1.Epstein-Barr virus and inflammatory bowel disease

Epstein-Barr Virus (EBV) is the first human virus related to oncogenesis.EBV infection is associated with inflammatory bowel disease (IBD) with unknown causality.The prevalence of EBV in intestinal tissue from patients with IBD is significantly higher and related to the exacerbation of the disease and refractory IBD.Immunosuppressive therapy has improved outcomes associated with IBD.However,it is also associated with an increased risk of opportunistic infection,and lymphoproliferative disorders (LDs) maybe due to EBV infection.Here we review our current understanding of the pathogenesis of EBV infection in colonic mucosal inflammation,EBV-induced disease exacerbation,lymphomagenesis in IBD,and clinical approaches therefrom.
2.Effects of Danshen injection on the malignant obstructive jaundice in the SD rat model.

To observe the effects of Danshen on the growth of hepatocellular carcinoma in the SD rats, a model of malignant obstructive jaundice was established by inoculation of transplanted tumor into the hepatic portal with the walker-256 hepatocarcine line, which resulted in the obstruction by the infiltration and metastasis of hepatocellular carcinoma. SD rats were divided into 4 groups: the rats were treated by 0.9 % NS (n=24, control group), inosine+vitamin C (n=40, InV group), Danshen (n=40, DS group) and 5-FU (n=40, 5-FU group), respectively. The liver function, morphological changes and the expressions of PCNA, VEGF and ICAM-1 in carcinoma foci, peri-carcinoma tissues, adjacent lobe (left-internal lobe) and lung tissues were observed after the treatment with the 4 agents. Our results showed that the protective effect of Danshen on liver function was significantly better than that of NS and 5-FU (P<0.01). No significant difference in protective effect was observed between DS group and InV group (P>0.05). Danshen also provided protective effect on the morphological damage of liver caused by obstructive jaundice. The rates of carcinoma-inhibition and metastasis inhibition were significantly higher than those of NS and inosine+vitamin C (P<0.01). No significant difference in this regard existed between DS group and 5-FU group (P>0.05). The expressions of PCNA,VEGF and ICAM-1 PCNA, VEGF and ICAM-1 in carcinoma foci, peri-carcinoma tissues, adjacent lobe (left-internal lobe) and lung tissues were lower than those in control group and InV group, with the differences being significant (P<0.01). No significant differences were found between DS group and 5-FU group in the expression levels of PCNA and VEGF (P>0.05) but ICAM-1 (P<0.05). It is concluded that Danshen injection not only has protective effects on liver injury caused by obstructive jaundice, but can inhibit the proliferation and growth of hepatocarcinoma, interfere with the vascularization of tumors, prevent recurrence and metastasis of hepatocarcineoma.
3.Determination of Serum Tryptophan and Kynurenine in Pregnant Women

[Objective] To establish a method for determination of serum tryptophan and kynurenine in pregnant women.[Method] RP-HPLC was applied to analysis.C18 column was used.The mobile phase was composed of methanol-0.001M KH2PO4(PH=4)(12∶88).The UV detection wave length was 245nm.[Results] The linear response ranged from 2.56~307.06?mol?L-1 for tryptophan, r=0.9999.The linear response ranged from 0.12~5.05?mol?L-1 for kynurenine, r=0.9997.The mean concentrations of serum tryptophan and kynurenine were 49.73?7.48?mol?L-1 and 0.74?0.33?mol?L-1 respectively in 30 pregnant women.[Conclusion] The method is sensitive and suitable for the determination.
4.Chitosan gel (Ⅲ) reduces the complications from Milligan-Morgan hemorrhoidectomy for treatment of severe mixed hemorrhoids

Objective To explore the effect of Chitosan gel (Ⅲ) on the complications from Milligan-Morgan hemorrhoidectomy for treatment of severe mixed hemorrhoids.Methods Seventy-six patients with severe mixed hemorrhoids were randomly divided into treatment group and control group with 38 cases in each group. All patients from the two groups were treated with Milligan-Morgan hemorrhoidectomy. Then Chitosan gel (Ⅲ) was used in the treatment group. The two groups were compared in terms of curative effect, pain level, complications and hospital stay.Results There was no significant difference in the curative effect (P>0.05). But the scores on pain level, hemtochezia, infection and edema of the treatment group were all significantly lower than those of the control group (allP<0.05) and the hospital stay was significantly shorter than that of the control group (P<0.05).Conclusions In the treatment of severe mixed hemorrhoids by Milligan-Morgan hemorrhoidectomy, Chitosan gel (Ⅲ) is effective in relieving pains, reducing hemorrhage, edema and preventing infections and therefore shortens the hospital stay. In view of nursing, it is critical to perform mental care as well as close observations on complications.
5.Exposure and Protection of ICU Care Workers

OBJECTIVE To explore the risk factors and protective measures on ICU attendants.METHODS According to the risk factors of ICU occupational exposure,we developed and standardized the implementation of the protective measures.RESULTS The occupational exposure protection awareness,prevention methods,and self-protection were improved.CONCLUSIONS The education about occupational of exposure can improve the management of occupational safety and ensure the protection of physical and mental health of attendants,and to prevent the occurrence of hospital infection.
6.Causes of stereoscopic function abnormality in elderly patients after the cataract extraction with intraocular lens implantation

