1.Application of health education of telephone visit in patients with chronic obstructive pulmonary disease

ObjectiveTo improve the compliance behavior of discharged patients with chronic obsturctive pulmonary disease(COPD)by health education of telephone visit.Methods240 COPD patients were divided randomly into the experimental group and the control group with 120 cases in each group.The experimental group accepted telephone visit regularly for six months after 1 week since discharged from hospital and the control group did not receive any intervention.Six months later,the compliance behavior and the readmission of two groups were compared.ResultsThe results indicated that the compliance behavior of the experimental group was significantly better than that of the control group (P<0.01).ConclusionsConducting the health education of telephone visit can strengthen the patients' compliance behavior,postpone the process of disease,prevent complications,lighten economic burden and improve their life quality.
2.Knowledge,attitude and related factors associate with HIV/AIDS among female prisoners in Shaanxi province

Objective To investigate the knowledge,attitude and related factors associated with HIV/AIDS among female prisoners in Shaanxi province.Method A cross-sectional survey with questionnaire was conducted in 837 female prisoners.Results Of the studied female prisoners 94% had heard of HIV/AIDS.The rate of correct answer on basic knowledge about AIDS was 19.8%-87.3%.The rate of correct answer on the transmission route was 11.6%-87.6%.The rate of correct answer on non-transmission route was 23.4%-48.6%.The rate of correct answer on prevention was 36.7%-57.5%.Of them 46.6%-55.7% were afraid of AIDS,and 26.6%-90.0% had prdjudice and stigma to HIV/AIDS and PLWHA(people living with HIV/AIDS).The factors affecting HIV/AIDS related knowledge included marriage status,career,education background and number of family members.The factors affecting attitude included age and education background.Conclusion The studied female prisoners have wide but superficial knowledge about HIV/AIDS and have expressed over fear for HIV/AIDS.They also cannot tolerate AIDS patients.

5.NOTCH3 gene diagnosis in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy

Objective To analyse the mutation types of the NOTCH3 gene with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy(CADASIL). Methods Including 4 probands of CADASIL,10 cases of CADASIL and 4 normal members from 3 CADASIL families and 100 healthy controls were recruited. Genomic DNA was extracted from white blood cell. The amplicons were analyzed by the denaturing high-performance liquid chromatography (DHPLC) technique. The positive samples which identified by the DHPLC were sequenced to determine the specific mutation or polymorphism, respectively. Results Three heterozygous missense mutations including Cys90Arg, Arg141Cys, Cys134Tyr located in the exon3 and exon4 were found in the 4 probands and 10 cases of CADASIL among the 3 families. 15 polymorphisms were also found. 2 members individual from family 1 and 2 were found to carry the same pathological mutations as in their proband but without clinical symptoms. They were identified as preclinical patients. Conclusions Mutation detection of NOTCH3 is the molecular genetic mechanical for CADASIL. The exon3 and exon4 are possible hot mutation spots in Chinese patients. The mutation of Cys134Tyr in exon4 is a novel mutation which has not been reported previously in China.
6.Relationship between the level of serum homocysteinaemia and carotid artery atherosclerosis and its change after intervention therapy

