1.Application analysis of ultrasound in diagnosis of early-onset breast cancer

Objective To observe the early onset breast cancer ( EOBC) sonographic performance character-istics and explore the diagnostic value of ultrasonography in EOBC .Methods 60 cases EOBC patients younger than 35 years old were seclected as the observation group .Older than 35 years of 50 patients with breast cancer were se-clected as the control group .Two groups were underwent breast color Doppler ultrasound .Results 59 cases in 60 pa-tients with preoperative ultrasound diagnosis of breast cancer ,one case was misdiagnosed as mastitis pseudotumor infil-trating ductal carcinoma ,lobular carcinoma ,ductal carcinoma in ultrasound showed more direct signs ,medullary carci-noma showed more indirect signs .Clumps were main types of breast cancer of the observation group and control group , respectively,accounted for 72.9% and 87.5%,the difference was statistically significant (χ2 =6.293,P<0.05). Two groups of color Doppler richness , lesion shape , boundary definition , without hyperechoic halo , calcification and glitches levy or levy Crab compared were not significant (P>0.05);Compared with the control group ,heterogeneous internal echo,rear attenuation of observation group had significant differences (P<0.05).Foci were observed group EOBC anteroposterior diameter and aspect ratio were significantly lower than that of the control group ,the difference was statistically significant(t =6.720,9.285,all P <0.05);diameter was significantly higher (t =34.962,P <0.05).Conclusion Early-onset breast cancer at diagnosis with color Doppler ultrasound large diameter aspect ratio is small,and the rear common heterogeneous internal echo attenuation less characteristic features ,screening younger women for breast cancer is important .
2.Observation of the effect of dexamethasone on postoperative inflammatory response of patients with aortic dissection stent

Objective To analyze the efficacy of prophylactic dexamethasone in the treatment of aortic dissection with stent graft.Methods The clinical data of 87 patients who received aortic dissection with stent implantation were retrospectively analyzed.The treatment group(39 cases)was treated with dexamethasone on the basis of antibiotic prophylaxis to prevent infection,and the control group(48 cases)received antibiotics to prevent infection perioperatively.The curative effect was compared between the two groups.Results The degree of inflammatory reaction in the treatment group significantly reduced,and the treatment group had obvious curative effect(x2=54.88,P<0.01).The total effective rate,significant effective rate and effective rate of the treatment group were 92.31%,61.52%,7.70%,respectively,which were significantly higher than those of the control group(13.00%,6.25%,6.25%),the difference was statistically significant(x2=54.88,P<0.01).Conclusion Prophylactic use of dexamethasone in the treatment of systemic inflammatory response after aortic dissection with stent implantation has significant effect.
3.Effects of the maltitol-gum on cariogenic bacteria

Objective: To determine the inhibitory effects of maltitol-gum on Streptococcus mutans, Lactobacilli and Actinomyces viscosus in dental plaque. Methods: Thirty 13-15 years old children with DMFS>4 were divided into three groups, maltitol chewing gums group(A group), xylitol chewing gum group(B group) and blank gum base group (C group). The plaque samples were collected and colony forming units were counted. Results: The levels of three-species cariogenic pathogens in three groups were statistically down-regulated when compared with the baseline(P<0.001).Moreover, A group and B group resulted in a higher decrease of Streptococcus mutans levels compare with C group(P<0.05). The levels of Lactobacilli and Actinomyces viscosus were not statistically different between groups(P>0.05). Conclusion: Maltitol-gum can lead to a significant suppression on Streptococcus mutans levels in dental plaque,while the inhibitory effect of the maltitol-gum on Lactobacilli, Actinomyces viscosus is not obvious.
4.Questionnaire analysis of related factors of male pattern alopecia and clinical efficacy of finasteride therapy in 320 patients

