2.Investigation of the cognitive levels and health education on elderly patients with acute myocardial infarction

Objective We observed the level of education of elderly patients with acute myocardial infarction (ami) influenced the level of awareness in the disease.Methods we used the questionnaire designed by ourselves to investigate the cognitive level of 80 cases with acute myocardial infarction.According to the education level of the patients,we divided them into different groups,including junior high group,secondary technical group,junior college group,and compared the different awareness levels of the three groups of patients with each other.Results The rate that the junior college group patients with AMI did not understand the risk factors were:diet70%,body weight 50%,smoking 70%,exercise40%,lipids 50%,blood pressure80%,blood sugar 50%,significantly lower than that of the secondary technical group of 38%,33%,47%,38%,42%,71%,42% and of the junior high school group,of 22%,18%,48%,16%,14%,45%,18% ( all P < 0.05 ),The awareness of the disease in the junior college group of AMI was significantly higher than the secondary technical group and the junior high school group( all P < 0.05 ).Conclusion The level of education and awareness of the disease in elderly patients with AMI was significantly related.
3.Clinical value of 18F-FDG PET/CT in patients with extranodal nasal type natural killer/T-cell lymphoma

Objective To analyze the image characteristics and clinical value of 18F-FDG PET/CT in patients with extranodal nasal type natural killer/T-cell lymphoma (ENKTL).Methods From January 2012 to March 2016,20 ENKTL patients (12 males,8 females;median age 53 years) who underwent whole-body 18F-FDG PET/CT were enrolled in this retrospective study.Eleven patients were newly diagnosed,and 9 were previously treated.Clinical data were collected for histopathology and bone marrow biopsy,laboratory results,PET/CT and radiological findings such as CT or MRI.The final diagnosis was based on histopathology and immunohistochemistry.Mann-Whitney u test was used to compare the SUVmax of newly diagnosed patients and patients with recurrence or disease progression after therapy.Spearman correlation analysis was used to explore the relation between diameter of tumor-invaded lymph nodes and SUVmax.Results ENKTL could be found in all parts of the body,but more frequently in the nasal cavity,nasopharynx and skin.All lesions showed high uptake of FDG on PET/CT.The SUVmax of newly diagnosed patients was significantly higher than that of patients (n=7) with recurrence or disease progression:6.5(4.3,10.4) vs 5.4(3.4,8.9);u=6 853.500,z=-2.039,P<0.05).The diameter of tumor-invaded lymph nodes showed weakly positive correlation with SUVmax(rs =0.290,P<0.01),suggesting that invasion of ENKTL might not be accurately evaluated by the size of lymph nodes.The staging by PET/CT was concordant with clinical final staging in 11 newly diagnosed patients,while the staging by CT or MRI was only correct in 6 patients.Conclusions PET/CT is superior to conventional imaging modalities in diagnosis and staging for patients with ENKTL.Since some lesions might be found in the limbs,limbs should be included in the scan field.
4.Autoantibodies to connective tissue in patients with auditory neuropathy

OBJECTIVE To study the causes and mechanism of auditory neuropathy. METHODS Auditory neuropathy is characterized by the DPOAE being normal, the shape of the pure tone loss being mostly in low frequencies, but the ABR being absent or the threshold elevated disproportionally to the pure tone threshold. Patients were screened from the deaf patients through asking the ill history and taking the exams of pure tone audiometry, auditory brain stem response, distortion product of otoaccoustic emissions (DPOAE). Thirty six patients were met the above standard. Deparaffined sections of cochlea of the guinea pigs were used as antigens to test whether the sera of patients had the autoantibodies with immunofluorescence method. RESULTS In the total of 36 patients with this type of hearing loss, autoantibodies were positive in 31 patients(86.1%). Twenty of the 31 patients had autoantibodiesto connective tissue of osseous spiral lamina where the nerve fiber connecting the hair cells and spiral ganglion cells go through. The autoantibodies to capsula surrounding the spiral ganglion and inner ear nerve fiber was also seen in these patients. In additional 7 patients, the autoantibodies to spiral ganglion cell nucleus and inner ear nerve fiber was detected. 5.Analysis of influence factors on neoadjuvant chemotherapy efficacy in inflammatory breast cancer

