1.Quality Analysis of Measles Breakout Events and Relative Cases in China in 2006

Objective To analyze the quality of measles breakout events and relative cases in order to offer the integrated,accurate,basic data for infectious disease anticipation and control.Methods Associate the measles breakout events with cases,the cases with organization type,and users'information with breakout events.As a result,the integrated data construct is created in order to analyze the index in detail. Results Measles breakout events substitute ration is 33.93%,measles breakout events without relative cases reporting rate is 12.50%.Relative cases reporting rate in time is 75.51% and substitute ratio is 12.07%.Conclusions There are still many problems in measles breakout events reporting and relative cases reporting.The local CDCs and hospitals should pay much attention to the data quality especially in the infectious disease outbreak and guarantee the data quality through more methods of quality control and evaluation.
2.Protective effect of recombination rat augmenter of liver regeneration on kidney in acute renal failure rats

Objective To investigate the protective effects of recombination rat augmenter of liver regeneration (rrALR) on tubular cell injury and renal dysfunction in rats with acute renal failure (ARF)induced by gentamicin. Methods One hundred fifty female Wistar rots were randomly divided into 5 groups: group A (control), group B (gentamincin 140 mg/kg+ saline 100 μg/kg), group C (gentarnincin 140 mg/kg +blank plasmid 100 μg/kg), group Dl (gentaminein 140 mg/kg+rrALR 80 μg/kg), group D2 (gentamincin 140 mg/kg +rrALR 160 μg/kg). Rats were sacrificed at day 4, 8, 12, 16 and 21 after gentamicin first injection. Blood urea nitrogen (BUN), serum creatinine (Scr) and urine N-aeetyl-β-D-glucosaminidase (UNAG) were measured. The histopathologic changes were observed by periodic aeid-Schiff (PAS) staining. Protein expression of ALR and PCNA in renal tissue was detected using immtmohistochemistry and Western blot. Results Compared with control rots, the levels of BUN, Scr and UNAG were significantly increased in ARF rats at day 4, 8, 12 and 16 (P<0.05), however, the renal dysfunction and the renal histopathologieal injury were significantly improved in ARF rats simultaneously administered with rrALR (group D) compared with ARF rats (group B and C). The protein level of ALR, the number of PCNA positive tubular ceils and the proliferation index (PI) in group D were significantly higher than those of group B and C (P<0.05). Conclusion rrALR can promote the regeneration of tubular cells in nephrotoxie ARF rats and ameliorate renal dysfunction.
3.A prospective randomized study of TOT and TVT procedure for female intrinsic sphincter deficiency(ISD)

Objective To compare the outcomes of TOT and TVT procedure treating female intrinsic sphincter deficiency (ISD).Methods From May 2010 to September 2015,42 stress urinary incontinence (SUI) patients whose abdominal leak point pressure was less than 60 cmH2O were enrolled in this study.Thirty-five patients were followed up.The mean age was (56.8 ±10.5) years,with a range of 30-80 years.The mean history was(10.5 ± 9.1)years,with a range of 4 months to 30 years,with 26 (74.3%) of them being postmenopausal,5 (14.3%) having a history of pelvic surgery,and no pelvic organ prolapsed or hormone replacement.All of them were randomly divided into 2 groups to undergo either TVT operation (13 cases) or TOT operation (22 cases).The baseline characteristics of the two groups including age,length of history,urodynamic parameters and scale scores showed no significant difference.The scales including urinary incontinence severity score (UISS),detrusor instability score (DIS),Quality of Life Scale Evaluation (I-QOL),lower urinary tract symptoms affect score (UDI-6) were used.The outcomes between TVT group and TOT group were compared.Result After procedure,patients in TVT group got a lower UISS score than TOT group(17.2 ± 2.2 vs.17.7 ± 3.1),and their severity of urinary incontinence improved significantly (P < 0.05).Patients from TVT group got a lower DIS score than TOT group (12.6 ± 4.2 vs.14.2 ± 3.5),and their detrusor instability symptoms improved more significantly (P < 0.05).Patients from TVT group got a higher I-QOL score than TOT group(17.5 ± 14.5 vs.16.1 ± 13.0),and their quality of life improved more significantly (P < 0.05).Patients from TVT group got a lower UDI-6 score than TOT group (10.1 ± 3.0 vs.11.2 ± 3.4),and their lower urinary tract symptoms improved more significantly (P < 0.05).Conclusion Urinary incontinence of female ISD patients were improved greater by TVT than TOT procedure.
4.Creatinine linear investigation in various assay instruments

