Late in-the-bag dislocation of intraocular lens bag complex is a rare but potential complication after cataract surgery,which can affect the quality of patients'life seriously,and usually need surgical treatment.People are paying more attention to this complication,because it has been reported with increasing frequency in recent years.This complication may present after a long time frame.There is a research showing that the interval between cataract surgery and the dislocation was 10.9 years.The cumulative risk of IOL dislocation at 10 years,20 years,and 25 years after cataract extraction was 0.1％,0.7％,and 1.7％ respectively.Late in-the-bag IOL dislocation is a result of progressive zonular dehiscence up to many years after cataract surgery,and its genesis is multifactorial.This article reviews the incidence,possible mechanism and risk factors of late in-the-bag IOL dislocation.

The explosive terror attack is the most frequent in all terror activities in the world. The authors reviewed its current situation and the challenges we are confronted with in China and elsewhere. In the present paper, the types, characteristics and emergency medical support (particularly classifying and emergency treatment, triage, transportation and evacuation, seeking and unearthing for buried victims) of explosive injuries in explosive attack site were systematically expounded.

Eight 1,6-O,O-diacetylbritannilactone (OABL) derivatives (compounds 1-8) were synthesized by esterification or reduction of 1-O-diacetylbritannilactone (ABL) isolated from Inula japonica.All derivatives were evaluated for their anti-inflammation activities through the determination of inhibition of nitric oxide (NO) production in lipopolysaccharide (LPS)-induced macrophages.As results,compounds 5-8 (IC50 ＜ 2 μmol/L) exhibited more potent inhibition of NO production activities than the lead compound OABL.

Objective To set up a method to determine cytoplasmic free calcium [Ca~(2+)]_(i) in human platelets with flow cytometry.Methods As a calcium indicator,Fluo 3-AM was used to determine the changes,induced by thrombin,of the level of platelet plasma free calcium [Ca~(2+)]_(i) in presence or absence of outside calcium [Ca~(2+)]_(o),in order to elucidate where the platelet plasma free calcium [Ca~(2+)]_(i) mainly come from and it's role in the activity of platelets.Results The geometric mean of fluorescence intensity of the platelet,labeled with Fluo 3-AM,was increased obviously.The concentration of platelet plasma free calcium [Ca~(2+)]_(i) was increased significantly by thrombin in a dose-and [Ca~(2+)]_(o)-dependent manner.Conclusion The increased level of platelet plasma free calcium [Ca~(2+)]_(o),induced by thrombin,mainly comes from [Ca~(2+)]_(o) and partly from the released [Ca~(2+)]_o of intra-platelet probably.

Objective:To analyze the polymorphism of urease B gene(ureB) from different Hp strains. Methods:The sequences of ureB gene from 14 different Hp strains were amplified by PCR to analyze PCR-RFLP with HaeⅢ digestion.At the same time,the sequence of ureB gene was analyzed with biochemical software to compare the homology of nucleotide and amino acid sequence and to profile the phylogenetic tree. Results: The results of 1.7 kb ureB gene digested with HaeⅢ showed there were 2-5 band types in 14 different Hp strains and formed five distinct RFLP types.The two reference strains had the same RFLP type as 3 clinical isolates while the other clinical isolates and 3 isolates from animal model belong to 4 different RFLP types respectively.The nucleotide sequences and putative amino acide sequences of Hp ureB were compared.The various ureB sequences had high homologies(more than 96.6% and 98% in nucleotide sequences and amino acide sequences respectively) among 14 Hp strains.Particularly,there was the highest homologies(100%) between CCS9801、CCS9806、M3 and M10.Phylogenetic tree analysis showed that two reference strains and other isolates from clinical patients and Hp-infected mice model were located in two different lineage respectively in phylogenetic tree of nucleotide sequences while there were some variance in phylogenetic tree of amino acide sequences. Conclusion: Hp ureB was high conserved and homologous in the gene level as well as in the protein level.

