Objective To investigate the safety and clinical significance of total thyroidectomy performed for benign thyroid disease. Methods Eighty-eight patients with benign thyroid disease were treated with total thyroidectomy. The postoperative complications were analyzed. Results With primary total thyroidectomy, the incidences of transient hypocalcemia and transient recurrent laryngeal nerve paralysis were 2.5% and 1.2% respectively. Objective To study the value of 99Tcm-DTPA dynamic renography in evaluating the function of duplex kidneys in pediatric patients.Methods Twenty-five pediatric patients with duplex kidneys diagnosed by ultrasound or MR urography (MRU) were included (9 males,16 females; mean age:(23.80 ± 20.97) months,range:2-72 months).Twenty patients (9 males,11 females; mean age:(32.95 ± 23.58) months,range:2-72 months) with urinary tract infection but without duplex kidneys confirmed by ultrasound or MRU were chosen as control group during the same period of this study.All patients and controls were divided into two subgroups according to their ages (group Ⅰ,0-24 months; group Ⅱ,25-72 months).The research was approved by the ethics committee,and all patients' parents (or guardians) signed informed consents.The time-activity curve was generated on the dynamic imaging data automatically with GFR calculated.The uptake rates of the upper and lower moieties were measured by drawing the corresponding ROIs in the duplex kidney.Dunnett-t test was used for statistical analysis.Results There were 25 patients with 26 duplex kidneys (1 case bilateral),16 on the left and 10 on the right.The time-activity curve of 6 cases was normal,9 with continuously upward type,4 with high level plateau type,2 with parabolic type and 5 with low level plateau type.There were 19 abnormal kidneys in group Ⅰ and 7 in group Ⅱ,and 20 kidneys in each control subgroup.The GFR of patients with normal renography was (78.81 ± 15.97) ml/min (group Ⅰ) and (64.68 ± 11.15) ml/min (group Ⅱ),continuously upward type was (72.11 ±22.76) ml/min (group Ⅰ) and (63.41 ± 16.42) ml/min (group Ⅱ),high level plateau and parabolic types were (68.74 ± 16.17) ml/min and (65.26 ± 15.27) ml/min in group Ⅰ,respectively.There was no statistically significant difference between the GFR of different renography type groups and that of the controls (group Ⅰ:(79.35 ±13.31) ml/min,group Ⅱ (76.46 ±9.69) ml/min;all P >0.05).The GFR of patients with low level plateau type was (45.83 ± 10.17) ml/min (group Ⅰ) and (45.53 ± 10.42) ml/min (group Ⅱ).There was statistically significant difference between the GFR of two subgroups of low level plateau type and that of control group,respectively (both P < 0.05).Among the 26 abnormal kidneys,23 could be separated into upper and lower moieties.Among the 23 duplex kidneys,15 cases had uptake rate less than 10% of that of the whole kidney,5 cases ranging from 10% to 30%,and 3 cases greater than 30%.Conclusions Quantitative evaluation of duplex kidney functions can be performed with 99Tcm-DTPA renography.It may provide important information for the management of pediatric patients with duplex kidneys.
3.Expression of centrosome-associated kinase Aurora-A, mt-P53, and c-myc in colorectal cancer and its significance

Objective:This study aimed to investigate the expression levels of centrosome-associated kinase Aurora-A, mutant type P53 (mt-P53), and c-myc in colorectal cancer. This study was also conducted to investigate the mutual relationship and functions of these factors in tumorigenesis and tumor progression. Methods:We examined the pathological specimens obtained from colorectal cancer, pericancerous tissues, and normal colorectal tissues by tissue microarray technique and immunohistochemistry (SP method) to determine the expression levels of Aurora-A, mt-P53, and c-myc proteins. The clinicopathological parameters were then analyzed. Re-sults: The positive rates of Aurora-A expression in normal colorectal tissues, pericancerous tissues, and colorectal cancer were 0%, 35%, and 69%respectively;by comparison, the positive rates of mt-P53 were 0%, 20%, and 57%, respectively. For c-myc, the positive rates were 0, 37%, and 76%, respectively. The expression levels of Aurora-A, mt-P53, and c-myc were significantly higher in tumor tis-sues than in normal colorectal tissues and pericancerous tissues (P<0.01). Aurora-A overexpression was related to the depth of invasion (P<0.05). Mt-P53 and c-myc overexpression was related to the depth of invasion, lymph node metastasis, and Dukes' classification (P<0.05). A strong positive correlation was observed between the expressions of Aurora-A, mt-P53, and c-myc in colorectal cancer (r=0.362, P<0.01; r=0.487, P<0.01). A strong positive correlation was also observed between the expressions of mt-P53 and c-myc in colorectal cancer (r=0.242, P<0.01). Conclusion:The overexpression of Aurora-A and c-myc and the mutation of P53 were important in tumorigenesis, tumor invasion, and metastasis of colorectal cancer. Thus, the co-detection of Aurora-A, mt-P53, and c-myc may be useful for the early diagnosis and prognosis of colorectal cancer.
4.Role of Endogenous Arginine Vasopressin in Physostigmine Induced Hypothermic Response in Rats