Objective To investigate the linearity on testing creatinine in various assay instntments,in order to evaluate creatinine testing ability of clinical laboratories. Methods Statistically standard linearity and nonlinearity were judged via polynomial regression analysis recommended by EP6-A guideline. The imprecision of analytical data were analyzed and the average deviation of best fitting curve from linearity were determined by Dr. Kroll's EP6-A extended methods. In accordance with the instruments used, the testing data were divided into four groups: Beckman LX (28 Labs), Beckman CX ( 14 Labs), Hitachi (62 Labs)and Olympus (72 Labs) groups. Results This research found that in samples analyzed by Beckman LX,Beckman CX, Hitachi and Olympus instruments, the imprecision ranged from 0. 30%-3.01%, 0.09%-3.46%, 0. 14%-4. 91% and 0. 17% -16. 44%, respectively. The imprecision in one laboratory in Olympus group exceeded the acceptable limit, while the imprecision in the remaining laboratories were precision for linear survey. The average deviations of best fitting curve from linearity ranged from 0%-2. 38%, 0%-2. 51%, 0% -5.46% and 0% -4. 66%, respectively. The linearity with statistical significance were 21.4% (6/28), 35.7% (5/14), 11.3% (7/62) and 18. 1% (13/72), respectively. The linearity with clinical significance were 78.6% (22/28), 64. 3% (9/14), 88. 7% (55/62) and 80. 6% (58/72), respectively.The percentages of laboratories whose creatinine testing met the linearity criteria were 100% (28/28),100% ( 14/14 ), 100% ( 62/62 ) and 98. 6% ( 71/72 ), respectively. Conclusion Majority of the laboratories involved in the linearity survey present good capability for creatinine testing.
5.Total hip arthroplasty for Crowe type IV developmental dysplasia of hip in adults

BACKGROUND:Total hip arthroplasty is an optimal choice for patients with late hip dysplasia. Crowe type IV developmental dysplasia of the hip increases the difficulty of the operation, and the surgery is controversial.
OBJECTIVE:To evaluate the clinical effects of total hip arthroplasty on Crowe type IV developmental dysplasia of the hip and the method of reconstruction of acetabulum and the treatment of proximal femur.
METHODS:A total of 12 patients (14 hips) with Crowe type IV developmental dysplasia of the hip underwent total hip arthroplasty. Preoperative Harris hip score was averagely (35.0±6.8) points. Al hips were treated with smal acetabular components combined with medial protrusion technique in acetabular reconstruction, as wel as subtrochanteric shortening osteotomy in femur. Joint function of hips was evaluated according to the Harris hip score.
RESULTS AND CONCLUSION:Al patients were fol owed up with an average of 4.6 years (ranged 1 to 7 years). Two cases (two hips) suffered from infraction of greater trochanter of femur during replacement, and it was fixed with wire. There was complete sciatic nerve injury in one case, which partial y restored after conservative treatment for 1 month. No infection, prosthesis loosening, or deep venous thrombosis with obvious clinical manifestations was visible. Bone union was observed at the site undergoing osteotomy at the side of femur. After replacement, final fol ow-up showed that Harris hip score was averagely (84.0±7.0) points. The mean amount of postoperative leg lengthening was 5 cm (range 4-6 cm). Shortened limbs were corrected satisfactorily. These results suggested that total hip arthroplasty using smal acetabular component, medial protrusion, and femoral subtrochanteric shortening osteotomy technique for the Crowe type IV developmental dysplasia of the hip can effectively restore hip function and leg length. The long-term curative effects require further investigations.
6.Diagnosis of thyroid microcarcinoma using contrast-enhanced ultrasound

