Objective To conduct a Meta analysis based evaluation of dual-source computed tomography (DSCT) in the diagnosis of coronary artery disease (CAD). Methods Literatures publicated from January 2006 to July 2009, including in English or Chinese languages, were searched in Medline,China National Knowledge Infrastructure (CNKI), and Chinese Medical Assosiation Digital Periodicals (CMADP). A study was enrolled if it: ( 1 ) used DSCT angiography as the diagnostic test for the detection of significant coronary stenosis ( ≥50% diameter stenosis) in patients with suspected CAD; (2) used coronary angiography as the reference standard. The pooled sensitivity and specificity of the 95% confidence interval (95% CI) were acquired based on the bivariate random-effects modol. Hierarchical weighted symmetric summary receiver-operating curve (HSROC) was also estimated. The pooled likelihood ratios were calculated based on the pooled sensitivity and specificity. Combined with the pooled likelihood ratios, the clinical utility of the results was estimated according to Bayes'theory. Results Total of ten articles enrolled in this study, included 1271 patients. The mean rate of nonevaluable patients and segments were 5.6% (33/590) and 2. 3% (271/11 745 ), respectively. The pooled statistical results were as follows: the sensitivity and specificity were 99% (97%-99% ) and 86% (79%-90% ), respectively; the positive and negative likelihood ratio were 6. 84 and 0. 01, respectively. If a pre-test probability was below 84%, the likelihood of disease was less than 5% on a negative diagnosis of DSCT. If a pre-test probability was above 13%, the likelihood of disease was more than 50% on a positive diagnosis of DSCT. Conclusions DSCT angiography presents good diagnostic performance in the detection of CAD; however, it could not completely replace conventional coronary angiography.

Chinese Journal of Tissue Engineering Research 2016;20(7):972-980
BACKGROUND:When the intervertebral disc is under stress, the hydraulic pressure generated inside the nucleus pulposus makes the annulus fibrosus extend outward and expand, and the annulus colagen fibers are stretched so that the extracelular matrix of annulus fibrosus cels is also under the pressure. In the intervertebral disc, aggrecan is the main component of proteoglycans, matrix metaloproteinase-2 is a major enzyme for extracelular matrix degradation, and tissue inhibitor of metaloproteinase is a multifunctional specific inhibition factor for matrix metaloproteinase activity. There is a mutual regulation between the latter two to keep the homeostasis between them.
OBJECTIVE: To investigate the mechanism of cyclic tensile strain in the metabolism of intervertebral disc annulus matrix.
METHODS:Rat anulus fibrosus cels were subjected to 2% or 10% cyclic tensile strain at 1.0 Hz for 2 and 12 hours using Flexcel4000 tension system. Then cels were colected and cultured in conditioned medium for gene and protein detection. Real-time quantitative PCR was used to detect mRNA expression of aggrecan, matrix metaloproteinases-2 and tissue inhibitor of metaloproteinase-2. Gelatin zymography was used to detect matrix metaloproteinases-2 activity.
RESULTS AND CONCLUSION:The use of 2% cyclic tensile strain had no obvious effect on the stress fiber of actin cytoskeleton, whereas actin cytoskeleton was depolymerized in response to 10% cyclic tensile strain. The 2% cyclic tensile strain raised the expression of Aggrecan at 12 hours; whereas raised the matrix metaloproteinases-2 and tissue inhibitor of metaloproteinase-2 at 2 hours, both of which were in homeostasis; matrix metaloproteinases-2 activity had no significant changes. 10% cyclic tensile strain had no effect on the mRNA expression of Aggrecan. No matter stretching 2 or 12 hours, the matrix metaloproteinases-2 was up-regulated, and the tissue inhibitor of metaloproteinase-2 was down-regulated, both of which were not in balance. Moreover, the matrix metaloproteinases-2 activity was not significantly changed. These findings indicate that the mRNA expressions of Aggrecan, matrix metaloproteinases-2 and tissue inhibitor of metaloproteinase-2 alter in response to cyclic tensile strain in rat anulus fibrosus cels, and the tensile strain induces different mechano-responses in the actin cytoskeleton.

[ ABSTRACT] AIM: To investigate the role of TNF-αand NF-κB in the mechanism of neuropathic pain due to chronically compressed dorsal root ganglion ( CCD) .METHODS:Based on the CCD model, von Frey filaments were used to quantify behavior test. The expression changes of TNF-αand NF-κB were determined by Western blotting, and the cor-relation between the expression of TNF-αand the 50%paw withdrawal threshold was also analyzed. Moreover, the location of TNF-αin dorsal root ganglion ( DRG) was observed with immunofluorescence double staining.RESULTS: We found 50%paw withdrawal threshold of CCD decreased at the first day after operation. The mechanical allodynia was the most ob-vious at postoperative 7~14 d and lasted longer than 35 d.The expression of TNF-αand NF-κB increased significantly in DRG after operation (P<0.01), especially at 7~14 d, and then restored gradually. Moreover, there was a correlation between the protein expression of TNF-αand the changes of neuropathic behavior ( P<0.05 ) .CONCLUSION: TNF-αand NF-κB are involved in the mechanism of mechanical allodynia after chronically compressed DRG.

Aim To study the regulatory effects of PMA,a PKC activator,on Kir 2.3 channel function expressed in Xenopus oocytes and COS-7 cells, and PIP2 involvement in these regulations. Methods Kir 2.3 channel was expressed in Xenopus oocytes and COS-7 cell by RNA microinjection and DNA transfection using calcium phosphate precipitate,respectively.Two-electrode-voltage-clamp and whole-cell patch clamp were used to record the Kir 2.3 current in Xenopus oocytes and COS-7 cell. The PIP2 hydrolysis was detected by confocal microscopy. Results PMA significantly inhibited Kir 2.3 current in Xenopus oocytes. But PMA had no effect on the Kir 2.3 current expressed in COS-7 cell,in which activation of M1 receptor, however, induced a significant inhibition of Kir 2.3 current. It was reported recently that PMA could trigger the PIP2 hydrolysis in membrane of oocytes. Thus PKC inhibition of Kir 2.3 current seen in oocytes could be the result of PIP2 hydrolysis.Following the same line, the inability of PKC inhibition of Kir 2.3 current seen in COS-7 cells would suggest PKC could not induce PIP2 hydrolysis in these cells. This hypothesis was tested by monitoring the PIP2 level in COS-7 cell membrane by confocal microscopy.Dynamic changes in membrane PIP2 level were imaged using GFP fluorescence signal that had been tagged to the PLC?1PH domain known to be able to bind PIP2 specifically. There was no significant change of PIP2 level on COS-7 cell membrane after longtime treatment of PMA,whereas again, the activation of M1 receptor by ACh induced a significant change in the PIP2 level. These results were in perfect agreement with the electrophysiological results. Conclusions PMA,through activation of PKC,inhibited Kir 2.3 current expressed in Xenopus oocytes but not in COS-7 cells.Similarly PMA induced significant reduction in membrane PIP2 level in Xenopus oocytes but not in COS-7 cells. PIP2 hydrolysis plays an important role in PKC-induced inhibition of the Kir channel currents.