Objective To study the subcutaneous injection of physostigmine (PHY) inducing hypothermic response and its relationship with endogenous arginine vasopressin ( AVP). Methods Core temperature and motor activity were monitored by telemetry in female rats maintained at an ambient temperature of 25℃. Tail skin temperature was measured at 30 min intervals with digital thermometer. The central cholinergic antagonist, scopolamine ( lmg/kg ) and AVP V_1 receptor antagonist were administered during the period of PHY (0. 25mg/kg) induced hypothermia. Plasma AVP concentration was measured at 50 min after administration of PHY. Results Subcutaneous injection of PHY led to a rapid reduction in core temperature concomitant with a marked elevation in the heat loss from the tail. The hypothermic response of PHY was blocked by scopolamine and AVP V1 antagonist. Plasma AVP concentration increased markedly at 50 min after PHY. Conclusion The results suggest that endogenous A VP could be involved in PHY -induced hypothermic processes. This may be a novel mechanisms of action for a reversible anticholinesterase drug ( such as PHY ) - induced hypothermia.

The amounts of iron and amino acids in dried pig powder were determined. It contained a large quantity of iron (230-260m/100g) and all essential amino acids. The blood fortified wheat flour biscuit containing 15mg Fe/100g was made and 30?2g of the biscuit was given to 65 primary school students (age 8-10) per capita per day during school day break for 40 days from Sep to Oct in 1984. For the control group, equal amount of the same biscuit without blood fortification was given to 34 subjects of the same age. AIM: To study the interaction effects of Astragalus polysaccharide(APS) and total glucosides of paeony(TGP) with various dosage on the platelet aggregation. METHODS: Platelet aggregation was estimated by the turbidimetric method. The best interaction effects were determined by uniform design method. RESULTS: APS showed apparent anti-platelet effects and better combination with TGP,and the ratio of 121 was the best one. CONCLUSION: APS concerted with TGP inhibits platelet aggregation.
7.Clinical application of stereotactic body radiation therapy for colorectal cancer with liver metastases

Objective To evaluate the efficacy of stereotactic body radiation therapy (SBRT) for hepatic metastases from colorectal cancer,and to collect data for the application of this technique.Methods A total of 28 patients from No.306 Hospital of PLA,including 17 male and 11 female with median age of 63.8 (range from 31 to 86),were treated with SBRT for colorectal cancer with liver metastases with 54 lesions in total.The GTV,CTV and PTV were delineated above the enhanced CT scans acquired during normal quiet respiration.CTV was obtained by adding 5 mm isotropic margin from GTV,and PTV was obtained by adding 5 to 10 mm isotropic margin from CTV.Prescription dose line covered 50%-60% of isodose curve at 3-6 Gy/fraction.The total dose was 39-45 Gy and the biologically equivalent doses(BED)was 50.7-65.3 Gy.The patients were followed-up beginning at 3 months after SBRT.The change in size of the lesion based on enhanced CT or MR scans was evaluated.Toxicity was evaluated and scored according to the RTOG criteria.Local control rate and survival rate were analysed.Results All patients completed the treatment.With median follow-up of 15.1 months (range frome 3 to 30 months),7 patients survived at the end of follow-up.The local control rate (LC) was 79.2%,and 1-and 2-year overall survival rate(OS) were 82.7% and 48.6%,respectively.There was a close corelation between the size of lesion and the LC.The LC (PR + CR) was much better at the size of lesion less than 14 cm3 than that at the size more than 65 cm3(x2 =4.17,P<0.05).When the size was more than 180 cm3,the LC was zero.Toxicity included fatigue (60.7%),grade 1 and 2 digestive system toxicity (28.6%),a transient grade 1 and 2 bone marrow suppression (46.4%) and a transient increase in transaminase(17.8%).No grade 3 toxicity and above and late toxicity were observed.Conclusions Stereotactic body radiation therapy could be suggested as the first choice for the selected patients who suffer form colorectal liver metastases,especially for those who cannot undergo surgery.
8.Establishment of a off-pump leukocyte depletion method in canines

