1.Application of Remifentanil in General Anesthesia in Cesarean Section

Objective Observing the effect of general anesthesia (GA) with remifentanil on parturients undergoing cesarean section and Apgar score of child.Methods 40 parturients were divided into two groups,GA combined remifentanil (group A,n =20) and standard GA(group B,n =20).Observe the change of hemodynamics parameter and Apgar score of child.Results In group A than in group B maternal hemodynamic index smooth,two groups of intubation after lmin SBP,DBP,HR decreased,while the5 min after intubation returned to normal.SBP DBP,HR before induction,the difference between groups was not statistically significant (P> 0.05),the two group after induction,SBP,DBP,HR intubation after 1 min were (120.8 ± 19.6 mm Hg,80.6 ± 11.2 mm Hg,98.1 ± 17.1 BPM),(128.1 ± 17.5 mm Hg,98.9 ± 13.8mm Hg,108.7 ± 20.7 BPM) after intubation of 5 min were (98.3 ± 24.7 mm Hg 75.3 ± 13.8 mm Hg,79.9 ±22.6 BPM),(121.5 ± 16.2 mm Hg,88.6 ± 13.7 mm Hg,110.5 ± 17.4 BPM) had a significant difference (P < 0.05),neonatal 1 min,5 min Apgar score,no significant differences between the two groups (P >0.05).Conclusion When remifentanil is used in the general anesthesia,its inducing is smoothly and it is no significant bad effect on the newborn.
2.Measurement of brain metabolites by 1H-MR spectroscopy in patients with Alzheimer disease: a Meta analysis

Objective To have a systemic review of the association between relative ratio in proton magnetic resonance spectroscopy (1H-MRS) and Alzheimer's disease (AD). Methods A search in Medline and China National Knowledge Infrastructure (CNKI) was performed,and relevant English and Chinese-language articles about assessing AD with 1 H-MRS were identifed.The data of relative metabolic ratios (NAA/Cr,Cho/Cr,mI/Cr ) from different brain regions (hippocampus,posterior cingulate gyrus,temporal lobe,parietal lobe,frontal lobe,occipital lobe) were extracted from the articles.The quality of the articles was evaluated according to the standard recommended by Newcastle-Ottawa criteria. The Meta-analysis was done with the Review Manager 4.2 to calculate pooled weighted mean difference (WMD) with 95% confidence interval (95% CI ),and linear correlation analysis between NAA/Cr ratio and mI/Cr ratio was done by SPSS 17.0.Results Thirty six articles (27 English articles,9 Chinese articles) were included. After heterogeneity test was done,fixed effects model or random effects model was selected. The meta-analysis showed that the NAA/Cr ratio in patients with AD was higher than that in controls ( WMD:-0.14,95%CI:-0.17 to -0.11 ).The mI/Cr ratio in patients with AD was lower than that in controls ( WMD:0.10,95% CI:0.07 to 0.13 ).There were greatest changes in NAA/Cr ratio and mI/Cr ratio on the hippocampus ( WMD of NAA/Cr: - 0.27,95% CI:- 0.36 to - 0.19 ; WMD of mI/Cr:0.21,95% CI:0.10 to 0.33). There were also no differences between patients with AD and controls with respect to the Cho/Cr ratio (WMD:0.01,95% CI:0.00 to 0.01,P >0.05). The NAA/Cr and mI/Cr changes are markedly correlated with each other in different brain regions ( r =0.947,P =0.004). Conclusion The hippocampus region is the first to present neuropathological changes in AD and the changes of NAA/Cr and MI/Cr might reflect the neurodegenerative process of AD.
3.Clinical and pathological observation of transcatheter embolization for uterine fibroids

Objective To inverstigate the efficiency and safety of transcatheter uterine artery embolization (TUAE) as well as the pathological changes and it′s significance after embolization. Methods 44 cases of uterine fibroids were treated by TUAE using lipiodol pingyangmycin emulsion (LPE) and Gelfoam sponge paeticles. Seven of 44 cases underwent hysterectomy or myomectomy from 1 to 6 weeks after TUAE.The specimens were studied to observe pathological changes of fibroids and myometrium. 37 of 44 patients were followed up for 1 to 6 months to observe the changes of fibroid size and uterine volume. The changes of ovarian function and myometrium after TUAE were found out by observation on mensis, test of ovarian hormone, premenstrual diagnostic curettage and pathology of the resected specimens. Results Total improvement of symptams occurred in 91.9%(34/37), including control of menorrhagia in 91.4%(32/35), disappearance of lumbago and lower abdominal pain in 85 3%(29/34),relief of freguency and urgency of micturition in 50.0%(7/14). Average decrease of fibroid and uterine volume at 6 months after TUAE was 61.3% and 50.6% respectively. No amenorrhea happened. Premenstrual diagnostic curettage showed secretory endomtrium. There was no significant difference in ovarian hormon level befor and after embolization. Pathological study showed that lipiodol was found to deposit in fibroids and was not seen in myometrium. Spotty necrosis 2 weeks occurred in fibroid and extensive patchy necrosis 3 weeks after embolization. The necrotic area became larger with elapse of time postoperative. Necrosis was not seen in myometrium. Conclusion TUAE is a new ,and effective procedure for uterine fibroid and LPE as an embolizing agent is safe without damage on myometrium.
4.Effects of preinfarction angina pectoris on severe ventricular arrhythmia and QTd in patients with first acute myocardial infarction