Objective To analyze the reasons and mechanisms of stereoscopic function abnormality in elder patients after the cataract extraction with intraocular lens implantation. Methods 150 cases with artificial lens implantation were randomly selected.Routine eye examinations were carried out and the associated refractive error and presbyopia were corrected.Bilateral simultaneous visual perception and fusion function were examined.The stereoscopic function was tested using "the stereoscopic function examination diagram"created by Yan Shao-ming.Ninety-seven patients who were found to have unrecover-ed or abnormal stereoscopic function were enrolled for analysis. Results In these 97 cases,53(54.6%)were found to have preexisting eye disorders that could affect visual acuity and binocular single vision before the operation.Macular problem was the most prevalent problem in this group.Twenty-seven(27.8%)patients had complicated with corneal astigmatism,after cataract,paralysing strabismus and diplopia as well as macular edema after the operation.In addition,the contralateral unoperated cataract in 17(17.5%)patients and post-operative anisometropia in 9(9.3%)patients were also the causes of stereoscopic function abnormality.There was no reason could be identified in 8 cases. Conclusions The pre-existing eye disorders before lenses implantation,complications of the operation,contralateral unoperated cataract and anisometropia are all the major factors that affect visual acuity recovery and bilateral stereoscopic function rehabilitation.
7.The analysis of clinical factors affecting the therapeutic efficacy of traumatic optic neuropathy

objective To evaluate the therapeutic efficacy of surgical and nonsurgical treatment and the clinical factors affecting the efficacy in traumatic optic neuropathy. Methods To analyse retrospectively the efficacy of recovery of visual acuity in 40 cases of traumatic optic neuropathy after treatment with transnasal endoscopic sphenoethmoidal optic canal decompression (28 cases) and drug therapy (12 cases). Results No significant difference existed between the therapeutic efficacy of surgery and that of drug therapy in patients with the visual acuity of LP~0.02. In surgery group,the therapeutic efficacy of the patients with visual acuity of LP~0.02 was better than that of the patients with no LP.The therapeutic efficacy of patients with duration shorter than seven days before sutgery is better than that of patients with duration longer than seven days. Conclusions The patients with serious traumatic comperssive optic neuropathy should not be treated with decompressive surgery and should not delay to at most seven days after injury.With or without the visual acuity of light perception of the affected eye surgery is usually an important factor affecting the therapeutic efficacy.
8.Methodological study of preparing iris pigment epithelial cells for autologous transplantation in rabbits

Objective To explore a better method in obtaining iris pigment epithelium (IPE) specimen for autologous transplantation in rabbits. Methods IPE was obtained from 20 black rabbits with method A, i.e., surgical peripheral iridectomy at 12:00 position obtaining a triangle iris tissue with the hemline of 4-5 mm in left eyes,and method B,i.e., surgical peripheral iridectomy at 11:00 and 1:00 positions obtaining two triangle iris tissues with the hemlines of 2-2.5 mm in right eyes . The IPE cells were isolated precisely with enzyme microdissection-enzyme isolation method, cultured in vitro, observed with light and electronic microscope, and identified with immunocytochemical staining. Results The success rate of cells culture were 65% for method A and 95% for method B. After 3-4 generations of culturing,the amount of IPE cells was enough for transplantation, and most of the functions of primary clutured IPE cells were kept still. Viability of IPE cells was 85%-93%. Conclusion The success rate of cells culture for method B is higher than that for method A. The third generation of cultured cells is available for autologous transplantation.
9.Analysis of pain management and satisfaction on pain management of home-based advanced cancer patients with pain

Objective:To detect the pain status and the pain management of home-based advanced cancer patients, and to investigate the key factors impacting the satisfaction on pain management.Methods:Totally the data of 138 cases from the hospice department in the duration from Jan. 2019 to Jun. 2019 were collected and analyzed by χ2 test with SPSS 23.0. Results:109 cases in 138 cases received analgesics treatment before, 54.13% of these participants expressed satisfaction with the effect of their pain management, and 45.87% showed unsatisfied on their pain management. Cancer clinical stage and PMI (pain management index) were both detected to be the significant factors for satisfaction of pain management ( P<0.05, P=0.029; P<0.01, P=0.001) . Conclusions:Cancer clinical stage and PMI are found to be related to the satisfaction for pain management. The percentage of patients who use opioids increased. while there was still a part of patients suffering from inadequacy of pain management. The pain management for home-based advanced patients needs to be paid more attention by medical staff.
10.Effect of visual function on posterior capsule opacification before and after Nd:YAG laser posterior capsulotomy

Objective To explore the influence of stereoscopic vision on the development of posterior capsule opacification (PCO) after intraocular lens implantation. The efficacy of Nd: YAG laser was studied. Design Case-controlled study. Participants 57 consecutive patients with POC(59 eyes with phacoemulsification and IOL implantation). Methods A prospective observational study was conducted comprising 57 patients (59 eyes) who had postoperative PCO. Neodymium: YAG (Nd: YAG) laser posterior capsulatomy was performed on all patients following a standardizes procedure. Preoperative and postoperative evaluation included LogMAR visual acuity, best-corrected visual acuity (BCVA), contrast sensitivity and stereoscopic vision. Main Outcome Measures LogMAR visual acuity, BCVA, contrast sensitivity and stereoscopic vision. Results The average LogMAR eyesight median of sick eye before laser treatment was (0.22?0.31) and the contrast sensitive degree of each frequency area was relatively lower than the opposite eye; After the treatment, there was evident improvement of the sick eye, and the LogMAR eyesight median of that was 0.00; The ratio of reaching the central spatial eyesight and the average sharpness degree both improve much than ahead of treatment. Those whose spatial eyesight sharpness don't get normal after the low best remedy eyesight. Irregular dipodic and previous eye disease. Conclusions LogMAR visual acuity, BCVA, contrast sensitivity and stereoscopic vision were damaged markedly with PCO. Our results indicate that Nd: YAG capsulotomy may affect the reconstruction of stereoscopic vision. Nd:YAG laser posterior capsulotomy is a valuable and clinically relevant method for clinical treatment of PCO.
