8.Effect of Tongxinluo on endothelial function and hypersensitive C-reactive protein in acute coronary syndrome patients undergoing percutaneous coronary intervention.

OBJECTIVE:
To determine the effect of Tongxinluo on the endothelial function and hypersensitive C-reactive protein (hs-CRP) in acute coronary syndrome patients undergoing percutaneous coronary intervention(PCI).
METHODS:
Thirty-three patients with unstable angina pectoris and 6 patients with acute myocardial infarction who underwent PCI for stenotic lesions of the coronary artery were enrolled. The patients were randomly assigned to a conventional group (n = 19) which took routine treatment or a tongxinluo group (n = 20) which took Tongxinluo(4 capsules once, 3 times per day) at the base of routine treatment after PCI. Nitric oxide synthase (NOS), nitric oxide (NO), endothelium-dependent vasodilation which was evaluated in the brachial artery flow mediated diameter(FMD) and hs-CRP were measured before the PCI and 24 hours and 3 months after the PCI. The correlation between NO and hs-CRP was analyzed.
RESULTS:
NOS, NO, and FMD in the 2 groups 24 hours after the PCI were significantly lower than those before the PCI(P < 0.05), but hs- CRP obviously increased (P < 0.05). NOS, NO, and FMD 3 months after the PCI in the 2 groups were significantly higher than those before the PCI (P < 0.05 or P < 0.01), but hs-CRP obviously decreased (P < 0.01).All indexes mentioned above in the Tongxinluo group showed greater changes than those of the conventional group(P < 0.05). NO was negatively correlated with hs-CRP (r = -0.3219, P<0.01).
CONCLUSION
Tongxinluo capsules have obvious beneficial effect on endothelial function and anti-inflammation in acute coronary syndrome patients undergoing PCI, by directly acting on the endothelium and indirectly inhibiting inflammation.

10.Analysis of serum allergen-specific immunoglobulin E in 3 051 patients with eczema or dermatitis in an allergy clinic

Objective:To analyze measurement results of serum allergen-specific immunoglobulin E (IgE) in patients with eczema/dermatitis.Methods:A retrospective analysis was conducted on serum allergen-specific immunoglobulin E (IgE) levels in 3 051 patients with eczema/dermatitis, who visited the allergy clinic of Huashan Hospital from April 1, 2021 to March 31, 2022. The serum allergen-specific IgE level was detected by using the Phadia allergen detection system, and positive rates of allergens were calculated to determine common inhaled allergens and food allergens in patients with eczema/dermatitis. Comparisons of enumeration data between groups were performed by chi-square test.Results:Among the 3 051 patients with eczema/dermatitis, there were 1 412 with atopic dermatitis and 1 639 were other eczema/dermatitis. Detection of serum allergen-specific IgE showed that 1 629 (53%) patients were positive for allergens, and the number of positive allergen-specific IgEs in each patient was 3.0 ± 1.6. The top 3 common inhaled allergens in patients with eczema/dermatitis were Dermatophagoides farinae (904/1 522, 59%) , Dermatophagoides pteronyssinus (891/1 513, 59%) and Alternaria alternata (206/1 068, 19%) , and the top 3 common food allergens were shrimps (251/1 432, 18%) , egg white (165/992, 17%) and cow milk (149/994, 15%) . Among the 3 051 patients, 25 (1%) were aged < 2 years, 571 (19%) aged 2 - 12 years, 285 (9%) aged 12 - 18 years, and 2 170 (71%) were aged > 18 years. The most common food allergens were both egg white in the age groups of < 2 years and 2 -12 years (77%, 37%, respectively) , and were both shrimps in the age groups of 12 - 18 years and > 18 years (31%, 17%, respectively) . Dermatophagoides pteronyssinus and Dermatophagoides farina were the top 2 common inhaled allergens in all age groups, with the positive rate ranging from 36% to 84%; in addition, the positive rate of molds was relatively high in the age group of 2 - 12 years (mold mixture: 37%; Alternaria alternata: 27%) . From April 2021 to March 2022, the positive rate of outdoor allergens ranged from 10% to 15% among outpatients in every month; the positive rates of tree pollen and grass pollen increased from April 2021, and peaked in October 2021. The patients with atopic dermatitis showed a significantly increased positive rate of allergens (73%) compared with those with other eczema/dermatitis (37%, χ2 = 389.36, P<0.001) , and the rank of common allergens in the patients with atopic dermatitis was basically the same as that in those with eczema/dermatitis. Conclusions:The common allergens were Dermatophagoides farina, Dermatophagoides pteronyssinus and Alternaria alternata in the patients with eczema/dermatitis. Food allergy was more common in infant patients, and inhalation allergy was more common in child, adolescent and adult patients. The positive rate of allergen-specific IgEs was markedly higher in the patients with atopic dermatitis than in those with other eczema/dermatitis.