Objective To study the related factors of male pattern alopecia(MPA) and efficacy of finasteride as well as to survey the related factors influencing the life quality of patients with MPA.Methods Questionnaire with dermatology life quality index(DLQI) and center for epidemiologic studies depression scale(CES-D) were adopted to assess the life quality and psychological changes of 320 outpatients with MPA.The 320 patients with MPA of Hamilton grades Ⅱ-Ⅶ took finasteride tablet of 1mg/day for over 4 months in this clinical tria1.We analyzed the related factors of MPA,tested the related indexes,and observed for changes of hair growth and adverse reactions.Results DLQI and CES-D scores of the 320 MPA patients obviously increased.Many factors including inheritance and psychological influence were correlated with MPA.The total efficacy rate after treatment for over 4 months was 80.8%.Conclusion Hair loss significantly affected the life quality of patients with MPA and brought negative effects on the patients' psychology,which can worsen hair loss.Therefore,appropriate psychotherapy and other treatments should be given to them.MPA is a kind of disease caused by multiple factors and oral administration of finasteride(1mg) is effective and safe for treating MPA.
5.Methyl Carboprost-NEGPF Administered at Different Time in the Prevention of Postpartum Hemorrhage

OBJECTIVE:To observe the efficacy of Methyl Carboprost-NEGPF administered per rectum at different time in preventing postpartum hemorrhage in patients with vaginal delivery.METHODS:A total of 300 parturients undergoing vagi-nal delivery with possible postpartum hemorrhage were enrolled:Methyl Carboprost-NEGPF 1 granule(1mg) per rectum (with a suitable inserting depth of 6 cm) were administered in Group A(n=100) when their uterine orifices were fully open and Group B(n=100) after fetal delivery,while Group C(n=100) were injected i.m with oxytocin 10IU after fetal delivery. 6.Diffrences in improved surgery and normal excision of large dermoid cyst by laparoscope

Objective To investigate the effectiveness and surgical techniques of the improved laparoscopic treatment of ovarian dermoid cyst to prevent the occurrence of intraoperative rupture of teratomat, so to preserve the ovarian function at the maximum. Methods The modified laparoscopic extra-cavity and cavity outside the teratoma removed surgical suture treatment from February 2005 to February 2009,39 cases with large dermoid cyst were trea-ted by improved surgery and 45 cases treated by normal removal of laparoscope. The rupture, surgery time, bleeding volume,frequency of use of electrocoagulation and the effect of body temperature on intestinal canal and hospitaliza-tion day after operation as well. Results 39 cases of dermoid cyst were successfully managed by improved laparo-scopie surgery, during which no cyst contents broke into the abdominal cavity, no one was placed on electrocoagula-tion to stop bleeding, bleeding volume was (35.13±5.49) ml, operative time was (36.07±12.53) min, significantly leas than that of normal laparoscopic surgery group [the cyst rupture rate was 46.7% (21/45), the frequency of elec-trocoagulation was (5.0±3.0) times, the duration of electrocoagulatian was (5.5±2.5) s, bleeding loss was (40.73±6.04) ml and the time for operation was (67.47±20.73) min], with significant difference between the two groups (P<0.05 or 0.01);howevere, there was no remarkable difference in the effect of the two types of surgery on postoperative body temperature,anal exhaust time,postoperative hospitalization day. Conclusions In addition to minimally invasive laparoscopic surgery in the treatment of gastro-intestinal effects of small, quick recovery, the im-proved laparoscopic surgery for dermoid cyst shortens the operation time. The improved abdominal cavity and suture outside the strip surgical method can effectively prevent the complication caused by intraoperative tumor capsule rup-ture,reduce electrocoagulation injury,and preserve the ovarian function at the maximum.
7.Effect of neurotrophin-3 on Akt expression during ropivacaine-induced neurotoxicity to spinal cord of rats