Objective: To observe the effects of angina pector is on severe ventricular arrhythmia and QTd in patients with first acute myocard ial infarction(AMI). Methods: One hundred and eight-four cases of first AMI were divided into 2 groups: PA group, angina pectoris occurred with in 24 h before AMI onset (n=58), NPA group, no preceeding angina pectori s occurred (n=126). Occurrence of complications and QTd were investigated du ring hospitalization. Results: The basic clinical characteristic s, coronary risk factors, medication before infarction, treatments after admissi on with antiarrhythmic agents, site of infarction, successful rate of thrombolys is and peak CK, CK-MB were not statistically different. Early QTd in PA group and NPA group were (56.22±18.40) ms vs (84.45±21.90) ms, respectively, P <0.05, late QTd in PA group and NPA group were (50.67± 16.34) ms vs (64.1 8(16.41) ms, respectively, P<0.05. Comparison with NPA group, incidence of severe ventricular arrhythmia, heart failure, cardiogenic shock and rate of car diac mortality in-hospital was lower in PA group. Conclusion: P reinfarction angina pectoris can significantly reduce the incidence of severe ve ntricular arrhythmia and QTd in the patients with first AMI, sugges ting that these favorable effects might be associated with protective effects of ischemic preconditioning on myocardium and ventricular pump function and improv ement of repolarizative asynchronism in ventricular myocardium.
5.The accuracy of functional magnetic resonance imaging and contrast-enhanced ultrasound on evaluation of breast cancer patients after neoadjuvant chemotherapy

Objective To investigate the accuracy of functional magnetic resonance imaging(fMRI) and contrast-enhanced ultrasound on evaluation of the breast cancer patients after neoadjuvant chemotherapy.Methods Forty-eight patients with breast cancer who were confirmed by biopsy,undergoing 4 cycles weekly dose-concentrated programs (wPC) neoadjuvant chemotherapy,evaluated the efficacy by fMRI and contrastenhanced ultrasound.Results In pathology,complete remission was 12 cases (25.0%,12/48),non remission was 13 cases (27.1%,13/48).In contrast-enhanced ultrasound,complete remission was 20 cases (41.7%,20/48),non remission was 10 cases (20.8%,10/48).In fMRI,complete remission was 17 cases (35.4%,17/48),non remission was 11 cases (22.9%,11/48).The P values more than 0.05 showed no statistically significant among three methods,the Kappa test showed that fMRI were superior to contrastenhanced ultrasound on evaluating the efficacy of neoadjuvant chemotherapy.Conclusions fMRI and contrast-enhanced ultrasound can be used in evaluating the efficacy of neoadjuvant chemotherapy,however,the data shows fMRI is superior to contrast-enhanced ultrasound.Contrast-enhanced ultrasound must be as the supplementary method.
6.Rifampicin resistance among elderly patients with pulmonary tuberculosisin Changzhou City

Objective:
To investigate the rifampicin resistance status and its influencing factors among elderly patients with pulmonary tuberculosis in Changzhou City, Jiangsu Province, so as to provide the basis for improving drug-resistant pulmonary tuberculosis prevention and control strategies.
Methods:
Patients aged over 60 years with pulmonary tuberculosis treated in tuberculosis designated hospital was selected. Demographic information, history of previous disease, history of diagnosis and treatment and drug sensitivity test were collected. Rifampicin resistance among elderly patients with pulmonary tuberculosis and its influencing factors was analyzed.
Results:
Totally 249 elderly patients with pulmonary tuberculosis were included, with an average of (69.75±4.36) years. There were 147 males and 102 females, with a gender ratio of 1.44∶1. There were 183 treatment-naïve patients (73.49%) and 66 retreated patients (26.51%). Rifampicin resistance was found in 21 cases, with a drug resistance rate of 8.43%. Among them, there were 7 treatment-naïve patients (3.83%), and 14 retreated patients (21.21%). Multivariable logistic regression analysis showed that retreated elderly patients with pulmonary tuberculosis had a higher risk of rifampicin resistance (OR=10.551, 95%CI: 1.344-82.857).
Conclusion
The rifampicin resistance rate of 249 elderly patients with pulmonary tuberculosis was 8.43% and was associated with the type of treatment.
7.Study on lingual mucosa carcinogenesis of C57BL/6 mice induced by 4-nitroquinoline 1-oxide.

