Objective The management of fluid infusion is crucial in severe sepsis/septic shock patients.The correlation of extravascular lung water index(EVLWI) versus oxygenation index ( PaO2/FiO2 ) and EVLWI versus intrathoracic blood volume index(ITBVI) were analysed in this present study. Method Totally 24 patients,admitted to the Intensive Care Unit of Second Affiliated Hospital of Zhejiang University, College of Medicine and diagnosed as severe sepsis/septic shock with acute lung injury and/or acute respiratory distress syndrome,were enrolled. ITBVI and EVLWI were detected with PiCCO technique. Correlation of EVLWI and PaO2/FiO2, ITBVI and EVLWI were analysed,respectively. Simple correlation and simple linear regression were used for statistical analysis. Results Significant negative correlation was found of EVLWI and PaO2/FiO2 ( r = - 0. 45, P ＜ 0.01).EVLWT = 14 mL/kg was defined as the cutoff value for the subgroup analysis. No correlation was found between EVLWI and PaO2/FiO2 in the subgroup with EVLWI≤ 14 mL/kg ( r = 0. 12, P = 0.243), but in the subgroup with EVLWI ＞ 14 mL/kg, significant negative correlation was found ( r = - 0. 47, P ＜ 0. 01 ). When EVLWI was higher than 14 mL/kg,EVLW should be decreased to improve oxygenation and other aspects should be taken into account. No significant correlation was found between ITBVI and EVLWI. A ITBVI value 1000 mL/m2 was also defined as the cutoff value for the subgroup analysis. No significant correlation was found in the subgroup with ITBVI≤ 1000 mL/m2( r = 0.13, P = 0.17), while significant positive correlation was found in the subgroup with ITBVI ＞ 1000 mL/m2. This result suggested that in patients of severe sepsis/septic shock with ALI/ARDS, when the blood volume is high, ITBV should be decreased to improve the oxygenation,however,it is not useful in the situation of high pulmonary vascular permeability. Conclusions Extravascular lung water has a important role in the fluid management in patients of severe sepsis/septic shock with ALI/ARDS.

Objective To explore the effect of PDCA circulation on the effective utilization of the disposable articles in the operating room.Methods 9,362 aseptic articles for 446 operations in the departments of general surgery and hepatobiliary surgery during January to May 2012 were put in the pre-experiment group,in which PDCA circulation was not used.Another 11,973 articles for 501 operations in the same departments during June to November 2012 were set as the post-experiment group,in which PDCA circulation was used.The two groups were compared in terms of the effective utilization rate and the degree of doctor’s satisfaction with the nurses in the operation room.Result After the implementation of PDCA circulation,the effective utilization rate and the degree of doctor’s satisfaction were both significantly higher than before the implementation(both P<0.05).Conclusion PDCA circulation for the management of sterile disposable articles in the operation room may effectively improve the utilization rate of disposable articles, reduce wasting of articles and safe resources.

Research of co-authorship network can reveal the scientific research collaboration network and can thus help us to have an understanding of it. Graphic database and Neo4J were described in detail due to the limitations of relationship database in processing the data of co-authorship network. Graphic database Neo4J-based research and practice of co-authorship network were analyzed with the Institutional Knowledge System of AMMS that we were involved in its construction as an example, and the advantages of Neo4J-based co-authorship network were summarized.

Objective To explore the effects of self-differentiation,social support,social adaptation on suicidal ideation.Methods A simplified cluster sampling method,involving the random selection of 3097 college students in 23 departments,was used to estimate the scores of Chinese version of Self Differentiation Inventory for university student(SDI),Social Support Rating Scale(SSRC),College Student Adaptability Scale (CSAS) and Adult Suicidal Ideation Questionnaire(ASIQ).The independent-samples t-test,Pearson correlation coefficients and stepwise regression analyses were used as statistical analysis techniques.Results The correlation coefficients were negatively significant between suicidal ideation and self-differentiation,social support and social adaptation (r=-0.24,-0.17,-0.29,P＜0.001).Social adaptation showed the strongest predictability for suicidal ideation (β=-0.19,P＜0.001),followed by self-differentiation (β3=-0.13,P＜0.001),social support (β3=-0.08,P＜0.001).Independent samples t-test showed that college students with suicidal ideation showed a significant reduction in interpersonal adaptation,social support,self-differentiation compared to those without suicidal ideation.Conclusion Self-differentiation level,social support and social adaptability may be effective predictors for suicidal ideation among college students.Future intervention programs may focus on enhancing the interpersonal skills of college students to reduce prevalence of suicidal behavior.

Objective To analyse clinical application of hepatitis B virus core antibody(HBcAb)detected by using the chemilu-minescence microparticle immunoassay.Methods A total of 1 6 830 specimen with positive HBcAb detected by using the two pairs of semi-hepatitis test from January 2012 to November 2014 were collected,and divided into three groups according to the cut off in-dex(COI)of detection results of HBcAb,including group 1.0-<9.0,group 9.0-<1 1.0 and group COI≥1 1.0,and detection re-sults were statistically analysed.The hepatitis B virus(HBV)DNA test was carried out in specimen with negative hepatitis B surface antigen(HBsAg)and hepatitis B surface antibody (HBsAb)and COI≥1 1.0.Results The detection rate of HBsAg(+)HBsAb(-) (13.84%)was significantly higher than other expression patterns in group ≥1 1.0(P <0.05).There was no statistically significant differences in positive rate among all expression patterns of HBsAg and HBsAb in the group 9.0-<1 1.0(P >0.05).The detec-tion rate of HBsAg(+)HBsAb(-)of group 9.0-<1 1.0 was significantly lower than that of the other two groups(P <0.05).A total of 304 specimen were HBsAg(-)HBsAb(-)and COI≥1 1,among them 64 specimen were HBV DNA postive and the posi-tive rate was 21.0%.Conclusion In the detection of HBcAb,COI≥1 1 and 1.0-<9.0 could be reference indicators for diagnosiing current and past HBV infection respectively,which should be combined with other laboratory indicators of HBV clinical data for comprehensive analysis.