Objective To evaluate the effect of neurotrophin-3 (NT-3) on the expression of serine/threonine protein kinase (Akt) during ropivacaine-induced neurotoxicity to the spinal cord of rats.Methods Healthy adult male Sprague-Dawley rats,aged 1-2 months,weighing 280-320 g,were used in the study.A catheter was inserted at L5,6 interspace into the epidural space of rats.A total of 108 rats,in which intrathecal catheters were successfully implanted,were randomly divided into 3 groups (n =36each):control group (group C),1% ropivacaine group (group R),and 1% ropivacaine + NT-3 group (group NT).The equal volume of normal saline was given in group C,1% ropivacaine 0.12 ml/kg was injected via the intrathecal catheter once every 1.5 h for 8 times in total in R and NT groups.In addition,NT-3 0.1 mg/kg was simultanenously injected via the intrathecal catheter in group NT.On days 1,3,5,7,14 and 28 after the end of administration (T1-6),6 rats were sacrificed in each group.Their lumbar enlargements were removed for determination of neuronal apoptosis (using TUNEL) and Akt expression (by immuno-histochemistry).The apoptotic rate was calculated.Results Compared with group C,the apoptotic rate was significantly increased at T1-4,and Akt expression was significantly up-regulated at T1-3 in group R,and the apoptotic rate were significantly increased,and Akt expression was significantly up-regulated at T1-3 in group NT (P<0.05).Compared with group R,the apoptotic rate was significantly decreased at T3,4,and Akt expression was significantly down-regulated at T2.3 in group NT (P<0.05).Conclusion The mechanism by which NT-3 reduces ropivacaine-induced neurotoxicity to the spinal cord may be related to down-regulation of the expression of Akt in rats.
8.Antioxidants protects against peripheral nerve damages induced by antiepileptic drugs in rats

Objective To explore the protective efficacy of antioxidants (vitamin E and Ginkgo biloba extract) on the peripheral nerve damages induced by antiepileptic drugs (AEDs). Methods Adult (2-month old) and infant SD rats (7-day old) were respectively treated with AEDs (phenytoin,or phenobarbital,or clonazepam) alone,or simultaneously with vitamin E or Ginkgo biloba extract for 4 weeks. All the rats were sacrificed,sciatic nerves and serum were collected. AIM:To compare the effects of remifentanil and fentanyl for posterior scoliosis correction and the influences on postoperative pain.METHODS:Forty patients scheduled for posterior scoliosis correction(ASAⅠ-Ⅱ,aged 11-18 years)were randomly divided into group remifentanil(group R)and group fentanyl(group F).Anesthesia was maintained with propofol,vecuronium bromide and sevoflurane.Intravenous remifentanil(0.2 ?g?kg-1?min-1)and fentanyl(1 ?g?kg-1?h-1)were continuously administered for analgesia.Vecuronium bromide and analgesics were discontinued around 30 min before the wake-up test.The wake up time from propofol and sevoflurane discontinuation to movement of the patient's toe were recorded.In group R,once remifentanil infusion was stopped before surgery ended,the patients were injected with fentanyl 2 ?g/kg.All of the patients received intravenous fentanyl(10 ?g?kg-1?min-1)for postoperative analgesia and were interviewed for pain degree during the first 24 h after surgery.RESULTS:The wake-up time were(12.3?5.7)min in group R and(21.6?6.5)min in group F(P
10.Clinical Fungal Infections and Their Drug Resistance

OBJECTIVE To analyze the clinical cases of fungal infections and drug resistance to provide a basis for the treatment of mycotic infection.METHODS A total of 215 cases of fungal strains were identified by API 20C AUX.Drug susceptibility was determined by Rosco slip diffusion.RESULTS In 215 fungal strains of specimens,Candida accounted for 87.9%,of which C.albicans accounted for 37.2%.The yeast-like fungi sensitivity rate to amphotericin B,nystatin and ketoconazole respectively was 100.0%,97.9% and 93.5%.CONCLUSIONS Candida are the most common pathogens in the 215 fungal stains.Yeast-like fungi is sensitive to amphotericin B,nystatin and ketoconazole.