Objective To establish a off-pump leukocyte depletion method in canines.Methods Twenty-one healthy adult mongrel dogs of both sexes, weighing 10-12 kg, were randomized into 2 groups: control group (group C, n = 9) and leukocyte depletion group (group LD, n = 12) . An extracorporeal leukocyte filtration end was constructed by aseptic puncture of the bilateral external jugular veins via a blood transfusion line and the leukocyte filter for extracorporeal leukocyte depletion. In group LD, blood was filtered with a MP-300 blood line and a SQ40S leucocyte depletion filter one end placed in the right external jugular vein (artery end) and the other end placed in the left external jugular vein (venous end). After heparin anticoagulation, a MP-300 blood line pump was used as the power. Blood was filtered at a rate of 75 ml/min and it was maintained for 60 min. The artery end was then closed, normal saline injected into the closed circuit, the remaining blood pumped into the body, and then the venous end closed. In group C, aseptic puncture of the bilateral external jugular veins was performed. Arterial blood samples were taken immediately before leukodepletion (baseline, T_0 ) , at 10, 20, 30, 40, 50 and 60 min of leukodepletion (T_(1-6) , depletion period), and at 10, 20, 30, 40, 50, 60, 120, 180 and 270 min after leukodepletion (T_(7-15) , recovery period) for determination of blood routine. MAP, HR, RBC.Plt and body temperature were recorded at T_0 , T_6 , T_(12) and T_(15) . Results There were no significant difference in MAP and RBC between the two groups ( P > 0.05 ) . HR, body temperature, and Plt were significantly lower in group LD than in group C (P < 0.05) .The leukocyte concentration was lower during depletion period in group LD than in group C ( P < 0.05) , while there was no significant difference in leukocyte concentration during recovery period between the two groups (P > 0.05 ). Conclusion The off-pump leukocyte depletion method is successfully established and has exact efficacy with less adverse effects in this study.
9.Analysis on volume modulated arc therapy for spinal metastases

Objective To evaluate clinical effect of volume modulated arc therapy ( VMAT) for spinal metastases .Methods Twenty patients with spinal metastases who had been treated with VMAT were chosen to participate in the study .The prescribed dose varied from 45 to 60 Gy within 15 -20 fractions, 3 Gy per fraction, and one fraction per day of VMAT .Pain and quality of life were measured before VMAT and at several time points up to 6 months after treatment , by the numerical rating scale (NRS) and verbal rating scale (VRS) and the quality of life scale for bone metastases (EORTC QLQ-BM22) questionnaire.In addition, Frankel grading was used to evaluate the neurological function of spinal cord.The primary endpoint was frequency and duration of complete pain relief , and the secondary endpoint was death.Results At the end of the follow-up, the number of patients reporting no pain from spinal metastases, as measured by the NRS, increased from 0 of 20 before VMAT to 10 of 14 ( t =20.24, P<0.05) .NRS-reported pain reduction from baseline to 1 weeks and 4 weeks after VMAT were clinically meaningful with NRS score (6.50 ±0.51) at baseline,(4.30 ±0.47)at 1 week and(2.50 ±0.50)at 4 weeks (t=15.98, 27.57, P<0.05).The pain reduction maintained until the end of the follow-up.These improvements were accompanied by significant reduction in opioid use during the first 6 months after VMAT (16 of 20 patients with strong opioid use at baseline vs.6 of 18 at 6 months; t =8.46, P <0.05). According to EORTC QLQ-BM22 questionnaire , patients obtained significant improvement in pain location , severity, and function with the score (46.50 ±1.50)at baseline, (38.35 ±0.98) at 4 weeks(t=21.51, P<0.05).However,no significant alleviation in social psychology was reported (P>0.05).After VMAT, the patients who suffered from nerve function impairment recovered to different extent . No one had radiation-induced injury , such as radiation myelitis , radiation pneumonitis , etc.Median survival time was 10 month.Conclusions VMAT is a safe and effective treatment method for spinal metastases .Significant reductions in patient-reported pain were observed , along with nerve dysfunction improved .The patients′quality of life was significantly improved .VMAT has no late spinal cord toxicities .