Objective To explore the relationship of molecular biology characteristic and the treatment outcome,and influence factors of neoadjuvant chemotherapy (NAC) in inflammatory breast cancer (IBC).Methods The clinicopathological data of 103 IBC patients who were treated with NAC from January 2005 to June 2013 were analyzed retrospectively.Immunohistochemical method was used to detect the expression of estrogen receptor (ER),progesteron receptor (PR),human epidermal growth factor receptor 2 (HER-2) and E-cadherin.The treatment outcome were evaluated.Results In 103 IBC patients,ER negative was 48 patients,PR negative was 51 patients,HER-2 positive was 45 patients,E-cadherin positive was 66 patients.The effective rate of chemotherapy was 72.8% (75/103).The effective rate of chemotherapy in taxane-based group was significantly higher than that in anthracycline-based group [80.6% (50/62) vs.61.0%(25/41)],and there was significant difference (P < 0.05).The effective rate of chemotherapy in ER,PR,E-cadherin negative patients was significantly higher than that in ER,PR,E-cadherin positive patients [83.3% (40/48) vs.63.6% (35/55),82.4% (42/51) vs.63.5% (33/52),83.8% (31/37) vs.66.7% (44/66)],and there was significant difference (P < 0.05).The effective rate of chemotherapy in taxane-based group with E-cadherin positive patients was significantly higher than that in anthracycline-based group with E-cadherin positive patients [77.5% (31/40) vs.50.0% (13/26)] (P <0.05).No correlation existed between the expression of HER-2 and the treatment outcome of chemotherapy (P > 0.05).Conclusion ER,PR and E-cadherin negative patients with IBC is chemosensitive to NAC.The positive expression of E-cadherin may be an important factor of chemotherapy resistance.For the patients with E-cadherin positive,taxane-based chemotherapy regimen can achieve a better effective rate.

Objective To explore the effect of applying personal digital assistant(PDA)in documenting medical orders and monitoring vital signs.Method There was significant diffenence the between two groups in dicumenting,inedical orders,monitoring vital signs(P<0.05).Result BPDA needed significantly shorter time for documenting medical orders and monitoring vital signs than the traditional method(P<0.05).Conclusions PDA may shorten the time for documenting medical orders and monitoring vital signs. It reduces the repetition of nursing routines,optimizes the work flow sheet so that nurses have more time for patients and the quality of nursing is improved.
7.Rapamycin and PD98059 collaborative inhibit mammalian target of rapamycin pathway in the prevention and treatment of mouse colorectal cancer

Objective To investigate the combined inhibition effect and the potential mechanism of rapamycin (mammalian target of rapamycin inhibitor) and PD98059 (mitogen-activated protein kinase/extracellular signal-regulated kinase kinase (MEK) inhibitor) on mouse colorectal cancer (CRC). Methods S-ICR mice were subcutaneously injected with 20 mg/kg of 1,2-dimethylhydrazine dihydrochloride in the nape for 20 weeks to induce CRC. From the 16th week, the mice were treated with alone or combined injection with 0.25 mg/kg rapamycin and 2.5 mg/kg PD98059. The drugs were administered for 8 weeks. Subsequently, the animals were sacrificed and dissected, the tumor sizes were measured, and the tumors were harvested for pathological assay. Furthermore, the phosphorylation of mTOR, p70S6K, and 4E-BPl proteins was detected by using immunohistoehemistry. Results The mice treated with rapamycin (44. 44 %) or PD 98059 (either alone (38.89%) or combination treatment (6.67%) were significantly less likely to develop cancer compared with mice receiving none of them (77.78%, P<0. 05). The average size of tumors was (6.15±2. 192), (8.85±3. 983), (2.917±0. 191), (16.36±6.855) mm3 respectively (P<0.05).The anti-cancer effect of the combination treatment was substantially significant. The proteins of phospho-mTOR, phospho-p70s6K and phospho-4E-BPl were significantly down-regulated after treatments (all P values < ,0.05). Conclusions Combined treatment was more effective than single-drug treatments of rapamycin or PD98059 alone for the prevention and treatment of mouse CRC. The mTOR signal pathway might be involved in the inhibitory mechanism.
8.Comparison of clinical application of ultrasound-guided and endoscopy-guided nasogastrojejunal tube placement