Objective To evaluate the diagnostic value of thyroid microcarcinoma using contrastenhanced ultrasound(CEUS).Methods The CEUS characteristics of 31 lesions in patients with thyroid microcarcinoma which were confirmed by surgery and pathology were retrospectively analyzed.[Results]The enhancement features of 31 lesions were hypoenhancement,morphological irregularity,and lack of a complete envelope.The time intensity curve showed late washed in and late washed out.The start-time of the lesions was later than that of normal thyroid tissue.The enhancement intensities of the lesions were lower than that of normal thyroid tissue.Conclusions The contrast enhancement patternshypoenhancement,suggested that the CEUS might provide a useful,quantitative analysis method for the diagnosis of thyroid microcarcinoma,which can improve diagnostic confidence in thyroid microcarcinoma.
7.Calibration verification for blood glucose test in automatic biochemistry analyzers

Objective To perform calibration verification for the blood glucose test in Hitachi and Olympus automatic biochemistry analyzers and present a method of calibration verification and evaluate the results.Methods The glucose test results were divided into two groups:Hitachi and Olympus groups according to the different analytical instruments.The calibration verification was performed following the current College of American Pathologists strategies.The slope and intercept of the calibration line were tested by one-sample t test and the percentage differences were calculated.The percentage differences were compared with allowable errors.If the slope and intercept had no statistically significant difference from 1 and 0 respectively and the percentage differences were within the limits of allowable errors,the calibration line was evaluated as verified 1 ; if the slope and intercept had no statistically significant difference from 1 and 0 respectively and at least one of the percentage differences exceeded the limits of allowable errors,the calibration line was evaluated as different 1 ; if the slope or intercept had statistically significant difference from 1 and 0 respectively and the percentage differences were within the limits of allowable errors,the calibration line was evaluated as verified 2; if the slope or intercept had statistically significant difference from 1 and 0 respectively and at least one of the percentage differences exceeded the limits of allowable errors,the calibration line was evaluated as different 2.Results In Hitachi group,verified 1,different 1,verified 2 and different 2 were 5.4% ( 4/74 ),0.0% ( 0/74 ),68.9% ( 51/74 ) and 25.7% ( 19/74 ),respectively.In Olympus group,verified 1,different 1,verified 2 and different 2 were 16.4% (12/73),0.0% (0/73),58.9% (43/73) and 24.7% ( 18/73 ),respectively.Conclusions The current College of American Pathologist strategies could be performed as a reasonable and feasible method for calibration verification.Most of participating laboratories ( 74.8%,110/147 passed the assessment of calibration verification.
8.Use of robust Z-score to assess creatinine proficiency testing data

ObjectiveTo evaluate creatinine proficiency testing data by robust Z-score analysis.MethodsThe data were collected from three proficiency surveys of routine biochemical test in 2009,to which 1 179,1 169 and 1 168 laboratories participated respectively.Creatinine data were divided into Jaffe group and enzymatic group based on the analytical method used.The results tested by both methods were compared using Mann-Whitney test.The outliers were deleted using the TUKEY fence established by quartile values.The normality of raw data and trimmed data was tested using one-sample Kolmogorov-Smirnov test.The performance of the laboratories was assessed using robust Z-score,whose values were considered satisfactory when | Z-score| ≤2,questionable when 2 < | Z-score | < 3 and unsatisfactory when | Z-score | ≥3.Results86.7% results tested using Jaffe and enzymatic methods were not comparable.The raw data in all research groups were not normally distributed.After deletion of outliers,73.3% trimmed data in most research groups were normally distributed.For the three proficiency tests in 2009,in Jaffe group,the satisfactory rates were 89.8%( 495/551 ),87.2%( 468/537 ) and 89.5%( 476/532 ) respectively,unsatisfactory rates were 3.3% ( 18/551 ),6.5% (35/537) and 4.5% (24/532) respectively; while in enzymatic group,the satisfactory rates were 88.8% (558/628),89.3% (564/632) and 88.1% (560/636) respectively,unsatisfactory rates were 5.6%( 35/628 ),5.2% (33/632) and 6.6%(42/636) respectively.Conclusion It is reasonable to choose robust Z-score as a proficiency testing assessment index,because it avoids the influence of the outliers on evaluation results.
9.Evaluation of Three-dimensional Volume Measurements of Pulmonary Nodules with 64-detector CT Compared with Two-dimensional Measurements

