1.The clinical analysis of patient controlled epidural analgesia for symptomatic lumbar disc herniation

Objective:To evaluate the curative effects of patient controlled epidural analgesia(PCEA)for symptomatic lumbar disc herniation.Methods:Lumbar disc herniation cases(n=98)were divided into two groups.The epidural drugs injection group(DEDI group,n=54)underwent the epidural drugs injection 4 times as one treatment course.The patient controlled epidural analgesia group(PCEA group,n=44)underwent the same treatment,but the first epidural drugs injection was followed by PCEA for 7 days.Evaluate the curative effects and the side effects.Results:VAS scores in both groups decreased significantly on the eighth day and at the end of the treatment,being 4.41?0.62 and 1.30?0.77 in the PCEA group and 5.09?0.88 and 1.65?0.97 in the DEDI group(P0.05).No significant side effect was found during the treatment.Conclusion:PCEA is an effective method of lumbar disc herniation,and it relieves the symptoms and signs more quickly in the early treatment compared with the DEDI.
2.Clinical research on the treatment of 50 cases of bronchial bsthma in the acute attack with Xiaoqinglong decoction combined with western medicine

Objective To observe clinical effect of treating bronchial asthma in the acute attack with xiaoqinglong decoction combined with western medicine. Methods 50 patients with bronchial asthma were randomly divided into two groups. The control group was treated with theophylline sustained release tablets, 0.1 g~0.2 g for each time, twice a day,salbutamol sulfate (inhaled), 200 μg (2 spray) for each time, twice a day, beclomethasone dipropionate aerosol (inhaled),100 μg (2 spray) for each time, twice a day, antibiotics, oxygen inhalation when necessary, infusion, and correcting water-electrolyte and acid-base imbalance if necessary. Based on the control group, the treatment group was treated with Xiaoqinglong decoction for continuous 3 courses, once a day, 7 days as a course. The clinical effect and adverse reaction were observed between control group and treatment group after the treatment. Results The total effective rate of the treatment group and the control group was 84.6% and 66.7% respectively. The clinical effect in the treatment group was significantly higher than that of the control group (χ2=2.762, P<0.05). Conclusion Xiaoqinglong decoction combined with western medicine has better effects bronchial asthma in the acute attack than western medicine.
3.Application of Repetitive Transcranial Magnetic Stimulation for Neuropathic Pain (review)

Repetitive transcranial magnetic stimulation (rTMS) may release neuropathic pain (NeP) by changing the excitability of cor-tex, improving the cerebral blood flow, regulating the expression of neurotransmitter, fitting the plasticity, reducing the overexpression of neuronal nitric oxide synthase and overaction of astrocyte, etc. It is used for management of post-herpetic neuralgia, thalamic pain after stroke, NeP after spinal cord injury, phantom limb pain, atypical facial pain post operation of trigeminal nerve, etc. The outcome is associat-ed with the location, frequency, pulses, intensity of stimulation, as well as the shape of the stimulator, etc.

OBJECTIVE Learned through the seeing about the infection management of the Japanese Oita Medical University affiliated hospital and analyzed our hospital infection management′s status,to understand the dynamic management of hospital infection.Moreover these suggested that we attach importance to the management of hospital infection and constantly explore new ways of hospital infection management.METHODS According to the comparative presentation of these two hospitals′ awareness and practices of infection management from China and Japan,this paper probed into the direction of our hospital infection management development.RESULTS Hospitals in every country of the world have attached great importance to their infection management,but their specific practices were different.CONCLUSIONS By comparing inspection,every hospital should maintain the individualized characteristics and learn from each others′ strong points to offset their own weakness in order to gain continuous improvement of hospital infection management.
5.Role of Sonic hedgehog signaling pathway in spinal cord in neuropathic pain in rats

Objective To evaluate the role of Sonic hedgehog (Shh) signaling pathway in the spinal cord in neuropathic pain (NP) in the rats.Methods Seventy-two male Sprague-Dawley rats,aged 8 weeks,weighing 250-300 g,were randomly divided into 2 groups (n=36 each) using a random number table:sham operation group (S group) and NP group.Spared nerve injury was produced by exposing the sciatic nerve and its branches and ligation and transection of tibial nerve and common fibular nerve in anesthetized rats.The mechanical paw withdrawal threshold (MWT) was measured before operation and at 1,4,7,14 and 21 days after operation.After measurement of the pain threshold at 1,4,7,14 and 21 days after operation,the animals were then sacrificed,and the lumbar segment (L46) of the spinal cord was obtained for determination of Shh,Patched (Ptch),Gli1 and glial fibrillary acidic protein (GFAP)expression (by Western blot),Shh,Ptch and Gli1 mRNA expression (by fluorescent quantitative real-time reverse transcriptase-polymerase chain reaction),and Shh and GFAP expression (by immunohistochemistry).Results Compared with group S,the MWT was significantly decreased,and the expression of Shh,Ptch and Gli1 protein and mRNA and GFAP in spinal cord tissues was up-regulated in group NP (P< 0.05).Shh was mainly expressed in the cytoplasm of spinal dorsal horn neurons and in the gap around glial cells.Conclusion Shh signaling pathway in spinal cord is involved in the development and maintenance of NP in the rats.
6.Role of reactive oxygen species in the apoptosis of human hepatocellular carcinoma HepG_2 cells induced by PIG11 protein overexpression

