1.A clinical analysis in 72 cases of perinatal mortality

Objective To study the causes of perinatal mortality aimed to pvevent and reduce it.Methods According to the medical records of perinatal mortality cases in our hospital from 2001 to October 2005 retrospective investigate was carried out.Results 72 cases of perinatal mortality occured during the last five years in our hospital comprised of 36 cases fetal death 24 cases dead labor and 12 cases newborn death the causes were the following in order,umbilical cord abnormalities,fetal anomalies placenta abnormalities,fetal asphyxia,newborn hypoxia,premature delivery and others.Conclusions To improve prenatal care combimed with spread health eduaction on better birth and better devdopment may reduce the prerinatal mortality.
2.Limitation of Staphylococcus Spp Test Kit ATB STAPH 5 from BioMerieux in Clinical Applcation

Objective To study the limitation analysis of Staphylococcus spp Susceptibility Test Kit ATB STAPH 5 from Bi-oMerieux in clinical application and to propose remedy.Methods The coverage area and break point of antibiotics ATB STAPH 5 from BioMerieux were analysed by using CLSI M100-S23 as standard.Results The kit ATB STAPH 5 did not include chloromycetin,oxazolidinones and lipopeptide three antibiotics that were recommend by CLSI.The 16 kinds of antibi-otics break point in the kit ATB STAPH 5 were all not inconsistent or did not contain with the standard of CLSI M100-S23. It may lead to error drug sensitivity tests,false sensitive or false drug resistance.Conclusion Only using this kit ATB STAPH 5 for Staphylococcus spp ,it may occurre errors or mistakes.Other experiments must be corrected before the re-port.
3.Extraction and purification of water-soluble active ingredient of Ferula sinkiangensis

AIM: To optimize the extraction and purification of water-soluble active ingredient of Ferula sinkiangensis.METHODS: First,the comprehensive scoring system was set up to reflect the change of the extraction rate and ferulic acid coutent.in combination with the uniform design.RESULTS: The optimized extracting process was as follows: adding 8 times water and extracting twice,every time lasting 2.5 h.The optimized centrifugation purificating process consisted of: concentration ratio (1 ∶ 3),35 minutes centrifugal time,4 000 r/min rotation speed.The optimized ZTC1 + l-Ⅱ clarificant purification process was as follows: concentration ratio was 1 ∶ 22,the content of pacificator was 22% ,purification time was 1 h.CONCLUSION: The optimized extraction is available,reasonable,and reproducible.The results of pharmacodynamic test show that centrifugation is suitable to the purification of extract liquid.
4.Effects of epididymal P34H gene silencing on expression of P34H and ac-tivity of hyaluronidase in mouse sperm

AIM:To investigate the effects of P34H gene silencing on the expression of P34H and activity of hyaluronidase (HYD) in mouse sperm.METHODS:The recombinant plasmid series of P34H targeted short hairpin RNA (shRNA) were constructed by GV248 plasmids vector.These recombinant plasmids were transformed into DH 5αcompetent cells, and the plasmids were taken from DNA sequencing analysis .The HEK293T cells were co-transfected with shRNA and lentiviral packaging plasmids .The 3 kinds of recombinant lentiviruses and negative control lentiviruses were used to in -ject into the mouse epididymis and the expression of P 34H at mRNA and protein levels was detected by real-time PCR and Western blot, respectively.The location of P34H protein on the mouse spermatozoa was determined by indirect immunofluo-rescent staining using P34H antibody.The positive rate and activity intensity of HYD was detected by modified sodium hya-luronate-gelatin membrane.RESULTS:DNA sequencing analysis confirmed that the 3 P34H-shRNA sequences were suc-cessfully inserted into the lentiviral vectors .P34H expression in epididymis tissue was significantly decreased at both mR-NA and protein levels compared with those of the non-transfected and normal control vectors (P<0.05).The GV-P34H-shRNA-1 played a significant role in reducing the percentage of P 34H positive rate and the activity of HYD in mouse sperm.The silencing effect did not significantly differ between the non-transfected and normal control vectors .CONCLU-SION:Silencing of P34H significantly inhibits the percentage of P 34H positive rate and the activity of hyaluronidase in mouse sperm.
5.Effects of Yindan Xinnaotong Soft Capsule on oxidative stress and vascular endothelial function in patients with type 2 diabetes mellitus

