1.Risk factor analysis of hospitalized patients with pulmonary thromboembolism and deep venous thrombosis

Objective:To investigate the risk factors related to pulmonary thromboembolism(PTE)and/or deep venous thrombosis(DVT)in hospitalized patients based on the analysis of PTE and DVT epidemiological data.Methods:The clinical data of218inpatients with PTE and/or DVT in recent10years were reviewed.The epidemiological data including tendency of incidence,demographic features,co-existing diseases,acquired risk factors and ABO blood type were analyzed.Results:There was an increasing trend in the number of inpatients with PTE and/or DVT.The male/female ratio was1.45,and was1∶1in patients≥45years old.The main risk factors of PTE and/or DVT were operation,trauma and fracture within3months.There was no significant difference in these main risk factors between relapse patients and non-relapse patients.The ratio of non-O blood type in inpatients with PTE and/or DVT was higher than in control group(P
2.Application value of cutter stapler in transumbilical single port laparoscopic left lateral lobectomy

Objective To investigate application value of cutter stapler in transumbilical single port laparoscopic left lateral lobectomy.Methods The retrospective cohort study was adopted.The clinical data of 26 patients who underwent transumbilical single port laparoscopic left lateral lobectomy at the Shengjing Hospital of China Medical University from January 2010 to February 2016 were collected.Nine patients who received liver parenchyma using ultrasonic knife were allocated into the ultrasonic knife group,17 patients who received liver parenchyma using cutter stapler were allocated into the cutter stapler group.Observation indicators included (1) operation situations:operation time,volume of intraoperative blood loss,postoperative complications,time of postoperative bowel function recovery,time of abdominal cavity drainage tube removal,duration of postoperative hospital stay.(2) Postoperative reexamination and follow-up:ultrasound or computed tomography (CT) examination was performed when necessary for detecting local exudation or encapsulated effusion.The patients were followed up at postoperative 1 to 3 months with telephone interview for whether with abdominal distension or abdominal pain till March 2016.Measurement data with normal distribution were presented as (x) ± s and analyzed by using t test.Measurement data with skewed distribution were presented as M (range) and analyzed by ranksum text.Comparison of count data was analyzed by the Fisher' s exact probility.Results (1) Operation situations:all the 26 patients received transumbilical single port laparoscopic left lateral lobectomy with no conversion to porous laparoscopic surgery or open surgery.The operation time was (114 ± 54) minutes,the volume of intraoperative blood loss was 100 mL (range,20-800 mL),and no intraoperative blood transfusion was adopted.The operation time and volume of intraoperative blood loss were (135 ±43)minutes and 200 mL (range,20-800 mL) in the ultrasonic knife group,(103 ±57)minutes and 100 mL (range,20-300 mL) in the cutter stapler group,respectively,showing no statistically significant difference between the 2 groups (t =1.500,Z =-0.961,P > 0.05).All the 26 patients recovered well after surgery,with no postoperative complications as postoperative hemorrhage,bile leakage,incision infection or death.The time of postoperative bowel function recovery,time of abdominal cavity drainage tube removal and duration of postoperative hospital stay was (1.5 ±0.4) days,(5.8 ± 2.0) days and (7.0 2.0) days in the ultrasonic knife group,(1.1 ± 0.3) days,(4.1 ±1.1) days and (4.9 ± 1.4) days in the cutter stapler group,respectively,showing statistically significant differences between the 2 groups (t =2.599,2.875,3.036,P < 0.05).(2) Postoperative reexamination and follow-up:of 26 patients,11 patients received ultrasound or CT examination after surgery and detected no obvious local exudation or encapsulated effusion,with no special treatment.The other 15 patients didn't receive ultrasound or CT examination.All the 26 patients were followed up for 1-3 months,with no occurrence of upper abdominal distension or abdominal pain.Conclusion Transumbilical single port laparoscopic left lateral lobectomy is safe and feasible,the application of cutter stapler is helpful to safety and success of the operation,further accelerating the postoperative recovery of patients.