Objective:To investigate the role of reactive oxygen species (ROS) in the apoptosis of human hepatocellular carcinoma HepG_2 cells induced by PIG11 protein overexpression. Methods:The fluorescent probe 2', 7'-dichlorofluorescin diacetate (DCFH-DA) was used to label the intracellular ROS in pLXSN-PIG11-HepG_2 cells,pLXSN-HepG_2 cells and HepG_2 cells.The intracellular fluorescence intensity was detected by using the flow cytometry (FCM). The apoptotic ratio of HepG_2 was determined by FCM after elimination of intracellular ROS with 10 mmol/L N-acetylcystine (NAC). Results:The intracellular content of ROS in pLXSN-PIG11-HepG_2 cells was significantly higher than that in pLXSN-HepG_2 cells[(15.60±0.92) vs (4.90±0.70), P<0.01)]. The apoptotic ratio of pLXSN-PIG11-HepG_2 cells was significantly decreased by pretreatment with 10 mmol/L NAC (P<0.01). Conclusion:The apoptosis-inducing effect of PIG11 over-expression is related with elevation of intracellular ROS levels.
7.The influence of systemic nursing intervention on the quality of life in patients with radical cystectomy and bricker operation

Objective To investigate the influence of systematic nursing intervention on the quality of life of patients with radical cystectomy and bricker operation.Methods 67 patients with radical cystectomy and bricker operation were randomly divided into control group ( n = 33) and observation group ( n = 34).Control group was routinely cared, while the observation group was cared with systematic nursing intervention by the nurse in charge of work.Then all the patients were surveyed with the core questionnaire of quality of life of cancer patients (QLQ-C30) in 6 months after the operation.Results The scores of global health status, physical function, role function,emotional function,cognitive function and social function of the observation group (64.8 ± 8.7,80.1 ±9.2,67.5 ± 10.1,82.1 ± 11.5,84.6 ± 14.2,66.4 ± 12.5 ) were higher than those of the control group ( 53.3 ±9.4,68.3 ± 8.8,56.5 ± 10.3,68.3 ± 12.2,67.1 ± 10.5,55.3 ± 12.3 ), respectively.The scores of fatigue, nausea and vomiting, insomnia, and appetite loss of the observation group (34.6 ± 8.9,19.4 ± 6.9,27.7 ± 7.6,25.7± 8.4) were lower than those of the control group (44.4 ± 10.3,30.5 ± 7.6,39.8 ± 7.8,34.8 ± 10.1 ), respectively.And the difference was significant (P < 0.05 ).Conclusion The systematic intervention can enhance patients' psychological, physical, and social adaptability, and improve patients' self-care skills and quality of life.
8.Isolation and Management of Special Infection Patients in General Hospital

OBJECTIVE To norm the isolation and management of special infection patients and eliminate the incidence of hospital infections.METHODS The measures,the implementation of training and personal management,and details of control were determined.RESULTS The system management and isolation measures were implemented to effectively control the incidence of hospital infections.CONCLUSIONS The isolation and control of special infection patients are a systematic and rigorous work,it is very important to form a scientific,complete and easy to implement workflow and management system for the control of hospital infections.
9.Effect of TRF1,TRF2 and telomeric stability on As_2O_3 induced MGC803 cell apoptosis

Aim To study the effect of As_2O_3(arsenic trioxide) on the expressions of telomeric repeat binding factor1,2 and telomeric stability of human gastric cancer MGC803 cells, and explore the mechanism of cell apoptosis. Method MGC803 cell growth inhibition was measured with MTT assay. Apoptosis was analyzed with flow cytometry. Influence on chromosome distal end was analyzed with chromosome end-end fusion analysis. Expressions of TRF1 and TRF2 were determined with Western blot analysis. Results MTT assay showed that As_2O_3 clearly inhibited the growth of MGC803 cells, depending on time and dosage. The apoptosis rates were significantly higher than those of the control group in a concentration and time-dependent manner. These changes were not found in the control group. After disposal with 5 ?mol?L -1 As_2O_3, chromosome fusion rate was obviously increased in 48 h. After 48 h of disposal with 5 ?mol?L -1 As_2O_3, the TRF1 of MGC803 cells was up-regulated while TRF2 was down-regulated. Conclusion As_2O_3 induced chromosome fusion and MGC803 cells apoptosis through up-regulating expressin of TRF1 and down-regulating expressin of TRF2.
10.Expression of tight junctions protein regulator PKC-θ in meningioma tissue

Objective To study the expression and clinical meaning of tight junctions protein regulator PKC θ in human meningioma tissue.Method The expression of PKC θ in 30 non-invasion and 30 invasion meningioma tissue is tested by using immunohistochemistry S-P method,with the goal of analyzing the relationship between the expression of PKC θ and pathology character.Results The positive rate ofPKC θ in invasion meningioma tissue is obviously lower than in non-invasion meningioma tissue.The difference is significant(P<0.05)Conclusions The lower expression of PKC θ in invasion meningioma tissue pmbably affects occurrence and invasion of the meningioma by ehang4ng the structure and function of TJs.
