Objective To study the changes of the blood Platelet activating factor (PAF)levels in neonatal hypoxic ischemic encephalopathy(HIE),and explore the relationship between PAF levels and severity of brain damage. Method The levels of blood PAF and SOD were measured in neonates with HIE by radio immunoassay and compared with that of normal term neonates. Results Blood PAF levels were much higher in acute stage of HIE,and PAF levels increased with the severity of HIE.Blood SOD and platelets were much lower compared with PAF. 3.Immunological Study on Neonatal Hypoxic-ischemic Encephalopathy

Objective To explore the immunologic changes and its immunologic mechanism in neonatal hypoxic ischemic encephalopathy(HIE).Methods T lymphocyte subpopulation, serum interleukin 2 receptor (SIL 2R), interleukin 6(IL 6), interleukin 8 (IL 8), tumor necrosis factor alpha (TNF ?), nitric oxide (NO), immunioglobin(Ig), complement (C 3), the percentage of RBC C 3b receptor rosette(E C 3b RR), RBC immune complex rosette ( E ICR) and cerebral artery hemodynamics was tested in newborns with HIE and normal controls. The umbilical blood samples and peripheral blood samples were obtained at the time of 1 day, 3 days, 7 days, 12~14 days and 26~28 days after birth. Results Birth asphyxia and HIE were associated with under regulated immune function, which include:(1) T Cell population disorder. In HIE and control group, CD 3 +were(62?8)%vs(65?10)%,CD 4 +(39?7)% vs (46?8)%,CD 8 +(33?6)% vs (19?5)%,CD 4 +/CD 8 + ratio (1.8?0.7) vs (2.5?0.7) on 1 to 3 days after birth. There were no signficant difference in both group on 26 to 28 days after bith ). (2)The IgM and C 3 were decreased.(3)The cytokins were abnormal. 10 years) groups.The form and function of ventricle were measured.And the blood pressure,hemoglobin,plasma albumin,pre-albumin,lipid,C responsive protein,calcium,phosphate and parathyroid hormone before dialysis were determined.Results The cardiac output(CO) of 1~2 years group was the highest.The left ventricular end-systolic and end-diastolic dimension(LVEDd/s)and left ventricular mass index(LVMI)of ≤1 year group were the lowest.With the increase of the dialysis ages,the blood pressure and the proportion of diabetes patients and those with LVH decreased,but the hemoglobin,plasma albumin,pre-albumin,calcium,phosphate and parathyroid hormone increased,while the CRP decreased.Conclusion With the increase of the dialysis ages,the left ventricular function of the uremic hemodialysis patients improves,which is probably associated with the improvement of anemia,malnutrition and inflammation.
5.Comparison of the Differences Between External Standard Method and Relative Correction Factor Method for Determination of the Flavonoids from Sorbus Tianschanica Rupr

Objective To compare the differences between external standard method and relative correction factor method for determination of the flavonoids from Sorbus tianschanica Rupr.Methods Using HPLC external standard method for determination of hyperoside,rutin,isoquercitrin,quercetin-3-O-(6″-O-malonyl)-β-D-glucoside,astragalin and Kaempferol-3-O-(6″-O-malonyl)-β-D-glucopyranoside in Sorbus tianschanica Rupr.,HPLC relative correction factor method was adopted to establish relative correction factor of the other five flavonoids above with hyperoside as reference.The difference was evaluated by comparing the external standard method with the relative correction factor method.Results There was no significant difference between the T test external standard method and relative correction factor method(P>0.05).Conclusion External standard method and relative correction factor method can be used for determination of the flavonoids from Sorbus tianschanica Rupr.,but in the case of lack of reference substance or mass detection,using the relative correction factor method for determination of rutin,hyperoside isoquercitrin,quercetin-3-O-(6″-O-malonyl)-β-D-glucoside,astragalin and kaempferol-3-O-(6″-O-malonyl)-β-D-glucopyranoside in Sorbus tianschanica Rupr.It was more feasible and it can be used as a new quality evaluation method in determination of flavonoid components from Sorbus tianschanica Rupr.
6.Risk Factors Analysis on Acute Respiratory Dysfunction Caused Death in Patients After Type A Aortic Dissection Surgery

Objective: To analyze the risk factors on acute respiratory dysfunction caused death in patients after type A aortic dissection surgery.
Methods: A total of 223 patients who received aorta replacement surgery in our hospital from 2010-01 to 2012-12 were retrospectively studied. 80 patients suffered from post-operative acute respiratory dysfunction including 61 male and 19 female with the mean age of (49.2 ± 11.6) years. Those patients were divided into 2 groups as Death group, n=18 and Survival group, n=62. We analyzed the most relevant risk factors for death, such as gender, age, histories of smoking, diabetes, hypertension, Marfan syndrome;pre-operative acute or chronic dissection, hypoxemia, mal-perfusion, LVEDD and LVEF;CPB time, aortic-clamping time;post-operative ICU retention time, mechanical ventilation time, permanent neurologic dysfunction, pulmonary infection, MACE, renal failure, hypohepatia, septicemia and wound mal-healing, et al.
Results: The early post-operative (< 3 days) respiratory dysfunction rate was 35.8% and the mortality was 22.5%(18/80). The relevant risk factors of death included female gender (P=0.019), haemorrhage (P<0.01), mechanical ventilation time (P=0.011), permanent neurologic dysfunction (P=0.013), pulmonary infection (P=0.001), MACE (P=0.022), renal failure (P<0.01), hypohepatia (P<0.01) and septicemia (P=0.001). Female gender and renal failure were the independent risk factors for respiratory dysfunction caused death in patients after type A aortic dissection surgery.
Conclusion: The occurrence and mortality were high in patients after type A aortic dissection surgery especially in those with female gender and post-operative renal failure.

