1.Changes of Cardiopulmonary Function of Patients with Spinal Cord Injury in Different Lesion Levels in Exercise Testing

Objective To investigate the changes of cardiopulmonary function of patients with spinal cord injury(SCI) in different lesion levels in exercies testing.Methods 15 SCI patients were divided into the high SCI level group(n=8) and low SCI level group(n=7),and all cases exercised on treadmill.The cardiorespiratory indexes of all patients were tested.Results The cardiopulmonary function of the low SCI level group were better than the high SCI level group(P<0.05~0.01).Conclusion The higher the lesion level is,the weaker the cardiopulmonary function is.
2.Assessment of myocardial viability with dobutamine stress echocardiography versus ~(99m)Tc single photon emission computed tomography

Objective To assess the value of dobutamine stress echocardiography (DSE) in predicting myocardial viability by comparing with single photon emission computed tomography(SPECT).Methods Taking improvement of ventricular wall motion as a standard,fifteen patients undergone percutaneous coronary artery intervention(PCI) were detected by DSE and SPECT respectively,comparing their accuracy in predicting myocardial viability.Results By DSE, difference of the sensitivity and negative forecasting value was remarkable between peak dose stage and low dose stage,comparing with DSE,SPECT had the highest sensitivity and the lowest distinctivity.Conclusions The two techniques have their own advantages and disadvantages in predicting myocardial viability,but DSE is more practical.

Objective To study the clinical application s of intravenous myocardial contrast echocardiography (IMCE) in assessing myocardial reperfusion in acute myocardial infarction (AMI) using intravenously infused Levovist. Methods IMCE was performed in 13 AMI patients before and after thrombolytic therapy. The power Doppler signals of the myocardial segments were analyzed and the time-amplitude curves were plotted during Levovist infusion and incremental triggering. 6.Place of county hospitals in the delivery of community health service and methods to be adopted

To do a good job of community health service, county hospitals ought to make efforts in the following aspects: restructuring of human resources, cooperation between various community health institutions, diversified training programs for personnel engaged in community health service and doctors at the grass-roots level, transformation of ideological concepts, and updating of knowledge and skills.
7.Treatment of Acute Myocardial Infarction by Allogeneic Transplantation of Rat Bone Marrow Mesenchymal Stem Cells

Objective To investigate the feasibility of acute myocardial infarction(AMI)treated with transplantation of bone marrow mesenchymal stem cells(MSCs).Methods Fifty female Wistar rats were randomly divided into the transplant group and control group with 25 animals in each group.The AMI animal model was made by liquid nitrogen freezing.Rat MSCs cultured and induced repeatedly was prepared for transplantation and injected into the infarcted parts of the transplant group.The same volume of DMEM solution was injected into the infarcted parts of the control group.Echocardiography was applied one day preoperation,and one week and four weeks postoperation to evaluate cardiac function.The animals of the transplanted group were executed in the fourth week after operation and the tissues of transplant part were examined by BrdU immunofluorescent stain.Results Some transplant cells expressed the cardiac-specific proteins,α-actin and troponin T.The cardiac function of the transplant group had better than that of the control group(P>0.05)in the fourth week postoperation.The BrdU-labeled cells were found in the histological sections of the transplant parts.Conclusion Rat MSCs cultured generation by generation and repeatedly induced by 5-aza in vitro can differentiate into cardiomyocyte-like cells,which if transplanted into the rat infarcted cardiac muscles will survive and help to improve the host's cardiac function.
8.Studies on Pharmacokinetics and Bioavailability of Puerarin in Yufeng Ningxin Tablet in Mice

