1.Analysis of anesthesia characteristic parameters based on the EEG signal.

All the collected original electroencephalograph (EEG) signals were the subjects to low-frequency and spike noise. According to this fact, we in this study performed denoising based on the combination of wavelet transform and independent component analysis (ICA). Then we used three characteristic parameters, complexity, approximate entropy and wavelet entropy values, to calculate the preprocessed EEG data. We then made a distinguishing judge on the EEG state by the state change rate of the characteristic parameters. Through the anesthesia and non-anesthesia EEG data processing results showed that each of the three state change rates could reach about 50.5%, 21.6%, 19.5%, respectively, in which the performance of wavelet entropy was the highest. All of them could be used as a foundation in the quantified research of depth of anesthesia based on EEG analysis.

2.Continuous passive joint motion following total knee replacement: 48 cases analysis

BACKGROUND: Early rehabilitation following total knee replacement has arisen more attention. The aim of continuous passive joint motion is to recover knee function, prevent anchylosis, accelerate blood and synovia circulation, therefore, enhance tissue repair. OBJECTIVE: To evaluate the efficacy of continuous passive joint motion following total knee replacement. METHODS: Totally 48 patients, who underwent artificial total knee arthroplasty at the Department of Orthopedics, Center Hospital of Minhang District, Shanghai Ruikang Hospital Group and Department of Rehabilitation Medicine, 309~(th) Hospital of Chinese PLA between December 2007 and October 2009 were selected. The patients were randomly divided into the experimental and control groups, with 24 cases in each group. The isometric contraction of qudraceps muscles, combined with physical agent assistant, was performed prior to total knee replacement. Patients in the experimental group was received continuous passive joint motion training at day 2 after operation, with 5°-10°increasing per day. Patients in the control group were treated with conventional methods. The knee functional score, and visual analog scale (VAS) was performed to evaluate knee functions and pains, in addition, perimeter between two legs; and range of motion were measured. RESULTS AND CONCLUSION: The knee functional score, VAS, perimeter between two legs, and range of motion were similar prior to and after total knee replacement (P > 0.05). Compared to the control group, the knee functional score of the experimental group was dramatically increased at days 3 and weeks 2 after operation (P < 0.01), the VAS and perimeter between two legs were significantly declined (P < 0.01), the range of motion was obvious improved after replacement, with greater excellent rate (P < 0.01). There was no deep venous thrombosis or infections. The results demonstrated that early rehabilitation following knee replacement is conductive to easing pain, eliminating swelling, and enhancing knee stability.
3.Pathogens and Their Antimicrobial Resistance in 105 Children with Community-acquired Bacteremia

OBJECTIVE To determine the pathogenic distribution and antimicrobial resistance of community acquired bacteremia in 105 children patients.METHODS Clinical profile,results of blood culture and antibiotic susceptibility test of 105 patients with community-acquired bacteremia treated from Jan 2003 to Dec 2006 were studied retrospectively.RESULTS The Gram-positive bacteria were from 58 cases which accounted for 55.2%.Most of them were Staphylococcus epidermidis,S.haemolyticus and S.hominis.The positive rate was 19.0%,15.5% and 10.3%,respectively.The Gram-negative bacilli were seen from 47 cases which accounted for 44.8%.Escherichia coli,Stenotrophomonas maltophilia and Klebsiella pneumoniae were the main microrganism in Gram-negative bacilli,the positive rate was 19.1%,14.9% and 10.6%,respectively.The resistant rate of Staphylococcus to penicillin was 100%.Their resistant rate to erythromycin was 94.7%.100% Gram-positive bacteria were susceptible to vancomycin,most of them were susceptible to ciprofloxacin,rifampicin and nitrofurantoin.The resistant rate of Gram-negative bacilli to ampicillin was 91.7%.Most of Gram-negative bacilli were susceptible to the third generation cephlosporin plus enzyme inhibitor,ciprofloxacin and imipenem.Resistance of the isolated pathogens to several commonly used antibiotics in pediatrics was observed.CONCLUSIONS We must focus on children aged under 3 years who are at risk of developing bacteremia.Empirical treatment with antibiotics is recommended only in life-threatening sepsis cases in pediatric emergency department.
4.Evaluation of a pediatric emergency observation unit:an analysis of 5471 observation pediatric patients

5.Clinical analysis of 5471 cases in a pediatric emergency observation unit

Objective To evaluate the role of an emergency observation unit(OU) in pediatric care.Methods A retrospective study was conducted to evaluate the medical service of the OU for 12 months.Results During 12 months,5?471 children were admitted to the OU,which was 0.78% of all visits to the outpatient department(OD) and emergency department(ED).Of OU patients,70.9% children were discharged home and 34.6% children were discharged under 48 hours.Of OU patients,65.6% children were under 2 years old.Median length of stay was 95.7 hours.The average admission rate was 15 children per day.The patient volume per month of OU was in positive relation with the visits volume per month of OD and ED (r=0.835,P=0.001).The commonest causes for children admitted to the observation unit were pneumonia(22.2%),enteritis(13.3%) and acute upper airway infection(11.4%).Conclusion The emergency pediatric OU should be set to assess and treat young children with a variety of conditions.It is effective in relieving the problems of limited inpatient hospital bed spaces and high-volume patient visits,and helpful for limiting inappropriate hospitalization.
6.Affect of ammonium glycyrrhizinate on extracellular matrix of chronic viral hepatitis