ObjectiveTo analyzing the psychological status of hospitalized patients of internal medicine and its impact factors.Methods Hospitalized patients of internal medicine underwent a survey by using Chinese Health Questionnaire (CHQ-12),Social Support Scale and Disease-Cognition Scale.Those with a score of >4 received further investigation of Symptom Check-List 90 (SCL-90).Correlation analysis was performed between all factors of SCL-90 and social support or disease-cognition scale score.Results There was no significant difference of psychological status between males and females ( P >0.05 ).All SCL90 factors were negatively correlated with social support,of which obsessive,paranoid,and phobic presented stronger negative correlations with social support and objective support (P < 0.05 ).Furthermore,all factors were negatively correlated with disease-cognition scale score.A significantly negative correlation between phobic factor and disease-cognition scale score was identified ( P < 0.05 ).Improvement was found in 26.2% patients after intervention.Conclusion Patients tend to show unhealthy emotion when they are under the stress of hospitalization.Hospitalization support system may need to be improved and patients' cognition of disease should be increased.
8.Evaluation of biatrial function after rapid right atrial pacing in a canine model by acoustic quantification technique

Objective To observe short-term rapid atrial electrical activation on atrial size and atrial systolic function. Methods Twelve anesthetized mongrel dogs were underwent insertion of a transvenous lead at the right atrial appendage and subjected to 5 hours of rapid right atrial pacing at 450 beat/min. Acoustic quantification(AQ) waveform on biatrial was recorded before and after 5 hours pacing with AQ technique. All measurements were made in sinus rhythm. Results After 5 hours rapid right atrial pacing, end systolic volume(ESV) of both left and right atrial enlarged significantly from ( 5.92 ? 1.51 )ml to ( 7.27 ? 1.86 )ml,from ( 3.13? 0.71 )ml to ( 3.58 ? 0.73 )ml, respectively; but biatrial function decreased markedly,left and right atrial ejection fraction decreased from ( 51.03 ? 9.45 ) to ( 38.23 ? 10.52 ),from ( 36.76 ? 5.36 ) to ( 30.71 ? 7.97 ), respectively. 9.Study on Quality Control of Guanxinshu Tablets

OBJECTIVE:To develop a method to control the quality of Guanxinshu tablets by HPLC.METHODS:Using HPLC as the determination method,the separation was performed on C 18 column(150mm?4.6mm,5?m)with a mobile phase of MeOH∶0.1%H 3 PO 4 (1∶2).The detective wavelength was320nm.RESULTS:The calibration curve was linear(r=0.9994)within the range of1.2~48?g/ml for ferulic acid and the average recovery was101.00%,RSD=4.56%(n=6).CONCLUSI_ ON:The method is simple and quick and it is suitable for the quality control of Guanxinshu tablets.

Objective To analyze the risk factors of postoperative acute renal injury (AKI) for acute Stanford type A aortic dissection in 137 cases.Methods From January 2010 to December 2011,137 patients with acute Stanford type A aortic dissection were received surgical operations in our hospital.There were 106 males and 31 females with their mean age of(46.8 ± 13.1)years and mean weight of (69.9 ± 18.0) kg.The postoperative acute renal injury diagnosis was according to AKIN diagnosis standard of acute kidney injury network working group in 2005.All patients were received surgical repair with cardiopulmonary bypass,including 120 patients with deep hypothermic circulatory arrest and selective cerebral perfusion.Among them,there were 54 cases with total arch replacement and 66 with right half arch replacement.The postoperative managements were include control the patients' mean arterial blood pressure at 80 to 90 mmHg (1 mmHg =0.133 kPa),supplement the blood volume timely,and correction of hypoxia and hypoproteinemia.The patients were received renal replacement therapy if still oliguria after medical treatments,or their blood creatinine raising continually more than 500 μmol/L.Results A total of 12 patients died in hospitalization with a total in-hospital mortality of 8.74% (12/137).76 cases had AKI in the first day after operations,including 38 cases (27.7%) with stage Ⅰ and 21 cases (15.3%) with stage Ⅱ and 17 cases (12.4%) with stage Ⅲ.There were 36 patients have acute renal failure (ARF) with morbility of 26.3% (36/137),and 34 patients among them were received renal replacement therapy.Single factor analysis showed that preoperative creatinine,total arch replacement,cardiopulmonary bypasstime,intraoperative day transfusion of concentrated red cells are risk factors of ARF.Logistic regression was used for multivariate analysis showed that total arch replacement and preoperative creatinine abnormalities are independent risk factors for postoperative AFR.Conclusion Total arch replacement and preoperative creatinine abnormalities were independent risk factors of AFR for acute type A dissection after operation.