Objective To evaluate the reproducibility of the three-dimensional volume measurement and two-dimensional measurement of pulmonary nodule volume with 64-detector computed tomography.Methods Thirty-three patients with pulmonary nodule who underwent the examination with 64-detector spiral CT(Lightspeed VCT,GE) were included,in whom sixty solid nodules with the diameter less than 3 cm were found.There were 18 women and 15 men.CT images were analysed by two radiologists,respectively,the volumes of pulmonary nodules were measured with ALA software continuously,and two-dimensional measurements which were separated a month apart were also done in the GE AW4.2 workstation independently.Intra-and inter-reader agreement were determined by using Bland and Altman method and the Spearman correlation coefficients,the variability between three-dimensional measurements and two-dimensional ones were identified.Results 57 nodules in 60 were successfully segmented and 3 were segmented incorrectly or unsuccessfully,the ratio of successful segmentation was 95%.Three-dimensional measurements between two readers were highly correlated with r of 0.998,and the r was 0.974 in the two-dimensional measurements.The difference between the two correlation coefficients was significant.The three-dimensional measurements between two readers had completely agreed with each other for nodules that of the diameter
10.The role of apparent diffusion coefficient value in evaluation of the response to preoperative chemoradiation in rectal carcinoma

Objective To assess response of rectal carcinoma to preoperative chemoradiation therapy(CRT)using DWI and tumor ADC values,and to investigate the value of ADC in predicting and monitoring therapeutic effect of CRT.Methods Twenty-six patients with primary rectal carcinoma undergoing preoperative CRT were recruited to the study.DWI was performed on a 1.5 T MR scanner in all patients at the time point of pre-therapy,the end of the 1st,2nd week of therapy and pre-operation,respectively.ADC values of the tumors were calculated on the workstation.Randomized block design was applied to analyze change in ADCs following treatment Results All patients were divided into T-downstaging group(n=12)and T-non-downstaging group(n=14).In T-downstaging group,the mean tumor ADC values were(1.10±0.13)×10~(-3),(1.32±0.19)×10~(-3),(1.35±0.13)×10~(-3),(1.32±1.00)×10~(-3) mm~2/s at the time point of pretreatment,week 1,week 2,pre-operation,respectively(F=16.420,P<0.01).The mean tumor ADC value in T-non-downstaging had a slight increase from(1.16±0.16)×10~(-3) mm~2/s to(1.23±0.13)×10~(-3) mm~2/s at the time of week 1(P>0.05).The ADC value in T-non-downstaging group continuously increased to(1.30±0.16)×10~(-3) mm~2/s at the end of the 2nd week of CRT(F=5.023,P<0.01)and appeared statistical difference.The evolution of tumor ADC values in the two groups was significantly different.Early increases in tumor ADC were observed in T-downstaging group.Regarding the increase percentage of ADC value at 1st week as a diagnostic marker of tumor downstaging,when it was set as 11.6%,the sensitivity,specificity,positive predictive value and negative predictive value is 75.0%,78.6%,75.0% and 78.6% respectively,the area under curve(Az)was 0.774(95% confidence interval:0.583 to 0.964).Conclusions An early significant increase of mean tumor ADC value in rectal carcinoma has a potential to predict therapeutic effect of CRT.One week after beginning CRT is an early time point to monitor therapy efficacy.