Objective To explore the relationship between the level of serum homocysteinaemia(Hcy) and the carotid artery atherosclerosis(CAA),and its change after intervention therapy with folate acid and VitaminB12 (VitB12). Methods The levels of serum Hcy,folate acid and VitB12 were measured in 169 patients with cerebral infarction(CI group) and 110 normal controls(NC group). The condition of carotid arteries was checked by color dopper flow image in CI group. The relationship between the level of serum Hcy and folate acid,VitB12 and CAA condition were analysed. Objective To analyze the causes and explore suitable treatments for the complica-tions induced by polyacrylamide hydrogel (PAHG) injection for augmentation rhinoplasty. Methods The causes of the complications of 52 cases who accepted PAHG injection for augmentation rhinoplas-ty were analyzed and summarized. All the patients were treated by surgical operations to remove the injected PAHG. Results The complications included infection, granuloma, skin ulceration, bad shape, pain and serious psychological stress. Each case had 1 to 4 complications. Satisfactory results were obtained after suitable treatments. Some cases had silicone or expanded polytetrafluoroethylene implant augmentation rhinoplasty at the same time or secondary to the PAHG removal. A few cases had sequelae and long time psychological stress or tend pain of nose. Conclusions The complications induced by PAHG injection for augmentation rhinoplasty are various, and may relate to the inherent character of PAHG and the anatomic features of the nasal soft tissue. Surgery can remove the PAHG as completely as possible, which is a better method to treat the complications of PAHG injection for augmentation rhinoplasty.
8.A study of T cell recombination excision circles levels in peripheral blood mononuclear cells of systemic lupus erythematosus patients

Objective To compare the T cell receptor recombination excision cycle (TREC) levels in peripheral blood mononuclear cells (PBMC) of systemic lupus erythematosus (SLE) patients with normal age- and gender- matched controls. To investigate the correlations between TREC levels of SLE patients and their clinical features. Methods We studied TREC levels in peripheral blood mononuclear cells (PBMC) of 21 SLE patients and 22 normal age- and sex- matched controls. TREC concentration was determined by real-time quantitative polymerase chain reaction (real-time qPCR) as the number of TREC copies/1000 PBMCs. The clinical features of the SLE patients such as systemic lupus erythematosus disease activity index (SLEDAI) , ESR, C reaction protein (CRP) , ANA, anti-dsDNA and complement levels and organ involvement were recorded and assessed. Results SLE patients had lower TREC levels [ (9.6 ± 7.5 )copies/1000 PBMC] than controls[ (16.1 ±11.1) copies/1000 PBMC,P = 0.033]. There was an inverse correlation between age and TREC levels in controls (r =- 0. 614, P = 0. 002) but not in SLE patients.There was an inverse correlation between SLEDAI and TREC levels in SLE patients(r =-0. 656, P =0. 001) and TREC levels seemed to have relations to skin lesions ( r = - 0. 620, P = 0. 003 ). No other clinical association was observed between TREC levels and clinical and laboratory SLE manifestations.Conclusion SLE patients had lower TREC levels than normal controls and there is a tendency that TREC level is reversely correlated with disease activity. The decrease PBMC TREC level is indicative of a low proportion of recent thymic emigrant (RTE) in SLE and could be caused by decreased RTE output and/or by increased peripheral T cell proliferation in this disease. The under-representation of RTE in the peripheral T cell pool may play a role in the immune tolerance abnormalities observed in SLE.

10.Gastric varices bleeding treated by tissue adhesive, D-TH glue injection

Objective To study on gastric varices bleeding treated by tissue adhesive, D-TH glue injection. Methods Fifty-nine consecutive cirrhotic patients with gastric varices were undergone endoscopic D-TH glue injection with sandwich approach. Among these 59 patients, bleeding was present in 46 patients (acute bleeding in 37, recent bleeding in 9) , no previous of gastric variceal bleeding in 13. Follow-up study on therapeutic results and complications were conducted. Results Ninety-six injections were performed in all patients with a mean of 1. 63 injections (1-2ml) per patient. Recurrent bleeding never occurred in 37 patients with acute gastric varices bleeding 72 hours after injection, and the rale of emergency endoscopic he mostasis was 100 per cent. Thirty (65. 2% ) of 46 patients with bleeding gastric varices were followed for 12 36 months, 24. 9 months on average. Recurrence of bleeding occurred in 2(6. 7% ) , 5 ( 16. 7% ) and 8 (26. 7% ) cases at 3 , 12, and 36 months respectively. The rate of survival was 86. 5 per cent. Besides fever occurred postoperatively in 2 cases, other severe complications never happened. Conclusion Endoscopic D-TH glue injection is a safe and effective method for treating gastric varices bleeding.