OBJECTIVEThis study aimed to induce carcinogenesis of lingual mucosa in C57BL/6 mice by feeding them 4-nitroquinoline 1-oxide (4NQO) solution.
METHODSA total of 85 C57BL/6 mice were randomly divided into distilled water control group (DD group, n=5), 1,2-propylene glycol control group (PG group, n=5), and experimental group (EP group, n= 75). The mice in the experimental group were medially fed in 15 cages. By contrast, the mice in DD, EP, and PG groups were watered with distilled water, 50 mg.L-1 4NQO solution, and 1,2-propylene glycol solution. The mice in EP group were executed every two weeks from week 0, and the mice in the control groups were sacrificed at the 28th week. The mice were weighed. Mucosal lesions were measured by macroscopic observation and histopathologic detection.
RESULTSOne mouse in EP group died of unknown reason. The weight of the mice in EP group presented weight loss compared with the mice in DD and PG groups after the 24th week. Seventy-nine macroscopic lesions were observed in the lingual mucosa, oral floor, and upper palatal and buccal mucosa. A total of 70 macroscopic lesions (88.6%) were located in the lingual mucosa. Mucosal lesions changed from simple hyperplasia to squamous cell carcinomas. Well-differentiated squamous cell carcinomas were observed in all mice of EP group by pathological section at the 28th week. No lesion was found in the mice of DD and PG groups.
CONCLUSIONThe animal model of lingual squamous cell carcinomas was successfully established. The periods from 12th to 16th week and 20th to 28th week were the ideal times for the research on pathogenesis of early and medial-advanced stage during carcinogenesis of squamous cell carcinomas.

8.MRI manifestations of normal pancreas in piglets

Objective To observe the MRI findings of normal pancreas in piglets. Methods Eight healthy piglets underwent MR examination, and the morphology, size, signal intensity of pancreas were observed. After MR imaging, all piglets were abdominally incised to observe the anatomy of pancreas and pancreatic adjacent structures. The opening of both common bile duct and pancreatic duct were detected during operation. Two piglets were sacrificed after operation and the whole pancreases were dissected for anatomic research. Results The pancreas of piglets was composed of three parts: right lobe, median lobe and left lobe. All the lobes were displayed clearly on MRI. The signal intensity of pancreas was higher than that of liver and spleen on T1WI, whereas lower than that of liver and spleen on T2WI. On MRCP, pancreatic duct was not presented, whereas the common bile duct could be seen clearly. The opening of common bile duct located at superior part of duodenum (nearby the pylorus) and the opening of pancreatic duct situated at duodenal papilla corresponding to pancreatic right lobe. Conclusion MRI can show the pancreas of piglets very well. The morphology of pancreas and features of common bile duct conjunction with pancreatic duct in piglet are different from those in human.
9.Selection of the most effective small interfering RNA inhibiting the expression of BDNF in microglial cells and detection of its inhibitory effect

Aim To pick out the siRNA which could most effectively inhibit the expression of brain-derived neurotrophic factor( BDNF) in microglial cells,to detect the cytotoxicity of the transfection complex,and to ob-serve the change of OX-42 expression,the microglial marker,after BDNF siRNA treatment. Methods Four siRNAs were chemically synthesized: three of them were used to inhibit BDNF expression in microglial cells,the rest was fluorescence-labeled mismatch siRNA as a negative control. They were all transfected into microglial cells,respectively. BDNF mRNA was detected 24 h after transfection by Real-Time PCR and itsprotein expression was observed done by Western blot 48 h later. The Sulforhodamine B( SRB) assay was used to investigate the drug-induced cytotoxicity. Co-expres-sion pattern of BDNF and OX-42 was determined by double-labeling immunofluorescence. 10.Adventitious bud induction and endogenous hormones changing of lamina from Zanthoxylum dissitum in vitro

Objective To establish a regenerative system in vitro and determine the endogenous hormones of Zanthoxylum dissitum. Methods Lamina of Z. dissitum was used as the explant. Callus adventitious bud differentiation was carried out by culturing on MS with different hormones. At the same time,in the course of callus induction,four endogenous hormone of GA3,IAA,ABA,and ZR were determined by ELISA. Results The optimum medium for adventitious bud regeneration of Z. dissitum was MS+6-BA 0.5 mg/L+NAA 0.1 mg/L,and the highest induction rate could reach to 61%. During the process of the differentiation of explants callus,the contents of ZR and IAA were correspondingly higher than other endogenous hormones,the content of ABA was always kept in low level during this process,the content of GA3 was kept in the trend of upgrading during the earlier differentiation period. Conclusion It could be considered that ZR and IAA should be the critical factors in the bud induction. According to the results,a proper adding of GA3in the culture medium could improve the differentiation rate of the adventitious bud. On the other side,ABA might be the negative regulation factor.
