1.Effect of two kinds of drainage operation on the short-term curative effect, mortality and complications risk in patients with single giant bacterial liver abscess

Objective To investigate the effect of two kinds of drainage operation including incision and drainage of abdominal operation and percutaneous puncture and drainage operation under ultrasound guidance on the short-term curative effect, mortality and complications risk in patients with single giant bacterial liver abscess. Methods Eighty patients with single giant bacterial liver abscess were selected, and all patients were treated with conventional therapy. Among them, 40 patients underwent incision and drainage of abdominal operation (control group), and 40 patients underwent percutaneous puncture and drainage operation under ultrasound guidance (observation group). The short-term curative effect, operation time, time of returning to normal body temperature, time of returning to normal white blood cell, hospitalization time, mortality and incidence of postoperative complication were compared between 2 groups. Results There were no statistical differences in total effective rate and mortality between control group and observation group: 92.5%(37/40) vs. 97.5%(39/40) and 5.0%(2/40) vs. 7.5%(3/40), P>0.05. The operation time, time of returning to normal body temperature, time of returning to normal white blood cell and hospitalization time in observation group were significantly lower than those in control group:(44.52 ± 5.68) min vs. (98.59 ± 12.70) min, (3.25 ± 0.74) d vs. (4.86 ± 1.32) d, (5.20 ± 1.35) d vs. (7.42 ± 1.79) d, (16.94 ± 3.61) d vs. (25.52 ± 5.28) d. The incidence of postoperative complication was significantly lower than that in control group:10.0%(4/40) vs. 27.5%(11/40), and there were statistical differences (P<0.05). Conclusions Two kinds of drainage operation in the treatment in patients with single giant bacterial liver abscess possesses the same clinical efficacy for short-and long-term curative effect; but percutaneous puncture and drainage operation under ultrasound guidance application can efficiently alleviate the clinical symptoms and signs, accelerate the recovery process of disease and be helpful to reduce the risk of postoperative complications.
2.Efficacy of high-dose ambroxol in elderly patients with aspiration pneumonias

Objective To investigate the efficacy of high-dose ambroxol in elderly patients with aspiration pneumonias. Methods Totally 62 patients aged 65 years and over with aspiration pneumonia were enrolled into 2 groups:conventional-dose group intravenously given ambroxol 60 mg/d (n=30) and high-dose group received 270 mg/d (n=32).The times of remission and fever disappearance,rate of adverse effects,average duration of hospitalization,rates of mortality and recurrence within 2 months were observed. Results There were no significant differences in clinical charecteristics between the two groups.The times of remission and fever disappear ance and average duration of hospitalization were lower in high-dose group than in conventional-dose group [(3.2±0.6)d,(54.2±19.5)h,(12.7±4.1) d vs.(3.8±1.1)d,(66.5±18.4)h,(13.5±3.1)d,t=2.11,2.36,2.04,all P<0.05].No differences were found in rates of mortality and recurrence within 2 months between the two groups (6.3%,3.1% vs.10.0%,13.3%,x2=0.01,0.87,both P>0.05).Adverse effects did not appear in the two dose groups. Conclusions High-dose ambroxol is efficient and safe for aspiration pneumonia in elderly patients.

Sphingosine-1-phosphate (S1 P) is a kind of bioactive messenger which is catalyzed by sphingosine kinases (SphK) and mediates various biological behaviors through binding to sphingosine-1-phosphate receptors(S1PR).Researches demonstrate that the aberration of SphK,S1P and S1PR is closely related to the occurrence and development of cancers,such as leukemia,breast cancer,colorectal cancer and lymphoma.The interaction between S1PR1 and signal transducer and activator of transcription 3 (STAT3) is shown to take part in the pathogenesis of malignant tumors as well.Therefore,they are considered to have the potential tumor-treatment value.
4.Intrathecal injection of dexmedetomidine attenuates neuron disruption induced by spinal cord ischemia-reperfusion injury in rabbits

