1.Nitric oxide, nitric oxide synthase and cerebral vasospasm after subarachnoid hemorrhage
International Journal of Cerebrovascular Diseases 2013;(1):74-78
Over the years, cerebral vasospasm has been regarded as one of the important factors influencing the prognosis in patients with subarachnoid hemorrhage.Studies have shown that cerebral vasospasm is associated with nitric oxide and nitric oxide synthase after subarachnoid hemorrhage, and regulating the synthesis of nitric oxide may play an active role in the treatment of cerebral vasospasm.

AIM: To observe the effects of the combination of positive end expiratory pressure (PEEP) and 80?10 -6 nitric oxide (NO) inhalation on oleic acid induced acute lung injury (ALI) in canine. METHEDS: 30 dogs were divided into 6 groups. Oleic acid was injected through Swan-Ganz catheter to induced ALI. Pulmonary and systemic hemodynamics, blood gas were measured in dogs before and after injection of oleic acid and the period of inhaled NO for 1-6 h. The methemoglobin(MHb) concentrations were measured. Histology and ultrastructure of the lung tissue were observed. RESULTS: (1) The combination of PEEP and 80?10 -6 NO inhalation rapidly reduced mean pulmonary arterial pressure (MPAP) and pulmonary vascular resistance (PVR), increased PaO 2/FiO 2, reduced A-aDO 2 without inducing significant change on systemic hemodynamics. Arterial blood levels of MHb did not change significantly. (2)The combination group was showed the lightest ALI change by HE stain and electron microscopy. CONCLUSION: The combination of PEEP and inhalation of 80?10 -6 NO significantly and rapidly increased PaO 2/FiO 2 without producing lung injury induced by high FiO 2 and high PEEP.

Southern blotting with a labeled and linearized pUC 19 DNA containing a specific fragment of 0. 24 kb DNA of Plasmodium vivax asexual blood stages (kindly offered by Dr. C. Kidson )was used for further identification of blood samples showing positive reaction by dot-blot hybridization. The results showed that those with positive reaction from patients with P. vivax, with P. falciparum or with fever but with negative microscopic findings were also positive by Southern blotting. It was confirmed that some of those positive with P. falciparum were likely to infect P. vivax at the same time. So did a part of those with fever but negative in the blood films (Figs. 1,2).

Objective To evaluate the clinical safety for outpatients over 80 years old after phacoemul sification surgery. Methods The 184 cases (226 eyes) of aged patients with cataracts were operated on with phacoemulsification and IOL implantation. The safety, complications during and after operation, visual recovery and others were followed up over that period. Results All eyes were operated on without any danger and no serious complications were found. The vision acuity was 0 5 or better in 198 cases (87 6%), 1 0 or better in 102 eyes (45 1%). The main complication was corneal edema in 58 eyes(26 2%), rupture of posterior capsule in 12 eyes (5 3%) and posterior capsular opacity in 16 eyes (7 1%). Conclusions The phacoemulsification and IOL for over 80 years outpatients were safe and effective. The key points are systemic physical examination and careful eye examination. Stimulations during operation and postoperative complications should be prevented.

Objective To study the effect of core stability training on motor function in patients with hemiplegia after stroke. MethodsSixty-eight patients with hemiplegia after stroke were randomly divided into a treatment group (34 cases) and a control group (34 cases). Both groups were given regular rehabilitation training. The patients in the treatment group also were taught core stability training. The trunk control test (TCT),Berg's balance scale (BBS),the modified Barthel index (MBI),functional ambulation categories (FACs) and the Fugl-Meyer assessment scale (FMA) were used to assess motor function before and after treatment. ResultsThere were significant differences between the two groups in average TCT scores,BBS scores,FACs,M BI scores and FMA scores after treatment. The gains in the treatment group were significantly superior to those in the control group. The patients'trunk control was positively correlated with the BBS,MBI,FAC and FMA results. Conclusion Core stability training can improve the motor function of patients with hemiplegia after stroke.

Objective To investigate the influence of hyperbaric oxygen treatment (HBOT) on blood pressure (BP) and the incidence of rebleeding in patients with hypertensive intracranial hemorrhage (HICH)Methods One hundred and twenty patients with HICH were treated for 60 min with 100% oxygen at 2.0 absolute atmospheres (ATA) daily when the condition was stable and BP was controlled ideally. Blood pressure was measured before the patients were sent into the HBO chamber and remeasured following completion of each HBOT session and 1 hour later. Rebleeding was monitored during and after each HBO session . Results HBOT caused a significant elevation of systolic BP in 25.83% (31/120) patients and a significant decrease in 16.67% (20/120) patients (P <0. 05 ),whereas the rest 57.5% (69/120) patients had no significant changes, when the BP was measured right after the HBOT session. The mean diastolic BP increased in 69 (57. 50% ) patients and decreased in 4. 17%(5/120) patients (P < 0. 05 ), whereas we found no significant changes in the rest 46 (38. 33% ) patients. No differences were found in the comparison of BP before and 1 hour after the HBOT session and no one suffered from rebleeding during and after HBOT session. Conclusions HBOT may cause temporal blood pressure changes in most patients with HICH, however, it will not cause an increasing incidence of rebleeding if the patient's condition is stable and the blood pressure has been well controlled.

Objective To screen for the miR-155 expression in synovial fibroblasts of rheumatoid arthritis (RASFs) and osteoarthritis (OASFs) and to evaluate the function of miR-155 on RASFs and its possible target mRNAs. Methods The expression levels of miR-155 in RASFs and OASFs were detected by real-time PCR. MiR-155 mimic and miR-155 inhibitor, as well as scrambled control were transfected into cultured RASFs by Lipofectamine 2000. Forty-eight hours later, MMP-3 levels in the cell culture supernatant were detected by ELISA and fibroblast proliferation was assayed by 3H -TdR incorporation test. Fibroblast invasive ability was tested by transwell system. IKBKE which previously identified as actual target of miR-155 was examined by real-time PCR. Comparisons between groups were performed with t test or one-way ANOVA analysis. Results It was shown that miR-155 was up-regulated in RASFs (1.79 ±1.94) and it was higher than that in OASFst (0.11±0.17), P<0.05]. Up-regulation of miR-155 could decrease MMP-3 levels (P<0.05). The proliferation and invasion of RASFs transfected with miR-155 were both evidently suppressed (P<0.05), while reducing the endogenous miR-155 could significantly enhance RASF proliferation (P<0.05). The expression of IKBKE of RASFs transfected with miR-155 was obviously down-regulated compared to those transfected with the scrambled control (P<0.05). Conclusion miR-155 is up-regulated in RASFs which may be a protective factor against the inflammatory effect, at least partially by attenuating the expression of IKBKK.