Objective To evaluate the pharmacokinetic parameters and bioavailability of puerarin in Yufeng Ningxin tablets in mice by determining dose-time curve and by comparing with the pharmacokinetics of puerarin injection.Methods NIH mice were randomized into different groups. 6 %HClO4 was used to precipitate plasma protein and the plasma concentration of puerarin in mice was determined by HPLC.The PK solutions 2.0 program,a non-compartmental model software,was applied to calculate the pharmacokinetic parameters and bioavailability.Results The main pharmacokinetic parameters of puerarin injection by i.v.in mice were:t1/2=98.359 min,CL=35.548 mL?min-1,AUC(0-∞)=281.3 ?g?min?mL-1;the main pharmacokinetic parameters of puerarin in Yufeng Ningxin tablets by o.p.were:t1/2 =35.562 min,CL=898.685 mL?min-1,Cmax=9.3 ?g?mL-1,Tmax=30 min,AUC(0-∞)=222.5 ?g?min?mL-1 ,F(bioavailability value)=3.95 %comparing to puerarin injection.Conclusion As compared with puerarin injection,the absorptive content of puerarin in Yufeng Ningxin tablets is very poor and the bioavailability is also low,indicating that enhancement of bioavailability of Yufeng Ningxin tablets will be beneficial to the clinical application.
9.Evaluation on Medication Route of Puerarin by Plasma Concentration- Time Cur ve and Histodistribution Characteristics in Mice

Objective The rational medication route of puerarin was explore d by studying the concentration- time curve and by comparing the histological and organic distribution difference of puerarin administered by intravenous injecti on or gastric gavage in mice, so as to supply a referential data for its rationa l application. Methods The NIH mice were used as experimental subject. The pu erarin concentrations in the plasma, tissue and organs at different time points were determined by HPLC. The PK solutions 2.0 program, a noncompartmental model software, was applied to calculate the pharmacokinetic parameters of puerarin an d to construct its plasma concentration- time curve. Results (1)The pharmacok inetic parameters of puerarin in mice were shown that the T1/2E of puerarin susp ension (0.2 mg? g- 1) by oral administration is 38.061min, CL =991.534 mL? mi n- 1, Cmax =3.6? g? mL- 1, Tmax =30 min, and AUC(0- ∞ ) =201.7? g? min? mL- 1, the bioavailability of puerarin suspension is 3.77 % compared to i.v puerarin injection. (2) Administered by intravenous injection (i.v), the puerari n distributed in the liver, kidney, plasma, spleen, muscle, lung, uterus and tes ticle rapidly, and the concentration of puerarin was the highest in the liver an d kidney and lower in the heart and brain. Distribution of puerarin suspension b y oral administration is similar to puerarin injection by i.v. However, the conc entration of puerarin in the tissues and organs by oral administration was lower than by i.v; the liver/heart, liver/brain, kidney/heart and kidney/brain concen tration ratios of puerarin by gavage administration were lower than those by i.v . Conclusion The bioavailability of puerarin by oral administration was poor, but the histological distribution characteristics of puerarin shows that the tox ic and side effects of puerarin are lesser by oral use than by intravenous injec tion.
10.Sorafenib for the treatment of unresectable hepatocellular carcinoma

Objective To observe the efficacy and safety of sorafenib in the treatment of the unresectable hepatocellular carcinoma.Methods According to the inclusion criteria,33 patients with the unresectable hepatocellular carcinoma were given sorafenib (400 mg for twice per day).During the course of treatment,dose was adjusted based on the degree of the adverse effects.Tumor response to sorafenib and safety was assessed every 6-8 weeks using the modified RECIST criteria.The survial curve for the time to progression (TTP) and overall survival (OS) were estimated.Results In this series,there was no patients achieved complete response (CR) and partial response (PR),1 1 patients were evaluated as with stable disease(SD),22 patients were with progressive disease (PD).The median TTP was 5.6 months (2.3-8.9 months).The median TTP was longer in patients with BCLC B than BCLC C stage.TTP was longer in good than in poor performance status patients,and shorter in extrahepatic metastasis than in no extrahepatic metastasis patients.The overall incidence of adverse events was 75.8%.The most common adverse events were hand foot skin reaction,diarrhea,hypertension and rash.Three patients had grade 3 adverse events.Conclusions Sorafenib can extend the median time to progression in patients with unresectable hepatocellular carcinoma.Patients with earlier stage of HCC and better performance status are hopeful for more positive response to the treatment of sorafenib.