4.Suppression of human glioblastoma in nude mice by siRNA Notch-1 gene therapy

Background and purpose:In recent studies, the Notch-1 gene has been found to play an important role in the development of human glioblastoma.Short interfering RNA(siRNA) was used to silence the Notch-1 gene and block its expression.The objective was to determine whether siRNA targeting Notch-1 would inhibit the formation and growth of tumors in nude mice that modeled human glioblastoma.Methods:The human glioblastoma cell line TJ905 were first cultured and transfected with Notch-1 siRNA or nonsense siRNA by OligofectamineTM.The TJ905 cells were divided into 3 groups:the Notch-1 siRNA transfected group, nonsense siRNA transfected group and the control group.Each group's cells were subcutaneously injected into 5 nude mice.The nude mice(males, 3-4 weeks old) were given subcutaneous injections of either 0.2 mL of transfected siRNA or with normal TJ905 cells suspended at a 1?107 cells/mL concentration in a DMEM medium without serum.One week later, when the tumors were palpable, they were directly injected with the Notch-1 siRNA complex, nonsense siRNA complex, or PBS every 4 days for 20 days.Tumor sizes were measured every 3 days and calculated by the formula:volume(mm3) =1/2 length?(width)2.After a 20-day follow-up period, the mice were exterminated.Immunohistochemistry was used to determine the expression of Notch-1 gene.Results:The final tumor volume was less in nude mice injected with Notch-1 siRNA(1 203?206) mm3 compared mice injected with the nonsense siRNA injection(2 241?401) mm3, P
5.Development of a new open “C”-shaped molar band with nanomaterial

BACKGROUND:In the field of orthodontics, molar bands play an important role. But traditional bands have some weakness difficult to be solved in clinical use, for example, trying on cumbersomely, having to separate the
adjacent teeth beforehand, occupying bigger space and tending to cause periodontitis of molars with bands. So some clinicians use a buccal tube instead of the molar band, but it has relatively high losing rate and cannot be fixed with Transpalatal Arch and Nance.
OBJECTIVE: To invent a new open “C”-shaped molar band with nanomaterial (patent number: ZL201110057699.1) to solve the above problems as a common molar appliance.
METHODS: We made some stainless steel strips of 0.18-0.20 mm thickness and a certain length. The stainless steel strips were made to the new bands composed of buccal, lingal, distal surface and bilateral extention caused by removable parts extending to mesial surface. In order to improve retention force, its inner surface was coated by nano-hydroxyapatite and it bound to the molar crown by resin reinforced glass ionomer cements in clinical use. RESULTS AND CONCLUSION: The new open “C”-shaped molar band with nanomaterial overcomes
shortcomings of conventional band and buccal tube. From the design, in contrast with traditional band, it not only has no need to separate teeth beforehand and to try on repeatedly, but also less stimulates periodontal tissues. So the new band can be more suitable for clinical use.
6.Analysis of allergens characteristic in 1172 patients with allergic rhinitis in Changzhou area.

OBJECTIVE:
To determine the distribution of allergens in patients with allergic rhinitis during from 2008 to 2015 in Changzhou area.
METHOD:
Allergy Screen method was used to detect the specifical-allergen IgE levels of 1172 patients with allergic rhinitis. Among the patients, the distribution of all allergens was analyzed. The positive rate was compared with age, gender, season and so on.
RESULT:
The most common allergens in allergic rhinitis patients in Changzhou were dust mite, fungus, house dust, milk, dander of dog, weed mixture, farina and dander of cat. The more higher positive rates of dust mite, fungus, house dust, milk, dander of dog, dander of cat, eggs were found in teenager group than adult group (P < 0.05). There were more higer positive rates of weed mixture, farina, cashew, cockroaches and crab in teenager group than adult group (P < 0.05). There was no significant difference of positive rate between female group and male group. There was significant difference of variation with seasons.
CONCLUSION
Dust mite, fungus and house dust were the most common allergens among patiens with allergic rhinitis in Changzhou area. The positive rates of allergens varied with ages and seasons.

Objective:To observe the effects of herbal cake.partitioned moxibustion and bran-partition moxibustion in improving symptoms of ulcerative colitis(UC)and the TNF-α and its receptor of colon mucosa.Method:67 UC cases were randomly allocated into herbal cake-partition moxibustion group of 35 cases and bran-partitioned moxibustion group of 32 cases,to compare the improvement and detect the TNF-α and its receptor with inlmunohistochemical method in both groups.Result:Herbal cake.partitioned moxibustion iS prior to bran-partitioned moxibustion in improving of diarrhea,flatus,lassitude,tenesmus and lumbar soreness;The expression of TNF-α,TNF-αR1,and TNF-αR2 are significantly decreased after treatment in herbal cake-partitioned moxibustion group,while in bran-partitioned moxibustion group only TNF-αR1 expression is significant decreased after treatment.Conclusion:Moxibustion can well improve the syndromes of UC.Herbal cake.partitioned Moxibustion is prior to bran-partitioned moxibustion in the improvement of diarrhea and flatus;Herbal cake-partitioned moxibustion could down-regulate the expression of TNF-α,TNF-αR1.and TNF-αR2.while bran-partitioned moxibustion could only down-regulate the expression of TNF-αR1.
9.Effects of photodynamic therapy on human lung adenocarcinoma cell line A549.