Objective To investigate application value of cutter stapler in transumbilical single port laparoscopic left lateral lobectomy.Methods The retrospective cohort study was adopted.The clinical data of 26 patients who underwent transumbilical single port laparoscopic left lateral lobectomy at the Shengjing Hospital of China Medical University from January 2010 to February 2016 were collected.Nine patients who received liver parenchyma using ultrasonic knife were allocated into the ultrasonic knife group,17 patients who received liver parenchyma using cutter stapler were allocated into the cutter stapler group.Observation indicators included (1) operation situations:operation time,volume of intraoperative blood loss,postoperative complications,time of postoperative bowel function recovery,time of abdominal cavity drainage tube removal,duration of postoperative hospital stay.(2) Postoperative reexamination and follow-up:ultrasound or computed tomography (CT) examination was performed when necessary for detecting local exudation or encapsulated effusion.The patients were followed up at postoperative 1 to 3 months with telephone interview for whether with abdominal distension or abdominal pain till March 2016.Measurement data with normal distribution were presented as (x) ± s and analyzed by using t test.Measurement data with skewed distribution were presented as M (range) and analyzed by ranksum text.Comparison of count data was analyzed by the Fisher' s exact probility.Results (1) Operation situations:all the 26 patients received transumbilical single port laparoscopic left lateral lobectomy with no conversion to porous laparoscopic surgery or open surgery.The operation time was (114 ± 54) minutes,the volume of intraoperative blood loss was 100 mL (range,20-800 mL),and no intraoperative blood transfusion was adopted.The operation time and volume of intraoperative blood loss were (135 ±43)minutes and 200 mL (range,20-800 mL) in the ultrasonic knife group,(103 ±57)minutes and 100 mL (range,20-300 mL) in the cutter stapler group,respectively,showing no statistically significant difference between the 2 groups (t =1.500,Z =-0.961,P ＞ 0.05).All the 26 patients recovered well after surgery,with no postoperative complications as postoperative hemorrhage,bile leakage,incision infection or death.The time of postoperative bowel function recovery,time of abdominal cavity drainage tube removal and duration of postoperative hospital stay was (1.5 ±0.4) days,(5.8 ± 2.0) days and (7.0 2.0) days in the ultrasonic knife group,(1.1 ± 0.3) days,(4.1 ±1.1) days and (4.9 ± 1.4) days in the cutter stapler group,respectively,showing statistically significant differences between the 2 groups (t =2.599,2.875,3.036,P ＜ 0.05).(2) Postoperative reexamination and follow-up:of 26 patients,11 patients received ultrasound or CT examination after surgery and detected no obvious local exudation or encapsulated effusion,with no special treatment.The other 15 patients didn't receive ultrasound or CT examination.All the 26 patients were followed up for 1-3 months,with no occurrence of upper abdominal distension or abdominal pain.Conclusion Transumbilical single port laparoscopic left lateral lobectomy is safe and feasible,the application of cutter stapler is helpful to safety and success of the operation,further accelerating the postoperative recovery of patients.

Objetive To investigate the effect of Fasudil on the hepatic ischemia repeffusion injury on rats of obstructive jaundice.Methods 160 mature SD rats were randomly assigned into 2 large groups,namely experimental group (Group A) and control group (Group B).While each large group was subdivided into 4 subgroups,including sham operation subgroups(Group A1 and B1),obstructive jaundice model subgroups(Group A2 and B2),ischemia reperfusion model subgroups (Group A3 and B3)and obstructive jaundice plus ischemia reperfusion model subgroups (Group A4 and B4).In Group A2 and B2,obstructive jaundice models were made on rats by double ligating and cutting the common bile ducts.In Group A3 and B3,the ischemia reperfusion injury models were built by blocking the hepatic portal vessel for 30 minutes before reperfusion.In Group A4 and B4,the ischemia reperfusion injury models were built in 1 week after building the obstructive jaundice models.For Group A,10 mg/kg fasudil was intraperitoneally injected at 30 minutes before ischemia.For Group B,equal amount of physiological saline was injected.Samples were gained on zero hour,1 hour,2 hour and 6 hour post reperfusion.The serum indexes for liver function and serum Endothelin 1 level were tested and analyzed through SPSS.Pathological changes of liver were viewed correspondingly.Results Compared to control groups (Group Bs),experimental groups (Groups As) have better liver function,lower Endothelin 1 level (P ＜ 0.05),as well as less severe microscopic histological damage.Conclusion Fasudil can effectively protect rats of obstructive jaundice from hepatic ischemia reperfusion injury.

Objective To investigate the effects of injecting allogeneic mesenchymal stem cells(MSCs) on the cellular immunity in rat in vivo.Methods Bone marrow-derived MSCs were isolated from Wistar rats.The purity of MSCs was identified by morphological examination with microphotography,and the phenotypes were identified with flow cytometry.Twenty SD rats were randomly divided into 4 groups.Different concentrations of MSCs(5?106/ml for group A,5?105/ml for group B,and 5?104/ml for group C,respectively) and PBS(for group D) were given to allogeneic SD rats via intravenous infusions.The suppressive effect of MSCs on lymphocytes proliferation in recipient rat was analyzed using mixed lymphocyte cultures(MLR) 10 days after cultivation.At the same time,proportions of CD4+,CD8+ and CD4+CD25+/CD4+ T-lymphocytes in peripheral blood and spleen were analyzed with flow cytometry.Results Proliferation rate of splenic lymphocytes in group A(5?106/ml MSCs,8.58%?0.27%) was markedly lowered compared with that in group D(PBS,24.40%?5.21%,P

Objective To explore the feasibility of establishing mini-pig model of type 1 diabetes by single intrave-nous injection of high dose streptozotocin .Methods Eight male Zhonghua mini-pigs (22.6 ±1.8 kg) were administrated with high-dose streptozotocin (150 mg/kg) into the ear vein .Before and 10, 30, 90 minutes, 1, 3 and 7 days after adminis-tration of streptozotocin , blood samples were obtained respectively , and used to dynamically monitor the fasting blood-glucose. C peptide and insulin levels were evaluated by IVGTT test .Results Since 24 hours after STZ administration , the fasting blood-glucose level was increased significantly compared with that of pre-administration and maintained at 16.7-20.6 mmol/L, while the C peptide and insulin levels were decreased significantly .IVGTT results showed that blood sugar levels at 1 h af-ter intravenous injection of 50%glucose were much higher than 11.1 mmol/L and failed to restore to fasting glucose levels until 2 h, insulin and c-peptide did no response after injection of glucose , always kept at a trace level .Conclusion A sin-gle high-dose streptozotocin injection can be used to establish a mini-pig model of type 1 diabetes successfully .