Objective To investigate the effects of Yindan Xinnaotong Soft Capsule on oxidative stress and vascular endothe‐lial function in the patients with type 2 diabetes mellitus(T2DM ) .Methods A total of 86 patients with T2DM were randomly di‐vided into the routine glucose‐reducing group(routine group ,40 cases) and the Yindan Xinnaotong Soft Capsule group(Yindan Xin‐naotong group ,46 cases) .On the basis of diet control and exercise ,the routine group was given the glucose‐reducing therapy for blood glucose reaching the standard for 12 successive weeks ,while on the basis of blood glucose reaching the standard by the rou‐tine therapy ,the Yindan Xinnaotong group was added with Yindan Xinnaotong Soft Capsule ,1 .2 g per time ,3 times daily for 12 successive weeks .The changes of blood lipids ,MDA ,SOD ,NO and ET were determined before treatment and after 12‐week treat‐ment .The flow mediated endothelium‐dependent diastolic function (FMD) and non‐flow mediated endothelium‐dependent diastolic function (NMD) in brachial artery were simultaneously detected using ultrasonography .Results After 12 weeks of treatment ,the levels of TC ,TG ,LDL‐C ,MDA and ET in the two group were obviously decreased compared with before treatment ,the levels of HDL‐C ,SOD ,NO and FMD in both groups were increased (P<0 .05);moreover the above indexes after treatment in the Yindan Xinnaotong group had significant changes compared with the routine group (P<0 .05) .Conclusion Yindan Xinnaotong Soft Cap‐sule can down‐regulate oxidative stress and regulate the lipid metabolic abnormality and obviously improve the injured vascular en‐dothelial function in T2DM .
7.A research on basic medical sciences' experimental teaching system and quality monitoring for in-novative talents' cultivation

This research is to construct a basic medical sciences’experimental teaching system in order to cultivate innovative talents. It is guided by cultivating innovative practical ability and post compe-tence and implements a teaching mode with “five combinations”by integrating teaching resources, strength-ening interdisciplinary combination , integrating curriculum and organ systems , and optimizing teaching modules and experiment content. A preliminary personnel training mode and experimental teaching system have been constructed for innovative talent cultivation, and correspondently a diversified experiment exami-nation system and teaching quality monitoring system have been constructed through teaching practice, which aims at continuously improving experiment teaching quality and talent training quality.
8.Changes of ocular aberration stability after correction with adaptive optics system

Background The use of adaptive optics (AO) system in ophthalmic clinic and basic studies has increased in recent years.However,there are few reports on the stability of ocular aberrations after correction.Objective This study was to analyze the stability of aberration after correction by observing the repeatability of ocular aberration measurements.Methods Forty-one postgraduate school students and volunteers who meet the conditions were included from February to April 2014.The Zernike aberration coefficients including astigmatism (Z2-2,Z22),defocus (Z02),trefoil (Z33,Z3-3),coma (Z3-1,Z13),spherical aberration (Z40) and the value of root mean square (RMS) including 3rd-order to 7th-order aberrations,total higher-order aberrations (HOAs) and total ocular aberrations (TOAs) were measured by using AO system.The repeatability and stability of these data after corrected with AO system were analyzed.The repeatability was evaluated by ANOVA,within-subject standard deviation (Sw),repeatability (r) and intra-class correlation coefficients (ICC).The stability was evaluated by the nonparametric Friedman's rank test.Results AO system showed excellent repeatability on Z2-2,Z22,Z20 and TOA RMS (ICC> 0.9),good repeatability on Z13,Z33,Z3-3,Z40,3rd-order RMS,4th-order RMS,HOA RMS (ICC > 0.75),poor repeatability on Z3-1,5th-order RMS,6th-order RMS,7th-order RMS (ICC < 0.75).Repeatability (2.77 Sw) values ranged from 0.009 mm (7th-order RMS) to 0.163 mm (Z31).After low-order ocular aberrations were corrected,It was founded that Z2-2,Z22 reached stable state at the 4th second;Z02 was stable at the 6th second;Z3-3 and Z33 reached stable state at the 4th second and third second,separately;Z13 was stable from 3rd-second to 9th-second,Z3-1 was stable at the 4th-second.Z40 and HOA RMS were stable at the third second and fifth second,respectively.The Z2-2,Z02,Z22,Z3-3,Z3-1,Z33,Z40 and HOA RMS were significantly different among different time points before and after low-order aberrations correction (all at P < 0.05).Z2-2,Z22,Z20 reached stable state at the 4th-second,3rd-second and 5th-second,respectively;Z3-3,Z33 reached stable state at the 2nd-second and 3rd-second,respectively;Z3-1and Z40 reached stable state at the 2nd-second;HOA RMS reached stable state at the 5th-second.Conclusions After correcting the human ocular aberration,different aberrations can reach stable state at different time.The time of Z02,Z22,Z3-3,Z3-1,Z40reaching stable state after 2nd-order to 5th-order ocular aberrations correction was earlier than those of lower-order aberrations correction.
9.Clinical observation of different wavelength laser in the treatment of severe non-proliferative diabetic retinopathy