BACKGROUNDTo observe the effects of photodynamic therapy (PDT) on human lung adenocarcinoma cell line A549 in vitro.
METHODSHuman lung adenocarcinoma cell line A549 was used to investigate the killing role of PDT with hematoporphyrin derivative (HPD) as photosensitizer and semiconductor successive laser as light source. The cultured cell was treated with different concentrations of HPD and different do-sages of laser, then MTT colorometric assay was applied to measure the OD492.
RESULTS5 mg/L of HPD was confirmed no effect. 10 mg/L of HPD could produce a marked therapeutical effect under the condition of same laser dosage. When the concentration of HPD was increased, the effect was not risen obviously. 10 J/cm² of laser could lead OD492 to decrease to platform under the condition of same HPD concentration. So the killing effect was saturated when the PDT parameters were selected as 10 mg/L of HPD and 10 J/cm² of laser. Under the same laser capacity density of 10 J/cm², A549 cells were exposured to three different duty-time fabrications which were 214 mw×10 min, 428 mw×5 min and 714 mw×3 min.The outcomes showed that OD492 had no statistical difference.
CONCLUSIONSPDT has a significant killing effect on human lung adenocarcinoma cell line A549. 10 mg/L of HPD and 10 J/cm² of laser were the best parameters of this experiment. The PDT effect is not influenced by different duty-time fabrications under the best laser capacity density.
10.Analysis of clinical index changes of right heart hypofunction in patients with primary myelofibrosis

Objective:To investigate the changes of related indicators of right heart hypofunction in patients with primary myelofibrosis (PMF).Methods:The clinical data of 55 PMF patients in the Second People's Hospital of Lianyungang in Jiangsu Province and Jiangsu Province Hospital from January 2015 to August 2019 were retrospectively analyzed. The differences in right heart function-related echocardiographic indexes and biochemical indexes between pre-fibrosis/early stage fibrosis patients and obvious stage fibrosis patients were compared. Single factor linear regression method was used to analyze the correlations of pulmonary artery pressure with biochemical indexes.Results:The hemoglobin level [119 g/L (47-224 g/L) vs. 78 g/L (33-182 g/L)] and platelet count [233×10 12/L (5×10 12/L-984×10 12/L) vs. 117×10 12/L (7×10 12/L-731×10 12/L)] of patients in the pre-fibrosis/early stage fibrosis group were higher than those in the obvious stage fibrosis group, and the differences were statistically significant (both P<0.05). Among 22 patients with complete results of cardiac ultrasound, 90.9% (20/22) patients had increased pulmonary artery pressure, 72.7% (16/22) patients had increased left atrial diameter, and 90.9% (20/22) patients had increased right ventricular diastolic diameter. There were no patients with abnormal ejection fraction. The pulmonary artery pressure [48 mmHg (46-90 mmHg) vs. 33 mmHg (20-50 mmHg) (1 mmHg = 0.133 kPa)], left ventricular diastolic diameter [46 mm (36-50 mm) vs. 47 mm (43-53 mm)] and fractional shortening rate [38.1% (36.0%-38.9%) vs. 35.4% (32.7%-37.8%)] of patients in the pre-fibrosis/early stage fibrosis group were higher than those in the obvious stage fibrosis group, and the differences were statistically significant (all P < 0.05). The pulmonary artery pressure of patients had positive correlations with age ( r = 0.590), serum ferritin (SF) ( r = 0.608), lactate dehydrogenase (LDH) ( r = 0.711) and soluble growth-stimulating expression gene 2 (ST-2) ( r = 0.580)(all P<0.05), and had negative correlation with platelet count ( r = -0.596, P = 0.003). Conclusion:PMF patients are prone to right heart hypofunction, the pulmonary artery pressure is higher in older patients and patients with high SF, LDH and ST-2 levels and low platelet count.