Objective To investigate the effect of rapid rehabilitation surgery nursing in patients in the department of hepatobiliary surgery.Methods A total of 160 patients in our department were randomly divided into observation group and control group, with 80 cases in each group.The control group was given routine nursing during perioperative period, and the observation group was given rapid rehabilitation surgery nursing, and surgical stress, postoperative rehabilitation and hospital costs of the two groups were compared.Results The levels of cortisol, epinephrine, CRP, HAMA after 1 d of the operation in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05).Postoperative bed-off time, anal exhaust time, defecation time, hospitalization time in the observation group were shorter than that in the control group, complications, hospitalization costs were less than that in the control group, the differences were statistically significant(P<0.05).Conclusion Rapid rehabilitation surgical nursing can reduce the surgical stress of patients in department of hepatobiliary surgery, accelerate the postoperative recovery, reduce the treatment cost, so it is worth promoting.

Objective To investigate and analyze the similarities of allergens in children of the same family with susceptibility to the same genetic background and environmental factors. Methods From January 2013 to December 2017,a total of 142 pairs of children with allergic diseases aged 0-15 years and their siblings ( 284 cases) were collected from the outpatient or hospitalized treatment of Pediatrics and otolaryngology department of the First People′s Hospital of Foshan City,the Third Affiliated Hospital of Sun Yat-sen University. The elder group was brother or sister (142 cases),and the younger group was brother or sister ( 142 cases ) . The serum allergen sIgE was tested by Allergy Screen allergen detection system (developed by Germany MEDIWISS Company) and the similarity of allergens was analyzed. Results A total of 142 siblings ( 284 cases ) had systemic symptoms of allergic diseases in varying degrees, including respiratory symptoms ( cough ( 47 cases in the old group,32 cases in the young group), χ2 ＝3. 946, P＝0. 047),nasal obstruction ( 41 cases in the old group,19 cases in the young group, χ2 ＝ 10. 227, P ＝0. 001),runny nose (46 cases in the old group,26 cases in the young group,χ2＝7. 442,P＝0. 006), gastrointestinal symptoms (abdominal pain (11cases in the old group,7 cases in the young group,χ2＝4. 63, P＝0. 031),skin symptoms(urticaria (18 cases in the old group,8 cases in the young group,χ2＝4. 234,P＝0. 037)) and so on,the difference was statistically significant. Some children have multiple organ system symptoms at the same time. There was no significant difference between the old group and the young group in the early use of antibiotics,mode of production and feeding mode within 6 months (all P＞0. 05). Among the two groups,house dust mite and cockroach (r＝0. 831,P＜0. 05),dog hair and house dust ( r＝0. 717,P＜0. 05),cypress,elm,willow,birch,oak,maple,walnut,sycamore,poplar and Dianqing,branching,yanqu, and heiqu ( r ＝ 0. 683, P＜ 0. 05 ) . Conclusion With the same genetic background and the same environmental factors,the allergens in siblings are similar.

Objective: To investigate the association between plasma selenium exposure and the risk of impaired glucose regulation (IGR). Methods: A case-control study was conducted to select IGR patients who were admitted to the outpatient clinic of the Department of Endocrinology to perform oral glucose tolerance test(OGTT) at the Tongji Hospital affiliated to the Tongji Medical College from September 2004 to 2016 as a case group. Participants with normal glucose tolerance recruited from an unselected group of population undergoing routine health examinations in the same hospital were selected as a control group. The control group was matched according to the age (±5 years old) and sex of the case group. The inclusion criteria for subjects recruited were as follows: age ≥30 years, body mass index (BMI) <40 kg/m², no history of a diagnosis of IGR or type 2 diabetes, and no history of receiving pharmacological treatment for hyperlipidemia or hypertension. Patients with any clinically systemic disease such as neurological or endocrine disease, acute illness, chronic inflammatory disease or infectious disease were excluded from the study. A total of 1 957 subjects, 897 in the case group and 1 060 in the control group, were included. Questionnaires were used to collect information of all subjects, and peripheral venous blood was collected after fasting and OGTT, respectively. Plasma selenium, fasting blood glucose, blood lipid (total cholesterol, triglycerides, high density lipoprotein cholesterol, and low density lipoprotein cholesterol) and 2 h OGTT plasma glucose concentration were detected, respectively. The subjects were divided into low, medium and high concentration groups according to the tertiles of plasma selenium concentration in the control group. The multivariate unconditional logistic regression analysis was performed to analyze the association between plasma selenium exposure and IGR. Results: The age (mean±SD) of the case and control group was (53.71±11.38) and (53.95±12.17) years old. The plasma selenium concentration [M (P₂₅, P₇₅)] in the case group was 92.81(77.07, 107.05) μg/L, which was significantly higher than the control group [88.73 (77.13, 100.88) μg/L] (P<0.05). The results of multivariate unconditional logistic regression analysis showed that after adjusting for age, sex, BMI, family history of diabetes and hypertension, the risk of IGR was higher in the high-concentration group and the low-concentration group compared with the middle-concentration group, the values of OR (95%CI) were 1.22 (95%CI: 0.94-1.59) and 1.81 (95%CI: 1.42-2.30), respectively. Conclusion The study suggested a U-shaped association between plasma selenium and IGR.