Objective To compare the clinical effects of 577 nm and 532 nm laser panretinal photocoagulation in the treatment of severe non-proliferative diabetic retinopathy (NPDR).Methods A prospective,controlled trial was conducted in 42 patients(64 eyes)with severe NPDR,who were randomly divided into 577 nm group and 532 nm group.All of patients received PRP with the single-point model.Preoperative and postoperative 1 day,1 month,3 and 6 months,the best corrected visual acuity (BCVA),fundus,optical coherence tomography (OCT) and full field flash electroretinogram (F-ERG) were examined.After treatment 3 and 6 months,fundus fluorescein angiography (FFA) examination was performed between two groups.Results In 577 nm group and 532 nm group,the average number of laser spot was (1969.25 ± 278.19) and (2098.16 ± 289.27) respectively;average laser power was (425.23 ± 50.15)mW and (438.15 ± 38.48)mW respectively;and average energy density was (7.54 ± 1.54) mW · ms · μm-2 and (7.68 ± 3.01)mW · ms · μm-2.There was no difference in number of laser spot(t =2.68),laser power (t =1.46) and energy density (t =2.15) between the two groups (all P > 0.05),and the differences of macular central thickness after treatment 1 month,3 and 6 months (t =1.98,1.88,1.81 respectively) approached no statistical significance between the two groups (all P > 0.05),while F-ERG a,b wave amplitudes after treatment 1 month,3 and 6 months (a wave:t =5.94,5.19,6.97;b wave:t =5.67,4.56,5.12) had significant differences between groups (all P < 0.05).The effective rate of treating 6 months after operation in the two groups were 87.5% and 46.9% respectively,with significant difference (x2 =7.56,P < 0.05).Conclusion 577 nm laser is more effective and has less damage to visual function than 532 nm laser in the treatment of NPDR.
10.Excess mortality risk associated with atmospheric PM2.5 exposure

Objective:
To evaluate the risk of fine particulate matter (PM2.5) on excess mortality among residents.
Methods:
The data of residential mortality in Yangzhou City, Jiangsu Province from 2015 to 2021 were collected from the Chinese Disease Prevention and Control Information System. The average daily mass concentration of PM2.5 and meteorology data were collected from the Yangzhou Environmental Monitoring Station and Yangzhou Meteorological Bureau, respectively. The effects of PM2.5 on non-accidental mortality, morality of respiratory diseases and mortality of circulatory diseases were evaluated using a generalized additive model. The risk of excess mortality was evaluated using excess risk (ER) and the number of excess mortality.
Results:
The median average annual mass concentration of PM2.5 was 38.00 (interquartile range, 31.95) µg/m3 in Yangzhou City from 2015 to 2021, decreasing from 51.75 (interquartile range, 32.82) µg/m3 in 2015 to 28.00 (interquartile range, 23.42) µg/m3 in 2021. The median average annual number of non-accidental mortality, mortality of respiratory diseases and mortality of circulatory diseases were 96 (interquartile range, 22), 9 (interquartile range, 5) and 38 (interquartile range, 13) cases, respectively. The greatest effects of per 10 μg/m3 increase in PM2.5 mass concentration on non-accidental mortality, mortality of respiratory diseases, and mortality of circulatory diseases were seen at a cumulative lag of 1 day (ER=0.528%, 95%CI: 0.293%-0.763%), a cumulative lag of 2 days (ER=0.917%, 95%CI: 0.125%-1.714%) and a cumulative lag of 1 day (ER=0.595%, 95%CI: 0.232%-0.961%), respectively. The number of excess mortality caused by PM2.5 on non-accidental mortality, mortality of respiratory diseases, and mortality of circulatory diseases in Yangzhou City from 2015 to 2021 were 2 125, 412 and 977 cases, respectively; compared with 2015, the number of excess mortality in 2021 decreased by 66.95%, 75.53% and 64.42%, respectively.
Conclusions
An increase in the mass concentration of atmospheric PM2.5 may elevate the risk of excess mortality among residents. Compared to 2015, the number of excess deaths attributed to exposure to atmospheric PM2.5 declined in 2021.