AIM:To study the affect of two kind preparations of ammonium glycyrrhizinate on serum extracellular matrix level of chronic viral hepatitis. METHODS: Seventy patients with chronic viral hepatitis were randomly divided into two groups. The first group of 41 patients (M36,F5;age 47a±s 13a) was treated with diammonium glycyrrhizinate 0.15 g, iv, gtt, qd×30 d. The second group of 29 patients (M23,F6; age 48a±12a) was treated with monoamonium glycyrrhizinate 0.2 g iv, gtt, qd×30 d. Serum ECM levels were assayed using RIA method. RESULTS: After treatment the serum ECM levels of both groups reduced remarkably and there was no differance in the reduction of serum ECM levels of the two groups except LN (P<0.05, P<0.01). CONCLUSION: Both preparations of glycyzzhizinate have the efficacy to reduce serum ECM level of chronic viral hepatitis patients and play some role in anti-hepatic fibrosis. Diammonium glycyrrhizinate is more potent than monoammonium glycyrrhizinate.
7.The clinical characteristic of 43 cases of neuroendocrine tumors with liver metastases

Objective To analyze the clinical characteristics of liver metastases of neuroendocrine tumors (NET) and its treatment outcome,so as to further cognition of NET.Methods The clinical data of patients with liver metastases of NET diagnosed by Peking Union Medical College Hospital during January 1996 to July 2010 were analyzed retrospectively.Results The ratio of male to female was 1∶1.15 (20∶ 23).The median age at onset of the patients with liver metastases of NET was 47.5 (26-70) years.The median duration from onset to diagnosis was 4 (0-120) months.The liver metastases were the first manifestation in 69.8% (30/43) cases.The detection rate of primary lesions with routine abdominal imaging (B-type ultrasonography,CT,MRI) was 65.1% (28/43),while increased to 90.7% (39/43) when combined the following one or more special examinations including somatostatin receptor scintigraphy ( SRS),PET-CT,endoscopic ultrasound (EUS) (P =0.004).The definite diagnosis methods mainly depended on surgical specimens (69.8%,30/43).The ratio of nonfunctional to functional NET with liver metastases was 1.87∶1(28∶ 15).The primary tumors were most commonly located in pancreas [39.3% (11/28) and 73.3%( 11/15 ) ],followed by stomach [ 21.4% (6/28) and 13.3% ( 2/15 ) ].Totally 88.4% ( 38/43 ) patients received operation,and 9.3% (4/43) patients had reoperation due to missed diagnosis of the primary tumors on earlier operation.Non-surgical treatments included octreotide acetate long-acting release,interventional therapy,chemotherapy and radiotherapy,which were difficult to be evaluated due to less follow-uped cases.Conclusions Liver metastases of NET are common and even the first manifestated symptom.Primary NET with liver metastases is the most commonly nonfunctional and located in digestive system.The detection rates of primary lesions are increased by special examinations including SRS,PET-CT and EUS. Surgical specimens are helpful to the final diagnosis,but it is necessary to improve the preoperative diagnostic rate of primary tumors to avoid repeat surgeries.
8.Determination of the Contents of Estriol in Estriol Suppositories by Three-wavelength Spectrophotometry

OBJECTIVE:A method was established for determining the content of estriol in estriol suppositories.METHO_ DS:Using three wavelength spectrophotometry.RESULTS:The detecting wavelengths were296nm,281nm and276nm;the average recovery rate of estriol was99.2%(RSD=0.5,n=6).CONCLUSION:The method is accurate,quick and suitable for detecting estriol in intermediate material of estriol suppositories.
9.Clinical analysis of children with acute symptoms of vomiting and abdominal pain

Objective To provide the practical basis for clinical diagnosis and treatment of patients who were admitted by green channel because of vomiting and abdominal pain. Method 268 cases with vomiting andabdominal pain ( from January 2007 to December 2008 ) who were admitted into the emergency observation unit through green channel were retrospectively analyzed, and various risk factors were considered with logistic regression model. Result Among 268 cases, 34 kinds of different diseases were found,while gastrointestinal diseases were the greatest proportion [60. 1% (161/268)] and gastroenteritis was most common reason 77.6%( 125/161 ). Between different age groups ( ≤ 3-year-old group and>3-yearold group), the overall incidence of various diseases and the profile of disease was different (P<0.05).The incidence of diseases in the digestive tract, infectious diseases and surgical between two groups was statistically different (P<0.05). Between inpatient admission and non-inpatient admission patients, single logistic regression showed that there was significantly different between the nine events (age, fever, shock,duration of symptoms, blood routine test( BRT), liver function, blood Na+, blood / urine amylase, electrocardiogram) ( P <0.05). In the regression model, all showed significant difference except BRT. Conclusion Most of patients with vomiting and abdominal pain were digestive system diseases. Patients ≤ 3years old had a higher incidence of infectious and surgical diseases. The patients who accompanied by fever,shock and laboratory tests such asliver function, blood Na +, blood / urine amylase, electrocardiogram were abnormal would have higher hospitalize rate.