Objective To compare ultrasound-and endoscopy-guided methods in nasogastrojejunal tube placement in critical ill patients at bedside.Methods A tot al of 95 critical ill patients planed to perform enteral nutrition (EN) were enrolled.They were divided randomly into ultrasound-guided group (48 patients)and endoscopy-guided group (47 patients).The success rate,the incidence of complications and the time they took were compared between two groups.Results Ultrasound guided group was compared to endoscopy guided group with a lower success rate (81.3% vs 100%,P =0.003),mainly because of the initial 24-stage having a very low success rate (66.7% vs 100%,P =0.000),but the success rate of the following 24 cases was significantly improved (95.8% vs 100%,P =0.338).Ultrasound guided group didn 't cause more complications (2.1% vs 0,P =1.000),and consumed less time [(13.3 ± 2.8)min vs (15.0 ± 1.4) min,P =0.000].Compared to the following 24 cases,the initial 24 cases of ultrasound guided group in nasogastrojejunal tube placement had the lower success rate (66.7% vs 95.8%,P =0.023),the longer time-consuming [(15.4 ±2.1)rin vs (11.2 ± 1.4) min,P =0.000],and but didnt cause more complications (4.2% vs 0,P =1.000).The success rate of different diseases [severe acute pancreatitis(SAP) vs cerebrovascular disease] hadn't statistically significant difference (94.9% vs 87.5 %,P =0.300),but the SAP group consumed less time [(12.6 ± 1.9)min vs (15.5 ± 2.0)min,P =0.000].Conclusions The placement of nasogastrojejunal tube under ultrasound guidance represented a safe,quick and effective method to provide enteral nutrition.
9.Value of gastrointestinal tract preparation with oral administration of anisodamine and isotonic mannitol in 18 F-FDG PET/CT imaging

Objective To investigate the value of gastrointestinal tract preparation with oral racean?isodamine tablets and isotonic mannitol in 18 F?FDG PET/CT imaging. Methods From July to September 2013, 129 patients with confirmed or suspected tumors who were referred for 18 F?FDG PET/CT imaging were divided into 2 groups. In the study group (30 males, 37 females, age (53.4±13.9) years), raceanisodam?ine tablets (10 mg) and 1 000-1 200 ml isotonic mannitol solution (2.5%) were orally taken at 10 min after in?jection of 18F?FDG;while in the control group (37 males, 25 females, age (60.0±12.8) years), 1 000-1 200 ml water was given. Mann?Whitney u test was used to compare the difference between the 2 groups in the filling degree of gastrointestinal lumen, delineation of tube wall, physiological uptake, matching degree of PET and CT images, delineation of mesentery, and the influence of gastrointestinal uptake on the identification of ab?dominal and pelvic lesions. χ2 test was used to compare the difference between the 2 groups in the uptake pattern of gastrointestinal tract and the incidence of side effects. Spearman correlation analysis was used to study the correlation between gastrointestinal lumen filling and PET/CT image matching. Results The gas?trointestinal lumen filling, delineation of tube wall, PET/CT image matching in the stomach, small intestine and colon (z: -5.096 to -2.665, all P<0.01),and delineation of mesentery (z=-2.781,P<0.01) were better in the study group than those in the control group. The filling degree of gastrointestinal lumen correla?ted with the corresponding PET/CT image matching (rs:0.521, 0.755, 0.239, all P<0.01). The physio?logical uptake of small intestine and colon was significantly higher in control group than that in the study group( z=-4.131, -3.095, both P<0.01) . But uptake in the stomach between the 2 groups was not signifi?cantly different(z=-0.776, P>0.05). There was statistically significant difference in the uptake pattern of small intestine between the two groups(χ2=12.884, P<0.01) , but not in the stomach and colon (χ2=0.559, 1.083,both P>0.05). The incidence of transient diarrhea (20?9%, 14/67) was higher in the study group than that in the control group (4.8%, 3/62;χ2=7.256,P<0.01). Conclusions The abdominal PET/CT image quality is improved by gastrointestinal preparation with oral raceanisodamine tablets (10 mg) and 1 000-1 200 ml isoton?ic mannitol solution (2.5%).
10.Cellulose hydrolysis and ethanol production by a facultative anaerobe bacteria consortium H and its identification.

The recalcitrance of lignocellulosic biomass makes its hydrolysis by cellulases less effective, and the consolidated bioprocessing (CBP) strategy that combines enzyme production, cellulose hydrolysis and fermentation, particularly the synergetic role of different microbes in attacking cellulose component could be a solution. In this article, a facultative anaerobe microbial consortium named H was isolated, which exhibited high stability even after 30 subcultures, with pH ranging from 6 to 9. Within three days, 0.5 g filter paper immerged in 100 mL PCS buffer was completely degraded, and 1.54 g/L ethanol was produced, correspondingly. Further analysis on the component of the microbe consortium was carried out though 16S rDNA and DGGE, and Clostridium thermosuccinogene, Clostridium straminisolvens and Clostridium isatidis that can directly convert cellulose to ethanol were identified, indicating that Clostridium spp. played important role in cellulose degradation through the synergistic coordination of different species, and the characterization of the consortium will benefit the analysis of the underlying mechanisms as well as the optimization of the CBP process for more efficient cellulose degradation and ethanol production.