Objective To investigate the beneficial effects and possible mechanisms of intrathecal injection of dexmedetomidine (DEX) in protection of spinal cord ischemia-reperfusion injury.Methods Spinal cord ischemia-reperfusion injury was induced in rabbits by infrarenal aortic occlusion for 30 minutes.Twenty-four rabbits were randomly divided into four groups:sham group,I/R group,DEX group (1 μg/kg dexmedetomidine by intrathecal injection),DEX + ATIP group (1 μg/kg dexmedetomidine and 0.5 μg/kg atipamezole (ATIP) by intrathecal injection).Physiological indices were assessed and motor neurons in the ventral gray matter were counted by histological examination.The expression of total extracellular signal-related kinase (t-ERK),phospho-extracellular signal-related kinase (p-ERK),and caspase 3 were assessed by Western blotting and real-time polymerase chain reaction (RT-PCR).The water content in spines was also measured.Results Intrathecal injection of dexmedetomidine minimized the neuromotor dysfunction and histopathological deficits(F =14.32,P < 0.05) and attenuated the influences of peroxidation at 24 hours after spinal cord ischemia-reperfusion injury.The physical indices during experiment were comparable among all groups.In addition,intrathecal injection of dexmedetomidine reduced the excessive expression of caspase 3 mediated by ERK signal pathway(F =15.45,P < 0.05).Conclusions Pre-emptive intrathecal injection of dexmedetomidine produced beneficial effects on spinal cord after spinal cord ischemia-reperfusion injury in a rabbit model of transient aortic occlusion.This beneficial effect was partly attributed to the inhibition of caspase 3 mediated by ERK signal pathway,which represents a potential therapeutic approach to mitigate spinal cord injury after aortic occlusion.
5.Effects of indole-3-carbinol on hepatic stellate cells activated by acetaldehyde in precision-cut liver slices

Aim To investigate effects of indole-3-carbinol (I3C) on hepatic stellate cells (HSCs) activated by acetaldehyde in precision-cut liver slices (PCLS). Methods PCLS were incubated with 700 ?mol?L-1 acetaldehyde and 200 ~ 800 ?mol?L-1 I3C for 6 h. The expression of ?-smooth muscle actin(?-SMA) in liver slices was analyzed by immunohistochemistry. The leakages of glutathione S-transferase (GST), lactate dehydrogenase (LDH) and content of transforming growth factor-?1 (TGF-?1) in media were assayed. Contents of malondialdehyde (MDA) and hydroxyproline (Hyp) in tissue were also determined. Expressions of matrix metalloproteinases-1 (MMP-1) and tissue inhibitor of metalloproteinase (TIMP-1) in media were analyzed by the western blot. Results The increase of activated HSCs due to acetaldehyde was inhibited by I3C (200~800 ?mol?L-1). Meanwhile, I3C treatment (200~800 ?mol?L-1) showed significant and concentration-dependent antagonistic actions on the increment of GST, LDH leakages into the media and MDA, Hyp contents in tissues induced by acetaldehyde. The increase of TGF-?1 was also remarkable inversed by I3C (200~800 ?mol?L-1). 6.Biomechanic study on reduction of thoracolumbar burstfracture with pedicle screw

Objective To supply safe mechanic refererence for reduction of thoracolumbar burstfracture with pediclc screws and lay biomechanic foundation for developing a new type of digitizing pediclc screws. Methods The change of stress and its scope of pediclc screws during reduction of thoracolumbar burstfracture were tested to determine the relationship of the degree of reduction with stress and strain. Results A total of 15 burstfracture specimens were obtained successfully. Stress was small and changed mildly at the beginning of reduction stage. Stress presented transparent elevation tendency at the stage of body height of 75% of intact until at the stage of 90%. When the height of vertebral body restored from 90% to 95%, the curve of stress changed gently. The maximum stress of left screw was 98.4?10.3 and the right was 101.2?13.4. The stress of bilateral screws did not differ significantly. It was calculated that the left strain was (34.64?3.84)N and the right one was (35.544?3.61)N, the moment was (5.20?0.58)Nm of the left and (5.33?0.54)Nm of the right. There was not significant difference between the strain and the moment of bilateral screws. Conclusion The safe range of reduction force is between 4.5Nm and 6Nm. The burstfracture of vertebral body can be both reduced successfully and prevented from overpropping.
7.Clinical characteristics of solitary plasmacytoma of temporal bone.

9.Bonding between nickel-based alloys and resin-veneers: strength and mechanism

Objective:To determine the shear bond strength and analyze the bonding mechanism of home-made and imported nickel-based alloys to resin-veneers.Methods: Home-made and imported alloy disks were cast by lost-wax technique and embedded in acrylic resin fabricating cylinders.Sixteen specimens of each alloy were divided into 2 subgroups: thermocycling group and non-thermocycling group.Specimens in thermocycling groups were submitted to thermocycling(300 cycles) and those in non-thermocycling groups were just stored in water at 37?C for 24 h(0 cycles).Then the shear bond strength values were determined by universal testing machine.The highest shear bond strength values were recorded and metallurgical analysis was performed on the interface between alloys and resin-veneers.Results: The shear bond strength values of both alloys to resin-veneers were relatively high either before or after thermocycling.The shear bond strength values of home-made alloy+thermocycling group,imported alloy+thermocycling group,home-made alloy+non-thermocycling group and imported alloy + non-thermocycling group were(10.50?2.72),(13.12?2.82),(15.51?3.04) and(18.44?3.15) MPa,respectively.The bond strength was significantly different before and after thermocycling for both alloys(P
