1.Pulmonary involvement in Crohn disease: a case report and review of the literature

Objective To describe the characteristics of pulmonary involvement in Crohn was described and relevant literature were reviewed. Results The patient showed chief complaints of cough,expectoration and fever. The chest CT scan revealed bilateral lung opacity shadows ,Ziehl-Neelsen acid-fast stain examination in sputum and cultures got negative results for many times. Chest CT scan revealed no dissipation after different antimicrobials treatment for 2 months. The fiberoptic bronchoscopy was performed, which showed nodular lesions in the trachea and beside the opening of right upper lobe. Histopathology showed polypoid hyperplasia of granulation tissue. The bilateral patchy opacity dissipated, the nodes in the trachea disappeared and the nodes beside the opening of right upper lobe diminished obviously after the patient was treated with oral prednisone and salicylazosulfapyridine (SASP). Pulmonary involvement in Crohn disease is very rare and it can involve any part of the lungs. The most common manifestations are cough, expectoration and fever. The lung and gastrointestinal system often share similar pathogenetic changes: granulomatous inflammation. These patients responded well to combination use of glucocorticoid and SASP. Conclusion Pulmonary involvement should be considered when patients with Crohn disease manifested with respiratory symptoms, which can be definitely diagnosed by clinical features and histopathological examintation.
2.Expression of survivin and bcl-xl in non-small cell lung cancer and their relationship

Objective To investigate the expression of survivin and bcl-xl in non-small cell lung cancer(NSCLC)and their relationship.Methods The NSCLC tissuechips including 32 cores were constructed and expression of survivin and bcl-xl was examined by immunohistochemical technique in 30 normal bronchial epithelium and 32 primary NSCLC.Results The positive rate of survivin and bcl-xl protein in NSCLC was 71.9% and 81.3% respectively,significantly higher than that in the normal ones(P0.05).The expression of bcl-xl protein was closely related to cell differentiation(P0.05).The over-expression of survivin and bcl-xl was positively correlated(P
3.Role of gastroesophageal reflux disease in exacerbations of chronic obstructive pulmonary disease

Objective To evaluate the effect of gastroesophageal reflux disease(GERD)on exacerbations of chronic obstructive pulmonary disease(COPD).Methods All of 74 patients with COPD accomplishing the reflux diagnostic questionnaire(RDQ)were.divided into GERD group(n=38)and non-GERD group(n=36)according the scores of RDQ.The clinical data including the frequency of exacerbations of COPD were compared between the two groups.Results The frequency of exacerbations of COPD was significantly higher in GERD group compared with that in non-GERD group[(2.68±0.77)times/year vs (1.38±0.49)times/year,t=8.74,P<0.05].The mean FEV;percentage was similar in the two groups [(39.82 4-10.95)%vs(37.11±12.24)%,t=1.00,P>0.05].The score of RDQ was probably as a factor of predicting the frequency of exacerbations of COPD(R2a=o.39,t=6.98,P<0.05).Conclusion The presence of GERD appears to be associated with increased exacerbations of COPD.
4.Study on the correlation between chronic sinusitis with nasal septum deviation.

OBJECTIVE:
Study on the correlation between chronic sinusitis with nasal septum deviation.
METHOD:
Randomly selected 722 patients with coronal sinuses CT, statistics the number of cases of nasal septum deviation, cases of nasal septum deviation with chronic sinusitis, the wide and narrow side cases of nasal septum deviation complicated with sinusitis. The number of sinusitis without deviation, and paired test.
RESULT:
The incidence of sinusitis between deviation of nasal septum and non deviation were 54. 13% and 44. 66%, the difference between two groups was statistically significant (P<. 05), the incidence of sinusitis with nasal septum deviation of wide and narrow side were 31. 65% and 32. 12%, no significant difference between the two groups (P>0. 01). The incidence of sinusitis high deviation and non high deviation were 59. 54% and 46. 97%, the difference between the two groups was statistically significant (P<0. 05). The number of wide side of upper nasal septum deviation with sinusitis was 54, the narrow side was 66, there is no significant difference between the two groups (P>0. 05).
CONCLUSION
The deviation of nasal septum is associated with the formation of chronic sinusitis, the high deviation is more prone to sinusitis, The incidence of sinusitis and nasal septum deviation on both sides was no different.

6.Transabdominal radical resection of cardiac carcinoma

Objective To investigate the feasibility and safety of transabdominal perposterior mediastinum radical operation in the treatment of cardiac carcinoma. Methods Twenty-six cases of cardiac carcinoma invading low esophagus underwent transabdominal posterior mediastinum radical resection. Among them 9 patients received radical proximal gastroesophagectomy, and 17 cases did radical total gastroesophagectomy. Results The procedures were all successful. The mean operation time for total gastrectomy was (189±39) min, proximal subtotal gastrectomy was (153±35) min. The averge blood loss of total gastrectomy was (200±80) ml, proximal subtotal gastrectomy was (168±76) ml. The harvest of celiac lymph nodes were (23.3±7.3), esophageal lymph nodes were (4.1±2.0). Pneumonia was complicated in 4 cases and there was no anastomotic hemorrhage, leakage nor stenosis. All patients were followed up from 5 to 51 months, liver metastasis was found in 4 cases, lung metastasis in 2 patients and death in 1. There was no anastomotic stoma tumor recurrence. Conclusion It was feasible and safe to perform transabdominal perposterior mediastinum radical operation for cardiac carcinoma patients, provided that diaphragms was not involved and tumor invaded esophageal length was less than 5 cm.

Objective To investigate the influence of HIWI gene silencing in biological behavior of T24 cells and explore the possibility for HIWI gene to be used as the molecular target of inhibiting bladder carcinoma cell proliferation with gene transfection and RNA interference(RNAi) technique.Methods T24 cells were divided into transfection group with pGenesil-2-HIWI,transfection group with pGenesil-2-HIWI2263,transfection group with pGensil-2-control,and two control groups transfected with PEI only,and PBS only,respectively.T24 cells were transfected with shRNA expression vectors targeting HIWI gene by PEI,and the cell proliferation and cell cycle were measured by MTT assay and FCM.Results At the time of post-transfection 24 h,the inhibitory rate of cell proliferation in transfection groups were 32.60% and 26.09%,they were lower than that in control group(3.54%).At the time of post-transfection 48 h,the percentages of cells at S phase in transfection groups were(29.39? 3.27)% and(30.87?10.88)%,they were lower than that in control group(39.36%?2.09%)(P
